  • The capability of design and engineering of global EPC companies has long been equalized through past similar construction experiences. Among various key factors in the success of EPC project, the capability of engineering is considered to be the most important factor since the engineering is preceding activities of EPC contract. The failure of engineering may adversely affect the subsequent procurement & construction activities and in turn may cause cost overrun or schedule delay. Therefore, an EPC company needs to continue to improve the engineering capabilities for the success of project. The engineering capabilities can be further improved if the EPC company should prevent recurrence of similar design faults that were previously committed. This study is intended to present how to make the most of the failure examples from previous projects towards a success of project. Failure is but a stepping stone to success. The EPC company can obtain useful lessons from the analysis of past failure examples.

  • A bendable electronic module has been developed for a mobile application by using a low-cost roll-to-roll manufacturing process. In flexible embedded electronic module, a thin silicon chip was embedded in a polymer-based encapsulating adhesive between flexible copper clad polyimide layers. To confirm reliability and durability of prototype bendable module, the following tests were conducted: Moisture sensitivity level, thermal shock test, high temperature & high humidity storage test, and pressure cooker tester. Those experiments to induce failure of the module due to temperature variations and moisture are the experiment to verify the reliability. Failure criterion was 20% increase in bump resistance from the initial value. The mechanism of the increase of the bump resistance was analyzed by using non-destructive X-ray analysis and scanning acoustic microscopy. During the pressure cooker test (PCT), delamination occurred at the various interfaces of the bendable embedded modules. To investigate the failure mechanism, moisture diffusion analysis was conducted to the pressure cooker's test. The hygroscopic characteristics of the encapsulating polymeric materials were experimentally determined. Analysis results have shown moisture saturation process of flexible module under high temperature/high humidity and high atmosphere conditions. Based on these results, stress factor and failure mechanism/mode of bendable embedded electronic module were obtained.

  • The failure probabilities of the reactor pressure vessel (RPV) for low temperature over-pressurization (LTOP) and cool-down transients are calculated in this study. For the cool-down transient, a pressure-temperature limit curve is generated in accordance with Section XI, Appendix G of the American Society of Mechanical Engineers (ASME) code, from which safety margin factors are deliberately removed for the probabilistic fracture mechanics analysis. Then, sensitivity analyses are conducted to understand the effects of some input parameters. For the LTOP transient, the failure of the RPV mostly occurs during the period of the abrupt pressure rise. For the cool-down transient, the decrease of the fracture toughness with temperature and time plays a main role in RPV failure at the end of the cool-down process. As expected, the failure probability increases with increasing fluence, Cu and Ni contents, and initial reference temperature-nil ductility transition ($RT_{NDT}$). The effect of warm prestressing on the vessel failure probability for LTOP is not significant because most of the failures happen before the stress intensity factor reaches the peak value while its effect reduces the failure probability by more than one order of magnitude for the cool-down transient.

  • Liquefaction-induced lateral spreading has been the most extensive damage to pile foundations during earthquakes. However, a case of pile failure was reported despite the fact that a large margin of safety factor was employed in their design. This means that the current seismic design method of pile is not agreeable with the actual failure mechanism of pile. Newly proposed failure mechanism of pile is a pile failure based on buckling instability. In this study, failure behavior of pile embedded in liquefied soil deposits was analyzed considering lateral spreading and buckling instability performing 1g shaking table test. As a result, it can be concluded that the pile subjected to excessive axial loads ($near\;P_{cr}$) can fail by buckling instability during liquefaction. When lateral spreading took place in sloping grounds, lateral spreading increased lateral deflection of pile and reduced the buckling load, promoting more rapid collapse. In addition, buckling shape of pile was observed. In the ease of pile buckling, hinge formed at the middle of the pile, not at the bottom. And in sloping grounds, location of hinge got loiter compared with level ground because of the effects of lateral spreading.

