In the core of the nuclear power plant of PWR, several cases of fuel failure by unknown causes have been experienced for various fuel types. From the common features of the failure pattern, failure lead time, flow conditions, and flow induced vibration characteristics in nuclear fuel bundles, it is deduced that the fretting wear failure of the fuel rod at the spacer grid position is due to the fluidelastic vibration. In the past, fluidelastic vibration was simulated by quasi -static semi-analytical model, so called the static model, which could not account for the interaction between the rods within a bundle. To overcome this defect and to provide for more flexibilities applicable to the fuel bundle, Tanaka's unsteady model was modified to accomodate the geometrical differences and governing parameter changes during the operations such as the number of rods, pitch to diameter ratio (P/D), spring force, damping coefficient, etc. The critical velocity was calculated by solving the governing equations with the MATLAB code. A comparison between the estimated critical velocity and the test result shows a good agreement. Finally, the level of decrease of the critical velocity due to the reduction in the spring force and reduced damping coefficient due to the radiation exposure is also estimated.

The present study aimed at investigating the effect of a special plaster on the out-of-plane behavior of masonry walls. A reference specimen, plastered with conventional plaster, and a specimen plastered with a special plastered were tested under reversed cyclic lateral loading. The specimens were identical in dimensions and material properties. The special plaster contained an additive, which increased the adherence strength of the plaster to the wall. The amount of the additive in the mortar was adjusted based on the preliminary material tests. The influence of the plaster on the wall behavior was evaluated according to the initial cracking load, type of failure, energy absorption capacity (modulus of toughness), and crack pattern of the wall. Despite having limited contribution to the ductility, the special plaster increased the ultimate load capacity of the wall about 25%. The failure mode of the wall with special plaster resembled the plastic failure mechanism of a reinforced concrete slab in the formation of yielding lines along the wall. The deflection at failure and the modulus of toughness of the wall with special plaster were measured to be in order of 60% and 75% of the corresponding values of the reference wall.
In this paper, the effect of normal load on the failure mechanism of echelon joint has been studied using PFC2D. In the first step, calibration of PFC was undertaken with respect to the data obtained from experimental laboratory tests. Then, six different models consisting various echelon joint were prepared and tested under two low and high normal loads. Furthermore, validation of the simulated models were cross checked with the results of direct shear tests performed on non-persistent jointed physical models. The simulations demonstrated that failure patterns were mostly influenced by normal loading, while the shear strength was linked to failure mechanism. When ligament angle is less than $90^{\circ}$, the stable crack growth length is increased by increasing the normal loading. In this condition, fish eyes failure pattern occur in rock bridge. With higher ligament angles, the rock bridge was broken under high normal loading. Applying higher normal loading increases the number of fracture sets while dilation angle and mean orientations of fracture sets with respect to ligament direction will be decreased.

