30-69세 성인의 공복혈당장애와 당뇨병의 유병률과 위험인자 분석 (Prevalence and Risk Factors for Diabetes Mellitus and Impaired Fasting Glucose of Adults)

  • This study was conducted to draw out prevalence and the risk factors of diabetes mellitus and impaired fasting glucose for adults,(age 30-69). The subjects were 2096 adults, who had regular health examinations between January and December of 1999 at K Hospital in Seoul. The data was analyzed using chi-square test, unpaired t-test and logistic regression. Diabetes Mellitus and impaired fasting glucose were diagnosed by ADA (American Diabetes Association, 1997) criteria. The results were as follows: 1. Mens' prevalence of Diabetes Mellitus was 7.9% and womens' prevalence of Diabetes Mellitus was 3.8%. Mens' prevalence of impaired fasting glucose was 10.4% and womens' prevalence of impaired fasting glucose was 6.5%. Prevalences of Diabetes Mellitus and impaired fasting glucose increased with age. 2. Prevalence of Diabetes Mellitus and impaired fasting glucose of obese subjects (relative body weight>=162) was higher than that of overweight subjects (110<=relative body weight<=119) in men and women. 3. The diagnoses of Diabetes Mellitus and impaired fasting glucose increased with systolic blood pressure and triglyceride. 4. Significant factors associated with diabetes in the logistic regression best gut model were age, relative body weight, systolic blood pressure, triglyceride in men, and systolic blood pressure in women. In conclusion, as age, weight, systolic blood pressure and triglyceride get higher, Diabetes Mellitus and impaired fasting glucose prevalence also increases, porportionally.

A Comparative Study of the Concentration of Salivary and Blood Glucose in Normal and Diabetic Subjects

  • Advantage of saliva analysis are the ease of sample collection and that samples can be collected more frequently with much less stress on the patient. The objective of the present study was to comparatively evaluate the concentrations of saliva and fasting serum glucose in both normal and diabetic subjects. The mean salivary glucose level in diabetic patients was $15.66{\pm}17.1$ mg/dl and $1.78{\pm}1.72$ mg/dl (P = 0.0006) in the control group. The mean fasting serum glucose level in diabetic patients was $202.12{\pm}66.91$ mg/dl, while that in the control group was $94.21{\pm}14.97$ mg/dl (P < 0.0001). The 0.95 degree of correlation between salivary and fasting serum glucose could be demonstrated. The concentration of salivary and fasting serum glucose was not significant different betweeen the measurements for male and female. In the oral glucose tolerance test (75g), the glucose concentration in saliva progressively increased during the first 30 minutes of the test and then progressively decreased, reaching at minutes 120 ~ 180 lowest point as like fasting serum glucose concentration. We can conclude that salivary glucose concentration was significantly higher in the diabetic subjects and that there was significant correlation between salivary and fasting serum glucose concentration. Measurement of salivary glucose could be a useful test having good correlation between salivary and fasting serum glucose concentration.

Predictive capability of fasting-state glucose and insulin measurements for abnormal glucose tolerance in women with polycystic ovary syndrome

  • Objective: The aim of the present study was to evaluate the predictive capability of fasting-state measurements of glucose and insulin levels alone for abnormal glucose tolerance in women with polycystic ovary syndrome (PCOS). Methods: In total, 153 Korean women with PCOS were included in this study. The correlations between the 2-hour postload glucose (2-hr PG) level during the 75-g oral glucose tolerance test (OGTT) and other parameters were evaluated using Pearson correlation coefficients and linear regression analysis. The predictive accuracy of fasting glucose and insulin levels and other fasting-state indices for assessing insulin sensitivity derived from glucose and insulin levels for abnormal glucose tolerance was evaluated using receiver operating characteristic (ROC) curve analysis. Results: Significant correlations were observed between the 2-hr PG level and most fasting-state parameters in women with PCOS. However, the area under the ROC curve values for each fasting-state parameter for predicting abnormal glucose tolerance were all between 0.5 and 0.7 in the study participants, which falls into the "less accurate" category for prediction. Conclusion: Fasting-state measurements of glucose and insulin alone are not enough to predict abnormal glucose tolerance in women with PCOS. A standard OGTT is needed to screen for impaired glucose tolerance and type 2 diabetes mellitus in women with PCOS.

A Comparative Study of the Fasting Blood Sugar between Homeless and Aged People of Nursing Home in Daegu City, Korea - Comparision of National Health Insurance Coporation Criteria -

  • This study was performed to investigate the average fasting blood sugar levels from various Homeless and the old at nursing homes. Subjects (297 Homeless and 190 aged people) were examined in April, 2011 to June, 2010 in Daegu city area and only fasting blood sugar was measured. Subjects consist of 274 (56.30%) men and 213 (43.70%) women but primarily comprise over 60s. Blood was collected by vein-puncture, centrifuged and analyzed by TBA-C8000. The number of each subject for blood sugar levels of men groups were shown 197 (40.50%) with under 110, 24 (4.90%) with 110~125, and 53 (10.90%) with over 126. On the other hand, 127 (26.10%) with under 110, 33 (6.80%) with 110~125 and 53 (10.90%) with over 126 from women groups. This means that the average prevalence rate of diabetes is approximately 10% at each sex. Depending on age, the fasting blood sugar level over 126 increased with advancing age but the significance of results was not shown. Two groups, Homeless and aged people at nursing homes, the fasting blood sugar level was alittle high (2.9%) from Homeless but not significant. Regular blood sugar test and HbA1c test for people could be play an important role to prevent diabetic disorders under the government's interest.

