A study on the extraction of risk factor and its application for senile dementia patient at home based on accidental cases (사고사례를 통한 재가치매환자의 위험요소 추출 및 그 활용에 관한 연구)

  • The purpose of this study was, first, to extract the risk factor by investigating several cases of accident of senile dementia patient at home, and second, based on these results to provide basic information for the determination of monitoring factor for the care of senile dementia patient. Basic and behavioral characteristics, Short form of Samsung Dementia Questionnaire (S-SDQ), Activities of Daily Living (ADL), and cases of accident were investigated with 55 senile dementia patient at home (16 male, 39 female). Based on these questionnaires, risk factors were extracted and frequency, cooccurrence frequency, and occurring place of risk factors, presence or not, region, and degree of injury were investigated. Frequency between risk factors and behavioral characteristics, ADL, and S-SDQ was analyzed by crosstabulation frequency analysis. Results showed that 12 risk factors were extracted, and the frequency of 'going out' was the highest, and risk factors for injury were 'tumble', 'bump', 'slip', and 'fall'. Cooccurrence frequency analysis showed that the occurrence of 'fall', 'going out', 'fire of gas', and 'violence' with other factors was relatively higher than others. The occurring place of risk factor was the highest in home neighborhood, and the region of injury in knee, and the degree of injury with bruise. Crosstabulation frequency analysis showed that factors which had difference in frequency of risk factor were behavioral disorder, disorder of daily living and ADL. Factor which had difference in frequency due to the degree of behavioral disorder and disorder of daily living was 'going out', and factors which had difference in frequency due to the degree of ADL were 'slip' and 'fire of gas'.

A Study of Industrial Patients from Selected General Hospitals in the Kyung Pook and Taegu City Areas (일부지역 산업재해환자 실태 연구 -대구, 경북지역 일부 종합병원 중심으로-)

  • The purpose of this study is to research the actual conditions of industrial accident patients and to produce worker satisfaction and a rational and effective counter measure pain. Direct interviews with 179 cases (in and out patients) were carried out during a three month period from April to July 1990, at six hospitals two general hospitals Sun Lin and Sung Mo in Po Hang, and four general hospitals in Taegu Kyung Pook University Hospital, Dong San Medical Center, Young Nam Medical Center and Catholic Hospital. The results of this study are summarized as follows: 1. Among the 179 cases, 51.6 % were male and 48.4 % were female. The two largest age groups were 30~39, 31.8 % and 20~29, 27.4 %. Among the 179 cases, 51.6% were married, the largest family number was 2 to 3, 41.1% and 4 to 5, 25.6%. Educationally, graduation from high school was the largest group, 46.4% among the patients, followed by middle school and primary school. The largest group income level was from 40~69만원, 45.2%. The largest group of patients who worked over 50 hrs. a week was 52.0%. The largest group of patients who worked less than 1 year was 44.7%, of the patients in work places of less than 100 people, 60.3% were injured and in work places of 100~299 people, 20.1% were injured. In manufacturing, the lagest group injured was 55.3%, the next group was transport, stroage, communication. The largest group of production workers injured was 40.2%. 2. The cause of injury in the largest group was facility problems, 33.5%. The next group was unsafe habits, 30.2% a lack of safety knowledge, 17.9% and insufficient supervision, 12.3%. The 30~39 year age group was head the highest number of injuries, 40.4% work places with more than 10 yeras of work, 44.4% work palces with more than 1000 people, 56.3% and mining accidents, 80.0%. Among these groups the highest cause of injury was due to facility problems. 3. The accident pattern showed machinery injuries 28.5% as the largest group, followed by falls & falling objects 17.3%, fire & electric 15.1%, struke by an object 14.5%, followed by overaction and vehicular accidents. The accident pattern showed 46.4 % among workers over the 50 year age group, workers in the 5~10 year group, 50.0 % places employing more than 1000 workers, 35.3 % : construction 73.7%, and construction workers 57.1%, among these fall & falling objects caused the greatest number of injuries. 4. The largest group of injuries was fractures 54.8%, trauma 14.5%, amputation 11.7%, open wound, and burns. The largest number of fractures occurred in people in the 30~39 year age group, 63.2 % over 10 years of work, 55.6% in work places of 300~400 people, 63.6% construction 63.2% and general workers 57.2 %. 5. The largest group of injuries was upper extremity 45.3%, lower extremity 24.0%, trunk 18.5 % and head or neck 12.2%. Of these groups, upper extremity injuries were the highest in those less 20 years old 75.0%, less than 1 years of work 59.5%, in work places of 500~999 people 60.0%, manufacturing 56.6 % and production workers 55.6%. 6. Periods of injury showed 34 people injured in September, to be the largest followed by October, 32 August, 22 people July, 19 people and the lowest December, 2 people. During the week, Friday had the largest group injured, 35 people followed by Saturday, 26 people and the lowest was Wednesday, 17 people, During the day 1400 hours had the largest group injured, 38 people followed by 800 hours, 31 people. 7. On a basis of 5 as the highest mark, the average, according to worker satisfaction showed facility safety 3.55, work environment 3.47, income 3.44, job 3.21 and treatment 2.98. 8. The correlation between general characteristics and injury showed that age was directly correlated to the duration of work(r=.2591) p<0.01, age was directly correlated to industry (r=2311) p<0.01, and the duration was directly correlated to occupation(r =.4372) p<0.001.

