낙상 예방 프로그램이 농촌지역주민의 낙상 효능감과 활동 특이적 균형 자신감에 미치는 효과 (Effects of Fall Prevention Program on Fall Efficacy Scale and Activities-Specific of Balance Confidence Scale in Rural Residents)

  • Purpose: The study evaluated the effects of a fall prevention program in a Korean rural area. Methods: A single pretest-posttest design group was studied. Thirty nine residents in the same rural area participated in a program comprised of education and exercise. The 5-week fall prevention program was performed for 60 minutes, once a week. Results: Fall Efficacy Scale (FES) score and Activities-specific of Balance Confidence Scale (ABC) score were significantly increased after the program, but the pre-and posttest FES and ABS scores were not significantly different between the fall experience group and non-fall experience group. Conclusion: The fall prevention program can improve rural residents' FES and ABS, and is recommended as a fall prevention intervention.

안구운동이 노인의 정적균형과 낙상효능감에 미치는 영향 (The Effects of Eye Movement Training on the Static Balance and Fall Efficacy in the Elderly)

  • Purpose: This study is to identify how eye movement influences the static balance and fall efficacy of the elderly who have experienced fall-related injuries. Methods: Thirty nine elderly who scored 24 points in the Korean mini mental state examination, were able to walk, and had no specific disease, were selected as the research subjects among elderly who have experienced fall-related injury more than once in the past year. Thus, 20 for the experimental group, and 19 for the control group were selected as subjects. The experiment was conducted for 6 weeks, including periods of evaluation before and after intervention. Results: The results with respect to the dependent variables are as follows: Substitute sample t-testing showed significant differences between each group in eye movement to check difference in balance performance and fall efficacy. When independent sample t-tests were conducted to compare static balancing performance and fall efficacy between the two groups after intervention, they showed significant differences in statistical terms (p<0.05). Conclusion: From the above results of the study, it was found that the application of eye movement combined with diverse fall prevention programs is effective, when enhancing static balance performance power and improving fall efficacy.

가상현실 프로그램이 뇌졸중 환자의 낙상 효능감과 일상생활활동에 미치는 영향 (The Effects of Virtual Reality Program on Fall Efficacy and Activities of Daily Living for Patients with Stroke)

  • 목적 : 본 연구는 가상현실 프로그램을 통해 뇌졸중 환자의 낙상 효능감과 일상생활활동에 미치는 영향을 알아보고자 하였다. 연구방법 : 뇌졸중 진단을 받은 환자를 대상으로 총 4주 동안 가상현실 프로그램 중재를 받는 실험군 10명과 작업치료 중재를 받는 대조군 10명으로 나누어 실시하였다. 치료 중재 전 후 낙상 효능감을 비교하기 위하여 낙상 효능감 척도(Fall Efficacy Scale: FES)를 시행하였고, 일상생활활동 수행능력을 비교하기 위하여 기능적 독립척도(Functional Independence Measure: FIM)를 시행하였다. 결과 : 실험군과 대조군의 치료 중재 전 후 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력은 통계학적으로 유의한 차이를 보였고(p<.05), 실험군이 대조군에 비해 통계학적으로 유의하게 낙상 효능감과 일상생활활동 수행능력이 향상되었다(p<.05). 결론 : 작업치료와 더불어 가상현실 프로그램을 적용할 때 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력을 향상시킨다는 것을 알 수 있었다. 따라서 가상현실 프로그램이 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력을 위한 치료방법으로 작업치료와 더불어 임상에서 유용하게 적용될 수 있을 것이다.

재가 관절염 노인의 낙상 예방행위 예측모형 구축 (A Structural Equation Model of Fall Prevention Behavior among Community-dwelling Older Adults with Osteoarthritis)

  • Purpose: The purpose of this study was to explain fall prevention behaviors of community-dwelling elderly with osteoarthritis based on the Health Belief Model. Methods: A total of 200 older adults with osteoarthritis residing in community was recruited from July 10 to August 30, 2013. The direct and indirect effects of perceived fall risk, perceived benefits and barriers, and self efficacy of fall prevention were examined on fall prevention behaviors. Data were collected with structured questionnaires and analyzed using SPSS/WIN 20.0 and AMOS 20.0 program. Results: The hypothetical model was a good fit for the data based on the model fit indices. Among socio-demographic variables, age and fall knowledge showed significant direct effects on fall prevention behaviors. The constructed model explained 34.2% of the variance of fall prevention behaviors, including perceived fall risk and efficacy of fall prevention behaviors as significant predictors. Conclusion: The findings revealed the need to develop an effective nursing intervention to promote fall prevention behaviors of community-dwelling elderly with osteoarthritis by focusing on perceived fall risk and efficacy of fall prevention behaviors. Knowledge about fall can also be increased by an age-based education program.