금식 여부에 따른 혈당치 분포와 당뇨병 선별을 위한 혈당조절지표의 평가 (The Assessment of Blood Glucose Distribution according to the Fasting State and Glycemic Control Indicators for Diabetes Screening)

  • 본 연구는 금식 여부에 따른 혈당치 분포 차이를 분석하였고 공복 혈당치와 혈당조절지표인 당화혈색소, 프록토사민, 1,5-AG의 상관성을 분석하여 전당뇨병 선별에 필요한 추가 정보를 제공하고자 하였다. 단국대학교병원에 내원한 707명의 외래 환자를 금식 군과 비금식 군으로 나누어 혈당 검사를 시행한 후 각 군의 혈당치 평균을 산출하고 성별, 연령별 및 임상과 별 결과치 분포를 분석을 하였다. 또한 건강검진을 목적으로 내원한 금식 상태의 153명에 대해 혈당치와 당화혈색소, 프록토사민, 1,5-AG을 측정하여 혈당치와 혈당조절지표의 상관성을 평가하였다. 비금식 군의 혈당 평균치는 111.9 mg/dL, 금식 군은 103.6 mg/dL로서 유의한 차이가 있었고 (p<0.05) 평균치 차이는 여자가 4.8 mg/dL로서 남자의 12.2 mg/dL보다 적었다. 연령대에 따른 중앙값의 차이는 비금식 군에서만 유의한 차이가 있었고(Kruskal-Wallis test, p<0.01) 금식 군에서는 유의하지 않았다. 공복 시 포도당 장애 선별 구간에서 혈당 농도와 상관성이 높은 검사 종목은 1,5-AG로 평가되었다. 저자들은 금식 여부에 따른 혈당치 분포의 차이를 외래 환자를 대상으로 분석하였으며, 건강검진 대상자의 결과치 분석을 통해 1,5-AG가 프록토사민, 당화혈색소보다 공복 혈당치와 유의한 상관성이 있는 것으로 평가하였다. 당뇨병 선별 시 혈당 측정과 함께 단기 혈당 조절능력을 반영하는 지표가 활용될 수 있을 것으로 사료된다.

절식요법의 체중감량효과와 체성분 변화에 대한 관찰 (The Clinical Study on the Effect of Reduction in Body Weight and the Change of Body Composition after Fasting Therapy)

  • Objectives : This study was aimed to investigate the change of the reduction in body weight and the change of body composition after fasting therapy, Chulsikyopub, which was put into practice in Dept. of oriental rehabilitation medicine at Kyung Hee oriental hospital. Method : We measured body weight, body composition, and the degree of obesity 2 weeks after fasting therapy and 10 weeks after fasting therapy with body composition analyzer(Inbody 2.0). Statistical comparisons between pre-therapy group and post-therpay group were done by paired t-test and Wilcoxon's signed rank test. Results : 2 weeks after fasting therapy, body weight was decreased 7.33kg(9.18%), body fat was decreased 2.47Kg(7.24%), muscle weight was decreased 4.66kg(11.37%), body fat ratio was increased 1.08%, and body metabolic rate(BMR) was decreased 118.23kcal(8.22%). 10 weeks after fasting therapy, each of body weight and fat was decreased 10.4Kg, muscle weight was increased 0.3Kg(0.7%), body fat ratio was decreased 7.9%, and BMR was increased 6.8Kcal(0.47%). But muscle weight and BMR did not satisfy statistical significance. Conclusions : fasting therapy for 10 weeks reduced body weight and body fat statistically significant without loss of muscle weight and BMR.

중년 남성의 공복혈당장애 관련 요인 (Factors Associated with Impaired Fasting Glucose in Middle-aged Men)

  • 본 연구의 목적은 우리나라 중년 남성의 공복혈당장애 유병률을 파악하고, 공복혈당장애 관련 요인을 규명하기 위함이다. 연구방법은 제8기 2차년도(2020) 국민건강영양조사에 참여한 대상자 중 당뇨병 유병자를 제외한 40-64세의 남성 545명의 자료를 이용한 서술적 조사연구이다. 자료분석을 위해 SPSS/WIN 23.0 프로그램의 복합표본분석방법을 사용하여 빈도와 백분율, 평균과 표준편차, Rao-Scott 𝛘2, t-test, 로지스틱회귀분석을 시행하였다. 연구결과, 공복혈당장애 유병률은 44.77%였으며, 음주빈도(OR=1.531, 95% CI: 1.01-2.33), 중성지방(OR=1.002, 95% CI: 1.00-1.01)이 공복혈당장애의 유의한 관련 요인이었다. 따라서, 중년 남성의 공복혈당장애의 예방과 관리를 위한 프로그램의 개발이 필요하며, 이에 음주습관과 중성지방 관리 전략을 포함해야 할 것이다.

