BACKGROUND/OBJECTIVES: Diabetes-specific oral nutritional supplements (ONS) have anti-hyperglycemic effects, while D-allulose exerts anti-diabetic and anti-obesity effects. In this study, we investigated the efficacy and safety of diabetes-specific ONS, including allulose, on glycemic and weight changes in overweight or obese patients with type 2 diabetes mellitus (T2DM). SUBJECTS/METHODS: A single-arm, historical-control pilot clinical trial was conducted on 26 overweight or obese patients with T2DM (age range: 30-70 yrs). The participants were administered 2 packs of diabetes-specific ONS, including allulose (200 kcal/200 mL), every morning for 8 weeks. The glycemic profiles, obesity-related parameters, and lipid profiles were assessed to evaluate the efficacy of ONS. RESULTS: After 8 weeks, fasting blood glucose (FBG) level significantly decreased from 139.00 ± 29.66 mg/dL to 126.08 ± 32.00 mg/dL (P = 0.007) and glycosylated hemoglobin (HbA1c) improved (7.23 ± 0.82% vs. 7.03 ± 0.69%, P = 0.041). Moreover, the fasting insulin (δ: -1.81 ± 3.61 μU/mL, P = 0.017) and homeostasis model assessment for insulin resistance (HOMA-IR) (δ: -0.87 ± 1.57, P = 0.009) levels decreased at 8 weeks, and body weight significantly decreased from 67.20 ± 8.29 kg to 66.43 ± 8.12 kg (P = 0.008). Body mass index (BMI) also decreased in accordance with this (from 25.59 ± 1.82 kg/m2 to 25.30 ± 1.86 kg/m2, P = 0.009), as did waist circumference (δ: -1.31 ± 2.04 cm, P = 0.003). CONCLUSIONS: The consumption of diabetes-specific ONS with allulose in overweight or obese patients with T2DM improved glycemic profiles, such as FBG, HbA1c, and HOMA-IR, and reduced body weight and BMI.

The effects of Okcheonsan powder on the body weights, the organ weights, the blood glucose level, the lipid and protein concentrations of serum and liver in diabetic rats were studied. Female rats (Sprague-Dawley, mean weight 313.6$\pm$18.5 g) were randomly assigned to one normal and two diabetic groups. They were fed experimental diets for 5 weeks. The diabetic groups were divided into the diabetic control (D-control group) and 3% Ok-cheonsan groups (D-Okcheonsan group). Rats were injected with streptozotocin intraperitoneally to induce diabetes. The body weights, the concentrations of total lipid and triglyceride of liver, the concentrations of total protein and albumin of serum in tole D-control and the D-Okcheonsan groups were significantly decreased compared with those in the normal group. The pancreatic weight in the D-control group was significantly more increased than that in the D-Okcheonsan group, but in the D-Okcheonsan group it was similar to that in the normal group. The fasting blood glucose levels and the atherogenic index in all the diabetic groups were significantly higher than those in the normal group. The concentrations of triglyceride, total cholesterol and LDL-cholesterol of serum, and the cholesterol of fiver in the D-Okcheonsan group were significantly lower than those in the D-control group. The concentration of HDL-cholesterol and HDL-cholestrol/total cholesterol ratio of serum were similar to those in all the groups. The concentrations of phospholipid of serum and liver in the D-Okcheonsan group were significantly decreased compared with those of the normal group, In conclusion, the Okcheonsan powder feeding could decrease the pancreatic weight, the concentrations of the triglyceride, the total cholesterol and LDL-cholesterol of serum, and the cholesterol of liver in the diabetic rats. But the concentrations of the blood glucose, the hepatic triglyceride and the atherogenic index seems to be not affected by it.
