경로당 이용 노인의 낙상경험과 낙상 예측요인 (Experience of Falls and Predictors of Falls in the Elderly at Senior Citizens' Centers)

  • Purpose: The purpose of this study was to investigate the experience of falls, the fear of falling and fall efficacy and identify the predictors of falls in the elderly at senior citizens' centers. Method: The subjects of this study were 106 elders who used senior citizens' centers in Chungcheongbuk-do, Korea. Data were collected from May to June, 2006 through a survey using a structured questionnaire. Result: Of the elderly sampled, 35.8% experienced falls during the last 1 year. Of falls experienced by the elderly, 53% occurred indoors, 47% outdoors, 15.8% in the bathroom or toilet. In addition, 57.8% of the elderly had injuries on the hip and waist related with falls. The fear of falling was higher in those with experience in falls than in those without. Factors affecting the elders' falls were age (OR=1.113, 95% CI=1.012-1.224), the number of chronic disease (OR=2.342, 95% CI=1.365-4.019) and the fear of falling (OR=4.279, 95% CI=1.901-9.634). The predictor of the frequency of falls was fall efficacy ($R^2=24.9$). Conclusion: As a result, it is essential to develop fall prevention programs based on the fear of falling, fall efficacy, chronic diseases and medication state. And in senior citizens' centers, fall prevention safety education should be activated as a health promotion program.

노인의 신체기능과 낙상두려움과의 관계 (Relation of the Physical Performance and Fear of Falls of the Elderly)

  • 본 연구는 지역사회 거주 노인의 신체기능과 낙상두려움간에 상호 어떻게 영향을 미치는가의 관계을 살펴보고자 시도하였다. 65세 이상 노인 460명을 대상으로, 연구기간은 2014년 1월 3일부터 3월 30일까지이며, 본 연구에 자발적으로 참여할 것을 동의한 노인에게 직접 설문지를 읽어주고 대상자가 대답하는 방식으로 조사하였다. 인구사회학적 특성과 낙상에 관한 정보, 신체기능과 낙상두려움에 대하여 조사하였고 지난 1년동안 낙상을 경험한 노인은 139명(30.2%)으로 조사되었다. 신체기능에서 한 발 들고 균형 잡고 서 있기, 협응성 유연성 검사, 보행능력 검사가 낙상두려움간에 유의한 상관관계가 있는 것으로 나타났다. 본 연구 변수의 회귀모형을 분석한 결과 유의한 것으로 나타났으며, 모형의 설명력은 16.9%이고 낙상 두려움에 가장 큰 영향을 미치는 요인은 앉았다 일어서기 검사, 보행능력 검사로 조사되었다. 본 연구는 낙상 예방을 위한 보다 효과적인 중재 프로그램 개발에 기여할 것으로 사료된다.

재가 노인의 낙상 경험 유무에 따른 신체적 기능과 정서 상태 (Physical Function and Psychological Status in the Elderly those who Experienced a Fall or not)

  • Introduction : The purposes of this study was to examine physical function and psychological status in the elderly caused by a fall. Method : The study was designed as a descriptive survey. Its subjects consisted of the 299 elderly over 65 years. Collected data were analyzed by the SPSS 10.0 program package. Results : The result of this study are as follows : 1) The score for ADL was significantly lower in the fall group. 2) Grip strength was significantly lower in the fall group. 3) Lapse of the motion sitting and standing was significantly longer in the fall group. 4) One-leg-standing time with the eyes open and closed was significantly shorter in the fall group. 5) Return time of 3m walk was significantly longer in the fall group. 6) The score for dizziness was significantly higher in the fall group. 7) The score for fear for a fall and depression were significantly higher in the fall group. 8) The score for falls efficacy was significantly lower in the fall group. Conclusion: ADL, muscle strength, balance, dizziness, fear for a fall, falls efficacy and depression turned out to be closely related to the fall of the elderly. Therefore, it would be required to develop and applicate the fall prevention program regarding these above risk factors.

