  • A flare from an Air Force fighter was abnormally dispensed during the landing. To determine the cause of the abnormal dispensing, fracture analysis, signal analysis and reproduction experiments based on physics of failures were performed. The primary cause of the failure was analyzed to be due to an intermittent fault of an internal circuit card in the AN/ALE-40 chaff/flare dispenser by a broken lid of a capacitor, and the root cause which had derived the primary cause was considered to be an improper handling during the domestic maintenance which were changed from the overseas maintenance due to the DMSMS problem. Therefore, the overall process of the maintenance capability development system was reviewed and alternative ways that considers maintenance error decision aid(MEDA) for system improvement were suggested to prevent further failures.

  • This paper shows an example of the Design Review and Common-Cause Failure (CCF) Modeling of mechanical Parts. Reliability should be continuously monitored during the entire period of design. Design Review is the procedure to improve the reliability for the product. We proposed the reliability assessment and design review method. CCF Model is the general dependent model considering the failure mode effects several component simultaneously. This study considers the computation of the network with dependent components. It is important that CCF model is applied for mechanical pars.

  • In modern plant lndustry, dignosis system is an essential implement because a human operator cannot check the state of system all the time. The recent facility needs a computer system which is able to replace and extense the function of the human expert. Checking the state of the plant system, the computer system uses signals form sensors attached to the plant systems. But, It is difficult to predict the cause of the failure from the sensing signals. Because the relationship among the signals cannot be easily represented by mathematical models. So expert system based on a fuzzy rule and Neural network method is sugguested. Expert system decide whether aa state of the system is ordinary of failure by the evaluation of the signals. If the state of the system is unstable, expert system preprocess the signals. When fault is occurred in the machine, the expert system dignoses the state of the system and find the cause as a primary tool. If the expert system dose not find the adequate cause successfully, neural network system uses the preprocessed signals as an input and propose a cause of the failure.

  • The author fractographically analyized the cause of metal failure(the first time this procedure has been used for this metal failure)and also analyized it clinically. In this study, I selected eight cases which have been analyized fractographically. In all these cases, the analysis was done after treatment of metal failure of implants internally fixed to femur shaft fractures at the Department of Orthopedic Surgery, Yeung-Nam University Hospital during the six year period from May 1983 to September 1989. 1. Metal failure occured in five dynamic-compression plates, one Jewett nail, one screw in Rowe plate, and one interlocking nail. 2. The clinical cause of metal failure was deficiency of medial butress in five cases, incorrect position of implant in one case, and incorrect selection of implant in two cases. 3. The time interval between internal fixation and metal failure was four months in one case, between five months to twelve months in six cases, three years in one case. 4. The fractographically analytical cause of metal failure was ; first, impact failure, one case, second, fatigue failure, six cases, machining mark(stress liser), four cases type : low consistent cyclic fatigue failure irregular cyclic fatigue failure third, stress corrosion crack, one case. 5. 316L Stainless Steel has good resistance to corrosion. However, when its peculiar surface film is destroyed by fretting, it shows pitting corrosion. This is, perhaps, the main cause of metal failure. 6. It is possible that mechanical injury occured in implants during the manufacturing of implants or that making a screw hole is the main cause of metal failure.

A Study of Life about Naturally Aged Nitrocellulose by Storage (자연 노화된 니트로셀룰로오스의 수명에 관한 연구)

  • During the safety inspection of nitrocellulose-made explosive containers stored for more than 10 years, cracks were found in the containers. Therefore, a failure cause analysis test was performed. First, the cause of failure through the failure tree analysis was conducted to select the factors that influenced failure. The changes in the properties of the container caused by the acceleration of the reaction were found to be the cause of the failure by confirming the influence on the environment and internal/external factors that may occur during storage. To confirm this, environmental tests, such as thermal shock test and vacuum thermal stability test, were performed using a naturally aged container to analyze the cause of failure, and an accelerated aging test was performed to reproduce the failure. Through this, the chemical reaction was accelerated by heat and charge, as in the result of the fault tree analysis, and it was confirmed that the physical properties of the container were changed. In addition, the service life of the container was estimated using the Arrhenius model for the storage life due to thermal aging.

