This study aimed to identify the change patterns of fatigue and quality of life during consecutive chemotherapies and to determine the relationship of these two variables. Method: Stomach cancer patients receiving chemotherapy were recruited from a university hospital in Seoul. Each chemotherapy, subjects were asked to respond to the questionnaires regarding their fatigue and quality of life. The number of subjects who completed 4 cycles and over was 11. Fatigue was measured with Lee's tool(1999). Quality of life was measured with a tool revised by the author based on Padilla et al(1983). Result: Most patients were in 1st stage(5 patients) or 3rd stage(5 patients). Fatigue was revealed at its highest level in the 3rd or 4th chemotherapy and at its lowest level in the 1st or 6th chemotherapy. A quality of life appeared at its highest level in the 5th or 6th chemotherapy and the lowest level in 3rd or 4th chemotherapy. Conclusion: Among 6 cycles of chemotherapy, in 3-4th chemotherapy the fatigue was the highest and the quality of life were the lowest. Many patients decided to stop treatment at the same period. Therefore we can recognize cancer patients receiving chemo- therapy are in the highest risk at the time of the 3-4 th chemotherapy.

This study was performed to find out the relationship between farmer's syndrome and self-conscious fatigue symptoms in farmers of suburban area. The questionnaire was prepared to survey the socio-demographic characteristics, health-related behaviors, farming activities, farmer's syndrome, and self-conscious fatigue symptoms of the farmers, and the survey was carried out from April 1 to June 30, 2011 by interviewing 296 farmers residing suburban area of Daejeon city. As a results, the ratio of farmers who had positive farmer's syndrome, doubtful syndrome, and no syndrome were 34.1%, 47.6%, and 18.2%, respectively. The farmer's syndrome was positively correlated with the self-conscious fatigue symptoms, and farmers with self-conscious fatigue symptoms in high risk group were at higher risk of having farmer's syndrome. The results of hierarchic multiple regression analysis indicated that sex, education, health, and self-conscious fatigue symptom were correlated with the farmer's syndrome. Among the affective factors variables, independently self-conscious fatigue symptom was the most influential. As a conclusion, the occurrence of farmer's syndrome was influenced by the socio-demographic characteristics, health-related behaviors, and self-conscious fatigue symptoms of the farmers. Especially, self-conscious fatigue symptom was independently influenced the occurrence of farmer's syndrome.
Objectives : The purpose of this systematic review is to confirm whether Gwibitang is beneficial in chronic fatigue syndrome (CFS). Methods : Clinical trials were searched from databases including Pubmed, Embase, Central Cochrane, CNKI, Wanfang, CQVIP, CiNii, OASIS, Koreamed, and NDSL. The eligible study design was limited into randomized controlled trial, quasi-randomized controlled trial and controlled clinical trial. The outcomes included general effectiveness as nominal scale, and fatigue severity, insomnia severity and quality of life as interval or ratio scale. The meta-analysis and assessment of risk of bias was performed based on the data extracted from the selected trials. Results : The results of eight randomized controlled trials (n=596) were included in the meta-analysis. The results of the synthesis showed Gwibitang is beneficial substantially for relieving and managing the general symptoms, and its heterogeneity was not in important level (RR 0.26 [95% CI 0.17, 0.39], Z=6.47, P<0.00001, I2=0%). Gwibitang was beneficial for alleviating fatigue (SMD -0.78 [95% CI -1.27, -0.30], Z=3.17, P=0.002), but its certainty was low. In case of insomnia, too few trials had been found and their risk of bias was substantial, so no conclusions had been brought to. Conclusions : We found an evidence that Gwibitang could be beneficial for managing and alleviating main symptoms in CFS patients.
Many risk factors exist for chemotherapy-induced nausea and vomiting (CINV). This study utilized a multivariate projection technique to identify which risk factors were predictive of CINV in clinical practice. A single-centre, prospective, observational study was conducted from January 2007~July 2010 in Singapore. Patients were on highly (HECs) and moderately emetogenic chemotherapies with/without radiotherapy. Patient demographics and CINV risk factors were documented. Daily recording of CINV events was done using a standardized diary. Principal component (PC) analysis was performed to identify which risk factors could differentiate patients with and without CINV. A total of 710 patients were recruited. Majority were females (67%) and Chinese (84%). Five risk factors were potential CINV predictors: histories of alcohol drinking, chemotherapy-induced nausea, chemotherapy-induced vomiting, fatigue and gender. Period (ex-/current drinkers) and frequency of drinking (social/chronic drinkers) differentiated the CINV endpoints in patients on HECs and anthracycline-based, and XELOX regimens, respectively. Fatigue interference and severity were predictive of CINV in anthracycline-based populations, while the former was predictive in HEC and XELOX populations. PC analysis is a potential technique in analyzing clinical population data, and can provide clinicians with an insight as to what predictors to look out for in the clinical assessment of CINV. We hope that our results will increase the awareness among clinician-scientists regarding the usefulness of this technique in the analysis of clinical data, so that appropriate preventive measures can be taken to improve patients' quality of life.