농촌지역 노인을 위한 낙상예방운동의 효과 (Effects of an exercise program for fall prevention of the elderly in rural communities)

  • 본 논문은 노인의 건강을 위협하는 가장 심각한 문제 중의 하나인 낙상을 예방하기 위하여 2014년 5월부터 6월 까지 1주에 2번씩 6주 동안 낙상을 예방하기 위한 운동을 시행하였고, 시행 전 후 TUG, FRT, BBS, ADL, 낙상공포감, 낙상효능감, 우울의 정도를 비교하여 운동의 효과를 분석하였다. 그 결과 본 연구의 가설 검정에서 제1가설 "낙상예방운동에 참여한 노인은 참여하기 전보다 FRT가 향상될 것이다"의 T-test 검정 결과는 사전 사후(F=3.14, p<.05)로 TUG 점수는 통계적으로 유의한 차이를 보였고, 제3가설 "낙상예방운동에 참여한 노인은 참여하기전보다 BBS(균형정도)가 향상될 것이다"의 T-test 검정 결과는 사전 사후(F=-5.74, p<.001)로 BBS의 점수도 통계적으로 유의한 차이를 보였다. 그 외 FRT, ADL, 낙상공포감, 낙상효능감, 우울의 점수는 통계적으로 유의하진 않으나 개선된 결과를 보여 본 연구에서 적용했던 운동이 농촌 노인의 낙상예방에 효과가 있었음을 확인하였다.

  • Purpose: The purpose of this study was to determine the effects of a fall prevention program on gait ability, one leg standing-right, one leg standing-left, fear of falling, fall efficacy, and depression in the elderly. Methods: A fall prevention program was consisted of fall preventing exercise with laughter therapy. Quasi-experimental design with one group pre- and post-test was used. Twenty-two elderly were participated in this study from a community health center located in a rural area, South Korea. SPSS/WIN 18.0 was used for descriptive analysis and paired t-test. Results: After 12 weeks with a fall prevention program, gait ability (t=3.44, p=.002), one leg standing-right (t=-3.73, p=.001), one leg standing-left (t=-4.17, p<.001) and fear of falling (t=2.12, p=.046) in the elderly were significantly improved. Conclusion: A fall prevention program was effective on physical and psychological functions. This study suggested that control groups and a larger sample should be included in order to validate the effects of a fall prevention program for the elderly. 낙상예방프로그램이 노인의 근력, 균형 및 낙상두려움에 미치는 효과 (Effects of Fall Prevention Program on Muscle Strength, Postural Balance, and Fear of Falling in Elderly)

  • Purpose: This study was to develop a fall prevention program and test the effects of the program on the muscle strength, postural balance and fear of falling for elderly. Methods: This study was conducted using a nonequivalent control group pretest-posttest design. There were 21 participants in the experimental group and 22 in the control group. Data were collected from July to September 2015. Questionnaires were completed three times (at pretest and after 8 and 10 weeks). Data were analyzed using $x^2$ test, Fisher's exact test, t-test, and repeated measure ANOVA. Results: Lower extremity muscle strength and postural balance scores were significantly improved in the experimental group compared to the control group. However, fear of falling was not significantly different between the experimental and control groups. Conclusion: The study results indicate that fall prevention programs for the elderly can be useful to improve muscle strength and postural balance.

