자연치유와 전인적 의학(Holistic Medicine)으로서의 단식요법(斷食療法)에 관한 고찰 (Consideration on Fasting Cure as Natural Healing and Holistic Medicine)

  • 본 연구에서는 자연치유와 전인적 의학으로서의 단식요법에 대하여 고찰하고자 한다. 연구방법으로는 종교사상에서 나타난 단식의 역사와 의의를 찾아 문헌적으로 고찰하였으며, 단식이 심신치유에 적용된 사례를 연구, 분석하였다. 연구결과 첫째, 단식은 종교사상에서 정신적인 중대한 역할을 함으로 단식행위가 인간의 삶에서 정신적으로 미치는 영향력이 큰 것으로 나타났다. 둘째, 단식은 인간의 심신을 치유하는 수단으로서 오랜 과거로부터 오늘날에까지 이어져 내려오는 자연치유적 의료행위로 중대한 역할을 하고 있는 것으로 나타났다. 결과적으로 단식요법은 자연치유적 방법에 의한 전인적 의학으로서 중요한 가치를 가진다는 점을 알 수 있었다. 본 연구의 시사점은 단식요법을 통해 현대사회에 각종 질병의 요인으로 작용하는 스트레스성 질병과 생활습관성 만성질환에 대하여 전인적 의학 차원에서의 효율적 치유의 방향과 근거를 제시하는데 기초 자료가 될 것으로 사료된다.

Role of Glucocorticoids in Fasting-induced Changes in Hypothalamic and Pituitary Components of the Growth Hormone (GH)-axis

  • To directly test if elevated glucocorticoids are required for fasting-induced regulation of growth hormone (GH)-releasing hormone (GHRH), GHRH receptors (GHRH-R) and ghrelin receptors (GHS-R) expression, male rats were bilaterally adrenalectomized or sham operated. After 7 days, animals were fed ad libitum or fasted for 48 h. Bilateral adrenalectomy increased hypothalamic GHRH to 146% and decreased neuropeptide Y (NPY) mRNA to 54% of SHAM controls. Pituitary GHRH-R and GHS-R mRNA levels were decreased by adrenalectomy to 30% and 80% of shamoperated controls. In shamoperated rats, fasting suppressed hypothalamic GHRH (49%) and stimulated NPY (166%) mRNA levels, while fasting increased pituitary GHRH-R (391%) and GHS-R (218%) mRNA levels. However, in adrenalectomized rats, fasting failed to alter pituitary GHRH-R mRNA levels, while the fasting-induced suppression of GHRH and elevation of NPY and GHS-R mRNA levels remained intact. In fasted adrenalectomized rats, corticosterone replacement increased GHRH-R mRNA levels and intensified the fasting-induced decrease in GHRH, but did not alter NPY or GHS-R response. These data suggest that elevated glucocorticoids mediate the effects of fasting on hypothalamic GHRH and pituitary GHRH-R expression, while glucocorticoids are likely not the major determinant in fasting-induced increases in hypothalamic NPY and pituitary GHS-R expression.

Upavasa (fasting) for health & longevity: an overview

  • Ayurveda is the science of life. It guides about the protection of health of the healthy and alleviation of ailments of diseased person. In Ayurveda Upavasa (fasting) is one among all Daivavyapashraya (spiritual) therapies and one among ten Langhana (that which produces lightness of the body) therapies. It is also stated in Ayurveda that fasting increases digestive fire and causes digestion of Ama-dosha (metabolic toxins). On religious grounds, Upavasa is a way to get closer to the almighty. Scientists have found fasting activates autophagy. Yoshinori Ohsumi, a Japanese cell biologist won the Nobel Prize in Medicine in 2016 for his research on how cell recycle and renew their content, a process called autophagy. Many published studies indicate positive effects of fasting on neuroprotection and longevity.

