대사증후군에 대한 식이와 운동 효과에 대한 메타분석 (Effects of Dietary and Physical Activity Interventions on Metabolic Syndrome: A Meta-analysis)

  • Purpose: This study identified effects of dietary and physical activity interventions including dietary interventions or physical activity interventions alone or combined dietary-physical activity interventions to improve symptoms in metabolic syndrome including abdominal obesity, high triglycerides, low high density lipoprotein cholesterol, elevated blood pressure, and elevated fasting glucose through meta-analysis. Methods: Articles on metabolic syndrome X published from 1988 to 2013 were searched through electronic databases, Google Scholar, and reference reviews. Methodological quality was assessed by the checklist, SIGN (Scottish Intercollegiate Guidelines Network). Results: In the meta-analysis, there were 9 articles reporting 13 interventions with 736 participants. Using random effect models, the dietary and/or physical activity interventions showed a lower mean difference in waist circumference ( - 1.30 cm, 95% CI: - 2.44~ - 0.15, p=.027). The combined dietary-physical activity interventions showed a lower mean difference in waist circumference ( - 2.77 cm, 95% CI: - 4.77~ - 0.76, p=.007) and systolic blood pressure ( - 5.44 mmHg, 95% CI: - 10.76~ - 0.12, p=.044). Additionally, interventions of over 24 weeks yielded a lower mean difference in waist circumference ( - 2.78 cm, 95% CI: - 4.69~ - 0.87, p=.004) and diastolic blood pressure ( - 1.93 mmHg, 95% CI: - 3.63~ - 0.22, p=.026). Conclusion: The findings indicate that dietary and/or physical activity interventions for metabolic syndrome reduce central obesity with no adverse effects. This finding provides objective evidences for dietary and physical activity management on metabolic syndrome as an efficient intervention.

Evaluation of fish oil-rich in MUFAs for anti-diabetic and anti-inflammation potential in experimental type 2 diabetic rats

  • The advantages of monounsaturated fatty acids (MUFAs) on insulin resistance and type 2 diabetes mellitus (T2DM) have been well established. However, the molecular mechanisms of the anti-diabetic action of MUFAs remain unclear. This study examined the anti-hyperglycemic effect and explored the molecular mechanisms involved in the actions of fish oil- rich in MUFAs that had been acquired from hybrid catfish (Pangasius larnaudii${\times}$Pangasianodon hypophthalmus) among experimental type 2 diabetic rats. Diabetic rats that were fed with fish oil (500 and 1,000 mg/kg BW) for 12 weeks significantly reduced the fasting plasma glucose levels without increasing the plasma insulin levels. The diminishing levels of plasma lipids and the muscle triglyceride accumulation as well as the plasma leptin levels were identified in T2DM rats, which had been administrated with fish oil. Notably, the plasma adiponectin levels increased among these rats. The fish oil supplementation also improved glucose tolerance, insulin sensitivity and pancreatic histological changes. Moreover, the supplementation of fish oil improved insulin signaling ($p-Akt^{Ser473}$ and p-PKC-${\zeta}/{\lambda}^{Thr410/403}$), $p-AMPK^{Thr172}$ and membrane GLUT4 protein expressions, whereas the protein expressions of pro-inflammatory cytokines (TNF-${\alpha}$ and nuclear NF-${\kappa}B$) as well as p-PKC-${\theta}^{Thr538}$ were down regulated in the skeletal muscle. These data indicate that the effects of fish oil-rich in MUFAs in these T2DM rats were partly due to the attenuation of insulin resistance and an improvement in the adipokine imbalance. The mechanisms of the anti-hyperglycemic effect are involved in the improvement of insulin signaling, AMPK activation, GLUT4 translocation and suppression of pro-inflammatory cytokine protein expressions.

