Correction of Post-traumatic Lower Lip Asymmetry Using Botulinum Toxin Type A (보톡스를 이용한 외상 후 하순 비대칭 교정)

  • Botulinum toxin type A (BTX-A) inhibits muscle contraction, which leads to reversible muscle atrophy and paralysis. Therefore, BTX-A injection can be an effective treatment of facial asymmetry that originated from the uncoordinated muscle movement. A 52-year-old patient was referred from another hospital for the correction of post-traumatic sequelae. The patient had prominent scar in the mandibular symphysis area with asymmetric lower lip movement. The reason for this asymmetric lower lip movement was due to damage in the lower lip depressor muscle. After the injection of BTX-A on the lower lip depressors, asymmetric lip movement has been improved.

Simultaneous Correction of Depressed Scar Using Dermofat Graft Harvested during Scar Revision (다발성 안면 반흔을 가진 환자의 반흔 절제술에서 얻은 진피지방을 이용한 함몰 반흔의 동시교정)

  • While no scar can be completely erased, most linear facial scars can result in a relatively good scar after revision. However, in case of round shaped depressed scar, the scar is often lengthened following an incisional technique. This study focuses on the technique of dermofat graft harvested from the adjacent scars for correction of depressed scars. 18 patients having multiple facial scars with a depressed scar among 375 patients who had undergone scar revision were treated from June 2003 to May 2004. Dermofat was harvested from the adjacent linear scar, then it was deepithelialized, reshaped, and grafted to the depressed scar through a small incision. Cosmetic results were generally good. Complications were overcorrection in 4 patients; hyperpigmentation occured in 1 patient. However, 13 patients were satisfied with the results. The advantages of our technique are as follows: it does not require additional operation; dermofat graft has low absorbable rate; it can maintain the volume with an adequate texture.

Correction of Class III malocclusion with alternate rapid maxillary expansions and constrictions using a hybrid hyrax-mandibular miniplate combination and simultaneous orthodontic treatment: A case report

  • In this report, we describe the successful use of alternate rapid maxillary expansions and constrictions with a hybrid hyrax-mandibular miniplate combination and simultaneous orthodontic treatment for the management of severe Class III malocclusion due to maxillary hypoplasia in an 11-year-old girl. The devices were removed after 20 months of treatment, and the family was instructed about a careful control and retention program that should be followed in accordance with the patient's growth. The final result included the correction of Class III malocclusion with adequate function and excellent facial esthetics, which restored the patient's self-esteem and provided personal motivation. The outcomes showed good stability after 24 months of retention. The decrease in the duration of active treatment is the most important finding from the present case. Considering that facial esthetics in adolescence is a determining factor for the development of a personality and interpersonal relationships, we recommend the use of this protocol for growing patients, who will exhibit not only an improved physical appearance but also a better quality of life.

  • Background: Mandibular deficiency leading to facial asymmetry causes cosmetic deformity as well as psychological stigma for the patient. Correction of these mandibular asymmetries is a major challenge. The study investigates the efficacy of bidirectional mandible distraction for the treatment of mandibular deficiency. Methods: This prospective study included six individuals aged between 17 and 24.4 years. Five patients had hemifacial microsomia and one had unilateral temporomandibular joint ankyloses. All patients underwent mandibular distraction osteogenesis. Postoperative skeletal changes in affected mandible, and changes in occlusal plane and oral commissure cant were evaluated using three-dimensional reconstruction. Patient satisfaction and understanding of the procedure were assessed through three questionnaires administered during pre-distraction, distraction and post-distraction phases. Results: In pre-distraction phase, aesthetic appearance seemed to be the primary indication for surgery. In distraction phase, pain while chewing was the primary handicap. In post-distraction phase all patients were satisfied with the aesthetic outcome. The facial deformity was improved through mandibular distraction osteogenesis. On the affected side in all the patients, height and length of the mandible increased. Canting of the occlusal plane and oral commissure was corrected. Conclusion: Bidirectional mandible distraction is an effective treatment for correction of mandible deformities in adult patients.

Orthodontic treatment in a patient with Moebius syndrome: A case report

  • Moebius syndrome (MBS) is a congenital neurologic disorder that causes cranio-facial abnormalities. It involves paralysis of the VI and VII cranial nerves and causes bilateral or unilateral facial paralysis, eye movement disorder, and deformation of the upper and lower limbs. The orofacial dysfunctions include microstomia, micrognathia, hypotonic mimetic and lip muscles, dental enamel hypoplasia, tongue deformity, open bite or deep overbite, maxillary hypoplasia, high arched palate, mandibular hyperplasia or features indicating mandibular hypoplasia. This case report presents a 7-year-old male patient who was diagnosed with MBS at the age 2 years. The patient displayed typical clinical symptoms and was diagnosed with Class II malocclusion with a large overjet/overbite, tongue deformity and motion limitation, and lip closure incompetency. Treatment was initiated using a removable appliance for left scissor bite correction. After permanent tooth eruption, fixed appliance treatment was performed for correction of the arch width discrepancy and deep overbite. A self-ligation system and wide-width arch form wire were used during the treatment to expand the arch width. After 30 months of phase II treatment, the alignment of the dental arch and stable molar occlusion was achieved. Function and occlusion remained stable with a Class I canine and molar relationship, and a normal overjet/overbite was maintained after 9.4 years of retainer use. In MBS patients, it is important to achieve an accurate early diagnosis, and implement a multidisciplinary treatment approach and long-term retention and follow-up.