Purpose : Impaired glucose tolerance (IGT) was standardized in 1979 by the National Diabetes Data Group and the World Health Organization as a risk factor for type 2 diabetes, replacing groups such as 'borderline' and 'chemical' diabetes. The main clinical significance of IGT is as a risk factor for type 2 diabetes and as a risk factor for cardiovascular disease and as a component of the metabolic syndrome. In 1997 the American Diabetes Association(ADA) was proposed the new classification and diagnostic criteria for diabetes, which is strict with the diagnostic baseline of Diabetes from 140mg/dl to 126mg/dl. And it's main purpose is to prevent chronic complications by early diagnosis and treatments. In the oriental medicine, Supungsunkihwan has been used in treatments of Diabetes including IGT & NIDDM, however there is not enough studies about the its objective hypoglycemic effect. so in order to investigate whether there is hypoglycemic effect of Supungsunkihwan, clinical studies were performed with IGT and mild NIDDM patients. Methods : Prior to the study, fasting blood sugar(FBS) and postprandial 2hrs(PP2hrs) glucose were checked. In addition, ECG, cholesterol, TG, HbA1c levels were measured, and the dinical characteristics of patients that can be influence to the blood sugar level such as age, gender, rest type, diet type were surveyed. Supungsukihwan was administered for 7 days, and FBS & PP2hrs were measured again after the therapy. Result : PP2hrs glucose levels were decreased by the administration of Supungsunkihwan. And It's hypoglycemic effects has the correlation with age. That is, the more age increase, the more hypoglycemic effect decrease. Conclusion : Supungsukihwan has hypoglycmic effects on IGT & mild NIDDM patients and it is more effective when administrated to young patients relatively.
Elevated high-sensitivity C reactive protein (hs-CRP) levels and metabolic syndrome are considered important predictors of cardiovascular mortality. This study examined the influence of the alcohol consumption level on the hs-CRP level and the prevalence of metabolic syndrome in South Koreans. The study subjects were 3,884 participants (${\geq}19$ years) from the KNHANES in 2015. The subjects were divided into four groups according to their alcohol consumption; None (none-alcohol consumption), Low (less than 15 g/day), Medium (15~29.9 g/day), and High (over than 30 g/day). The odds ratio for high-risk hs-CRP of men was significantly lower in the Low or Medium groups compared to the None group. In women, the odds ratio for high-risk hs-CRP was not decreased in the Low or Medium groups but was significantly higher in the High group than in the None group. Compared to nondrinkers, the prevalence of metabolic syndrome was lower in the Low group and Medium group in both men and women, whereas it was significantly higher in the High group in men only. In particular, the prevalence of low HDL-cholesterol was lower in all drinking groups. When the confounding factors were adjusted, the odds ratio for low HDL-cholesterol appeared to decline in the Low groups in both men and women. On the other hand, in the Medium group, women showed an elevated odds ratios of high blood pressure (HBP) and high fasting plasma glucose but men showed an elevated odds ratios of HBP and high triglyceride levels. The overall results suggest that low alcohol consumption is more appropriate than medium alcohol consumption. Nevertheless, more study will be needed to evaluate the appropriate alcohol consumption level.