The anti-tumor efficacy of 20(S)-protopanaxadiol, an active metabolite of ginseng, according to fasting on hepatocellular carcinoma

  • Background: 20(S)-protopanaxadiol (20(S)-PPD), one of the main active metabolites of ginseng, performs a broad spectrum of anti-tumor effects. Our aims are to search out new strategies to enhance anti-tumor effects of natural products, including 20(S)-PPD. In recent years, fasting has been shown to be multi-functional on tumor progression. Here, the effects of fasting combined with 20(S)-PPD on hepatocellular carcinoma growth, apoptosis, migration, invasion and cell cycle were explored. Methods: CCK-8 assay, trypan blue dye exclusion test, imagings photographed by HoloMonitorTM M4, transwell assay and flow cytometry assay were performed for functional analyses on cell proliferation, morphology, migration, invasion, apoptosis, necrosis and cell cycle. The expressions of genes on protein levels were tested by western blot. Tumor-bearing mice were used to evaluate the effects of intermittent fasting combined with 20(S)-PPD. Results: We firstly confirmed that fasting-mimicking increased the anti-proliferation effect of 20(S)-PPD in human HepG2 cells in vitro. In fasting-mimicking culturing medium, the apoptosis and necrosis induced by 20(S)-PPD increased and more cells were arrested at G0-G1 phase. Meanwhile, invasion and migration of cells were decreased by down-regulating the expressions of matrix metalloproteinase (MMP)-2 and MMP-9 in fasting-mimicking medium. Furthermore, the in vivo study confirmed that intermittent fasting enhanced the tumor growth inhibition of 20(S)-PPD in H22 tumor-bearing mice without obvious side effects. Conclusion: Fasting significantly sensitized HCC cells to 20(S)-PPD in vivo and in vitro. These data indicated that dietary restriction can be one of the potential strategies of chinese medicine or its active metabolites against hepatocellular carcinoma.

Role of Glucocorticoids in Fasting-induced Changes in Hypothalamic and Pituitary Components of the Growth Hormone (GH)-axis

  • To directly test if elevated glucocorticoids are required for fasting-induced regulation of growth hormone (GH)-releasing hormone (GHRH), GHRH receptors (GHRH-R) and ghrelin receptors (GHS-R) expression, male rats were bilaterally adrenalectomized or sham operated. After 7 days, animals were fed ad libitum or fasted for 48 h. Bilateral adrenalectomy increased hypothalamic GHRH to 146% and decreased neuropeptide Y (NPY) mRNA to 54% of SHAM controls. Pituitary GHRH-R and GHS-R mRNA levels were decreased by adrenalectomy to 30% and 80% of shamoperated controls. In shamoperated rats, fasting suppressed hypothalamic GHRH (49%) and stimulated NPY (166%) mRNA levels, while fasting increased pituitary GHRH-R (391%) and GHS-R (218%) mRNA levels. However, in adrenalectomized rats, fasting failed to alter pituitary GHRH-R mRNA levels, while the fasting-induced suppression of GHRH and elevation of NPY and GHS-R mRNA levels remained intact. In fasted adrenalectomized rats, corticosterone replacement increased GHRH-R mRNA levels and intensified the fasting-induced decrease in GHRH, but did not alter NPY or GHS-R response. These data suggest that elevated glucocorticoids mediate the effects of fasting on hypothalamic GHRH and pituitary GHRH-R expression, while glucocorticoids are likely not the major determinant in fasting-induced increases in hypothalamic NPY and pituitary GHS-R expression.

절식요법을 시행한 입원환자의 후향적 연구 (The Retrospective Study on Modified Fasting Therapy in Inpatients)

  • Objectives : The purpose of this study was to investigate the effects of modified fasting therapy on body composition, blood cell count and blood chemistry in inpatients. Methods : The modified fasting therapy program consists of three phases, a period of reducing food intakes, fasting and refeeding. Body weight, body composition, and the degree of obesity were estimated in before fasting therapy and after refeeding. Blood cell count and blood chemistry were estimated in before fasting therapy, before refeeding and after refeeding. Results : After modified fasting therapy, body weight, body fat mass, percent body fat, body mass index(BMI), body matabolic rate(BMR) and muscle mass was decreased significantly. WBC count and platelet count decreased significantly, RBC count and hemoglobin increased significantly and hematocrit was not changed significantly but within normal limit. AST and ALT increased significantly, especially patients group over BMI 25 within normal liver function. This study was not founded correlation between decreased muscle mass and increased liver function test. ALP, BUN, Total-cholesterol, and triglyceride decreased significantly. Creatinine increased significantly, but within normal limit. Sodium was not changed significantly, potassium decreased significantly but within normal limit. Conclusions : These results suggest that modified fasting therapy will be beneficial if several measures complement.