A RETROSPECTIVE CLINICAL STUDY OF CONDYLAR FRACTURES OF THE MANDIBLE IN A 4-YEAR PERIOD (하악 과두 골절에 대한 4년간의 후향적 임상연구)

  • The present study was performed to evaluate the function of the mandible according to the pattern of fracture and treatment methods of condylar fractures of the mandible and help operators in making a treatment plan. Sixty patients (average follow-up period was $7.8{\pm}9.4$ months) who were treated for condylar fracture from June, 2002 to May, 2006 at the Department of Oral and Maxillofacial surgery, Chonnam National University Hospital were reviewed. The common causes of the condylar fracture were traffic accident and fall-down (35.0%). In concomitant injuries, laceration was 46.7% and the fracture of the mandibular symphysis was highest incidence (60.0%). The common site of the fracture was the condylar head (47.8%), followed by subcondyle (36.2%) and condylar neck (15.9%). Under 15 years old patients, the closed reduction was performed in 87.5% out of the patients. All of the condylar fragments were fixed to the mandible with titanium miniplates in cases of open reduction. The mean period of intermaxillary fixation (IMF) was $14.2{\pm}6.5$ days in closed reduction and $10.0{\pm}4.2$ days in open reduction. The old patient with bilateral condylar head fractures, who were treated by closed reduction with IMF for 3 weeks, showed the limitation of mandibular movements. But, there was no significant different results between open reduction and closed reduction with the respect of the Helkimo's mandibular mobility index and clinical dysfunction index (DI). Complications, such as fibrous ankylosis and resorption of the mandibular condyle, were not observed in all patients. These results suggest that the good results can be obtained by closed reduction with proper IMF periods and functional exercise in most condylar fractures of the mandible except severely displaced extracapsular fractures.

Lifejcket-Integrated Antenna for Search and Rescue System (탐색 및 구조 시스템용 구명조끼 내장형 안테나)

  • When the crew or passengers fall into the water due to marine accident of vessel, it is very important to rescue them quickly. In the case of marine accidents, most people in distress have been wearing a lifejacket, so if the GPS and Cospas-Sarsat communication module will be integrated within the lifejacket, it is easy to rescue them. In this paper, development of the dual band lifejacket-integrated antenna for GPS and Cospas-Sarsat communication is discussed. The antenna with the FR-4 substrate of 0.2mm thickness for flexibility was designed that it can be fitted close to the shoulder of the life jacket and operate at 1.575GHz and 406MHz. The GPS communication antenna was implemented with a ring-slot antenna having a circular polarized characteristic and a meander type linear polarized antenna is used as Cospas-Sarsat communication. The two antennas are fed by a single microstrip line and an open stub is used to minimize the mutual interference between the two antennas. The performance of the fabricated antenna attached to the life vest is confirmed by the measurement of the return loss at GPS and Cospas-Sarsat frequency bands.