For repairable software systems, the Mean Time Between Failure (MTBF) is used as a measure of software system stability. Therefore, the evaluation of software reliability requirements or reliability characteristics can be applied MTBF. In this paper, we want to compare MTBF in terms of parameter estimation using Makeham life distribution. The parameter estimates used the least square method which is regression analyzer method and the maximum likelihood method. As a result, the MTBF using the least square method shows a non-decreased pattern and case of the maximum likelihood method shows a non-increased form as the failure time increases. In comparison with the observed MTBF, MTBF using the maximum likelihood estimation is smallerd about difference of interval than the least square estimation which is regression analyzer method. Thus, In terms of MTBF, the maximum likelihood estimation has efficient than the regression analyzer method. In terms of coefficient of determination, the mean square error and mean error of prediction, the maximum likelihood method can be judged as an efficient method.
This paper aims at developing the knowledge on the seismic behavior of dowel beam-to-column connections, typically employed in precast buildings in Europe. Despite the large diffusion of the industrial buildings, a high seismic vulnerability was exhibited by these structures, mostly due to the connection systems deficiencies, during some recent earthquakes (Emilia 2012, Turkey 2011). An experimental campaign was conducted on a typical dowel connection between an external column and a roof beam. In this paper, the performed cyclic shear test is described. According to the experimental results, the seismic response of the system is evaluated in terms of strength, stiffness and failure mechanism. Moreover, the complete damage pattern of the test is described by means of the instrumentations records. The connection failure occurred due to the concrete cover failure in the column (splitting failure). Such a mechanism corresponds to a negligible energy dissipation capacity of the connection, compared to the overall seismic response of the structure. The experimental results are also compared with the results of a similar monotonic shear test, as well as with some literature relationships for predicting the strength of dowel connections under horizontal (seismic) loads.
This paper aims to study the behavior of short reinforced concrete columns confined with external glass Fiber Reinforced Polymers (GFRP) sheets under eccentric loads. The experimental part of the study was achieved by testing 9 specimens under eccentric compression. Three eccentricity ratios corresponding to e/t = 0, 0.10, 0.50 in one direction of the column were used. Specimens were divided into three groups. The first group was the control one without confinement. The second group was fully wrapped with GFRP laminates before loading. The third group was wrapped under loading after reaching 75% of failure loads of the control specimens. The third group was investigated in order to represent the practical case of strengthening a loaded column with FRP laminates. All specimens were loaded until failure. The results show that GFRP laminates enhances both failure load and ductility response of eccentrically loaded column. Moreover, the study also illustrates the effect of confinement on the first crack load, lateral deformation, strain in reinforcement and failure pattern. Based on the analysis of the experimental results, a simple model has been proposed to predict the improvement of load carrying capacity under different eccentricity ratios. The predicted equation takes into consideration the eccentricity to cross section depth ratio, the ultimate strength of GFRP, the thickness of wrapping laminate, and the time of wrapping (before loading and under loading). A good correlation was obtained between experimental and analytical results.
This report details a case of 8-year-old girl showing failure of odontogenesis after chemo-radiation therapy for the rhabdomyosarcoma at the age of 4. The observed results were as follows : 1. Past history revealed that she had received for a total radiation dose of 4430cGy, 29 fractions in 6 weeks and chemotherapy with vincristine, actinomycin D and cytoxan, followed as maintenance phase for 2 years. 2. The patient was symptom -free and appointed for the treatment of multiple dental caries. 3. Oral examination showed hypoplastic enamel on whole erupted permanent teeth and showed retarded eruption. 4. Conventional radiograms showed failure of root development including abrupt cessation of root formation and root agenesis, and microdontia, missing teeth, irregular enamel, dislocation of the impacted teeth. Additional finding showed good healing bone pattern on the left mandibular ramus and angle area. 5. Cephalometric analysis revealed failure of bite raising due to incomplete eruption of all the first molars and made it possible to suspect entrapped mandibular growth and then Class II tendency growth. 6. There was correlation between the time of chemo-radiation therapy and the damage of the teeth.
Cooling by the flow of water through an embedded cooling pipe has become a common and effective artificial thermal control measure for massive concrete structures. However, an extreme thermal gradient induces significant thermal stress, resulting in thermal cracking. Using a mesoscopic finite-element (FE) mesh, three-phase composites of concrete namely aggregate, mortar matrix and interfacial transition zone (ITZ) are modeled. An equivalent probabilistic model is presented for failure study of concrete by assuming that the material properties conform to the Weibull distribution law. Meanwhile, the correlation coefficient introduced by the statistical method is incorporated into the Weibull distribution formula. Subsequently, a series of numerical analyses are used for investigating the influence of the correlation coefficient on tensile strength and the failure process of concrete based on the equivalent probabilistic model. Finally, as an engineering application, damage and failure behavior of concrete cracks induced by a water-cooling pipe are analyzed in-depth by the presented model. Results show that the random distribution of concrete mechanical parameters and the temperature gradient near water-cooling pipe have a significant influence on the pattern and failure progress of temperature-induced micro-cracking in concrete.
Rock as a mass generally exhibits discontinuities, commonly witnessed in rock slopes and underground structures like tunnels, rock pillars etc. When these discontinuities experiences loading, a new crack emerges from them which later propagates to a macro scale level of failure. The failure pattern is often influenced by the nature of discontinuity, geometry and loading conditions. The study of crack growth in rocks, namely its initiation and propagation, plays an important role in defining the true strength of rock and corresponding failure patterns. Many researchers have considered the length of the discontinuity to be fully persistent on rock or rock-like specimens by both experimental and numerical methods. However, only during recent decades, there has been a substantial growth in research interest with non-persistent discontinuities where the crack growth and its propagation phenomenon were found to be much more complex than persistent ones. The non-persistence fractures surface is generally considered to be open and closed. Compared to open flaws, there is a difference in crack growth behaviour in closed or narrow flaws due to the effect of surface closure between them. The present paper reviews the literature that has contributed towards studying the crack growth behaviour and its failure characteristics on both open and narrow flaws subjected to uniaxial and biaxial compression loading conditions.

Thirty patients underwent bypass operation with autogenous saphenous vein or synthetic graft with or without concomitant lumbar sympathectomy. Remaining 5 patients were operated on with sympathectomy only, Bypass procedures were anatomic bypass in 22 cases: aortoiliac artery bypass in 11 cases, femoropopliteal artery bypass in 10 cases, sequential femoropopliteal artery bypass in one case and extra-anatomic bypass in 8 cases, axillary-bifemoral artery bypass in one case and femorofemoral artery bypass in 7 cases. Postoperative complications which mainly composed of superficial wound infection[5 cases] which were treated without any significant sequel in all cases and thrombosis[2 cases]. Three patients died whose causes of death were acute renal failure in 2 cases and myocardial infarction in other, The overall patency, rate was 70Zo in 5 years. In conclusion, the clinical pattern and operative outcome were similar to he western pattern and all cases of death did not related to operative procedures and ischemic symptoms were relieved by bypass operations except several cases. I think and recommend that all patients suffering chronic arterial insufficiency by atherosclerosis obliterans ought to be managed with urgent and adequate operative procedure.