慢性 皮膚疾患 患者에 對한 節食療法 治驗例 (A Reports on Chronic Dermatitis Treated by fasting Therapy)

  • A Reports were done on 3 cases of proriasis, 1 case of chronic urticaria which were admition and treated by fasting therapy in Dept. surgery, opthalmology & otolaryngology, college of Oriental Medicine, Kyungsan University, Pohang, Korea, from March 1999 to October 2000. The results were as follows: 1. In the case chronic urticaria, symptom and sign(jtching, wheal, erythema), except of pigment, were showed conciderable improvement. 2. In the case proriasis, it is different of person, bellow 5cm sized erythematous popules were disappeared during fasting therapy, above 5cm sized erythematous papules and plaque were improved by using medication after fasting therapy. Silver-white colored scales was improved about $50-60\%$, itching was disappeared during fasting therapy. 3. After fasting therapy there was a recurrence, but about $40-50\%$ decreased symptoms were showed. 4. Besides upper diagnosis and treatment based on overall of symptoms and signs considered by documentary investigation, medications by Sasang prescription also can be expected good treatment effect.

Enzyme Activities Related to Lipid Metabolism in the Liver and Adipose Tissue of Tsaiya Ducks under Fasting and Ad libitum Feeding Conditions

  • The study investigated the lipid metabolism of Tsaiya ducks under fasting and ad libitum feeding conditions. Sixty Tsaiya ducks in their growing period (8-12 wk-old) and sixty Tsaiya ducks in their laying period (26-30 wk-old, 10-14 weeks after the onset of laying) were randomly divided into ad libitum feeding and 3-day fasting groups. The activities of lipid metabolism related enzymes were determined. Experimental results indicated that fasting depressed the activities of lipogenesis related enzymes such as fatty acid synthetase and NADP-malic dehydrogenase in both periods (p<0.05). Fasting also increased the activities of liver fatty acid $\beta$-oxidation enzymes (p<0.05). However, the activities of lipoprotein lipase in adipose tissue, heart and ovarian follicle in both periods and the hormone-sensitive lipase of adipose tissue in the growing period were decreased by fasting (p<0.01).

Regulation of Lipoprotein Lipase by Fasting in Epididymal and Mesenteric Adipocytes of Rats

  • There are marked variations in the activity of lipoprotein lipase (LPL) among adipose depots. The aim of this study was to compare the mechanisms of 24 h of fasting on LPL regulation between epididymal (EPI) adipocytes and mesenteric (MES) adipocytes in rats. 1-Day fasting consistently decreased activities of heparin-releasable LPL, total extractable LPL and cellular LPL markedly in both EPI and MES fat pads. LPL activity in MES fat pads was relatively lower than in the EPI fat pads. Consistent with data on LPL activity, the levels of expression of LPL mRNA in both nutritional states were lower in MES than EPI adipose tissue and isolated adipocytes. The decreased LPL activity after 1 day of fasting in MES adipocytes was explained mainly by a 50% decrease in the relative abundance of LPL mRNA level and a parallel 50% decrease in relative rate of LPL synthesis. In contrast, fasting of 1 day in EPI adipocytes decreased total LPL activity by 47% but did not affect LPL mRNA level or relative rate of LPL synthesis. A decrease in overall protein synthesis contributed to the decreased LPL activity after 1 day fasting both in EPI and MES adipocytes. In MES adipocytes the decrease in LPL activity, LPL mRNA and LPL synthesis were comparable, but in EPI adipocytes the changes in LPL activity were substantially larger than the changes in LPL mRNA level and LPL synthesis. Therefore, fasting decreased fat cell size, LPL activity, LPL mRNA level and relative rate of LPL synthesis in rats, and these effects were more marked in the MES adipocytes. These results clearly demonstrate the regional variations in the metabolic response of adipose tissue and LPL functions to fasting.

절식요법을 시행한 입원환자의 후향적 연구 (The Retrospective Study on Modified Fasting Therapy in Inpatients)

  • Objectives : The purpose of this study was to investigate the effects of modified fasting therapy on body composition, blood cell count and blood chemistry in inpatients. Methods : The modified fasting therapy program consists of three phases, a period of reducing food intakes, fasting and refeeding. Body weight, body composition, and the degree of obesity were estimated in before fasting therapy and after refeeding. Blood cell count and blood chemistry were estimated in before fasting therapy, before refeeding and after refeeding. Results : After modified fasting therapy, body weight, body fat mass, percent body fat, body mass index(BMI), body matabolic rate(BMR) and muscle mass was decreased significantly. WBC count and platelet count decreased significantly, RBC count and hemoglobin increased significantly and hematocrit was not changed significantly but within normal limit. AST and ALT increased significantly, especially patients group over BMI 25 within normal liver function. This study was not founded correlation between decreased muscle mass and increased liver function test. ALP, BUN, Total-cholesterol, and triglyceride decreased significantly. Creatinine increased significantly, but within normal limit. Sodium was not changed significantly, potassium decreased significantly but within normal limit. Conclusions : These results suggest that modified fasting therapy will be beneficial if several measures complement.

