삼차 신경절 액조내 글리세롤 주입에 의한 삼차신경통 치험(12예 보고) (Retrogasserian glycerol Injection as a Treatment of Tic Doulouruex -Report of twelve cases-)

  • In 1983, Sten H$\ddot{a}$kanson first reported the clinical safety and efficacy of retrogasserian glycerol injection as a treatment of typical trigeminal neuralgia in 96 of 100 patients during a follow-up period of 1~6 years. Since September 1987, we have injected sterile pure glycerol into the trigeminal cistern using an anterior percutaneous approach via the foramen ovale (H$\ddot{a}$rtel route) for treatment of tic douloureux in 12 patients who were suffering from attacks pain of following discontinuation of carbamazepine. The results were as follows; 1) Eight patients were completely free from pain attacks with a single dose of glycerol (0.4 ml). The remaining four patients needed a second dose (0.4 ml) several days later following the single dose. The degree of patient's subjective satisfaction by those injections was very good in 11 and fair in one. 2) During the follow-up period (1~13 months), persistent sensory deficit as determined by the pin prick test, appeared to be mild in 10 and moderate in one patient. There was no sensory deficit in one patient. further attacks of pain from those injections were still noted. 3) As a transient complication, there was headache in all patient, facial hematoma in 4, nausea and vomiting in two each, and vertigo and herpes simplex in one each. In conclusion, we confirmed that the above glycerol injections into the trigeminal cistern were clinically very effective as a treatment of tic douloureux even though the follow-up period was short.

Treatment of non-odontogenic orofacial pain using botulinum toxin-A: a retrospective case series study

  • Background: The purpose of this study was to evaluate the clinical outcomes of treatment of non-odontogenic atypical orofacial pain using botulinum toxin-A. Methods: This study involved seven patients (seven females, mean age 65.1 years) who had non-odontogenic orofacial pain (neuropathic pain and atypical orofacial pain) and visited the Seoul National University Bundang Hospital between 2015 and 2017. All medication therapies were preceded by botulinum toxin-A injections, followed by injections in the insignificant effects of medication therapies. Five of the seven patients received intraoral injections in the gingival vestibule or mucosa, while the remaining two received extraoral injections in the masseter and temporal muscle areas. Results: In five of the seven patients, pain after botulinum toxin-A injection was significantly reduced. Most of the patients who underwent surgery for dental implantation or facial nerve reconstruction recovered after injections. However, the pain did not disappear in two patients who reported experiencing persistent pain without any cause. Conclusions: The use of botulinum toxin-A for the treatment of non-odontogenic neuropathic orofacial pain is clinically useful. It is more effective to administer botulinum toxin-A in combination with other medications and physical therapy to improve pain.

Evidence-based use of cold for plantar fasciitis

  • Objective: The purpose of this study was to examine the effect of cold applied the night before or in the morning on pain and symptoms of plantar fasciitis. Design: Experimental study. Methods: Thirty subjects with plantar fasciitis were recruited for this study. Subjects with plantar fasciitis either had no intervention, cold applied (20 minutes) at night before bed, or 20 minutes in the morning upon wakening. Plantar fascia tenderness and pain were evaluated. There were ten subjects in each group. Measures included visual analog scale, plantar facial thickness via high resolution ultrasound, algometer measure, and range of motion of the ankle and foot. There were 3 groups of 10 subjects, control (no intervention), cold the night before bed, and cold in the morning before rising. Results: The greatest relief of symptoms was cold used at bedtime the night before the measurements. Cold used in the morning was not as effective as cold used in the evening before bed. Cold use reduced the thickness of the plantar fascia and irritation. There was a 13% reduction in plantar fascia thickness with cold the night before (p<0.05), a 44% reduction in pain and an 86 % increase in the force that could be applied to the bottom of the foot without pain (p<0.05). Conclusions: Cold applied for 20 minutes prior bedtime is effective for reduced symptomology caused by plantar fascia inflammation.

Fiberoptic bronchoscope and C-MAC video laryngoscope assisted nasal-oral tube exchange: two case reports

  • In cases of multiple facial trauma and other specific cases, the anesthesiologist may be asked to convert an oral endotracheal tube to a nasal endotracheal tube or vice versa. Conventionally, the patient is simply extubated and the endotracheal tube is re-inserted along either the oral or nasal route. However, the task of airway management can become difficult due to surgical trauma or worsening of the airway condition. Fiberoptic bronchoscopy was considered a novel method of airway conversion but this method is not useful when there are secretions and bleeding in the airway, or if the anesthesiologist is inexperienced in using this device. We report a successful airway conversion under the aid of both, a fiberoptic bronchoscope and a C-MAC video laryngoscope.