Relationships among Knowledge, Self-efficacy, and Health Behavior of Osteoporosis and Fall Prevention in Old Aged Women

  • Purpose: This study was conducted among older women to (1) identify their levels of knowledge, self-efficacy, and health behavior in dealing with osteoporosis and falls and (2) explore the relationships between the study variables based on a health-beliefs model. Methods: With a cross-sectional survey design, we recruited 94 older women of ages from 65 to 74 at a community setting via convenience sampling. The study participants completed two sets of structured questionnaires (on osteoporosis and fall prevention). Results: The general characteristics of the study participants demonstrated that the women were at high risk for osteoporosis and falls. Overall, the levels of knowledge about osteoporosis and falls, their self-efficacy, and their preventive behaviors were average or slightly above. The relationships between the study variables showed that self-efficacy and healthy behavior, such as doing osteoporosis exercise, eating an osteoporosis diet, and avoiding falls, were related (r=38, p<.001; r=.33, p<.05; r=.26, p<.05). In addition, there were statistically significant relationships between osteoporosis and fall prevention knowledge (r=.37~.46, p<.001), self-efficacy (r=.50~.53, p<.001), and preventive behaviors (r=.50, p<.001). Conclusion: The women's scores on osteoporosis and fall knowledge, self-efficacy, and preventive behaviors suggest an urgent need for the implementation of educational programs for older women. A close relationship between self-efficacy and health behaviors implies a need for transformation of a traditional one-way lecture form.

  • Purpose: The purposes of this study was to develop a comprehensive community-based fall prevention program and to test the effects of the program on the muscle strength, postural balance and fall efficacy for elderly people. Methods: The design of this study was a nonequivalent control group pretest-posttest design. There were 28 participants in the experimental group and 29 in the control group. The program consisted of balance exercises, elastic resistance exercises and prevention education. The program was provided five times a week for 8 weeks and each session lasted 90 minutes. Data were analyzed using ${\chi}^2$-test, independent t-test and paired t-test using the SPSS program. Results: Muscle strength of the lower extremities, postural balance and fall efficacy scores significantly improved in the experimental group compared to the control group. Conclusion: These results suggest that this program can improve lower extremity muscle strength, postural balance and fall efficacy in elders. Therefore, this program is recommended for use in fall prevention programs for elders living in the community.

  • Purpose: This study aimed to verify the effects of a community-based fall prevention exercise program for older adults on lower extremity muscle strength, balance ability, and fall efficacy. Methods: This study recruited 30 participants, and the program was conducted for 10 weeks. The program included sessions on strength and balance exercises, conducted for 50 minutes a day and at least three times a week. The collected data were analyzed using non-parametric (Wilcoxon signed-rank test, generalized estimated equation) and parametric statistics (paired t-test, rmANOVA). Results: After 12 weeks of exercise program, lower extremity muscle strength and static-dynamic balance ability significantly increased. The fall efficacy increased from an average score of 2.48 points before the test to an average score of 2.91 points after the test. Regarding general characteristics, there were no significant differences in effect before and after the program, except for fall efficacy. Conclusion: The above results showed that the intervention of a community-based fall prevention exercise program for older adults was effective. Therefore, it is suggested that this exercise program be organized regularly within the center to provide periodic and long-term services.