고지방식이로 비만이 유도된 C57BL/6 마우스에서 Metformin과 황금추출물의 병용 투여 시 비만관련 지표에 미치는 영향 (The Effect of Scutellariae Radix Combined with Metformin on Obesity-Relating Biomarker in High Fat Fed C57BL/6 Mice)

  • Objectives: The purpose of this study was to investigate anti-obesity effect of Scutellariae Radix extract combined with metformin. Methods: Male C57BL/6 mice, 4 weeks of age, were used to set high-fat diet induced obesity model. They were grouped NOR (normal control), HFD (high fat diet control), MET (metformin, 100 mg/kg/day), MH2 (metformin 50 mg/kg/day+Scutellariae Radix 200 mg/kg/day), and HG4 (Scutellariae Radix 400 mg/kg/day). MET, MH2, and HG4 were orally administered for 10 weeks. Body weight was measured every week. Fasting blood glucose, triglyceride, total cholesterol (TC), high density lipoprotein, glutamic oxaloacetic transaminase (GOT), and glutamic-pyruvic transaminase (GPT) were measured before sacrifice. Oral glucose tolerance test (OGTT) was also performed. Organ weight and internal fat weight were measured after sacrifice. Results: MH2 group showed a reduction of body weight when compared with HFD group. MH2 group showed stable blood level control which was calculated areas under the curves by OGTT. TC, GOT, GPT level, internal fat, and organ weight in MH2 group reduced. Conclusions: The combined treatment of Scutellariae Radix and Metformin has impact on treating obesity. Further studies are needed to evaluate the effect of Metformin and herbal medicine combination therapy.

Evaluation of Antidiabetic and Antihyperlipidemic Activity of Luffa tuberosa (Roxb.) Fruits in Streptozotocin Induced Diabetic Rats

  • Fresh unripe whole fruits of Luffa tuberosa (Roxb.) or Momordica tuberosa (Roxb.) Cogn. (Cucurbitaceae) were evaluated for the antidiabetic and hyperlipidemic potential in streptozotocin (STZ) induced diabetic rats. Diabetes was induced by administration of intra-peritoneal injection of streptozotocin at a dose of 55 mg/kg body weight. After the induction of diabetes aqueous extract of L.tuberosa (AELT) was administered orally at doses of 250 and 500 mg/kg. body weight/day for a period of 14 days. The Fasting blood glucose (FBG) levels, serum insulin levels, changes in body weight, food and liquid intake were measured. In diabetic rats, the AELT exhibited significant reduction in blood glucose levels. Biochemical assay of plasma total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDLc), low-density lipoprotein cholesterol (LDLc), glycogen content and glucose-6-phosphatase activity in liver were assessed. Antihyperlipidemia in diabetic rats after the extract supplementation was confirmed by significant reduction in the levels of above mentioned hyperlipidemic indicators. This study focus on the efficacy of L.tuberosa fruits for the management of experimental STZ induced diabetic rats and provides the scientific basis of ancient herbal therapy and use of these fruits as vegetable.

LC/MS/MS를 이용한 가바펜틴 300 mg 캡슐의 성인 지원자에 대한 생물학적 동등성 및 약물동태 연구 (Bioequivalence and Pharmacokinetic study of Gabapentin 300mg Capsules using Liquid Chromatography-Tandem Mass Spectrometry (LC/MS/MS) in Volunteers)