A CLINICAL AND STATISTICAL STUDY OF MANDIBULAR FRACTURES (하악골 골절의 임상통계학적 연구)

  • This is a clinical study on 235 inpatients who sustained a total of 350 mandibular fractures and who were treated in our department during the period of Oug., 1989 through Dec., 1993. The results obtained are as follows : 1. The mumber of patients has not been increased year after year. In respect of incidence, there were the highest frequency as 67 patients(28.5%) in 1991 and the least frequency as 16 patients(6.8%) in 1989 and the highest frequency as 28 patients(11.9%) in October and the least frequncy as 12 patients(5.2%) in February. 2. The age frequency was the highest in the second decade(30.6%). The youngest patient was 8 months and the eldest patient was 80 years and mean age was 27.8 years. The ratio of men to Wemen was 4 : 1. 3. The most frequent cause was fall down as 86 patients(36.6%) and the next was fist blow 4 as 75 patients(31.9%), traffic accident as 56 patients(23.9%) in order. 4. The most common location was symphysis as 141 numbers(40.3%) and the next was condyle as 96 numbers(27.4%), angle as 94 numbers(26.8%), body as 14 numbers(4.0%) in order. The sum of fracture sites were 350 and there were 1.49 fracture sites per one patient and multiple fractures occured in 100 patients(40.5%) of the 235 patients. The ratio of Left to Right except for symphysis was 1.71 : 1. 5. Among the 333 associated injuries, facial laceration was 136(57.9%) and tooth injury was 58(24.7%) and extremity was 31(13.2%). 6. Among the 350 mandibular fractures, complications after treatment occured in 51(14.6%) and infection as 20(5.7%) was the most frequent complication. 7. In respect of treatment of mandibular fracture, close reduction was 64 patients(27.2%) and open reduction was 164 patients(69.8%) and other were 7 patients(3.0%) of all and so open reduction was the most common in type of treatment.

ENDODONTIC TREATMENT WITH CALCIUM HYDROXIDE OF REPLANTED TOOTH : A CASE REPORT (재식한 치아에서 수산화 칼슘을 사용한 근관치료)

  • Tooth avusion implies total displacement of teeth out of its socket. Its frequency range from 0.5 to 16% of traumatic injuries. Replantation procedure is used for the treatment of avulsed tooth. However, its major complications are pulp necrosis, inflammatory root resorption and replacement root resorption. This paper describes 10 years and 2 months old male patient whose both maxillary central incisors were avulsed due to fall-down with slightly underdeveloped root apices. Teeth were replanted 2 hours after accident. Right central incisor's pulp tissues were extirpated and filled with $Vitapex^{(R)}$(calcium hydroxide) at 3-4 weeks after replantation, but left central incisor was filled at 8-9 weeks. Right central incisor showed little inflammatory resorption in apical portion, whereas left central incisor showed severe root resorption. During 18 months' follow-up period, left central incisor showed slightly ankylosis while right central incisor did not. Based upon the above-mentioned results, the following conclusions can be drawn: 1. Inflammatory resorption could be suppressed by endodontic treatment with calcium hydroxide. 2. When apex formation is doubted in replanted tooth due to avulsion, early endodontic treatment with calcium hydroxide seems to act positively for better prognosis.