폐경 후 여성의 낙상예방행위 예측모형 (A Predictive Model of Fall Prevention Behaviors in Postmenopausal Women)

  • Purpose: This study was done to propose and test a predictive model that would explain and predict fall prevention behaviors in postmenopausal women. The health belief model was the theoretical basis to aid development of a nursing intervention fall prevention program. Methods: Data for 421 postmenopausal women were selected from an original data set using a survey design. The structural equation model was tested for 3 constructs: modifying factors, expectation factors, and threat factors. Expectation factors were measured as relative perceived benefit (perceived benefit minus perceived barrier), self-efficacy, and health motivation; threat factors, as perceived susceptibility (fear of falling) and perceived severity (avoiding activity for fear of falling); and modifying factors: level of education and knowledge about fall prevention. Data were analyzed using SPSS Windows and AMOS program. Results: Mean age was 55.7 years (range 45-64), and 19.7% had experienced a fall within the past year. Fall prevention behaviors were explained by expectation and threat factors indicating significant direct effects. Mediating effect of health beliefs was significant in the relationship between modifying factors and fall prevention behaviors. The proposed model explained 33% of the variance. Conclusion: Results indicate that fall prevention education should include knowledge, expectation, and threat factors based on health belief model.

독거여성노인의 낙상경험에 따른 낙상두려움과 관련요인 (Fear of Falling and Related Factors in Elderly Living Alone Based on Fall Experience)

  • 본 연구는 상관성 조사연구 설계를 이용하여 낙상경험 유무에 따른 낙상두려움 영향 요인을 파악하기 위해서 수행하였다. 연구대상자는 전라남도 1개시에 거주하는 낙상군 148명, 비낙상군 256명 총 404명을 대상으로 구조화된 설문지를 이용 하여 편의표집 하였으며 조사 기간은 2013년 4월 22일부터 6월 9일까지이었다. 연구결과 낙상률은 36.6%였으며 낙상군과 비낙상군은 연령, 만성질환 수, 영양위험도, 보조기구 사용, 시력장애, 청력장애에서 두 집단 간 유의한 빈도 차이를 보였다. 일반적 특성에 따른 낙상두려움 정도는 낙상군은 영양불량위험도, 보조기구 사용, 시력장애, 거주형태, 주거환경문제수에서 통계적으로 유의한 차이를 나타냈으며, 비낙상군은 연령, 종교, 영양위험도, 보조기구 사용, 시력장애, 청력장애, 거주형태, 주거환경문제수에서 통계적으로 유의한 차이를 나타냈다. 낙상군은 비낙상군에 비해 낙상두려움, 일상활동장애, 우울은 유의하게 더 높았고 낙상효능감과 주거환경 문제수는 낮았으며 정적균형, 복합이동능력, 생활만족도는 두 집단간 유의한 차이가 없었다. 상관관계를 측정한 결과 낙상군은 낙상두려움과 생활만족도, 낙상효능감 간에 유의한 음의 상관성을 보였으며, 일상활동장애, 우울, 주거환경문제수와는 유의한 양의 상관성을 보였다. 비낙상군은 낙상두려움과 생활만족도, 낙상효능감은 유의한 음의 상관성을 보였고 일상활동장애, 복합이동능력, 우울, 주거환경문제수와는 유의한 양의 상관성을 보였다. 낙상군의 낙상두려움 영향요인은 일상생활장애, 우울, 낙상효능감, 정적균형, 보조기구사용으로 46.1%의 설명력을 보였으며, 비낙상군은 낙상효능감, 주거환경문제수, 일상활동장애. 영양위험도, 주거형태, 현기증, 보조기구사용 등으로 55.2%의 설명력을 보였다. 이상의 결과를 바탕으로 독거여성노인 낙상두려움은 신체적 특성과 심리적 특성 및 주거 환경적 특성과 관련이 있는 것으로 나타났지만 낙상경험에 따라 차이가 있으므로 독거노인 의 낙상두려움을 감소시키기 위해서는 낙상경험을 고려한 중재가 필요함이 확인되었다. 그러나 낙상두려움이 재낙상의 중요한 관련 변수이기는 하지만 교정가능성이 높은 점을 고려하여 낙상두려움 정도 및 위험요인에 대한 주기적 평가를 통해 고위험군을 발견하도록 하고, 이들을 대상으로 낙상경험에 따른 통합적 중재 프로그램이 맞춤형으로 개발 제공되어 져야 할 것이다.