Structural behavior of cable-stayed bridges after cable failure

  • This paper investigates the change of structural characteristics of steel cable-stayed bridges after cable failure. Cables, considered as the intermediate supports of cable-stayed bridges, can break or fail for several reasons, such as fire, direct vehicle clash accident, extreme weather conditions, and fatigue of cable or anchorage. Also, the replacement of cables can cause temporary disconnection. Because of the structural characteristics with various geometric nonlinearities of cable-stayed bridges, cable failure may cause significant change to the structural state and ultimate behavior. Until now, the characteristics of structural behavior after cable failure have rarely been studied. In this study, rational cable failure analysis is suggested to trace the new equilibrium with structural configuration after the cable failure. Also, the sequence of ultimate analysis for the structure that suffers cable failure is suggested, to study the change of ultimate behavior and load carrying capacity under specific live load conditions. Using these analysis methods, the statical behavior after individual cable failure is studied based on the change of structural configuration, and distribution of internal forces. Also, the change of the ultimate behavior and load carrying capacity under specific live load conditions is investigated, using the proposed analysis method. According to the study, significant change of the statical behavior and ultimate capacity occurs although just one cable fails.

  • In general, capacitors have a large influence on the life of the system due to frequent charging and discharging. In this paper, we analyze the cause of the core failure of high voltage, high current HVDC sub-module film capacitor and analyze the precautions of the capacitor design and manufacturing process. First, the cause of the fault, the failure mode, and the effect are analyzed through the FMEA of the capacitor. To quantitatively evaluate the causes and effects of faults that have the greatest effect on the failure of a capacitor, a fault tree for the capacitor is presented and the failure rate is analyzed according to the design parameters and the driving conditions. It is verified that the main cause of capacitor failure is the capacitance change, and it is necessary to minimize the temperature rise, corona occurrence, electrode expansion, and insulation distance decrease during capacitor design and manufacturing process in order to reduce the failure rate of the capacitor.

Diencephalic syndrome: a frequently neglected cause of failure to thrive in infants

  • Purpose: Diencephalic syndrome is an uncommon cause of failure to thrive in early childhood that is associated with central nervous system neoplasms in the hypothalamic-optic chiasmatic region. It is characterized by complex signs and symptoms related to hypothalamic dysfunction; such nonspecific clinical features may delay diagnosis of the brain tumor. In this study, we analyzed a series of cases in order to define characteristic features of diencephalic syndrome. Methods: We performed a retrospective study of 8 patients with diencephalic syndrome (age, 5-38 months). All cases had presented to Seoul National University Children's Hospital between 1995 and 2013, with the chief complaint of poor weight gain. Results: Diencephalic syndrome with central nervous system (CNS) neoplasm was identified in 8 patients. The mean age at which symptoms were noted was $18{\pm}10.5$ months, and diagnosis after symptom onset was made at the mean age of $11{\pm}9.7$ months. The mean z score was $-3.15{\pm}1.14$ for weight, $-0.12{\pm}1.05$ for height, $1.01{\pm}1.58$ for head circumference, and $-1.76{\pm}1.97$ for weight-for-height. Clinical features included failure to thrive (n=8), hydrocephalus (n=5), recurrent vomiting (n=5), strabismus (n=2), developmental delay (n=2), hyperactivity (n=1), nystagmus (n=1), and diarrhea (n=1). On follow-up evaluation, 3 patients showed improvement and remained in stable remission, 2 patients were still receiving chemotherapy, and 3 patients were discharged for palliative care. Conclusion: Diencephalic syndrome is a rare cause of failure to thrive, and diagnosis is frequently delayed. Thus, it is important to consider the possibility of a CNS neoplasm as a cause of failure to thrive and to ensure early diagnosis.

A classification of electrical component failures and their human error types in South Korean NPPs during last 10 years

  • The international nuclear industry has undergone a lot of changes since the Fukushima, Chernobyl and TMI nuclear power plant accidents. However, there are still large and small component deficiencies at nuclear power plants in the world. There are many causes of electrical equipment defects. There are also factors that cause component failures due to human errors. This paper analyzed the root causes of failure and types of human error in 300 cases of electrical component failures. We analyzed the operating experience of electrical components by methods of root causes in K-HPES (Korean-version of Human Performance Enhancement System) and by methods of human error types in HuRAM+ (Human error-Related event root cause Analysis Method Plus). As a result of analysis, the most electrical component failures appeared as circuit breakers and emergency generators. The major causes of failure showed deterioration and contact failure of electrical components by human error of operations management. The causes of direct failure were due to aged components. Types of human error affecting the causes of electrical equipment failure are as follows. The human error type group I showed that errors of commission (EOC) were 97%, the human error type group II showed that slip/lapse errors were 74%, and the human error type group III showed that latent errors were 95%. This paper is meaningful in that we have approached the causes of electrical equipment failures from a comprehensive human error perspective and found a countermeasure against the root cause. This study will help human performance enhancement in nuclear power plants. However, this paper has done a lot of research on improving human performance in the maintenance field rather than in the design and construction stages. In the future, continuous research on types of human error and prevention measures in the design and construction sector will be required.
