Toothache Caused by Sialolithiasis of the Submandibular Gland

  • Sialolithiasis is the most frequent disease of the salivary glands, causing swelling and/or pain of the affected site. We report a 44-year-old woman who presented with severe pain in the lower left second molar region without swelling. Sialoliths on her left submandibular gland were confirmed by radiographic examinations. After robot-assisted sialoadenectomy, the pain did not recur but remained facial paralysis and unaesthetic scar.

  • This thesis is written about the causes, the processes and the treatments of edema. It consists of 31 chapters. Covering 11 chapters, there are prescriptions and constructions. When one of the organs fails to work metabolizing water, you can have edema. It is the result of the accumulation of excess fluid under the skin. Edema most commonly occurs in the head facial, the palpebral portion, the feet and legs or all the body. Sometimes it is accompanied with ascites. There are three classifications in the thesis. First, according to the cause and the symptom of disease, it is divided into 5 types. Type 1. The swelling caused by the disorder of the lungs from the troubling wind. Type 2. The swelling caused by the weak spleen. Type 3. The swelling caused by abnormal condition between the kidneys and the lungs. Type 4. The swelling in abdomen caused by the weak kidneys. Type 5. The swelling (Yellow sweat) caused by the heat inside body as water outside. Second, the swelling and the abnormal symptom of five viscera. Third, the swelling accompanied with menstruation in female and with chilliness. In short, the original textbook is showed that the treatment of edema focuses on the cause and the region of fluid accumulation. When the swelling is in the upper body or the cause of diseaseis outside, the useless humidity can be expelled through sweating. On the other hand, when the swelling is in the lower body or the cause of disease is inside, it is pushed out by urination. In this book, the author presents further treatments. And these treatments are often cited by doctors after ages. Even one of them suggested that the swelling be divided by cosmic dual forces. In spite of many researches, it is too complicate for us to understand the writer’s intend. For that reason, comparing with other medical books and referring to guides explained by doctors, I have studied the textbook to help understand.

  • This article discusses the diagnosis, anatomic consideration, and surgical management of masseter muscle hypertrophy. Surgical correction is advised for patients who have esthetic complaints. Esthetic improvement can be achieved by surgery and not by conservative treatment. Recently, the intraoral method, which leaves no scar on the face and minimizes the possibility of injury to the marginal branch of the facial nerve, has been supported by many surgeons. Patients who complained of marked swelling of unilateral or bilateral mandibular angle area and showed abnormal bony growth at the mandibular angle area and enlarged masseter muscle received mandibular angle shaving and excision of the inner layer of masseter muscle with intraoral approach. After operation, physiotherapy was done with EAST(eletrical acupuncture stimulation therapy) for encouraging the mouth opening and reducing the swelling. They showed early maximum mouth opening and reduction of swelling.

Treatment of Nasal Bone Fracture using Wire Fixation in a Horse

  • A 7-year-old, 453 kg, thoroughbred-cross mare showed depression, severe nasal bleeding, diffuse swelling, and distortion of the facial contour. Physical and radiographic examinations revealed an open nasal bone fracture. Surgery was conducted under sedation with the horse in a standing position. Two holes were drilled directly above and below the fracture line. The nasal bone was raised to the normal position by inserting a periosteal elevator through the holes. Orthopedic wire was used to secure the fractured nasal bones through the holes. The horse recovered from sedation without complication. Sutures were removed three weeks after surgery. The nasal bone fracture appeared well-recovered morphologically and the recovery was confirmed by radiography. A wire fixation method is recommended to treat nasal bone fracture in a horse.

Masticator Space Tumor Mimicking Temporomandibular Disorder Presenting Facial Swelling and Trismus: A Case Report

  • Temporomandibular disorders (TMD), involving the masticator system of temporomandibular joint (TMJ) and masticator muscle, can be characterized with the cardinal signs and symptoms of jaw pain, noises and limitation of mandibular range of motion. However, TMD requires differential diagnosis due to its heterogeneous characteristics with various causes despite the similar clinical profiles. Oral cancer involving TMJ and the masticator system, although infrequent, can be one of these causes and should be considered one of the most life-threatening disease mimicking TMDs. This report introduces a case of masticator space tumor originally diagnosed as TMD in a 73-year-old Korean female with previous history of brain tumor. The clinical signs and symptoms closely mimic that of TMD which may have disrupted differential diagnosis. We discuss here key points for suspecting TMDs of secondary origin, namely, that of cancer and the implications it has on dental clinicians.