보툴리눔 독소를 이용한 코골이 및 수면무호흡 치료 (Treatment of Snoring and Sleep Apnea with Botulinum Toxin)

  • 보툴리눔 독소는 사시, 안검경련, 뇌성마비, 경부근 근육긴장이상, 다한증, 얼굴의 주름 같은 미용 치료, 그리고 만성 편두통 치료에 미국 식약청의 승인을 받아 사용되고 있다. 이외에도 강직유발 통증, 포진후 신경통, 근막통증, 아프타성 구내염 등과 같은 통증을 조절하는 데 효과가 있을 것이라 기대되어 다양한 의료영역에서 연구가 진행되고 있는데, 최근 코골이 및 수면무호흡 치료에 있어서 보툴리눔 독소의 효과에 관한 연구들이 보고된 바 있어 그 유용성에 관해 고찰해 보고자 한다.

Integrative Korean Medicine Treatment for Idiopathic Glossopharyngeal Neuralgia: A Case Report

  • Glossopharyngeal neuralgia is a skin-sensitive condition/disease that causes severe pain in the facial area predominantly innervated by the glossopharyngeal nerve. A 51-year-old man diagnosed with glossopharyngeal neuralgia who was hospitalized with severe pain that limited his daily life activities. From November 18, 2021, to March 4, 2022, he received inpatient (10 days) and outpatient treatment (35 times) using acupuncture, pharmacopuncture, and herbal medicine. His symptoms were assessed using the Numerical Rating Scale, Neuropathic Pain Scale, and the Self-report Leeds Assessment of Neuropathic Symptom and Signs. The pain reduced after 4 days of initiating Korean medicine treatment and was completely resolved within 4 months of treatment (Numerical Rating Scale score 4 to 0; Neuropathic Pain Scale score 49 to 8; Self-report Leeds Assessment of Neuropathic Symptoms and Signs score 18 to 0). Korean medicine treatment could be an option for treating patients with neuropathy who rely on analgesics.

말초성 안면신경마비 경과에 대한 임상적 예후인자 및 신경생리검사의 유용성 분석 (An Analysis of Clinical Prognosis Factors of Peripheral Facial Palsy and the Effects of Electrodiagnostic Test)

  • Objectives : Facial Nerve Paralysis is one kind of common diseases and it can be treated by natural therapy and the efficiency of treatment is relatively high. In clinical trial, it is not difficult to find patients who were not completely recovered from Facial Nerve Paralysis, so the symptoms are fixed permanently. This leads many doctors and patients to have interests in the progress and prognosis of the disease, so this study was to analyze clinical prognosis factors and verify the effects of Electrodiagnostic Test. Methods : The 378 subjects were chosen from 987 patients who were suffering from Peripheral Facial Palsy, diagnosed with Bell's palsy and Ramsay Hunt Syndrome and had admission treatment. They got Oriental-Western Medicine Treatment within two weeks after outbreaks of the disease and treated at least over 3 weeks using Oriental-Western Medicine Treatment. Results : 1. There was a significant difference in the results of treatment according to gender, age, types of Facial Palsy, existence of Post Auricular Pain, existence of Labyrinth Symptom, HBGS, and existence of onsets of recovery as clinical prognosis factors of Peripheral Facial Palsy, However, a statistically significant difference was not shown in the results of treatment according to the position of Facial Palsy(left or right), existence of a relapse, and diabetes, hypertension. 2. As a result of overall treatment, 77.2% of patients were recovered almost entirely and 22.8% were not, and the quelae of incomplete recovery were Synkinesis, facial contracture, facial spasm, crocodile tears and scheroma in order of frequency. 3. The results of electrodiagnostic test represented useful correlation to predict the final effects of treatment. Conclusion : Based on the above results, the prognosis factors, the degree of recovery, and the sequelae of incomplete recovery were analysed and the effects of electrodiagnostic test was verified.

파노라마 및 정모방사선사진을 이용한 저작형태에 따른 안면비대칭에 관한 연구 (A Study of Facial Asymmetry and Masticatory Pattern Using Panorama and PA Cephalograph.)

