Applications of digital subtraction angiography in the management of penetrating injuries of the maxillofacial region: A case report

  • This report presents a clinical case of trauma due to assault with a knife, and describes the importance of using the correct imaging modality in cases of facial penetrating trauma involving the superficial and deep anatomical planes. Penetrating wounds in the maxillofacial region are rare and poorly reported, but can result in serious complications that are difficult to resolve and may compromise the patient's quality of life, especially when large blood vessels or other vital structures are involved. Thus, it is essential to determine the extent of the affected blood vessels and the proximity of the retained object to the anatomical structures. In this case, digital subtraction angiography was the imaging modality chosen. The use of appropriate imaging examinations allows a proper map of the surgical field, reducing the chances of vascular damage during the surgical procedure.

  • Intramuscular hemangioma is a rare vascular benign proliferation that can occur within any muscle, particularly in the trunk and extremities. In the head and neck region, the masseter muscle is most commonly involved, followed by the periorbital and sternocleidomastoid muscles. Diagnosing intramuscular hemangioma is challenging because there are no characteristic symptoms; instead, magnetic resonance imaging is the best imaging modality to diagnose these lesions. Complete surgical resection is the treatment of choice, although the local recurrence rate is high. Herein, we report a rare case of intramuscular hemangioma located in the zygomaticus minor muscle, which is related to smiling and usually runs along the orbicularis oculi muscle. Distinguishing or separating these two muscles is challenging. However, based on the muscle vector of the midface and radiological findings, the two muscles were successfully separated. The zygomaticus minor was cut very slightly to approach to the lesion and the muscle fibers were split to excise it. A follow-up examination revealed no nerve damage or muscle dysfunction at 4 weeks postoperatively. This rare case may serve as a reference for managing intramuscular hemangioma in the head and neck region.

Botulinum neurotoxin injection for treating plunged nose and post-rhinoplasty: anatomical perspectives of depressor septi nasi, nasalis, leveator labii superioris alaeque nasi muscle

  • Botulinum neurotoxin (BoNT) injection for the treating plunged nose, post-rhinopasty and hyaluronic filler migration is common procedures in clinical settings. However, the lack of thorough anatomical understanding makes it difficult to locate the nose region muscles. The anatomical considerations concerned with BoNT injection into the nasalis, levator labii superioris alaeque, and depressor septi nasi muscles were reviewed in this study. The injection spots have been presented for the nasalis, levator labii superioris alaeque, and depressor septi nasi muscles, with the recommended injection technique for each muscle. We have suggested the ideal injection sites in association with outer anatomical landmarks of the nose region. Moreover, these proposals would support a more accurate procedure of BoNT injection in relieving plunged nose, preventing post-rhinoplasty deviation, and migration of the hyaluronic acid filler.

  • This study was performed to analyze the physical attributes of the faces and affective words on the fares. For analyzing physical attributes inside of a face, 36 facial features were selected and almost of them were the lengths or distance values. For analyzing facial contour 14 points were selected and the lengths from nose-end to them were measured. The values of these features except ratio values normalized by facial vortical length or facial horizontal length because the face size of each person is different. The principal component analysis (PCA) was performed and four major factors were extracted: 'facial contour' component, 'vortical length of eye' component, 'facial width' component, 'eyebrow region' component. We supposed the five-dimensional imaginary space of faces using factor scores of PCA, and selected representative faces evenly in this space. On the other hand, the affective words on faces were collected from magazines and through surveys. The factor analysis and multidimensional scaling method were performed and two orthogonal dimensions for the affections on faces were suggested: babyish-mature and sharp-soft.

  • The osteochondroma, also known as osteocartilagenous exostosis, is one of the most common benign tumors of the axial skeleton, but is rarely found in the facial bones. When present, the tumor is most often reported to affect the mandibular coronoid process. Osteochondroma of the mandibular condyle is extremely rare and may cause signs and symptoms like those seen in patients with temporomandibular joint dysfunction. Sometimes, differentiation between osteochondroma and condylar hyperplasia is not possible on histologic grounds alone, but the radiographic and intraoperative findings together are usually sufficient to establish a definite diagnosis. This report reviews the literature concerning osteochondroma, especially of the maxillofacial region, and describes a case of osteochondroma of the condyle.

  • The patient, 19 years old female, complained of protrusion of upper and lower anterior teeth. Teeth lining was good except slight crowding in lower anterior teeth. Teeth lining was good except slight crowding in lower incisors, but distocclusion in the region of right buccal segment was present. Cephalometric analysis revealed normal relation between maxilla and cranial base. The labial inclination of upper and lower anterior teeth was severe, so diagnosed as bialveolar protrusion case. She was treated by means of multibanded system under the extraction of four first bicuspids. After 1 year and 4 months. She gained good interdigitation of buccal segments and attractive facial profile because the labioversion of incisors was reduced properly.

