골다공증 여성의 골절위험도와 낙상예방행위와의 관계에 미치는 낙상위험지각의 매개효과 (The Mediating Effect of Fall Risk Perception on the Relationship between Fracture Risk and Fall Prevention Behaviors in Women with Osteoporosis)

  • Purpose: This study aims to establish a theoretical framework for the fall prevention behavior improvement program by verifying the associations between fracture risk, fall risk perception, and fall prevention behaviors in women with osteoporosis. Methods: A survey was conducted in 122 women who were diagnosed as osteoporosis by having T-score of the femur bone density below -2.5 standard deviation from the bone density examination performed in 2 orthopedic clinics located in B metropolitan city between July 2014 and September 2014. The risk of fracture, level of fall risk perception, and fall prevention behaviors were measured. Results: Fall risk perception had a complete mediating effect on the relationship between the fracture risk and fall prevention behaviors in women with osteoporosis. Conclusion: The perception of fall risk is important to enhance fall prevention behaviors in women with osteoporosis, and the development of various education programs to improve awareness of fall risk is needed.

방문간호를 받는 재가노인의 낙상위험 (Fall Risk in the Community-dwelling Elderly who Received Home Care Services: Focused on Residential Environment and Perception of Fall Risk)

  • Purpose: The purpose of this study was to investigate the factors increasing fall risk in the residential environment risk and the perceived fall risk among the older adults who received home care services to provide information for developing a comprehensive falls intervention program. Methods: The subjects were 227 community-dwelling elderly aged 65 years and over who were taken care of by home-visiting nurses of the national health centers. The data were collected from July to August in 2012 using the Choi's residential environmental risk scale (2010) and the Hong's fall risk scale (2011). Results: Requires an assistive devices to walk, modified residential environment, health security, approval certificate of LTC, residential safety perception, residential environment risk, and perception of fall risk were statistically significant risk factors. A multiple logistic regression analysis showed that room & kitchen, physical perception, medication & ADL perception, floor-related environmental perception, and daily living tool-related perception were statistically significant predictors of fall. Conclusion: The results showed that the residential environment and the perceived fall risk were associated with fall experiences among the elderly. It is necessary to develope multifactorial intervention programs considering both environmental and perceived risk factors as well as physical risk factors to reduce and prevent falls among the elderly.

침대 자세 기반 입원 환자의 낙상 위험 예측 모델 설계 (Predictive Modeling Design for Fall Risk of an Inpatient based on Bed Posture)

  • 본 논문에서는 환자의 자세를 기반으로 행동을 예측하여, 의료진에 의해 입력된 개인의 병력 중심의 프로파일과 신체정보, 침상의 기본 정보를 모두 조합하여 침대에서의 낙상 위험을 예측하는 모델을 설계하고, 위험의 수준을 판단할 수 있는 알고리즘을 제시한다. 낙상 위험 예측은 크게 환자의 프로파일을 활용한 정성적 낙상 위험 노출도 평가와 실시간 낙상 위험 측정 단계로 구분된다. 정성적 낙상 위험 노출도는 의료진이 낙상 위험과 관련된 환자의 건강 상태를 점검하여 위험 노출도를 평가함으로써 위험 등급이 결정된다. 실시간 낙상 위험 측정 단계에서는 환자의 침대에서의 자세를 인식하고 환자의 정성적 위험등급 정보가 고려된 낙상 위험 측정을 위한 규칙 기반 정보를 추출한다. 인식된 환자 자세 정보와 정성적 위험평가 정보를 모두 조합하여 시그모이드 함수를 활용하여 최종 낙상 위험 수준을 예측한다. 본 연구에서 제시된 절차와 예측 모델은 입원 환자를 위한 낙상 사고 예방과 환자 안전을 위한 개인화 서비스에 크게 기여할 것으로 기대된다.