Among patients who visited each clinical department for oriental medical treatments, anthropometric measurement, blood pressure, fasting blood glucose and blood lipid profile level were measured at their first initial visit. 55 subject patients who were diagnosed as having metabolic syndrome and 150 mg/dL or more of triglyceride were selected as subjects whose fasting blood glucose, triglyceride, total cholesterol, HDL cholesterol and LDL cholesterol were measured after fasting. According to each patient's disease, the subject received treatments such as herb medicine, acupuncture, moxibustion, cupping therapy, physical therapy and rehabilitation therapy from each clinical department, and after an average of 4.10${\pm}$0.31 weeks, another test was performed yielding the following results. Serum triglyceride was 243.72${\pm}$13.05 mg/dL before the oriental medical treatment and 188.11${\pm}$12.17 mg/dL after the treatment where although it continued to show an abnormal value even after the treatment, there was statistically significant decrease compared to pre treatment(P<0.05). Serum total cholesterol was 207.50${\pm}$5.89 mg/dL before the oriental medical treatment and 192.37${\pm}$5.53 mg/dL after the treatment which was statistically insignificant compared to pre treatment(P>0.05). Serum HDL cholesterol was 51.19${\pm}$3.95 mg/dL before the oriental medical treatment and increased to 52.53${\pm}$1.49 mg/dL after the treatment although it was statistically insignificant compared to pre treatment(P>0.05). Serum LDL cholesterol was 110.66${\pm}$5.86 mg/dL before the oriental medical treatment and decreased to 106.12${\pm}$4.82 mg/dL after the treatment although it was statistically insignificant compared to pre treatment(P>0.05). In regards to the change of triglyceride for each sex, it was 221.84${\pm}$14.01 mg/dL before the treatment and 187.00${\pm}$15.47 mg/dL after the treatment for men, and it was 271.50${\pm}$22.78 mg/dL and 189.53${\pm}$19.76 mg/dL respectively for women where even though men and women showed the decrease of 34.84${\pm}$12.79 mg/dL and 81.96${\pm}$20.01 mg/dL respectively, both men and women continue to show abnormal values after the treatments. However, there was statistically significant decrease compared to pre treatment(P<0.05). In regards to the change of total cholesterol for each sex, with 198.24${\pm}$7.60 mg/dL for men before the treatment and 188.93${\pm}$7.45 mg/dL after the treatment, values for both before and after the treatment were within the normal range where the change value was 9.30${\pm}$5.86 mg/dL and statistically insignificant(P>0.05). For women, it was 219.26${\pm}$8.87 mg/dL and 196.73${\pm}$8.43 mg/dL respectively for women where with 22.53${\pm}$7.60 mg/dL, it decreased to the normal level after the treatment, and there was a statistically significant decrease compared to pre treatment(P<0.05). With such results, serum triglyceride and cholesterol levels of patients who have been diagnosed as having metabolic syndrome were observed to decrease after the oriental medical treatment. Especially, for both men and women, abnormally high triglyceride level decreased while the effect of lipid profile improvement for women was more significant compared to men.

Objectives : SimJeok-Hwan (CP, Cardiotonic Pills) is the traditional remedy to treat patients with coronary arteriosclerosis, angina pectoris and hyperlipidemia. This study was designed to investigate the effects of CP on changes in serum cholesterol levels in hyperlipidemic rats. Methods : In this study, we investigated the effects of CP on changes in body weights, food and water uptake, fasting blood glucose, total cholesterols, HDL-cholesterol, and triglyceride in hyperlipidemic rats. Results : In our results, body weights of hyperlipidemic rats induced by high fat diet did not changed, and treatment with CP did not affect body weights in hyperlipidemic rats. For experimental period, food and water uptake in CP administered group were the same as those in hyperlipidemic control group. In this experiment, treatment with CP decreased total cholesterol and triglyceride in serum which elevated by high fat diet respectively. Conclusions : These results suggest that CP is useful to treat patients with disease related to hyperlipidemia because CP can decrease cholesterol in serum
Purpose: This study aimed to estimate the effects of a regular walking exercise program on metabolic syndrome, cardiovascular risk factors, and depressive symptoms among the elderly with diabetic mellitus (DM) based on the Theory of Reasoned Action (TRA). Methods: This study has randomized and stratified experimental design with experimental and control groups. We developed a regular walking exercise program suitable for the elderly with DM based on the guidance of AAHPERD. The experimental group participated in the regular walking exercise program, which contains walking exercise 3 times a week and 50 minutes each time for 3 months and education on controlling diet and preventing complications once a week and 20 minutes each time for 4 weeks. Post-test was conducted after 3 months to estimate metabolic syndrome, cardiovascular risk factors, and depressive symptoms. Results: The regular walking exercise program was effective for decreasing the waist size, the level of fasting blood glucose (FBG) and triglyceride (TG), cardiovascular risk factors and the severity of depressive symptoms among the elderly with DM. Conclusion: The incidence of complications would be decreased by applying a regular walking exercise program.
