Prevalence, Awareness, Control, and Treatment of Hypertension and Diabetes in Korean Cancer Survivors: A Cross-Sectional Analysis of the Fourth and Fifth Korea National Health and Nutrition Examination Surveys

  • Background: Management of hypertension and diabetes in cancer survivors is an important issue; however, not much is known about the level of management of such chronic disease in Korea. This study therefore assessed the prevalence, awareness, control, and treatment of hypertension and diabetes in Korean cancer survivors compared to non-cancer survivors. Materials and Methods: A cross-sectional design was employed, wherein data were obtained from standardized questionnaires completed by 943 cancer survivors and 41,233 non-cancer survivors who participated in the Fourth and Fifth Korea National Health and Nutrition Examination Surveys (2007-2011). We calculated adjusted proportions for prevalence and management of hypertension and diabetes in non-cancer survivors and cancer survivors. We also assessed the associated factors with prevalence and management of cancer survivors. Results: Cancer survivors are more likely than the general population to have higher prevalence, awareness, treatment, and control of hypertension. However, diabetic management was not significantly higher in cancer survivors than in non-cancer survivors, despite their having a higher prevalence. Several factors, such as, age, drinking, years since cancer diagnosis, self-perceived health status, and specific cancer types were found to affect to management of hypertension and diabetes. Conclusions: These data suggest that cancer survivors appear to be better than non-cancer survivors at management of hypertension, but not diabetes. There is a need for healthcare providers to recognize the importance of long-term chronic disease management for cancer survivors and for the care model to be shared between primary care physicians and oncologists.

파킨슨병 환자의 삶의 질과 가족지지 관련요인 분석 (An Analytical Study on Quality of Life and family Support of Patients with Parkinson's Disease)

  • Purpose: Investigation of the quality of life (QoL), family support and associated variables in patients with Parkinson's disease. Methods: The data were collected from 102 patients registered in the neurology department of C University Hospital. QoL was assessed using the Parkinson's Disease Questionnaire (PDQ-39) and Family Support Scale. Modified Hoehn and Yahr (H & Y) stages were obtained from an interview and clinical examination by a neurologist. Collected data were analyzed using the SAS program. Results: The participants' PDQ-39 average score was 34.34. The scores were significantly higher for participants who were older, who had a job and who were over 2.5 on the H & Y stage. The average for Family support scores was 41.6. Participants who had a living spouse, religion and a job scored higher than those who did not. PDQ-39 score had a positive correlation with age, job, and H & Y stage. Family support and PDQ-39 score were also positively correlated. Conclusion: It is recommended a program involving family that improves family support and be developed and that it be varied according to different H & Y stages, in order to improve QoL.

  • Recent domestic findings indicate steep increase of geriatric diseases such as obesity, diabetes, and high blood pressure among children and adolescents. It is, therefore, necessary to establish a new system for constitution inspection that reflects domestic adolescents' health conditions by the introduction of feasible items and systematic methods in the inspect ion. The objective of this study is to grasp the problems and to present proper ways of improving the system. As the method of the research during the period from September 2001 to March 2002 we refer to the domestic and foreign literature, and ask the advice of the specialists in pediatrics, family medicine, endocrine, cardiology. As on-site research we us e survey of nationwide elementary, middle, and high schools, visit representative local schools and have an interview with school nurses. Through the interview we try to find practical problems of school heath care and find proper method of students physic al examination. The abstract of the problems of student examination system is as following. To the question about the extent of the result of physical examination 46. 46.4%(school doctor) and 42.6%(parents and students) of the respondents answered "don't know". It means neglectfulness about examination itself. 62% of the school nurses responded "unsatisfactory" people who take charge of practical affairs have complaint about pre sent examination method. Because of the short time of the examination, and numerous students, the examination was incomplete. Several questions, method of physical examination, content of physical examination and list of physical examination, were raised. Reform measures for physical examination system of students were presented by physician, school nurses, students and parents of student.

만성질환에 대한 가족력의 유병률과 위험도에 관한 연구: 국민건강영양조사(2019)를 중심으로 (A Study on the Prevalence and Risk of Family History for Chronic Diseases: Findings from the Korea National Health and Nutrition Examination Survey 2019)

  • 본 연구는 국민건강영양조사 2019 자료를 이용하여 만성질환 가족력에 대한 만성질환의 유병률과 위험도를 실증적으로 도출하고자 하였다. 연구 대상자는 5,691명으로 만성질환 가족력과 관계있는 인구사회학적 변수들을 통제한 후, 만성질환 유병에 대한 만성질환 가족력의 위험도인 오즈비를 검증하기 위해 로지스틱 회귀분석을 수행하였다. 주요 결과는 성별, 나이, 소득수준, 교육수준, 거주지역을 통제한 경우, 만성질환 가족력이 없는 집단 대비 가족력이 있는 집단에서 만성질환이 발병할 위험도인 오즈비는 고혈압 2.623, 이상지질혈증 1.868, 당뇨병 1.964, 관절염 1.435로 유의하게 나타났다. 이러한 결과는 가족력 검사에 대한 표준화 도구를 개발하고 보건의료의 예방의학 차원에서 가족력이 있는 구성원에 대한 건강과 질병 관리 시스템이 필요함을 시사한다. 본 연구는 만성질환 유병률에 영향을 미치는 인구사회학적 변수를 통제하고, 만성질환에 대한 가족력의 위험도를 실증적으로 확인하여 보건의료 측면에서 실천방안을 제언했다는 점에서 의의가 있다.