Aircraft MLG and wing structures have been recognized as fatigue critical structures and exposed to the risk of fatigue crack initiation and propagation. Furthermore, these structures are frequently subjected to serious dynamic loading condition during a Hard Landing which may lead to their failure. Especially, structural integrity of MLG and wing components is decreased as the flight time increased because of the fatigue damage accumulated on the aircraft. In this study, the effects of Hard Landing on the MLG and wing components of aging aircraft were evaluated by using numerical approach. To achieve the aim, a finite element model has been developed and simulations were conducted by varying the landing conditions. As a result, it was revealed that the high stress concentration phenomenon was occurred at the lower Side Brace of MLG. Thereby, the intensified inspection for the lower Side Brace should be considered to prevent unexpected aircraft mishap.

국내외 해양사고 통계에서 선원 피로가 해양사고의 주요 원인 중 하나로 꼽히고 있다. USCG의 보고에 의하면 선박충돌 및 좌초사고를 포함한 중대한 해양사고에서 인간과실의 원인으로 선박승무원의 피로가 약 16% 기여한 것으로 조사되었고, 인명손상사고의 약 33%가 피로에 의한 것으로 나타났다.(USCG, 1996). 또한 영국 해양사고조사국(MAIB)에 의하면 1994년부터 2003년까지 10년간 전체 해양사고의 1/3에서 선원피로가 사고원인으로 기여하였음이 확인되었다(MAIB, 2004). 2003년 스웨덴에서의 연구결과 약 73% 정도의 당직근무자들이 당직근무 기간 중 최소 1회 또는 그 이상 졸음에 빠져들었다는 사실이 확인되었다(KMST, 2012). 최근 개최된 IMO 인적요소훈련당직 전문위원회(Sub-Committee On Human Element, Training And Watchkeeping ; HTW) 회의에서는 MSC 74차에 회의에서 최종 승인된 "피로에 관한 지침서(Guidelines on Fatigue)"에 대한 개정 논의가 이루어지고 있다. 본 발표에서는 현재 IMO에서 논의 중인 "피로에 관한 지침"의 개정안과 개정안의 핵심 내용인 피로위험관리시스템(Fatigue Risk Management System)에 대한 연구개발 방향을 소개하고자 한다.
Aviation safety is the State and industry's top priority and more scientific approaches for fatigue management should be needed. There are lately various studies and regulation changes for crew fatigue management with ICAO, FAA and EASA. ICAO issued the provisions of fatigue management for flight crew since 1st edition, 1969, of Annex 6 operation of aircraft as a Standards and Recommended practice(SARPs). Unfortunately, there have been few changes and improvement to fatigue management provisions since the time they were first introduced. However the SARPs have been big changed lately. ICAO published guidance materials for development of prescriptive fatigue regulations through amendment 33A of Annex 6 Part 1 as applicable November 19th 2009. And then ICAO introduced additional amendment for using Fatigue Risk Management System (FRMS) with $35^{th}$ amendment in 2011. According to the Annex 6, the State of the operator shall establish a) regulations for flight time, flight duty period, duty period and rest period limitations and b) FRMS regulations. The Operator shall implement one of following 3 provisions a) flight time, flight duty period, duty period and rest period limitations within the prescriptive fatigue management regulations established by the State of the Operator; or b) a FRMS; or c) a combination of a) and b). U.S. FAA recently published several kinds of Advisory Circular about flightcrew fatigue. U.S. passed "Airline Safety and FAA Extension Act of 2010" into law on August 1st, 2010. This mandates all commercial air carriers to develop a FAA-acceptable Fatigue Risk Management Plan(FRMP) by October 31st, 2010. Also, on May 16, 2012, the FAA published a final rule(correction) entitled 'Flightcrew Member Duty and Rest Requirements; correction to amend its existing prescriptive regulations. The new requirements are required to implement same regulations for domestic, flag and supplemental operations from January 4, 2014. EASA introduced a Notice of Proposed Amendment (NPA) 2010-14 entitled "Draft opinion of the European Aviation Safety Agency for a Commission Regulation establishing the implementing rules on Flight and Duty Time Limitations and Rest Requirements for Commercial Air Transport with aeroplanes" on December 10, 2010. The purpose of this NPA is to develop and implement fatigue management for commercial air transport operations. Comparing with Korean and foreign regulations regarding fatigue management, the provisions of ICAO, FAA, EASA are more considering various fatigue factors and conditions. Korea regulations should be needed for some development of insufficiency points. In this thesis, I present the results of the comparative study between domestic and foreign regulations in respect of fatigue management crew member. Also, I suggest legal proposals for amendment of Korea Aviation act and Enforcement Regulations concerning fatigue management for crew members. I hope that this paper is helpful to change korea fatigue regulations, to enhance aviation safety, and to reduce the number of accidents relating to fatigue. Fatigue should be managed at all level such as regulators, experts, operators and pilots. Authority should change surveillance mind-set from regulatory auditor to expert adviser. Operators should identify various fatigue factors and consider to crew scheduling them. Crews should strongly manage both individual and duty-oriented fatigue issues.