  • 최근 가상환경에서 아바타의 활용이 빠르게 증가하면서 아바타 애니메이션에 대한 연구가 활발히 진행되고 있다. 특히 아바타의 사람과 같은 자연스러운 얼굴 애니메이션(Facial Animation)은 사용자에게 아바타가 살아 있는 듯한 느낌(Life-likeness)과 사실감(Believability)을 심어주어 보다 친숙한 인터페이스로 활용될 수 있다. 이러한 얼굴 애니메이션 생성을 위해 얼굴의 특징 점을 추출하는 기법이 끊임없이 이루어져 왔다. 그러나 지금까지의 연구는 실시간으로 사람 얼굴로부터 모션을 생성하고 이를 바로 3D 얼굴 모델에 적용 및 모션 라이브러리를 구축하기 위한 최적화된 알고리즘 개발에 대한 연구가 미흡하였다. 본 논문은 실제 사랑 얼굴 모델로부터 실시간으로 특징 점 인식을 통한 애니메이션 적용 및 라이브러리 생성 기법에 대친 제안한다. 제안 기법에서는 빠르고 정확한 특징 점 추출을 위하여 색 정보를 가공하여 얼굴 영역을 추출해내고 이를 영역 분할하여 필요한 특징 점을 추출하였으며, 자연스러운 모션 생성을 위하여 에러 발생 시 대칭점을 이용한 복구 알고리즘을 개발하였다. 본 논문에서는 이와 같은 색 정보 기반의 영역분할 및 영역 대칭 기법을 제시하여 실시간으로 끊김이 없고 자연스러운 얼굴 모션 라이브러리를 생성 및 적용하였다.

Incidental finding of an extensive oropharyngeal mass in magnetic resonance imaging of a patient with temporomandibular disorder: A case report

  • In this report, we describe the incidental finding of an oropharyngeal mass in a patient who presented with a chief complaint of temporomandibular pain. The patient was initially evaluated by an otorhinolaryngologist for complaints of headaches, earache, and sinus congestion. Due to worsening headaches and trismus, he was further referred for the management of temporomandibular disorder. The clinical evaluation was uneventful except for limited mouth opening (trismus). An advanced radiological evaluation using magnetic resonance imaging revealed a mass in the nasopharyngeal/oropharyngeal region. The mass occupied the masticatory space and extended superioinferiorly from the skull base to the mandible. A diagnostic biopsy of the lesion revealed a long­standing human papilloma virus (HPV­16)­positive squamous cell carcinoma of the oropharynx. This case illustrates the need for the timely radiological evaluation of seemingly innocuous orofacial pain.

PANAGRAPHIC STUDY OF MAXLLlOFACIAL REGION (Panagraph에 의한 악안면에 관한 연구)

  • The author has studied maxillo-facial anatomical landmarks using Status X with two methods. The one has performed by application of contrast media on the human dry skull, the other has performed on living human skull as control group. Comparing the panagraphs taken by two methods, the author has drawn following results: 1. The panagraphs revealed the undistorted, highly sharp panoramic shadows of each jaw on a film. 2. Diminishing the inserted anode tube overlapping-free representation of the anterior teeth was taken. 3. Alternating the head position of the objects, direction of anode tube and film placing, the shadows of temporo-mandibular joint and zygomatic arch were taken without overlapping the other bone tissues. 4. In the panagraphs applied various shaped contrast media to each anatomical landmark, a radio-anatomical atlas which is necessary to interpret various bone tissues was taken. 5. In order to interpret panagraphic shadows easily, the author has tried this study by comparing the films of the living human skull with the films of the human dry skull applied contrast media.

Microsurgical Composite Tissue Transplantation in Reconstruction of the Head and Neck Tumor (미세수술로 복합조직 이식술을 이용한 두경부 종양의 재건)

  • Although regional and myocutaneous flaps are still the standard in head and neck reconstruction. The capacity to transfer distant tissues by microsurgical technique expands the reconstructive surgeon's ability to restore severe deformities of the head and neck. From 1981 to 1985, thirteen patients with extensive defects of the head and neck after extirpative surgery of tumor have been reconstructed utilizing the technique of microsurgical composite tissue transplantation. In this series of patients, six presented with reconstructions in the different region of the head and neck with review of literatures. The results of case analysis are summarized as follows: 1) Transplantation was successful in twelve among thirteen patients (92%). 2) Seven patients presented with malignancy and six with benign tumor. 3) Frequent sites of reconstruction were face and mandible. 4) Fourteen free composite tissue transplantations for reconstruction of thirteen patients were obtained six different tissue compositions from eight different donor sites. 5) The recipient artery and vein was most often superficial temporal or facial vessels. 6) The average time of operation was ten hours.