유방자가검진에 관한 관련요인분석 (The Affecting Factors on Breast Self-Examination(BSE) of Korean)

  • The purpose of this study is to analyze the affecting factors on breast self-examination of middle-aged women in Korea. The subjects were 141 women on Jeju Island, and materials were collected through an organized questionnaire from March to August 2002. The data were analyzed by using descriptive statistics, $x^2$-test. A-nova and Discriminant analysis by the SAS program. The major results are as follows: 1. 53% of the subjects performed the breast self-examination. The practice of breast self-examination was correlated with the education level, income, low salt diet, more than 30 minutes of regular exercise. 2. Attitudes towards breast self-examination were not a significant practice regarding breast self-examination. However, the cognition about difficulty of breast self-examination partly showed an important barrier factor to the positive attitude. 3. The average self-efficacy concerning breast self-examination was 3.03. The stages of change related to the subjects showed that a contemplation stage was high. 4. With a stepwise multiple regression, the most significant factor on self-efficacy related with breast self-examination was the contemplation stage, 30-40 aged women. high income, regular exercise, low salt diet, family cancer history, etc. 5. With the discriminant analysis, the most important factors related with the practice of the breast self-examination were income and self-efficacy. The power of discriminant was 59%. The results of this study suggests that programs should be developed to promote practice and the self-efficacy related with breast self-examination.

서울시내 중고등학교 결핵이환학생에 대한 결핵관리실태 및 지식에 관한 조사연구 (A Study on Knowledge and Disease Management of Tuberculosis by Themselves of Tuberculosis Patients Among the Middle and High School Students in Seoul.)

  • The purpose of this survey was to find out the knowledge and the disease management of the Tuberculosis patients by themselves among the first grade middle and high school students in Seoul city during the period of June 15-July 19, 1986. Questionaires were used to collect the data and were analysed from answers of 188 students at the 113 schools. The results were as follows: 1. The ages of the students were distributed as follows: in middle school, 13 years old was $70.0\%$. 14 years old, $20.0\%$, and 15 years old, $6.7\%$. In high school, 16 years old was $66.5\%$, 17 years old, $18.4\%$, and 15 years old, $10.1\%$. 2. In X-ray mass examination by school, the rate of execution was $50.4\%$ in middle school and $96.7\%$ in high school, and in X-ray mass examination by student, it was $50.1\%$ in middle school and $97.3\%$ in high school. 3. The prevalence of Tuberculosis among the middle school students was $00.3\%$and high school students, $0.15\%$. 4. Of the total, $77.1\%$ of the respondents did not realized Tuberculosis bdore X-ray mass examination 5. The perfect cure rate of the respondents was $52.7\%$. 6. General characteristics of the respondents: a. The educational background was varied as follows: of the total $47.9\%$ of the fathers had the level of high school education and $37.2\%$ of the mothers had the level of middle school education. The educational background of the parents had no statistical significant to the medical cure rate of Tuberculosis. (P>0.05). b. The average monthly income of the family was as follows : above five hundred thousand won was $21.8\%$, three or four hundred thousand won was $22.9\%$, and below two hundred thousand won was $10.6\%$. The most frequent family size was 5-6 persons. $(59.6)\%$. 7. The actual situation of Tuberculosis control and the variables related to the treatment: a. $69.1\%$ of the respondents wanted mental support from their surroundings. $48.7\%$ of the respondents answered that their parents or the other family helped treatment as mental supporter, b. As a medical service, $53.2\%$ of the respondents were treated at Health Center, $38.8\%$ were treated at a hospital. A medical service was statistically significant to the medical cure (P<0.01). c. Family members of $61.7\%$ of the respondents had checked chest X-ray. A X-ray examination of family was statistically significant to the medical cure (P<0.005). d. $73.9\%$ of the respondents had taken the Anti-Tuberculosis-drugs regularly. Regular taking of Anti-Tuberculosis drugs was statistically significant to the medical cure (P<0.005). e. $89.4\%$ of the respondents had received a regular examination during the treatment. A regular examination was statistically significant to the medical cure (P<0.05). f. The period of perfect cure was that $50.0\%$ of the respondents took from half a year to one year, $25.2\%$ took below half a year and $16.2\%$ took from one year to one year and a half. g. The rate of the respondents who abhored to let anyone know their disease was $93.1\%$. 8. Knowledge related with Tuberculosis: a .$63.3\%$ of the respondents answered that Tuberculosis is a communiable disease. b. $89.9\%$ of the respondents answered that there is a preventive method of Tuberculosis. Among them, $28.4\%$ answered that it is B.C.G. vacination. c. $96.8\%$ of the respondents belived they can be cured perfectly. d. $42.4\%$ of the perfect curer answered that they had have permanent immunity of Tuberculosis. According to the results of above study, it is desired to be practiced X-ray mass examination to the total middle school students. Nurse teachers and the responsible persons who participated to the helping of disease management to the Tuberculosis patients must make an offer knowledge of Tuberculosis to the Tuberculosis patients. And also, it will be very helpful to the cure of Tuberculosis patients if they do their best and to have a mental supporter.