입원한 노인환자의 낙상위험도, 낙상두려움 및 사회적 지지가 낙상예방행위에 미치는 영향 (Influence of Fall Risk, Fear of Falling, and Social Support on Fall Prevention Behaviors among Older Inpatients)

  • Purpose: This study aimed to investigate the influence of fall risk, fear of falling, and social support on older inpatients' fall prevention behaviors. Methods: The participants were 105 older inpatients. Data were analyzed using descriptive statistics, independent t-test, One-way ANOVA, Pearson's correlation coefficients, and multiple regression analysis using the SPSS program. Results: The mean fall risk, fear of falling, social support, and all prevention behaviors were 58.05±28.27, 2.47±0.67, 3.58±0.63, and 2.93±0.48, respectively. There were significant differences in fall prevention behaviors by sex, and fall related to education experience. Fall prevention behaviors were associated with fall risk, and fear of falling. The factors influencing the fall prevention behaviors of the older inpatients were sex, fall related to educational experience, fall risk, and fear of falling. It was found that 21% could explain fall prevention behaviors. Conclusion: The results indicate a need to develop a fall prevention education program, including intervention, to reduce the fear of falling among older inpatients.

일 종합병원 입원환자의 진료과별 낙상특성 및 위험요인 (Fall Risk Factors and Characteristics of an Acute Hospital Setting across Clinical Departments)

  • Purpose: This study was done to determine inpatient fall rates in an acute hospital setting and to explore risk factors and characteristics across clinical departments. Methods: The medical records and standardized fall reports of 416 patients admitted between January 1 and December 31, 2012 were reviewed. Descriptive statistics and statistical tests were used, including: t-test, ${\chi}^2$-test, ANOVA. Results: The total fall rate per 1,000 inpatient days was 0.49. Fall rate, fall risk factors and characteristics such as age, type of fallers and mean MFS (Morse Fall Scale) differed significantly among clinical departments. Conclusions: The analysis results show that the fall rates, fall risk factors and characteristics of acute hospital inpatient falls varied significantly across clinical departments. The findings of this study suggest that hospitals should consider differences in fall related characteristics across clinical departments when implementation fall prevention strategies and interventions.

종합병원 입원 환자의 낙상 위험 요인 및 낙상 위험도 평가 (Fall Risk Factors and Fall Risk Assessment of Inpatients)

  • Purpose: The purpose of this study was to identify the fall risk factors and to evaluate the effectiveness of the Morse Fall Scale(MFS) as an assessment tool among hospitalized inpatients. Methods: The medical records of a total of 294 patients who admitted to hospital from January 1 to December 31, 2010 were reviewed. One hundred forth seven patients who had experienced fall were matched with 147 patients who have never experienced fall. The fall information was obtained from electronic medical records and fall reports. Results: There were significant differences in visual disturbances, pain, emotional disturbances, sleep disorder, urination problems and elimination disorder at admission between fallers and non-fallers. Patients who had higher MFS scores at admission were more likely to fall as compared to the patients with lower MFS scores. When falls did occur, these occurred within five days following admission, in the patient room, among patients with alert mental status, and among patients who were ambulant with some assistance. Conclusion: The findings of this study support the need of using risk assessment tool for predicting risk for falls. This finding can be used as a useful resource to develop nursing intervention strategies for fall prevention at the hospital.

  • Construction fall accidents have been investigated by many researchers. Construction workers are prone to fall when elevations of the construction site is high. And falls are the most fatal accidents: it can be directly linked to the death. Construction fall accidents might be reduced by predetermining several areas which are highly probable to have fall accidents and by controlling such areas until the completion of the building construction. In this paper, a fall prevention system is suggested which can identify the areas where the focus on fall protection is perhaps most needed from the process characteristics. Main methodologies for this research are summarized as follows: 1. A data base on elements and types of falls is constructed from the data analysis of last 10 years fall accidents history. 2. Guideline is derived by identifying the highly probable areas of fall accidents with respect to the specific construction process. 3. Developed system is verified by applying the system to construction sites. 4. Finally a fall prevention system is suggested by utilizing the fall accidents data.