  • This paper attempts to develop the analytical model of estimating the fatigue damage using a linear elastic fracture mechanics method. The stress history on a welding member, when a truck passed over a bridge, was defined as a block loading and the crack closure theory was used. These theories explain the influence of a load on a structure. This study undertook an analysis of the stress range frequency considering both dead load stress and crack opening stress. A probability method applied to stress range frequency distribution and the probability distribution parameters of it was obtained by Maximum likelihood Method and Determinant. Monte Carlo Simulation which generates a probability variants (stress range) output failure block loadings. The probability distribution of failure block loadings was acquired by Maximum likelihood Method and Determinant. This can calculate the fatigue reliability preventing the fatigue failure of a welding member. The failure block loading divided by the average daily truck traffic is a predictive remaining life by a day. Fatigue reliability analysis was carried out for the welding member of the bottom flange of a cross beam and the vertical stiffener of a steel box bridge by the proposed model. Results showed that the primary factor effecting failure time was crack opening stress. It was important to decide the crack opening stress for using the proposed model. Also according to the 50% reliability and 90%, 99.9% failure times were indicated.

  • This study was conducted to investigate the effect of the factors of delivery app service failure on consumption well conflict emotion and complaining behavior targeting consumers with experience using delivery apps, and the results of the study are summarized as follows. First, when looking at the demographic analysis, the proportion of men was 58.1% and women were 41.9%, and the age group was the highest with 91.9% in their 20s. Second, as a result of testing hypothesis 1 that the factor of delivery app service failure will have a significant influence on consumer conflict emotions, failure to provide app information and failure to deliver/food service showed a significant positive (+) relationship. Third, as a result of the verification of Hypothesis 2, that consumer conflict emotions will have a significant effect on the consumer's continued use intention, it was found to be insignificant and rejected. Lastly, hypothesis 3 that consumer conflict emotions will have a significant effect on consumer complaints behavior is that negative word of mouth behavior, complaint behavior from service providers, and complaint behavior from service managers were adopted, but the complaint behavior was rejected by third parties.

  • We analyze the failure rate change of a conventional bidirectional converter and a modified one which moves an output capacitor towards propulsion battery. We analysis of the circuit structural homogeneity and the difference between both converters, and confirm that the capacitor working voltage is reduced by changing the capacitor position. After obtaining the capacitor failure rate according to voltage stress factor and operating temperature, it is applied to the fault-tree of the bidirectional converter to obtain the overall failure rate of the converter. We analyzes the advantages and disadvantages of design changes by comparing and analyzing the failure rate and mean time between failures (MTBF) according to operating temperature and capacitance value.

  • In comparison with the existing static reliability analysis methods, the dynamic reliability analysis(DyRA) method is more suitable for estimating the failure probability of a structure subjected to earthquake excitations because it can take into account the frequency characteristics and damping capacity of the structure. However, the DyRA is known to have an issue of numerical stability due to the uncertainty in random sampling of the earthquake excitations. In order to solve this numerical stability issue in the DyRA approach, this study proposed two earthquake-scale factors. The first factor is defined as the ratio of the first earthquake excitation over the maximum value of the remaining excitations, and the second factor is defined as the condition number of the matrix consisting of the earthquake excitations. Then, we have performed parametric studies of two factors on numerical stability of the DyRA method. In illustrative example, it was clearly confirmed that the two factors can be used to verify the numerical stability of the proposed DyRA method. However, there exists a difference between the two factors. The first factor showed some overlapping region between the stable results and the unstable results so that it requires some additional reliability analysis to guarantee the stability of the DyRA method. On the contrary, the second factor clearly distinguished the stable and unstable results of the DyRA method without any overlapping region. Therefore, the second factor can be said to be better than the first factor as the criterion to determine whether or not the proposed DyRA method guarantees its numerical stability. In addition, the accuracy of the numerical analysis results of the proposed DyRA has been verified in comparison with those of the existing first-order reliability method(FORM), Monte Carlo simulation(MCS) method and subset simulation method(SSM). The comparative results confirmed that the proposed DyRA method can provide accurate and reliable estimation of the structural failure probability while maintaining the superior numerical efficiency over the existing methods.