안면부 골절에 사용된 흡수성 보형물의 지연성 이물반응: 증례보고 (Delayed Foreign Body Reaction around the Absorbable Implants in Facial Bone Fracture: A Case Report)

  • Purpose: Absorbable implants are frequently used to fix facial bone fractures, because they are radiolucent and compatible with magnetic resonance imaging (MRI). Despite their increasing usage, however, there have been few reports about their long-term side-effects. In this paper, a case in which absorbable implants led to a foreign body reaction 17 months after their insertion is presented. Methods: A previously healthy 19-year-old male fell from a flight of stairs and visited the authors' hospital with right periorbital pain. Zygomaticomaxillary fracture involving right orbital floor was detected via a facial bone computed tomography (CT). Internal fixation with absorbable implants was followed by open reduction. 17 months after the surgery, the patient complained of localized periorbital swelling. Removal of the granulomatous lesion including the absorbable implants along with their biopsy and culture was performed. Results: The granulomatous lesion around the implants was firm and extended into the maxillary sinus. The histologic finding showed a microabscess with a foreign body reaction. Methicillin-sensitive Staphylococcus aureus growth was confirmed in the culture. No definite abnormal symptoms ensued after the complete removal. Conclusion: A microabscess-forming granulomatous lesion around the absorbable implant can cause delayed symptomatic foreign body reaction despite its rare occurrence. Complete removal of the lesion including implants is expected to have a successful outcome if it is encountered.

안면골 골절로 인한 시신경 손상 (OPTIC NERVE INJURY DUE TO FACIAL FRACTURES)

  • Optic nerve injury serious enough to result in blindness had been reported to occur in 3% of facial fractures. When blindness is immediate and complete, the prognosis for even partial recovery is poor. Progressive or incomplete visual loss may be ameliorated either by large dosage of steroid or by emergency optic nerve decompression, depending on the mechanism of injury, the degree of trauma to the optic canal, and the period of time that elapses between injury and medical intervention. We often miss initial assessment of visual function in management of facial fracture patients due to loss of consciousness, periorbital swelling and emergency situations. Delayed treatment of injuried optic nerve cause permanent blindness due to irreversible change of optic nerve. But by treating posttraumatic optic nerve injuries aggressively, usable vision can preserved in a number of patients. The following report concerns three who suffered visual loss due to optic nerve injury with no improvement after steroid therapy and/or optic nerve decompression surgery.

Subcutaneous emphysema after uncommon traumatic and iatrogenic events: a report of two cases

  • Cervicofacial subcutaneous emphysema is defined as the abnormal introduction of air into the subcutaneous tissues of the head and neck. It is mainly iatrogenic and traumatic in origin. Our two case reports are also due to the same cause, but the features of the trauma and the site of the dental treatment are different from the existing reports. A 29-year-old man visited our hospital with facial swelling and pain after experiencing facial trauma in a soccer game. Another 55-year-old woman visited with similar symptoms after replacement of her maxillary anterior fixed prosthesis. In the two cases presented, subcutaneous emphysema was gradually treated with no complications during antibiotic prophylaxis and supportive care. In this paper, we report two cases of traumatic and iatrogenic subcutaneous emphysema and their diagnoses, etiologies, complications, and treatments based on a literature review.

  • The authors have observed a case of eosinophilic granuloma which occurred in 10 years old girl 1.The lesion which occurred swelling and facial disfigurement , was located in the right side of ramus mandible, but toothache and loosening of tooth was absent. 2. Roentgenograph showed bean-sized monocystic radiolucent shadow in mandible border.3. Microscopically,few hemorrhagic araas were existed, and numerous eosinophilic leukocytes eosinophilic myelocyteswith inflammatory cells were infiltrated.
