말초성 안면마비 후유증 환자에서 침 치료와 바깥목동맥으로의 분지 영역에 시행한 고주파 병행 치료의 안면부종 감소 효과: 증례보고 (Effect of Acupuncture and High Frequency Therapy Applied to the Region Branching to the External Carotid Artery on Reduction of Facial Edema in Patients with Sequelae of Peripheral Facial Palsy: A Case Report)

  • This study was performed to evaluate the effect of high frequency therapy applied to the region branching to the external carotid artery for peripheral facial paralysis sequelae. A patient suffering with facial edema due to facial paralysis sequelae had been treated with acupuncture, high frequency therapy on the branch area to the external carotid artery for 7 weeks. The evaluation of clinical outcome was done by degree of swelling by measuring the distance of the face and skin temperature of face through digital infrared thermographic imaging. After treatment, the patient's degree of swelling and the temperature difference between the affected side and normal side was decreased. In addition, the temperature was changed in the entire facial area as well as the treatment point of high frequency therapy. This result shows that acupuncture combined with high frequency therapy at the region branching to the external carotid artery could be an effective way to improve facial blood flow, although further clinical studies will be needed.

상 경부 대상포진에 병발한 안면신경 마비 -증례 보고- (Facial Palsy Accompanied with Herpes Zoster on the Cervical Dermatome -A case report-)

  • We treated a 56 year old male ailing of painful herpetic eruption on his 2nd, 3rd and 4th left cervical spinal segment. On the 18th day, patient also suffered an abrupt left facial palsy, accompanied with ongoing postherpetic neuralgia even though the skin eruption had been cured. This patient visited our pain clinic on his 46th day of illness and was teated with continuous cervical epidural block for 9 days, and stellate ganglion block plus oral analgesics and antidepressant for 12 days. The combination of treatments resulted in marked improvement of facial palsy and postherpetic neuralgia. A possible explanation of facial palsy accompanied with herpes zoster on cervical spinal segment could be related to Hunt's hypothesis that geniculate ganglion forms a chain connecting the high cervical ganglion below. Another possibility may be related to a compression injury of the facial nerve by long-term severe edema on the soft tissue of the face, the periauricular area and parotid gland around the facial nerve, and edema on the facial nerve itself emerging out from the cranium.

Effect of a vertical incision on postoperative swelling after an impacted mandibular third molar extraction: two cases with split-mouth designed magnetic resonance imaging analysis

  • This study examined the effects of a vertical incision on postoperative edema after third molar extraction. The study design was that of a comparative split-mouth approach. Evaluation was performed via magnetic resonance imaging (MRI). Two patients with homogeneous bilateral impacted mandibular third molars were enrolled. These patients underwent facial MRI within 24 hours after simultaneous extraction surgery. Modified triangular flap and enveloped flap incisions were made. Postoperative edema was evaluated by MRI and was assessed according to anatomical space. The two pairs of homogeneous extractions demonstrated that vertical incisions were associated qualitatively and quantitatively with extensive postoperative edema. The edema associated with these incisions spread toward the buccal space, beyond the buccinator muscle. In conclusion, a vertical incision with mandibular third molar extraction was related to edema in the buccal space and the fascial space, which contributed to clinical facial swelling.

Lymphaticovenular anastomosis for Morbihan disease: a case report

  • Morbihan disease (MD) is a very rare condition characterized by rosaceous or erythematous lymphedema on the upper two-thirds of the face. A definitive management strategy for MD is lacking, and treatment is challenging. Herein, we present a case of persistent bilateral eyelid edema treated by lymphaticovenular anastomosis (LVA) and lymph node-vein bypass surgery. The patient experienced persistent bilateral eyelid edema. Indocyanine green lymphography was performed, and the diagnosis of bilateral facial lymphedema was confirmed. On the right side, a preauricular lymphatic vessel was anastomosed to a vein. On the left side, lymphostomy on the preauricular lymph node was done, with anastomosis to the transected proximal end of the concomitant vein of the transverse facial artery. Furthermore, a preauricular lymphatic vessel was anastomosed to a vein. Eyelid edema decreased and progressively improved on both sides. The outcome of this case suggests that LVA and lymph node-vein bypass surgery are appropriate for treating persistent eyelid edema related to MD.

Efficacy of local hyaluronidase administration in guided bone regeneration surgery: a randomized controlled trial

  • Objectives: Hyaluronoglucosaminidase (hyaluronidase) increases the local intercellular permeability of the peripheral lymphatic channel and capillaries, which may help reduce edema. In the present study, the effects of hyaluronidase on postoperative edema and pain reduction were evaluated. Materials and Methods: The study included 38 patients who underwent guided bone regeneration (GBR) surgery before implantation. Patients were randomly assigned to either the control group (n=20) or the test group (n=18). Hyaluronidase was injected into the GBR site of subjects in the test group. Postoperative edema was evaluated by measuring the distance between specific facial landmarks immediately after surgery (T1) and 2-4 days after surgery (T2). The degree of pain at T2 and at 10-14 days after surgery (T3) was assessed. Results: In the test group, the degree of swelling was lower than in the control group, however, only two measurements, from the tragus to the mouth corner and from the outer canthus to the mouth corner, showed statistically significant differences (P=0.012 and P=0.001, respectively). The anti-edema effect of hyaluronidase was more effective in the maxilla than in the mandible. In the maxilla, the percentage of facial swelling was significant for three measurements. However, in the mandible, the percentage of facial swelling was significant for only one measurement. Low levels of pain that were similar at T2 and T3 were reported in both groups. Conclusion: The results indicate the degree of swelling was lower in the test group and hyaluronidase appeared to be more effective in the maxilla. The degree of pain reduction was similar between groups. Further in vivo and randomized controlled trials with larger sample sizes are warranted.