Family Members of Cancer Patients in Korea Are at an Increased Risk of Medically Diagnosed Depression

  • Objectives: Family members are often cancer patients' primary source of social and emotional support and make a major contribution to how well patients manage their illness. We compared the prevalence of depression in the family members of cancer patients and the general population. Methods: This study used the data from the fourth, fifth, and sixth rounds of the Korea National Health and Nutrition Examination Survey. The variable of interest was the presence of a cohabitating cancer patient in the family and the dependent variable was the presence of diagnosed depression. Results: The odds of having medically diagnosed depression in those with a cohabitating cancer patient in the family were significantly higher than among those who did not have cancer patients in their families (odds ratio [OR], 1.56; 95% confidence interval [CI], 1.12 to 2.17; p=0.009). The OR for females was 1.59, and this increase was statistically significant (95% CI, 1.09 to 2.31; p=0.02). Conclusions: We need to invest more effort into diagnosing and managing depression in the family members of cancer patients. This will have an impact both on their quality of life and on the well-being of patients, as supporters and caregivers play an instrumental role in helping patients manage their illness.

우리나라 가족계획사업(家族計劃事業)에 있어서의 문제점(問題點)에 대(對)한 고찰(考察) (Selected Problems in the Korean Family Planning Program)

  • An examination of avalable data relevant to several problems in the Korean family planning program led us to the following conclusions: 1. The high incidence of discontinuance in the use of IUD appears to be one of the foremost problems which effectiveness of the family planning program in Korea. Thus, the development of technologies and program strategies to decrease the discontinuance rate is badly needed to increase effectiveness and efficiency of the program. 2. The monthly supply of oral pill was found inadequate in terms of timing at the end and beginning of the year. 3. The frequency of home visiting showed discrepancy between service statistics and survey result. Evidences indicate that the latter figure is more correct. Program strategies should be developed to make home visiting activities more effective. 4. It was observed that the unit cost of the family planning program was increased fairly rapidly. It calls for the necessity of increasing cost efficiency of the program.

19세기 후반 전의감(典醫監) 의학생도(醫學生徒)에 대한 고찰 - 『전함생도안(前銜生徒案)』을 중심으로 - (A Research on the Jeonuigam medical cadet in the late 19th century)

  • Jeonuigam(典醫監) is one of the Big Three of medical government office through the Joseon dynasty era. But Jeonuigam literature is extremely rare. This paper is to analyze 362 cadets of "Jeonham-saengdoan(前銜生徒案)" by writer of Jeonuigam origin. Jeonuigam filled cadet on a three-year cycle because the national examination for medical officials is conducted on same cycle. Most cadets came from a few meritorious family of medical career officials. Ratio of ten major family stand at 34.91 percent. 373 of the 392 cadets are confirmed as family member of medical bureaucrat. They have a very closed society. Wancheon(完薦) and cadet system strengthen the monopholy power of a few family. The analysis of study confirmed that in the 19th century Joseon(朝鮮) dynasty had monopolized the medical technicians.

The Association between Family Mealtime and Depression in Elderly Koreans

  • Background: Several studies have revealed the frequency of family mealtimes to be inversely associated with depressive symptoms in adolescents. However, there have been few studies in older populations. This cross-sectional study investigated the association between family mealtime frequency and depressive symptoms in elderly Koreans. Methods: This study analyzed 4,959 elderly men and women (aged 65 years or older) who participated in the Korea National Health and Nutrition Examination Survey. Self-administered questionnaires were used to assess depressive status, family mealtime frequency, and covariates. Multiple logistic regression analysis was performed to evaluate the association using the eating alone group as a reference. Results: After adjusting for all covariates, participants who had family meals 3 times a day had fewer depressive symptoms than the eating alone group; adjusted odds ratios (ORs) (95% confidence intervals [CIs]) were 0.72 (0.58-0.89) for point depressiveness/anxiety and 0.73 (0.56-0.94) for depressiveness lasting for at least 2 weeks. In suicidal ideation, the OR (95% CI) of eating with family twice a day was significant after full adjusting for covariates at 0.67 (0.50-0.88). Conclusion: Family mealtimes were closely associated with depressive symptoms in elderly Koreans, which suggests that maintaining intrafamilial bonding is important for mental health in an older population.
