MORPHOLOGIC STUDY FOR SAGITTAL SPLIT RAMUS OSTEOTOMY USING 3-D IMAGE IN MANDIBULAR PROGNATHISM (하악전돌자에서 3차원영상을 이용한 하악지시상분할골절단술과 관련된 하악골의 해부학적 연구)

  • Sagittal split ramus osteotomy(SSRO) has been commonly performed in the mandibular prognathism. The previous studies of the mandibular anatomy for SSRO have mostly been used in dry skull without consideration of age, sex or jaw relationship of patients. This study was performed to evaluate the location of mandibular canal and the anatomy of ramus, such as the location of mandibular lingula and the ramal bone marrow, which were associated with SSRO procedures, in the patients with mandibular prognathism and normal young adults by using computerized tomographs(CT) and 3D images. The young adults at their twenties, who were considered to complete their skeletal growth, and seen in the Department of Orthodontics and Oral and Maxillofacial Surgery in Chonnam National University Hospital between March 2000 and May 2003, were selected. This study was performed in 30 patients (15men, 15women) who were diagnosed as skeletal class I normal relationship, and another 30 patients (15men, 15women) who were diagnosed as skeletal class III relationship upon clinical examination and lateral cephalometric radiographs. The patients were divided into 2 groups : Class I group, the patients who had skeletal class Ⅰ normal relationship(n=30, 15men, 15women), and Class III group, the patients who had skeletal class III relationship(n=30, 15men, 15women). Facial CT was taken in all patients, and pure 3D mandibular model was constructed by V-works version 4.0. The occlusal plane was designed by three points, such as the mesiobuccal cusp of both mandibular 1st molar and the incisal edge of the right mandibular central incisor, and used as a reference plane. Distances between the tip of mandibular lingula and the occlusal plane, the sigmoid notch, the anterior and the posterior borders of ramus were measured. The height of ramal bone marrow from the occlusal plane and the distance between mid-point of mandibular canal and the buccal or lingual cortex of the mandible in the 1st and 2nd molars were measured by V-works version 4.0. Distance(Li-OP) between the occlusal plane and the tip of mandibular lingula of Class III Group was longer than that of Class I Group in men(p<0.01), but there was no significant difference in women between both groups. Distance(Li-SN) between the sigmoid notch and the tip of mandibular ligula of Class III group was longer than that of Class I Group in men(p<0.05), but there was no significant difference in women between both groups. Distance(Li-RA) between the anterior border of ramus and the tip of mandibular lingula of Class III Group was shorter than that of Class I Group in men and women(p<0.01). Distance(Li-RP) between the posterior border of ramus and the tip of mandibular lingula of Class III Group was slightly shorter than that of Class I Group in men(p<0.05), but there was no significant difference in women between both groups. Distance(RA-RP) between the anterior and the posterior borders of ramus of Class III Group was shorter than that of Class I Group in men and women(p<0.01). Longer the distance(SN-AN) between the sigmoid notch and the antegonial notch was, longer the vertical ramal length above occlusal plane, higher the location of mandibular lingula, and shorter the antero-posterior ramal length were observed(p<0.01). Height of ramal bone marrow of Class III Group was higher than that of Class I Group in men and women(p<0.01). Distance between mandibular canal and buccal cortex of Class III Group in 1st and 2nd lower molars was shorter than that of Class I Group in men and women (p<0.05 in 1st lower molar in men, p<0.01 in others). These results indicate that there are some anatomical differences between the normal occlusal patients and the mandibular prognathic patients, such as the anterior-posterior length of ramus, the height of ramal bone marrow, and the location of mandibular canal.