  • Gabapentin, 1-(aminomethyl-1-cyclohexyl)acetic acid, is anew antiepileptic drug related to ${\gamma}-aminobutyric$ acid(GABA) currently being introduced in therapy worldwide. The bioavailability and pharmacokinetics of gabapentin capsules were examined in 22 volunteers who received a single oral dose in the fasting state by randomized balanced $2{\times}2$ crossover design. After dosing, blood samples were collected for a period of 24 hours and analyzed by liquid chromatography-tandem mass spectrometry (LC/MS/MS). Time course of plasma gabapentin concentration was analyzed with non-compartmental and compartmental approaches. $WinNonlin^{(R)}$, the kinetic computer program, was used for compartmental analysis. One compartment model with first-order input, first-order output with no lag time and weighting by $1/(predieted\;y)^2$ was chosen as the most appropriate pharmacokinetic model for the volunteers. The major pharmacokinetic parameters $(AUC_{0-24hr},\;AUC_{inf},\;C_{max}\;and\;T_{max})$ and other parameters $(K_a,\;K_{el},\;V_d/F\;and\;Cl/F)$ of $Gapentin^{TM}$ (test drug) and $Neurontin^{TM}$ (reference drug) were estimated by non-compartmental analysis and compartmental analysis. The 90% confidence intervals of mean difference of logarithmic transformed $AUC_{0-24hr}\;and\;C_{max}$ were $log(0.9106){\sim}log(1.l254)\;and\;log(0.8521){\sim}log(1.0505)$, respectively. It shows that the bioavailability of the test drug is equivalent with that of the reference drug. There was no statistically significant difference between the two drugs in all pharmacokinetic parameters.

A Korean patient with Fanconi-Bickel Syndrome Presenting with Transient Neonatal Diabetes Mellitus and Galactosemia : Identification of a Novel Mutation in the GLUT2 Gene

  • Fanconi-Bickel Syndrome (FBS) is a rare autosomal recessive disorder of carbohydrate metabolism recently demonstrated to be caused by mutations in the GLUT 2 gene for the glucose transporter protein 2 expressed in liver, pancreas, intestine, and kidney. This disease is characterized by hepatorenal glycogen accumulation, both fasting hypoglycemia as well as postprandial hyperglycemia and hyperglactosemia, and generalized proximal renal tubular dysfunctions. We report the first Korean patient with FBS diagnosed based on clinical manifestations and identification of a novel mutation in the GLUT 2 gene. She was initially diagnosed having a neonatal diabetes mellitus due to hyperglycemia and glycosuria at 3 days after birth. In addition, newborn screening for galactosemia revealed hypergalactosemia. Thereafter, she has been managed with lactose free milk, insulin therapy. However, she failed to grow and her liver has been progressively enlarging. Her liver functions were progressively deteriorated with increased prothrombin time. Liver biopsy done at age 9 months indicated micronodular cirrhosis with marked fatty changes. She succubmed to hepatic failiure with pneumonia at 10 months of age. Laboratory tests indicated she had generalized proximal renal tubular dysfuctions; renal tubular acidosis, hypophosphatemic rickets, and generalized aminoaciduria. Given aforementioned findings, the diagnosis of FBS was appreciated at age of 2 months. The DNA sequencing analysis of the GLUT 2 gene using her genomic DNA showed a novel mutation at 5th codon; Lysine5 Stop (K5X).

우리나라 65세 이상 노인의 고혈압 유병률과 관련요인: 제4기 국민건강영양조사자료 분석(2007~2009) (Prevalence of Hypertension and Related Risk Factors in the Elderly: Data from the 4th Korean National Health & Nutrition Examination Survey, 2007~2009)

  • The purpose of this study was to estimate the prevalence of hypertension, and to assess the risk factors associated with hypertension in elderly (over 65 years old) Koreans, using data from the 4th Korean National Health & Nutrition Examination Survey (the 4th KNHANES), 2007~2009. A total of 1,887 participants were analyzed for assessing the risk factors after excluding those who took hypertension medicines or underwent diet therapy (hypertension perceivers). On the other hand, in analyzing prevalence, a total of 3,526 people were analyzed, including hypertension perceivers. Hypertension was more frequently found in female (64.1%) and urban (62.0%) groups compared to male (55.6%) and rural (57.4%) groups. The mean age of the hypertensive group was significantly higher than that of normal group. The mean values of total cholesterol levels were also significantly higher in the hypertensive group, while body weight, waist circumference, fasting blood glucose, hemoglobin, hematocrit, LDL-cholesterol, and HDL-cholesterol were not. Multiple logistic regression showed that smoking was significantly related to the prevalence of hypertension, but alcohol drinking, stress, obesity, hypercholesterolemia, hypertriglyceridemia, diabetes, anemia, and nutrient intakes were not. The results of this study does not support clear relations of hypertension with chronic diseases including obesity, hyperlipidemia, anemia, and diabetes as well as nutrient intakes among a Korean elderly population. A prospective long-term research study is needed to establish the effects of these factors on hypertension.

