보건의료서비스 방향 재정립: 일차의료에서 건강증진 서비스 (Reorienting health services: Health promotion services in primary care)

  • Objectives: Health screening in Korea is very active in both the public and private sector. However, primary prevention for health promotion has not been activated yet. Quaternary prevention is the prevention of unnecessary medical interventions or the prevention of overmedicalization. Methods: Data was collected after a search of the literature focusing on keyword. The curriculum guidelines for family medicine residents were collected from the homepage of the American Academy of Family Physicians. Results: This quaternary prevention is just beginning. The first step to enhance the health promotion services in the community is to identify the barriers pertaining to the delivering of health promotion activities. These barriers are related to the patient, the physician, attitudes, health promotion programs and the healthcare system. The second step is to establish new changes, such as expansion of insurance coverage, improvement of medical payment system, provision of consumer-oriented services, connection and integration of providers, and the service provider education and training. Conclusions: In order to enhance the health promotion services in the community we need to identify the barriers and to establish several changes to overcome them.

Knowledge of Turkish Mothers with Children in the 0-13 Age Group about Cancer Symptoms

  • Background: There have been few studies in Turkey on the incidence of childhood cancers. A mother's knowledge about signs and symptoms of cancer is important for early diagnosis, effective treatment, and improvement of life expectancy. This study was conducted with a group of mothers of children, aged 0-13, at a Family Health Center (FHC) in Turkey's Eastern Black Sea Region, to analyze their knowledge about cancer symptoms in childhood. Materials and Methods: The study group of this descriptive/cross-sectional research comprised 2,061 mothers, ages 19-49, at an FHC in the Eastern Black Sea Region in February 1, 2011 - June 1, 2011. Before the study, permission was obtained from the local ethics board and the institutions concerned. A value of p<0.05 was accepted as statistically significant. Results: Of the mothers, 34.9% were between the ages of 40-47, 40.5% had three children, 73.8% had no experience with children with cancer, 45.9% said they learned about cancer on television, 39.7% stated that the primary reason for childhood cancer was the mother's smoking during pregnancy, 68.8% said that early diagnosis would save a child, and 98% wanted to learn about childhood cancer. Conclusion: It was determined that the mothers' knowledge of cancer was deficient.

한국 의료법에서 후견주의 이념의 수용, 변형 그리고 거부 - 치료중단에 대한 법원 판결을 중심으로 - (Acceptance, Modification and Rejection of Paternalism in Korean Medical Law)

  • 이 글은 연명치료의 중단에 관해 서로 상반된 결론을 내렸던 보라매병원 판결(대법원 2002도995)과 신촌세브란스병원 판결(대법원 2009다17471)이 전통적으로 의료사회를 지배했던 의사후견주의 혹은 가족주의적 후견주의의 이념을 어떠한 방식으로 수용하거나 변형 또는 거부하고 있는지를 분석한다. 보라매병원 사건에서 법원이 '의사'의 자연법적 의무를 강조한 것은 의사가 자연법 발견의 능력이 있음을 전제하는 전통적인 의사후견주의적 인식에서 출발한 것이긴 하다. 하지만 법원은 종국적으로는 자연법 발견의 최종적 주체를 '법원'으로 상정함으로써 스스로를 환자에 대한 독자적 후견인으로 규정한 셈이 되었다. 뿐만 아니라 법원은 환자 가족의 결정 역시 법원의 자연법적 결정 뒤로 물러나게 함으로써 가족주의적 후견주의로부터 탈피했지만, 법원의 우월성을 드러낼 뿐 가족의 결정이 가질 수 있는 의미를 충분히 존중하지는 못했다. 신촌세브란스병원 사건에서는 이와는 달리 환자의 자기결정권이 갖는 의미가 무엇인지를 좀더 명확히 언급한다. 뿐만 아니라 자기결정권의 행사범위는 '내용적'으로, 그리고 '시간적'으로도 확대된다. 하지만 이 판결 역시 진정한 의미에서의 의료적 자율성에 대한 인식을 충분히 보여주진 못했다. 법원은 의사나 병원윤리위원회의 결정의 중요성을 인식하면서도 행위에 대한 실체적인 판단의 권한을 여전히 유지하고 있는 듯하며, 환자가족의 결정을 중시하긴 하지만 여전히 정황에 대한 (법원의) '객관적' 판단을 강조함으로써 결정주체로서의 권위를 포기하지 않는다.