A comparative study on head posture and craniofacial morphology between koreans and scandinavian caucasians (한국인과 스칸디나비아계 백인의 두부자세와 두개안면구조의 형태에 관한 비교연구)

  • The purpose of this study was to evaluate racial differences In head posture and the influence of head posture to the craniofacial morphology. The sample group of this study was made up of 51 Korean males and 120 Scandinavian Caucasian males. From the comparison of the cranio-cervical angle and the variables of craniofacial morphology between them, the following results were obtained. 1. The cranio-cervical angle (NSL/OPT) was on average 9.28 degrees larger In Koreans. 2. The length of the anterior cranial base (N-S) was on average 4.66mm shorter in Koreans. 3. The length of the maxillary base (sp-pm and ss-pm) were on average 2.75mm and 4.65mm shorter in Koreans respectively, the anterior maxillary height (n-sp) was on average 2.60mm longer, the posterior dimension (s-pm) was found to be 2.06mm longer in Koreans, and the maxillary inclination (NSL/NL) was identical in both samples. 4. The mandibular body length (pg-tgo) and ramus height (ar-tgo) were identical in the two groups, but the genial angle (ML/RL) was 3.22 degrees smaller and the mandibular plane inclination (NSL/ML) was 2.44 degrees larger in Koreans 5. The maxillary prognathism (s-n-sp and s-n-ss) and the mandibular prognathism (s-n-sm) were identical in both samples. 6. The sagittal jaw relationship (ss-n-pg) was 1.44 degrees larger in the Korean sample, but the vortical jaw relationship (NL/ML) was not significantly different. 7. The anterior facial height (n-gn) was 5.57mm longer in the Korean sample. 8. The mandibular alveolar prognathism (CL/ML) was 5.71 degrees greater and the interincisal angle (ILs/ILi) was 3.08 degrees more acute in Koreans. Taken together these results, craniofacial morphology can be influenced by the head posture defined by cranio-cervical angulation.

  • Orthodontic patients are individuals that grow and develop ; therefore selection of the proper time for orthodontic treatment is considered to be one of most difficult and yet difficult factor. Since the development of cephalometric X-ray, amount and Pattern of craniofacial growth change with aging could be predicted and be came useful in the process of orthodontic treatment. The relationship between the mean values of cephalometric measurements and body height and weight was studied among the groups(boys and girls) of Korean children from the ages 6-years to 17-years. 409 boys and 437 girls with no abnormality in growth and development and no history of orthodontic treatment from the ages of 6 years to 17 years were chosen as subjects Cephaloment X-ray were taken for 3 years and hard tissue analysis based on Burstone's COGS, which was devided into measurements of 6 parts(Cranial base, Maxillar and Mandible, Dental measurements). The relationship between craniofacial growth and height & weight was studied. The following conclusions were obtained : 1. The maximum growth in the measurements of cranial base, N-Ar(FH), N-Ba(FH) corresponded with the age with the maximum increase in body height & weight in both boys and girls. 2. Genial angle gradually decreased with aging in both boys and girls. 3. N-ANS(L) showed greater amount of growth than ANS-Ne(L), and this had greater influence on facial profile. 4. N-A-$Pog^{\circ}$ decreased with aging, and mandibular growth exceeded maxillary growth in amount and rate. 5. Length of Y-axis Increased, but Y-axis to FH plane remained constant. This show that mandible grows at a constant angulation to cranial base. 6. As permanent teeth erupt, interincisal angle deceased.