저항전분이 인체 혈당 조절기능에 미치는 영향 (Effect of Resistant Starch on Human Glycemic Response)

  • In order to observe the effects of resistant starches on human glycemic response, nine female university students were investigated using cellulose (CED), resistant starch 3 (RS3D) and resistant starch 4 (RS4D) diets. Each woman's blood sugar and insulin, triacylglycerol and free fatty in plasma concentration were measured at fasting state, then 15, 30, 45, 60, 75, 90 and 120 minute after each test diet feeding. Glycemic indices of the Cellulose diet (CED: 57.9 $\pm3.00$), the Resistant starch 3 diet (RS3D: 52.6 $\pm7.9$) and the Resistant starch 4 diet (RS4D: 52.9 $\pm10.2$) were similar to each other, but they were significantly lower in comparison with those of white wheat bread diet (WWBD: 100). Insulinemic indices of the CED (49.8 $\pm8.2$), RS3D (50.0 $\pm7.3$) and RS4D (72.4 $\pm7.7$) were significantly lower in comparison with the white wheat bread diet (WWBD: 100), but among the dietary fiber diets, the insulinemic index of RS4D was significantly higher than the CED and the RS3D. Plasma triacylglycerol contents of the CED, RS3D and RS4D including WWBD showed gradual increase in tendency after lowering in early stage of each test diet feeding, but not significantly different in each dietary fiber added diet. Plasma free fatty acid contents of the CED, RS3D and RS4D including WWBD showed gradual decrease in tendency after each test diet feeding, but not significantly different by each dietary fiber added diet. In above results, we speculate that resistant starch 3 controls rapid elevation of blood sugar by delaying intestinal digestion and absorption of cellulose, but the result appears to be different from RS4 in comparison. Thus, RS3 intakes may contribute to the diet therapy of diabetic humans, but more studies on RS4 is needed in the future. (Korean J Community Nutrition 9(4): 528∼535, 2004)

Role of Glyco-Persica® in Targeting Diabetes Type 2: an Integrative Approach

  • Objectives: The objective of this study was to examine how an integrated approach to type 2 diabetes mellitus treatment could improve glycemic control and immune-potentiating activities adherent to oral hypoglycemic agents along with a botanical compound, among primary care patients. Methods: In this study, we used the self-control and the group-control methods. Candidates meeting the trial conditions were selected from among volunteers who had taken the test substance for 45 days. During the trial, all groups were on a controlled diet; neither were the original medications nor their dosages changed. Results: The results showed that the botanical compound (Glyco-Persica$^{(R)}$) significantly reduced the main clinical symptoms in diabetes type 2. In the treatment group, 36 of 52 patients (69.23%) and in the control group 10 of 52 patients (19.23%) showed reduced symptoms, and this difference was statistically significant (P < 0.05). The fasting blood sugar in the treatment group after treatment compared with that before treatment and with that in the control group after treatment was statistically different (P < 0.05). The post-prandial glucose in the treatment group after treatment was significantly different from that before treatment and from that in the control group after treatment (P < 0.05); the post-prandial blood sugar in the treatment group was reduced by 8.98%. Conclusions: The results revealed that the botanical compound (Glyco-Persica$^{(R)}$) has significant hypoglycemic properties which affect main clinical symptoms in diabetes type 2. Body weight, blood pressure, heart rate, routine blood, stool and urine tests showed no meaningful negative changes after the course of treatment. There was no significant adverse reaction during the trial.