Acutrak Screw Fixation for Radial Head Fracture -7 Cases Report- (Acutrak 나사를 이용한 요골두 골절의 치료 -7례 보고-)

  • Purpose: To evaluate the efficiency of Acutrak screw fixation for radial head fracture (Mason type 2) without considering the safe Bone of radial head. Materials and Methods: Consecutive seven radial head fracture of Mason type II underwent internal fixation with Acutrak screws from May 2001 to February 2003. The mean follow-up period was 1.2years (ranged, $6 months{\sim}2.5 years$). The mean age of patients was 47 years old (ranged, $36{\sim}60years$ old). The cause of injury were fall down -4 cases and traffic accident -3 cases. The results were evaluated by Mayo Clinic results scoring system. Results: Functional Rating Index of Mayo Clinic was excellent- 2 cases and good- 5 cases. There were no nonunion, loosening, heterotopic ossification, infection or degenerative changes. The postoperative range of motion in elbow joint is nearly full for flexion, extension, pronation and supination in this study Conclusion: Consideration of safe zone is not necessary when Acutrak screws are used for radial head fracture. It seems to be a useful method that Acutrak screw fixation for radial head fracture (Mason type II) could achieve good radiologic and clinical results without influencing proximal radio-ulnar joint and has powerful fixation.

Clinical Features of Distal Tibial Fractures and Treatment Results of Minimally Invasive Plate Osteosynthesis (원위 경골 골절의 임상양상 및 최소 침습적 금속판 고정술의 결과)

  • Purpose: The purpose of this study is to analyze the clinical features of distal tibia fractures and to evaluate the treatment outcomes of minimally invasive plate osteosynthesis (MIPO). Materials and Methods: From January 2004 to December 2009, 84 cases of 81 patients treated with plate fixation for distal tibia fracture were enrolled in this retrospective review. We investigated age, sex, injury mechanism, fracture patterns, and complications, and the clinical features were analyzed. To evaluate the treatment outcomes of MIPO, we divided into two groups. MIPO group consisted of 55 patients were treated with MIPO technique and conventional group consisted of 18 patients were treated with open reduction and internal fixation with conventional anterolateral plating. The results were compared between two groups by assessing bony union time, operation time, amount of blood loss, range of ankle motion, clinical score by American Orthopaedic Foot and Ankle Society (AOFAS) score, and post-operative complications. Results: The mean age of 81 patients with distal tibia fracture was 54.8 years. According to AO classification, A1:2:3 were 16, 20, 16 patients, B1:2:3 were 2, 8, 7, C1:2:3 were 1, 3, 11 patients. According to injury mechanism, slip down injury was patients, traffic accident was 26, fall from height injury was 14 patients respectively. The type A fractures were lower energy trauma and more older patients. The type C fractures were higher energy trauma and younger patients. MIPO group was better than conventional group in operative time, blood loss, bony union time, and ankle joint motion. In complications, MIPO group showed no nonunion and infection, one malunion, one skin necrosis, nine skin irritations, and one screw breakage. Conventional group showed two nonunion, four infections, two skin necrosis, and one metal failure. Conclusion: Distal tibial fractures caused by low energy trauma were on the increase. Minimal invasive plate osteosynthesis was shorter bony union time and operation time, less blood loss, and larger ankle motions than conventional open reduction and plate fixation.

Clinical Observation and Treatment of Fracture-Dislocation of Talus (거골 골절 및 탈구의 임상적 고찰)

  • Talus is a important structure of the ankle joint and its function is critical for ambulation and weight bearing. The talus fracture is rare, but the complications of fracture and dislocation are serious and resulting in avascular necrosis, osteoarthritis. So its treatment is carefully considered at initial status. Authors reviewed 11 cases of fracture and dislocation of the talus treated at Yeungnam university hospital from 1984 to 1991. The longest follow up was 8 years and shortest, 1 year. The results were as follows. 1. There were all males, the average age was 30 years old. 2. The most common cause was fall down(8 cases), and next traffic accident(2cases), sports injury(1 case). 3. According to Marti-Weber classification, 1 cases was type I, 1 in type II, 4 in type III and 5 in type IV. 4. The method of treatment were open reduction and internal fixation in 6 cases, the others were closed reduction in 5 cases. 5. Final results(by Hawkins grading system) were as follows, 3 cases were excellent, 4 cases were good, 1 case was fair and 3 cases were poor. 6. Complications were AVN in 2 cases, degenerative arthritis in 8 cases, malunion in 1 case. 7. The range of motion of the ankle joint was relatively preserved(74%), but in the subtalar joint it was decreased(43%).