Effect of Fall Prevention Exercise Program on Fall Efficacy, Depression, and Quality of Life of Elderly in the Community

  • Objective: This study aimed to investigate the effects of fall-prevention exercise programs on fall efficacy, depression and health-related quality of life in elderly. Design: a randomized controlled trial Methods: A total of 57 participants over 65-years-old have been allocated to control and experimental groups. The experimental group received 8 weeks of fall-prevention exercises that included strengthening of lower extremities, balance and gait training. They received the exercises twice a week for 50 minutes as a group.Both groups were assessed using Modified Falls Efficacy Scale (MFES), Geriatric Depression Scale (GDS), and 36-Item Short-Form Health Survey (SF-36) prior and post to the intervention. The control group also received the same exercise program after the post evaluation. Results: The experimental group showed overall improvement in MFES, GDS, and physical components of SF-36 (p<0.05). The MFES was significantly increased in the experimental group after the intervention (p<0.05). The GDS was significantly decreased in the experimental group after the intervention (p<0.05). The SF-36, only physical Function, Role limitations due to physical health, general health, and energy and fatigue categories were improved in the experimental group after the intervention (p<0.05). Conclusions: The results showed 8 weeks of fall-prevention exercise programs can increase fall efficacy and physical related quality of life while reducing depression of elderly over 65.

지역사회 노인의 요실금과 수면의 질이 낙상 효능감에 미치는 영향 (The Effects of the Urinary Incontinence and Quality of Sleep on Fall Efficacy of the Community Dwelling Elderly)

  • 지역사회 노인의 요실금과 수면의 질이 낙상 효능감에 미치는 영향을 살펴보고자 하였다. 도시에 거주하는 65세 이상 노인 112명을 대상으로, 인구사회학적 특성과 낙상에 관한 정보, 요실금 증상, 수면의 질과 낙상 효능감에 대하여 조사하였고 연구기간은 2013년 1월 3일부터 1월 30일까지였다. 지난 1년동안 낙상 발생률은 40.2%였고, 2회 이상 낙상을 경험한 노인은 낙상 경험자의 26.7%로 나타났다. 주로 겨울(53.3%)과 오후(53.3%)에 거리(44.4%)에서 많이 넘어졌으며, 낙상의 원인은 미끄러진 경우가 37. 8%로 가장 높게 나타났다. 본 연구 변수의 회귀모형을 분석한 결과 유의한 것으로 나타났으며(F=5.71, p<.01), 모형의 설명력은 9. 6%이고 낙상 효능감에 가장 큰 영향을 미치는 요인은 수면의 질(${\beta}$=.24, p<.05)인 것으로 나타났다. 노인에게서 낙상은 건강한 생활을 유지하고 각종 안전사고와 밀접한 연관이 있다는 것을 고려하여 지역사회 노인의 낙상 효능감을 사정하는 것은 낙상을 예방하는데 필요할 것으로 사료된다.

상상연습이 노인의 균형 능력과 낙상효능감에 미치는 영향 : 단일대상연구 (Effects of Mental Practice on Balance Ability and Fall Efficacy in Dwelling Elderly Population: Single Subject Design)

  • 목적 : 상상연습(mental practice)이 노인의 균형 능력에 미치는 효과를 규명하고 노인 낙상 예방을 위한 효과적인 중재 방법으로써 적용 가능성에 대해 알아보는 것이었다. 연구방법 : 본 연구는 2013년 7월부터 10월 까지 인지에 문제가 없고 낙상에 경험이 있지만 보조 도구 없이 자립보행이 가능한 65세 이상 노인 3명에게 상상연습을 제공하였다. 개별 사례 실험 연구 중 ABA' 설계를 사용하여 기초선 과정 4회기, 중재 과정 5회기, 재 기초선 과정 4회, 총 13회기로 진행되었다. 독립변수는 노인의 균형 능력 및 낙상 효능감 증진을 위해 상상연습을 사용하였고, 종속변수로는 전반적 균형능력의 변화를 알아보기 위해 버그 균형 척도(Berg Blance Scale; BBS), 낙상효능감에 변화를 알아보기 위해 낙상효능감척도(Fall Efficacy Scale: FES), 정적, 동적 균형에 변화를 측정하기 위해 각각 외다리기립검사(One-Leg Standing test; OLS), 일어나 걸어가기 검사(Timed Up & Go test : TUG)를 실시하였다. 결과 분석은 평균값과 시각적 그래프를 통해 제시하였다. 결과 : 상상연습 기법 적용 후 모든 대상자의 낙상효능감이 기초선 과정에서 측정된 수치 보다 증가된 결과를 보였으며 또한 전반적 균형 능력과 정적 동적 균형 능력 또한 증진 또는 유지 되었다. 결론 : 본 연구는 상상연습이 노인의 낙상효능감과 균형능력의 향상과 유지에 효과가 있는 것으로 나타났음으로 노인의 낙상 예방 활동으로 상상연습의 적용을 추천한다.