Objectives : A precedent research has documented that occupational stress is closely associated with increased the risk of fatigue and decreased in job satisfaction. This study was conducted in an effort to assess the relationship of occupational stress to self-perceived fatigue and job satisfaction by using job strain model. Methods : The number of respondents was 122 dental hygienist who work in dental clinic and period of the investigation was July 2009 through september 2009. A structured questionnaire was employed to evaluate the participants' sociodemographics, job-related factors, health-related behaviors, occupational stress, job satisfaction and self-perceived fatigue. Occupational stress and self-perceived fatigue were assessed using the Korean Occupational Stress Scale-Short form (KOSS-SF) and the Multidimensional Fatigue Scale (MFS), respectively. Results : In job strain model, the ratio of Q2(High Strain Job) group that more susceptible to disease by stress than other group was 16% and the proportion of high fatigue group(Q3, Q4 group) and low job satisfaction group(Q3, Q4 group) was 48%, 45% respectively. In logistic regression analyses, a High Strain Job group was associated with higher odds of react factor(fatigue, job satisfaction) and the odds was down by 3.3%~7.5% after adjustment for age, smoking, alcohol drinking, regular exercise. So the effective strategy for fatigue, job satisfaction reduction for dental hygienist requires additional program focusing on innovated work environment that provide a enough leisure time and exercise program considering the personality traits. Conclusions : The results of this study suggest that occupational stress is a determinant predictor of self perceived fatigue and job satisfaction. Thus, a stress management program for the reduction of occupational stress, and the promotion of dental hygienist impact assessment health and quality of life is strongly recommended.

Purpose : Proprioceptive position sense plays a key role in providing joint stability, and multiple factors are related to proprioceptive position sense. Thus, this study aimed to determine the effects of body composition, particularly skeletal muscle mass on proprioceptive position sense following muscle fatigue. Methods : Healthy female subjects agreed to have their body composition analyzed. Only subjects who had 18.5-22.9 kg/㎡ of BMI (body mass index) were included in this study, and the participants were divided into two groups by skeletal muscle mass level. The experimental group had a level of skeletal muscle lower than the standard level (n=9), while the control group showed a standard or high level of skeletal muscle mass (n=11). To determine the change in proprioceptive position sense of the knee joint, the absolute angle error (AAE) was evaluated following muscle fatigue on low extremity. The muscle fatigue was induced by isokinetic resistance exercise program of Biodex system. AAE was measured by the Biodex system and compared the result before and after muscle fatigue. Results : The experimental group showed a significant AAE difference between before (3.16±2.48 °) and after (5.40±2.61 °) muscle fatigue. In addition, there was a AAE difference between the experimental (5.40±2.61 °) and control groups (3.53±1.67 °) after fatigue; however, there was no significance. Those results indicated that low level of skeletal muscle mass might influence the proprioceptive position sense of the knee joint after muscle fatigue. Conclusion : Thus, maintaining the proper level of skeletal muscle mass is pivotal to reduce the risk of injury following muscle fatigue in ADL or sport activities.
Background: Fatigue and sleepiness are inter-related and common among road transport drivers. In this study, sleep deprivation and fatigue among chemical transportation drivers were examined. Methods: A cross-sectional study surveying 107 drivers from three hazardous types of chemical production and transportation industries (nonflammable gases, flammable gases, and flammable liquids) was conducted. Data on sleep deprivation were collected using questionnaires of the Stanford Sleeping Scale and the Groningen Sleep Quality Scale. Fatigue was assessed using an interview questionnaire and a flicker fusion instrument. Results: Chemical drivers had a mean sleeping scale (Stanford Sleeping Scale) of 1.98 (standard deviation 1.00) and had a mean score of 1.89 (standard deviation 2.06) on the Groningen Sleep Quality Scale. High-risk drivers had higher scores in both the Stanford Sleeping Scale and the Groningen Sleep Quality Scale with a mean score of 2.59 and 4.62, respectively, and those differences reached statistical significance (p < 0.05). The prevalence of fatigue, as assessed through a critical flicker fusion analyzer, subjective fatigue question, and either of the instruments, was 32.32%, 16.16%, and 43.43%, respectively. Drivers who slept <7 hours and had poor sleep quality were found to have more fatigue than those who slept enough and well. Drivers who had a more sleepiness score resulted in significantly more objective fatigue than those who had a less sleepiness score. Conclusion: Sleep quality and sleeping hour can affect a driver's fatigue. Optimization of work-rest model should be considered to improve productivity, driver retention, and road safety.