Febrile seizures

  • Febrile seizure (FS) is the most common seizure disorder of childhood, and occurs in an age-related manner. FS are classified into simple and complex. FS has a multifactorial inheritance, suggesting that both genetic and environmental factors are causative. Various animal models have elucidated the pathophysiological mechanisms of FS. Risk factors for a first FS are a family history of the disorder and a developmental delay. Risk factors for recurrent FS are a family history, age below 18 months at seizure onset, maximum temperature, and duration of fever. Risk factors for subsequent development of epilepsy are neurodevelopmental abnormality and complex FS. Clinicians evaluating children after a simple FS should concentrate on identifying the cause of the child's fever. Meningitis should be considered in the differential diagnosis for any febrile child. A simple FS does not usually require further evaluation such as ordering electroencephalography, neuroimaging, or other studies. Treatment is acute rescue therapy for prolonged FS. Antipyretics are not proven to reduce the recurrence risk for FS. Some evidence shows that both intermittent therapy with oral/rectal diazepam and continuous prophylaxis with oral phenobarbital or valproate are effective in reducing the risk of recurrence, but there is no evidence that these medications reduce the risk of subsequent epilepsy. Vaccine-induced FS is a rare event that does not lead to deleterious outcomes, but could affect patient and physician attitudes toward the safety of vaccination.

말기의 의료적 의사결정에 관한 임상간호사의 갈등경험 (Nurse's Conflict Experience toward End-of-life Medical Decision-making)

  • Purpose: The purpose of this study was to explore clinical nurse's reported conflict experience toward end-of-life medical decision making. Methods: Data were collected by in-depth interviews with eight nurses from three different wards of university hospital in D city of Korea. Conventional qualitative analysis was used to analyze the data. Results: Results were three major themes and twelve categories from the analysis. The three major themes were prioritization of treatment, non-disclosure of diagnosis, and hierarchical and power relations. Conclusion: The results of this study suggest that shared decision making in end of life among patient, family members, physician, and nurse may contribute to improve end-of-life care performance as well as dignified dying of patient in end of life.

서울지역 성인들의 비타민.무기질 보충제 섭취 실태에 관한 연구 (The Experience of Nutrient Supplement Use among Adults in the Seoul Area)

  • This study examined the experience of vitamin/mineral supplement among adults in Seoul. Using a self-completed questionnaire including demographic information and use of vitamin/mineral supplement, 618 participants completed the study. The majority of respondents were female (72.2%), 40~49 years old (42.2%), and over college graduates (53.4%). Among the 618 respondents surveyed, 56% had experience of supplement use. The reasons for vitamin/mineral supplement use were health maintenance (56%), fatigue recovery (18%), disease prevention (8%), disease treatment (8%), and nutrition (8%) in order. The main considering factors for selecting vitamin/mineral supplement were side effect, cleanliness and reliability of company. The information sources for vitamin/mineral supplement were physician/pharmacist (45%), TV & advertisement (29%), family and relatives (19%). The main purchasing place for their supplement reported by the most of the respondents (84%) was from pharmacy.

Melissa parviflora Benth. A Review on its Ethnobotany, Phytochemistry and Pharmacological profile

  • Melissa parviflora Benth. is an aromatic perennial herb of Lamiaceae family. It is one of the most influencial plant and used from centuries in Unani system of medicine for the treatment of various malady such as Epilepsy (mirgi), hemiplegia (falij), migraine (shaqeeqa), insomnia (sehar), indigestion (sue hazm) and palpitation (khafqaan) etc. The Persian physician Avicenna endorsed it for heart problems. It has antitubercular, antipyretic, analgesic and stomachic properties, also used to remove bad breath from mouth, strengthen the gums but its main action is as a tranquillizer and nervine relaxant, it is greatly esteemed for its calming properties. Preliminary performed phytochemical analysis revealed that tannin, flavonoid and saponins are the major components of the plant extract. The plants containing saponins or flavonoids exhibit anticonvulsant activity whereas the flavonoids show various biological activities including antioxidant, anti-inflammatory and cytotoxic-antitumor etc. Keeping in view the tremendous medicinal importance of the plant Badranjboya in Unani Medicine, this review provides updated information on its phytochemistry, therapeutic uses and pharmacological properties.