구강건강관리와 공복혈당의 관련성 (Relationship of oral health care with impaired fasting glucose)

  • Objectives: The purpose of this study was to determine the importance of oral health care in the control of diabetes mellitus. Methods: The raw data of the National Health and Nutrition Examination Survey were utilized and 4,445 adults aged 19 and over were surveyed and examined. Complex samples crosstabs and general linear model analysis were carried out. Results: In total, 69.6 percent of the subjects were normal, 21.4 percent had impaired fasting glucose, and 9.0 percent had diabetes. By sex, 25.8 percent of the men had impaired fasting glucose and 10.8 percent had diabetes. Women with impaired fasting glucose accounted for 17.2 percent and 7.2 percent of women had diabetes. Impaired fasting glucose and diabetes were more common among men. Fasting glucose values were higher in the respondents who were male, were younger, whose monthly mean household income was lower, and who were less educated. Fasting glucose values were higher when the frequency of toothbrushing was lower and when dental floss and a dental brush were not in use. Fasting glucose was lower when there was no periodontal disease, when there was less difficulty in mastication, and when there were no implants in the upper and lower jaws. Conclusions: Oral health care workers should develop a variety of programs to motivate patients to take care of their own oral health actively and responsibly.

Replication of Interactions between Genome-Wide Genetic Variants and Body Mass Index in Fasting Glucose and Insulin Levels

  • The genetic regulation of glucose and insulin levels might be modified by adiposity. With regard to the genetic factors that are altered by adiposity, a large meta-analysis on the interactions between genetic variants and body mass index with regard to fasting glucose and insulin levels was reported by the Meta-Analyses of Glucose- and Insulin-related trait Consortium (MAGIC), based on European ancestry. Because no replication study has been performed in other ethnic groups, we first examined the link between reported single-nucleotide polymorphisms (SNPs) and fasting glucose and insulin levels in a large Korean cohort (Korean Genome and Epidemiology Study cohort [KoGES], n = 5,814). The MAGIC study reported 7 novel SNPs for fasting glucose levels and 6 novel SNPs for fasting insulin levels. In this study, we attempted to replicate the association of 5 SNPs with fasting glucose levels and 5 SNPs with fasting insulin levels. One SNP (rs2293941) in PDX1 was identified as a significant obesity-modifiable factor in Koreans. Our results indicate that the novel loci that were identified by MAGIC are poorly replicated in other ethnic groups, although we do not know why.

선천성 심장병 신생아의 개심술 후 금식기간에 따른 영양공급량, 생화학적 영양지표 및 환아결과 (Nutrition Supply, Biochemical Nutrition Indexes and Patient Outcomes in New Born Babies with Open Heart Surgery according to Post Operative Fasting Period)

  • Purpose: The purpose of this study was to examine and analyze the post operative fasting period of neonates in the intensive care unit (ICU) after receiving open heart surgery in order to provide optimal nutrition support for these neonates. The variables included biochemical nutrition indexes (albumin, total lymphocyte count, total cholesterol) and patient outcomes (duration of mechanical ventilation, ICU stay, hospital stay, infectious complication). Methods: The participants were 124 neonates in ICU after receiving open heart surgery, and the design of this study was to investigate their post operative fasting period retrospectively to analyze the biochemical nutrition indexes and patient outcomes according to post operative fasting period. Results: The results for 4 groups according to post operative fasting period showed that the group with less 48 hours of fasting had the best biochemical nutrition indexes and patient outcomes, followed by the 48-72 hour group, the 72-144 hour group, and the over 144 hour group. Conclusion: The results indicate that for new born babies receiving open heart surgery, the period of fasting after the operation should be minimized and tube feeding should be started as soon as possible.