  • The clinical application of constitutional Diagnosis is the most important part of Sasang constitutional medicine. It has been studied in various way. The study of morphologic characteristics on the head and face has been identified but didn't considered the Variations of age and sex. For the statistic analysis of the correlation between the sasang constitution and the shape of the face, the head-facial part of 182 cases(the group of throughout the age) and 69 cases(the group of age 30 to 40) were measured by Martin's measurement and analysis of a) the measurement value of height and the component ratio from the Gnathion to each part of face by constitution. b) the measurement value of depth and the component ratio from T-projected to each part of the face by constitution. c) the measurement value of breadth and component ratio between each parts of the facial breadth by constitution. d) the characteristics on each part of face by constitution. e) the result of discriminant analysis about the constitution Authors obtained the results from the study as follows: 1. Taeum-In group is characterized by the value of variables had a tendency to maximum value in throughout the age, the charateristics that the total group show is the shape of face is wide shape in horizontally and flat, the nasal breadth is wider than other constitutions, the lips is narrower than Soyang-In in horizontally and thick shape in vertically, and the biogonial breadth is more developed than other constitutions, so lower face is developed. The charateristics that only the age of 30 40 group show is that the lips is thicker than Soeum-IN. The projection of inter-eyebrows is more projected than Soyang-IN. 2. Soeum-IN group is characterized by the value of variables had a tendency to minimum value in throughout the age. The characteristics that the total group show is the lips is thiner than other constitutions, the breadth of eyes is wider than other constitutions, the difference between sellion and nasal breadth is to be little. The charateristics that only the age of 30 40 group show is that the ratio of upper face in physiognomic face is lager than Soyang-IN. 3. The total age group of Soyang-In is characterized by the shape of face is long in vertically and narrow in horizontally, the total eyebrow breadth and lips and philtrun is wider than other constitutions. The charateristics that only the age of 30 40 group is characterized by the projection of sellion is more projected than Taeum-IN. 4. The values which showed significance in both age group V76, V52, V54, V55, V57, V59, V64, V65, V67, V88, V89, V148, V150, V151, V155, V160, V161, V28, V50, V99, V102, V167, V169, V173, V175, V177, V181 are 27 in all. 5. The values which was significant in the age 30 to 40 group V77, V78, V79, V109, V140, V142, V143, V166, V174 are 9 in all.

A Study on the Skeletal and Profile Change after Using the Activator in Class II Malocclusion (II급 부정 교합자의 Activator 치료 후 골격 및 안모 변화에 관한 연구)

  • To establish the diagnosis and treatment plan for skeletal Class II malocclusion, patient's skeletal morphology, prognosis as well as the treatment effect is one of the important factor to consider. Therefore, the present study classified analyzed the difference between initial(T1) and after use of activator(T2), and after finish of direct multi-bonding system treatment(T3) for Class II malocclusion during growth period according to the treatment result(effective body length) and morphology of vertical skeletal type. The experimental group was classified into two groups(1 group, 2 group) according to the effective body length change between before and after use of activator, showed good treatment effect of activator for patient with small mandible and large differential between maxilla and mandible, and short anterior facial height. And the difference between 1 and 2 group in the experimental group before treatment(T1) disappeared in the finished treatment(T3). But in contrast, the initial difference of T1 stage between a and b group in the control group did not disappear in the finished treatment(T3). In short, experimental group's treatment effect was much better than contrast group and the treatment effect was maintained and got stable results at comparison experimental group with contrast group. Through this study, we can find activator's treatment effect and stable retention of that in growing Class II malocclusion patients. By estimate of activator treatment effect through these results, we can establish the correct diagnosis and treatment plan for adolescent Class II malocclusion estimate of activator treatment effect and lead the ideal facial growth pattern.

  • It is well recognized that a cross-bite tends to occur more frequently among Japanese, Chinese and Koreans more than among Europeans. A variety of functional appliances have already invented for the treatment of cross-bites. One of them is the FR III(by Rolf $Fr\ddot{a}nkel$) which applies a quite different philosophy based on the using vestibular region compared with other methods which utilize muscular forces applied directly on the teeth. The FR III also increases both the lack of muscular tension against the dental arch and the dentition and muscular forces which can then influence the teeth indirectly. This mechanism can achieve favorable developments with the basal bone, teeth, and alveolar bone. After using FR III to the anterior cross-bite patient with the eruption of a maxillary incisor the results were as follows: 1. Forward growth of maxilla and proclination of the upper incisor 2. Downward and backward rotation of the mandible 3. Increase the facial height 4. The case which was normal mandible, underdevelopmental maxilla, deeper overbite and more nagative overjet exhibited good prognosis