Informational Needs of Women with Breast Cancer Treated with Chemotherapy

  • Background: Research in the field of informational needs of breast cancer patients is scarce. In the few published articles, these needs were usually not satisfied. The main objective of this study was to evaluate satisfaction regarding informational needs in women with breast cancer. The long-term goal was to guide physician-patient communication to meet these needs. Materials and Methods: A survey with 21 questions was completed by 84 female patients receiving chemotherapy in a one-day hospital in Beirut, Lebanon. All patients were aware of their disease and agreed to participate in the survey. Results: The doctor was the major source of information for patients followed by media (radio and television). The level of knowledge of patients concerning their disease was proportional to the number of information sources. Women aged younger than 45 years, diagnosed during the last three months before the survey and certified from high school were less satisfied with information given by the oncologist. The missing information was in relation with the steps of the treatment after the chemotherapy regimen, the risk of a family member (sisters and daughters) of developing the disease and management of lymphedema. Conclusions: This study generated a scale for the degree of satisfaction of information received by women with breast cancer from their oncologist. The physician can use this scale to improve his or her skills of communication to patients and diminish their level of fear and anxiety.

Identifying Women's Knowledge about Risk Factors of Breast Cancer and Reasons for Having Mammography

  • The aim of this study was to identify breast cancer risk factors and reasons for having mammography of the women who applied for mammography, as well as to determine their level of knowledge about risk factors and level of risk perception, and anxiety concerning breast cancer. This cross-sectional descriptive study was conducted from June 15, 2010 through September 10, 2010, in a university hospital in Ankara, Turkey. A questionnaire prepared by the researchers was used to collect the data. The mean age of the women was $52.1{\pm}9.98$ years. Sixteen percent of the women had a family history of breast cancer. The majority of participants had mammograms (75.8%) before and had gained knowledge about breast cancer and its screening (73.7%). The leading source of information about breast cancer was physicians (46.2%). Physician recommendations, having breast-related complaints, and family history of breast cancer were important reasons to obtain mammography. The mean knowledge score about risk factors of breast cancer was $4.15{\pm}2.73$ and the mean anxiety score was $1.65{\pm}1.61$. It was found that some socio-demographic and obstetrical characteristics of women, their family history, and risk perceptions about breast cancer affect their knowledge and anxiety scores about breast cancer. In conclusion, the present study identified a number of factors affecting mammography participation for women. The results of this study can be helpful in promoting screening for breast cancer.

퇴원후 뇌졸중환자의 재적응과 사회적 지지와의 관계분석 (Readjustment and Social Support of the Post Hospitalized Stroke Patients)

  • An explanatory design was employed to identify the relationship of physical, emotional & social readjustment and social support of post hospitalized stroke patients and their caregivers. A convenient sample of 254 patients who given follow-up care at the outpatient department after discharge and 225 caregivers were recruited. Mental Status Questionnaire (MSQ), Social Support Inventory Stroke Survivors (SSISS), Illness intrusiveness(II), Instument Activity of Daily Living(IADL), Center of Epidemilogic Studies-Depression(CES-D), social activity and caregiver burden were used for measurement in this study. Results showed patient's physical level measured by IADL and psychological level measured by depression were high. But social activity was low. Cognitive function, depression & social activity were not significantly different by the posthospitalized period, but IADL was. The source of professional support was mostly the physician at the outpatient department. The family support was found significantly related to patient's depression & social activity and caregiver's subjective burden. Professional support was found significantly related to patient's IADL & depression. Illness intrusiveness as a mediating variable was a sig nificantly predicting power on patient's IADL & depression. The path analysis was used to identify the variables to predict the physical, emotional, and social status of patients. As a result, patient's age, cognitive function, illness intursiveness and professional support significantly predicted the level of IADL ; patient's cognitive function, illness intrusiveness and family support significantly predicted the level of depression ; and patient's age and family support significantly predicted the level of social activity of posthospitalized stroke patients. Based upon these results, the rehabilitation programs to reduce the illness intrusiveness and improve cognitive funtion were recommended for the readjustment of the stroke patients. This model of the readjustment of the posthospitalized stroke patients is recommended as the framework for care of the stroke patients.