Osteoporosis is a disease that increases the fracture rates and a major cause of increased mortality and morbidity in the elderly people. This study is to determine which components of body composition and metabolic syndrome risk factors are important to bone health, we analysed the relationship among bone mineral density (BMD), body composition and metabolic syndrome risk factors in females. Totally 630 females participated in a medical check-up program (mean age 47 years) were selected for this study. Body composition analysis was performed by segmental bioelectrical impedance method, muscle mass, and percent body fat were measured. We also measured metabolic syndrome risk factors including abdominal obesity, HDL-cholesterol, triglyceride, blood pressure and fasting glucose level. Metabolic syndrome was defined by NCEP-ATP III criteria. The lumbar spine and femoral neck BMD were measured using the dual energy X-ray absorptiometry. Osteopenia and osteoporosis were observed in 180 and 51 persons, respectively. Muscle mass and HDL-cholesterol decreased in osteopenia and osteoporosis groups compared to the control group, and the grade was shown progressively by the symptoms. Significant positive correlation between BMD and muscle mass was observed. Multi variable regression analyses showed that % body fat and muscle mass were independent predictors of BMD after adjustment of age, height and weight. In conclusion, the BMD showed negative correlation with the metabolic and body composition was associated with BMD.
The purpose of this study was to investigate the differences in anthropometric, hematologic, and lipidologic data between normocholesterolemia and hypercholesterolemia and influencing factors in hypercholesterolemia in middle-aged men in Kwang-ju. 179 middle-aged men were involved. Anthropometric data(body weight, height, body circumferences and skinfold thickness) and hematologic data(blood pressure, fasting blood glucose, albumin and hemoglobin) were measured. Bleed lipid profile(total cholesterol, tr- iglyceride, phospholipid, HDI-cholesterol, LDL-cholesterol and VLDL-cholesterol) were analyzed. To determined using questionnaire. The subjects were classified as hypercholesterolemic based on The Guideline for Korean Hyperlipidemia. body mass index, systolic blood pressure, waist and hip circumference, and triceps, subcostal, abdomen, subscapular and suprailliac skinfold thicknesses of the hypercholesterolemic group were significantly higher than in the normocholesterolemic group. The concentrat- ion of cholesterol, triglyceride, phospholipod. HDL-cholesterol, LDL-cholesterol and VLDL -cholesterol of the hypercholesterolemic group were significantly higher than normocholesterolemic group. The concentration of cholesterol has a significantly positive correlation with body mass index, diastolic blood pressure, waist circumference, waist/thigh ratio and skinfold thickness(triceps, subcostal, abdomen, and supraillic skin- fold thickness). The general tendency of socioeconomec status, frequencyu of meat intake, smoking amounts and duration and duration and drinking frequencies and amount were higher in the hypercholesteolemic group but not significant. In conclusion, hypercholesterolemia in middle-aged men was closely correlated with upper body fat obesity and most of them had an accompanied high triglyceride level tendency.
To evaluate the hypoglycemic effect of propolis, streptozotocin (STZ) induced diabetic rats were divided into 4 groups such as, diabetic control group, low dose of propolis (0.1 ml) group, medium dose of propolis (0.3 ml) group and high dose of propolis (0.9 ml) group and feeded with propolis extracts for 30 days. After experiment, oral glucose tolerance test (Oral GTT) was carried, and 16 hours fasting blood sugar levels, body weights, blood lipid levels were measured. Finally, pancreatic histopathological study was performed. In conclusion, the propolis is effective to the treatment diabetes due to the reduction of the blood sugar level and the regeneration of the damaged $\beta$-cells shown in streptozotocininduced diabetic rats.