  • Class III malocclusion or anterior crossbite is commonly seen in Asian. This problem is easily recognized by dentists and parents. During the primary dentition period, anterior crossbite with functional shift and deep overbite could develop to skeletal protrusive mandible. So, early and proper diagnosis of anterior crossbite which needs prompt treatment is important. These cases showed the early management of crossbite with functional shift in primary dentition using intraoral removable appliance resulting in improvement of intermaxillary relationship. And I analyzed the positional change and the dimensional change during treatment with lateral cephalometric x-ray analysis. Our patients showed vertical dimensional change of lower anterior facial height and clockwise rotation which results crossbite correction in 1 year of treatment period.

Comparison of cephalometric norms between Mongolian and Korean adults with normal occlusions and well-balanced profiles (조화로운 측모를 가진 몽골인과 한국인 성인 정상교합자의 측모두부계측방사선학적 비교 연구)

  • Objective: The purposes of this study were to establish cephalometric norms of Mongolian adults, which have not previously been reported in orthodontic journals, and to compare them with cephalometric norms of Korean adults. Methods: Lateral cephalometric radiographs of 74 Mongolian adults (35 men, 39 women) and 95 Korean adults (52 men, 43 women) with normal occlusions and well-balanced lateral profiles were obtained. The subjects were chosen by orthodontists of the same ethnic background. Forty craniofacial variables were measured, and groups were compared by analysis of covariance. Results: The cephalometric norms for male and female Mongolian and Korean adults with normal occlusions and well-balanced lateral profiles were established. Sexual dimorphism was found in linear skeletal measurements and vertical skeletal relationships. Compared with Korean adults, Mongolian adults had shorter anterior facial height, more prominent chins, and more upright upper incisors. There were few differences in soft tissue measurements. The differences were statistically significant, but their clinical importance may be limited. Conclusions: Cephalometric differences can be considered, but little difference may exist in the orthodontic diagnosis and treatment planning of Mongolian and Korean adults.

A cephalometric study on the relationship between mandibular opening movement and morphology of craniofacial skeleton (아동의 개구운동과 두개안면골격형태의 상관성 -측모두부방사선 계측법적 연구-)

  • Lateral cephalometric X-ray films in maximal intercuspation and maximal opening of 68 children were taken and analyzed to examine the pattern of condylar movement and to study the relationship between opening movement and morphologic factors of craniofacial skeleton. The results were as follows : 1. The mean value of maximal opening capacity was 47.1mm, condylar moving distance was 18.1mm, horizontal condyle movement was 17.5mm, vertical condyle movement was 3.8mm and condylar moving angle was $13.1^{\circ}$. 2. The maximal opening capacity had positive relationship with the length of anterior cranial base, mandible and maxillary complex and with posterior facial height and had negative relationship with articular angle, sagittal jaw relationship. 3. Vertical condyle movement and condylar moving angle had positive relationship with articular angle and had negative relationship with gonial angle. 4. Horizontal condyle movement and condylar moving distance had positive relationship with the length of maxillary complex.

  • To enhance the esthetic appearance, the maxillary anterior area is important. It is possible to improve the esthetic appearance through the treatment of maxillary anterior area, which includes altering the color, form, and arrangement of teeth. When planning these treatments, clinicians should individualize personal demands, by using the information obtained from facial, dento-labial, dental, and gingival analysis. It is essential to properly prepare the gingival structure, which includes the height of gingival margin, the location of zenith, reconstruction of the interdental papillae, emergence profile, and symmetry. Clinicians often face unfavorable condition of the gingiva and the edentulous ridge, and appropriate management of the gingival structure is needed. In this case report, the patients were treated to improve the gingival conditions surrounding maxillary anterior teeth. By using conservative treatment without surgical intervention, such as application of pink porcelain, subgingival contour modelling and modification of pontic base, satisfactory esthetic results were gained.