폐경여성의 낙상 위험과 관련요인 (Fall Risk related Factors in Postmenopausal Women)

  • Purpose: The purpose of this study was to understand the risk of falling associated with postmenopausal women and to identify the relationships between this risk and factors such as lifestyle, metabolic syndrome, and bone mineral density. Methods: The sample was 128 postmenopausal women between 50 and 65 from one menopausal clinic in an urban city. The Risk Assessment for Falls Scale II, developed by Glydenvand and Reinboth (1982) and adapted by Park Young-Hye (2003), was modified and used for this study. Results: The average fall-risk score in postmenopausal women was 7.2 out of 33, the fall-risk score associated with lifestyle was higher in women exposed to stress frequently or who favored spicy or salty foods. The fall-risk score associated with metabolic syndrome was higher in groups with HBP or with a waist circumference of 80cm or greater. The fall-risk score in groups with three or more factors of metabolic syndrome was the highest. Conclusion: The risk of fall in post-menopausal women was higher in groups with only elementary education, unemployed, reported two or more chronic diseases or reported frequent exposure to stress and for women who preferred spicy or salty foods or exhibited three or more factors of metabolic syndrome.

재가 허약노인의 낙상효능감, 낙상위험지각 및 낙상예방행위와의 관계 (Relations among Fall Efficacy, Perception of Fall Risk and Fall Prevention Behavior in the Frail Elderly at Home)

  • 본 연구의 목적은 65세 이상의 허약노인을 대상으로 낙상효능감, 낙상위험지각 및 낙상예방행위와의 관계를 규명하기 위함이다. 자료 수집은 재가 허약노인 300명을 대상으로 설문조사로 이루어졌다. 본 연구 결과는 다음과 같다. 1)낙상효능감은 $65.4{\pm}23.62$점이었으며, 낙상위험지각은 $34.3{\pm}11.5$점이었고, 낙상예방행위는 $17.7{\pm}2.75$점이었다. 2)낙상효능감(t=2.905, p<.05), 낙상위험지각(t=2.704, p<.05), 낙상예방행위(t=2.706, p<.05) 모두 낙상경험에 따라 유의한 차이가 있었다. 3)낙상효능감과 낙상위험지각정도와는 역상관관계(r=-.440, p<.001), 낙상효능감과 낙상예방행위(r=.326, p<.001) 및 낙상위험지각과 낙상예방행위(r=.436, p<.001)와는 순상관관계에 있었다. 본 연구를 통하여 볼 때 노인의 낙상예방을 위한 프로그램 개발은 노인의 낙상효능감 증진 및 지각을 증진할 수 있도록 개발되어야 할 것이다.

치매노인의 낙상위험요인에 관한 메타분석 (A Meta-analysis of the Risk Factors related to Falls among Elderly Patients with Dementia)

  • Purpose: The purpose of this study was to provide data about the risk factors related to falls among elderly patients with dementia using meta-analysis. Methods: Key words used for search through electronic database (CINAHL, PubMed, Ovid-MEDLINE, RISS, KISS, DBPIA, National Assembly Library) included 'dementia', 'Alzheimer', 'fall'. Twenty studies met the inclusion criteria for the meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that risk factors related to falls were identified as the demographic (age, gender, education), dementia-related (disease duration, cognition), physical (body mass index, walking, balance, activity of daily living, use of walking aids, number of medications including psychotropic drugs, musculoskeletal problems, parkinsonism, comorbidity), psychological (neuropsychiatric symptom, depression), environmental (Physical environment), and fall-related (fall history, high risk group of fall) factors. The effect size of risk factors such as high risk group of fall (r=.35), use of walking aids (r=.33), depression (r=.31), psychotropic drugs (r=.27), Musculoskeletal problems (r=.25) were higher than the other risk factors. Conclusion: Based on the findings of this study, strategies to improve elderly patient's depression, intensive care for high risk group of fall, and adequate training with walking aids are needed for prevention of falls in elderly patients with dementia.