Case report: Correction of nasolabial fold with needle embedding therapy (매선침법을 이용한 비순구 주름 개선 5례)

  • Objective : Needle embedding therapy is a newly induced therapy which uses specialized tools. The purpose of this study is to report the nasolabial folds correction effect of needle embedding therapy on 5 cases. Method : Five women with no disease were received needle embedding therapy four to six times over one to two months. The photos of each patient were taken before and after the treatment. Also, Glogau photoaging classification were assessed before and after the treatment. Result and conclusion : Needle embedding therapy may be an effective and safe treatment for the correction of nasolabial folds.

A New Method of Intraoral Le Fort II Osteotomy for Correction of Midface in Cleft Patients (구순 구개열 환자에서의 새로운 구강내 접근 Le Fort II 골절단술)

  • Midfacial retrusion is one of characteristics of cleft patients due to underdevelopment of the midface. In this case, Le Fort II advancement is indicated. But there has not been reported on intraoral approach to Le Fort II osteotomy for correction of midface, which traditional technique resulted in facial scar. To overcome the problems, Le Fort II osteotomy via only intraoral approach named 'Intraoral Le Fort IT osteotomy' was developed by Choung in 1994, this technique was applied to cleft patients. A new technique of Intraoral Le Fort IT osteotomy for correction of midface in cleft patients was successfully carried out with good results. This technique showed no noticeable relapse nor complications, which is reported.

Correction of mandibular ramus height with frontal and lateral ramal inclinations in cephalograms and its effects on diagnostic accuracy of asymmetry (2차원 방사선 규격사진에서 하악골 상행지 고경의 보정분석에 관한 3차원 CT 영상 연구)

  • Defining right and left side differences in mandibular ramus height is one of the key elements in the diagnosis of facial asymmetry. The purpose of the present study was to evaluate the effect of correction of ramus height with frontal and lateral ramal inclinations (FRI and LRI) in 2-dimensional cephalograms and observe how this affects the diagnostic accuracy of asymmetry. Methods: Frontal and lateral cephalograms were obtained in 40 individuals with chin deviation. FRI and LRI were measured on each side and ramus height measurement was corrected with these inclinations using Pythagorean's theorem. The results of diagnosis before and after correction on cephalograms were compared with the results in 3D CT images. Results: Both FRI and LRI showed greater values in the contralateral side than in the chin-deviated side and these contributed to an increase in the right and left side ramus height differences. After comparison of diagnostic results before and after correction on cephalograms with the results on 3D CT images, the sensitivity increased significantly (from 74 to 94 %) whereas the specificity decreased (from 44 to 22 %). Overall accuracy increased from 68 to 78 % with the correction using FRI and LRI. Conclusions: The results of the present study indicate that correction of ramus height with FRI and LRI is useful for an accurate diagnosis of facial asymmetry on frontal cephalograms.

A STUDY ON TREATMENT EFFECTS OF MAXILLARY SECOND MOLAR EXTRACTION CASES (상악 제 2 대구치 발거에 의한 교정치료의 효과)

  • Orthodontic treatment in conjunction with second-molar extraction has been a controversial issue among orthodontists over many decades. The aim of this study was to investigate the treatment effects of upper second molar extraction cases. The sample included 19 upper second molar extraction orthodontic cases(ten Angle's Class I's and nine Class II's, average age=13Y 6M) cared at Kyung-Hee University Department of Orthodontics. Lateral cephalometric radiographs were taken before and immediately after treatment. Seventy-nine points were digitized on each cephalogram and 38 cephalometric parameters were computed comprising 22 angular measurements, 13 linear measurements, and 3 facial proportions. The data obtained from each malocclusion group were analyzed by paired t-test. The statistical results disclosed that there was no significant change in skeletal pattern after treatment except for that accountable by growth while there was statistically significant change in dentoalveolar and soft tissue patterns. There were no significant changes in Bjork sum, posterior facial height /anterior facial height and lower anterior facial height /anterior facial height. No significant changes in anteroposterior position of maxilla and palatal plane were manifested. Although facial axis and lower facial height was slightly increased and the mandible was rotated backward and downward, there was no remarkable change in the mandibular plane. There were statistically significant changes in distal movement of upper first molar, molar key correction and overjet reduction while there was no change in the occlusal plane. The upper lip was slightly retracted simultaneously with slight increase in nasolabial angle. These results signify that distalization of upper dentition with the second molar extraction does change occlusal relationship without gross modifications in the craniofacial skeletal configurationson. Henceforth the second molar extracted would be recommended to treat severe anterior crowding and protrusion with minor skeletal discrepancy.

Development of a Web-based Presentation Attitude Correction Program Centered on Analyzing Facial Features of Videos through Coordinate Calculation (좌표계산을 통해 동영상의 안면 특징점 분석을 중심으로 한 웹 기반 발표 태도 교정 프로그램 개발)

  • In order to improve formal presentation attitudes such as presentation of job interviews and presentation of project results at the company, there are few automated methods other than observation by colleagues or professors. In previous studies, it was reported that the speaker's stable speech and gaze processing affect the delivery power in the presentation. Also, there are studies that show that proper feedback on one's presentation has the effect of increasing the presenter's ability to present. In this paper, considering the positive aspects of correction, we developed a program that intelligently corrects the wrong presentation habits and attitudes of college students through facial analysis of videos and analyzed the proposed program's performance. The proposed program was developed through web-based verification of the use of redundant words and facial recognition and textualization of the presentation contents. To this end, an artificial intelligence model for classification was developed, and after extracting the video object, facial feature points were recognized based on the coordinates. Then, using 4000 facial data, the performance of the algorithm in this paper was compared and analyzed with the case of facial recognition using a Teachable Machine. Use the program to help presenters by correcting their presentation attitude.