  • 최근에 안모에 관심이 증가하면서 안모비대칭에의 불편을 호소하는 환자가 증가하고 있으며 안모 비대칭 평가에는 고가의 3차원적인 영상이 많이 이용되었다. 본 연구에서는 치과영역에서 쉽게 접할 수 있으며 비교적 저렴한 파노라마방사선 사진과 정모방사선 사진을 이용하였다. 안모 비대칭에 대한 골격적인 평가를 하고자 본 연구에 지원한 대상자 중에 특이할만한 전신 질환자나 특이한 교합을 지닌 대상을 제외한 35명을 택하여 파노라마 방사선 사진과 정모 방사선 사진을 촬영하였으며, 저작 형태와 안모 비대칭을 인지하는 정도를 설문지 형식으로 조사하여 다음과 같은 결과를 얻었다. 1. 파노라마 방사선 사진상의 좌우측 비교에서 하악의 수직적 측정 항목인 Co-Go(p<0.05), Co-Ag(p<0.05)에서 하악각 측정 항목에서는 Co-Go-Ag(p<0.05), Go-Me-Ag(p<0.01) 유의한 차이를 보였다. 2. 파노라마방사선 사진에서 저작측에 따른 평가에서는 하악의 수직적 길이측정치에서 우측 저작자는 Co-Ag(p<0.01) 좌측 저작측에서는 Co-Go(p<0.05)에서 유의성을 보였다. 3. 정모 방사선사진 상의 좌우측 안모비대칭 비교에서는 Cg-Go(p<0.01), 우측 저작자에서는 Co-Ag(p<0.01) 좌측 저작자에서는 Cg-Go(p<0.05)에서 유의한 차이를 보였다. 4. 안모비대칭이 있다고 인지하는 군에서 하악의 수직적 측정 항목인 Co-Go(p<0.05)에서 유의성을, 하악각의 측정항목인 Co-Si-Cr(p<0.05), Go-Me-Ag(p<0.05)에서 유의성을 보였다. 안모가 좌측이 더 길다고 인지하는 군에서는 Go-Ag(p<0.05)의 수평적 계측 항목이 우측이 더 길었으며, 우측안모가 길다고 인지하는 군에서는 Co-Go(p<0.05)와 Co-Ag(p<0.01)의 하악의 수직적 측정치에서 유의성을 보였다.

상복부(上腹部) 통증완화(痛症緩和)를 위한 복강신경총차단(腹腔神經叢遮斷) (Alcohol Neurolysis of the Celiac Plexus of Upper Abdominal Pain Relief)

  • 상북부 통증을 호소하는 암성통증 환자 18예, 만성 췌장염환자 4 예에서 통증제거 목적으로 60% dehydrated ethanol 30~50ml을 방척추법으로써 복강신경총의 지각전도를 차단한 후 얻은바 결론을 다음과 같이 요약한다. 1. 위암, 간암, 췌장암등으로 인한 상복부 통증에 대해서 86%의 우수한 진통효과가 있었다. 2. 만성췌장염 환자에서도 양호한 효과를 얻을 수 있었다. 3. 진통효과의 지속시간은 55%에서 약 4개월 내지 7개월간 지속되었다. 4. 시술후 일시적인 기립성 혈압하강이 16%에서 나타났다. 5. 시술후 합병증은 경미한 정도에서 안면홍조 혈압하강 요통등이 있었다. 6. 시술전 마약성 약물 사용이 습관화된 경우는 시술후 마약성 약물 투여금단 효과를 억제하기 어려웠다.

The Algorithm-Oriented Management of Nasal Bone Fracture according to Stranc's Classification System

  • Background: Nasal bone fracture is one of the most common facial bone fracture types, and the surgical results exert a strong influence on the facial contour and patient satisfaction. Preventing secondary deformity and restoring the original bone state are the major goals of surgeons managing nasal bone fracture patients. In this study, a treatment algorithm was established by applying the modified open reduction technique and postoperative care for several years. Methods: This article is a retrospective chart review of 417 patients who had been received surgical treatment from 2014 to 2015. Using prepared questionnaires and visual analogue scale, several components (postoperative nasal contour; degree of pain; minor complications like dry mouth, sleep disturbance, swallowing difficulty, conversation difficulty, and headache; and degree of patient satisfaction) were evaluated. Results: The average scores for the postoperative nasal contour given by three experts, and the degree of patient satisfaction, were within the "satisfied" (4) to "very satisfied" (5) range (4.5, 4.6, 4.5, and 4.2, respectively). The postoperative degree of pain was sufficiently low that the patients needed only the minimum dose of painkiller. The scores for the minor complications (dry mouth, sleep disturbance, swallowing difficulty, conversation difficulty, headache) were relatively low (36.4, 40.8, 65.2, 32.3, and 34 out of the maximum score of 100, respectively). Conclusion: Satisfactory results were obtained through the algorithm-oriented management of nasal bone fracture. The degree of postoperative pain and minor complications were considerably low, and the degree of satisfaction with the nasal contour was high.