성형 수술 후 부종에서 호박탕(琥珀湯)의 효과 : 2 증례보고 (Effect of Succium Decoction on Edema after Plastic Surgery: Two Case Report)

  • Objectives : In traditional Korean medicine literature, Succium(琥珀) is known to be effective in eliminating edema. However, pumpkins are misused in folk remedies to manage edema, because succium and pumpkin is homophony to 'Hobak' in Korean. The purpose of this study is to report the effects of Succium decoction on two patients with edema after plastic surgery. Methods : We treated two patients with edema after plastic surgery using Succium decoction. Numeric rating scale (NRS) of edema, photograph of changes in edema before and after treatment were analyzed to evaluate the effects of treatment. Results : After 2 weeks of administration, NRS for facial edema decreased from 5 to 3 and 2 to 1, respectively. Photograph of edema decreased after treatment, and there were no adverse events. Conclusions : This case report suggests that Succium decoction can be effective in treating edema after plastic surgery.

벌독으로 인한 혈청병 양상의 두드러기 및 맥관부종 치험 1례 (A Case of Urticaria and Angioedema Caused from Serum Sickness Reaction by Bee Venom)

  • Objective : This study was performed to prove the effect of the oriental medical treatments on urticaria and angioedema caused from serum sickness reaction by bee venom. Method : We treated one case of urticaria and angioedema with oriental medical treatments such as 8 constitution acupuncture, herbal medicine, venesection therapy and facial pack therapy. We took pictures through the treatment to evaluate the improvement. Six items, itching, burning sensation, pain, wheal, redness, edema were used to diagnose urticaria and angioedema. Result & Conclusion : After treatments the urticaria and angioedema were improved. The symptom of itching, heating, pain, wheal, redness of whole body and facial edema disappeared.

자하거(紫河車) 약침치료를 이용한 양측 동시성 Bell's palsy 환자 1례에 대한 증례보고 (A Clinical Study on 1 Case of Patient with Bilateral Simultaneous Bell's Palsy Treated by Hominis Placenta Herbal-Acupuncture)

  • Objective : This study was carried out to investigate the progress of bilateral simultaneous facial palsy and the effect of Hominis Placenta herbal-acupunture and the other oriental medical therapies. Method : We used two methods to research the progress of disease. 1. Diagnosis - Facial muscle test, Taste test, Hearing test, Photographies, Lab-finding 2. Treatment - Acupuncture, Herbal-acupuncture, Electroacupuncture, Herb-med Edema rate, Pain endurance, WBC, Hemoglobin, Platelet, Total protein, Albumin, Globulin, RA factor, CRP Results : The onset of Rt. facial palsy was earlier than Lt. facial palsy 3days. The reaction on the treatment of Rt. facial palsy was more dull than Lt. facial palsy. In terms of treatment period, Rt. facial palsy was very longer than Lt. facial palsy. Conclusion : According to the above results, we discoveried that Hominis Placenta herbal-acupunture and the other oriental medical therapies had good influence on the bilateral simultaneous facial palsy. In the future, we should endeavor to know influence between Rt. and Lt. face in case of bilateral simultaneous Bell's palsy.

  • Angioneurotic edema is rarely seen in dental practice and manifested by acute attacks of swelling of the extremities, face, airway, or abdominal visera, occuring spontaneously and suddenly or secondary to trauma. Two types are recognized : hereditary and nonhereditary. Prophylatic therapy may be used by fresh frozen plasma or antifibrinolytic agents in hereditary type. Good supportive care for acute attacks, together with a knowledge of course of the disease, can prevent asphyxiation from airway obstruction. A case of acute angioneurotic edema of the facial area in a 19-year-old man is presented in conjunction with a review of the literature. Angioneurotic edema attacks him acutely after the extraction of the lower, right 3rd molar.

안면골 골절 후 시신경 손상에 관한 고찰 (THE CLINICAL STUDY OF THE OPTIC NERVE INJURY AFTER FACIAL TRAUMA)

  • Orbital injuries are common with facial trauma. Direct injuries to the globe are not rare but it can result in complications such as chemosis, subconjunctival hemorrhage and hyphema. Periorbital trauma or injuries to the extraocular muscles and blow-out fracture may result in lid edema, ecchymosis & ptosis and diplopia or limitation of ocular motion respectively. Indirect injuries to the optic nerve come up without any injuries but its complication is irreversible and severe such as loss of vision. The aim of this study is to review the literature on blindness or ptosis following facial trauma and present the cases of blindness after facial trauma and ptosis after mandibular fracture without specific clinical findings.