태극운동이 낙상 위험 노인의 신체적, 심리적 기능 및 낙상발생에 미치는 효과 (The Effects of Tai Chi Exercise on Physiologic, Psychological Functions, and Falls among Fall-Prone Elderly)

  • This study was a quasi-experimental study of non-equivalent control group pretest and posttest design. The purpose of this study was to determine the effect of Tai Chi exercise program on physical functions, psychological functions, and fall among the fall-prone elderly. The data were collected from September 19, 2001 to January 31, 2002. The study, conducted at two facilities located in Kwang-ju, was targeted to the ambulatory aged 60 years or older who had at least one of the key fall risk factors. Experimental group participated in Tai Chi exercise for 40 minutes per one time and three times a week for 12weeks at an auditorium. Fifty nine fall-prone elderly were assigned to 12-week Tai Chi exercise program (n=29) and control group (n=30). They underwent tests of lower muscle strength, time for chair stand, balance, flexibility, depression, falls efficacy, fear of falling, and numbers of fall at the baseline and at the 12th week. Numbers of fall and fall injuries were monitored for 16 weeks(12-weeks intervention plus 4-week follow-up periods) using fall calendar. Each participant was given a calendar to record the numbers of fall per day for a month. The calendars were collected at the last week of each month. 1. Tai Chi exercisers showed significant improvement in the strength of knee flexors, and ankle dorsiflexors and plantarflexors compared to the control group. The experimental group had improvement in the strength of knee extensors while the control group did not, with no statistical significance. 2. Tai Chi exercisers reported positive change in the average time of chair stand as compared to the control group. 3. Tai Chi exercisers had significant improvement in flexibility as compared to the control group. 4. There was no significant difference in the depression between the two groups, even though the Tai Chi exercisers maintained depression score in the same level while the others were increased. 5. Tai Chi exercisers showed significant improvements in the falls efficacy as compared to the control group. The falls efficacy was significantly improved among the experimental group while the opposite was identified among the control group. 6. The experimental group reported the significant reduction of the fear of fall, whereas control group reported the opposite. 7. Of the 59 subjects for 16weeks(12weeks intervention period and 4weeks follow up), 9 (31.0%) of the 29 in exercise group and 15 (50%) of the 30 in the control group fell (relative risk=0.62. 95% CI 0.32-1.19), even with no statistical difference. The results suggest that the Tai Chi exercise program can improve the strength of knee flexors, ankle dorsiflexors and plantarflexors, chair stand. flexibility, falls efficacy, and fear of falling for the fall-prone elderly.

낙상 위험요인에 대한 타이치 운동 효과의 메타분석 (Effects of Tai Chi on Fall Risk Factors: A Meta-Analysis)

  • Purpose: This study was done to analyze the effects of Tai Chi on fall-related risk factors through meta-analysis of randomized clinical trials published in English and Korean between 2000 and 2010. Methods: Using health related database and hand search of references and Google, 28 randomized studies were collected from doctoral dissertation and published peer reviewed articles. The Comprehensive Meta-analysis version 2.0 was used for the analysis. Results: The effect sizes for Tai Chi for 3 months were significant with ES=0.54 for static balance, ES=0.24 for dynamic balance, ES=0.69 for balance measured by scale, and ES=0.40 for flexibility, ES=0.48 for muscle strength, ES=0.71 for ADL, and ES=0.37 for fear of falling. Also, the effect sizes of Tai Chi for 6 months were significant for most fall-related variables. The 6 month data for flexibility was not analyzed since only one study was published. Conclusion: The analysis of studies of randomized clinical trials indicate that Tai Chi is effective in improving balance, flexibility, muscle strength, activities of daily living, and fear of falling when applied for 3 or 6 months. The findings provide the objective evidence to apply Tai Chi as a fall preventive intervention.
