노인틀니 국민건강보험의 틀니제작행위 및 비용에 관한 인식 (The cognition of denture fabrication activities and the costs in National Health Insurance for elderly denture)

  • Purpose: The purpose of this study was to investigate the cognition of denture fabrication activities and its cost in National Health Insurance for elderly denture. Methods: A self-administered questionnaire was completed by 41 dental laboratories' owner who was research subjects of HIRA(Health Insurance Review & Assessment Service)'s policy research for elderly denture in 2011. The questionnaire consisted of general characteristics of the subjects, dental technicians' knowledge of national health insurance coverage of elderly denture, job-related changes after national health insurance coverage of elderly denture including validity of denture fabrication activity classification and the cost for each service. Each question was measured by Likert 5 point scale or frequency. The collected data were analyzed by SPSS 16.0. Results: Most of the research subjects had been fabricating national health insurance coverage denture(92.7%), also had ample knowledge of national health insurance coverage denture for elderly. Job-related changes after national health insurance coverage of elderly denture revealed marginal differences in the quality. The validity of fabrication activity classification of resin based complete denture was $3.71{\pm}1.023$ by Likert 5 point scale. Conclusion: The goal of national health insurance for elderly denture is to promote elderly's health and well-being. To fabricate denture is a very important part of the denture treatment. For this reason, denture fabrication activity classification and the cost analysis plan should be duly reflected in the policy of national health insurance for elderly denture.

Modified soft tissue cast for fixed partial denture: a technique

  • In process of fabrication of a fixed partial denture, dies are trimmed to expose margins of the preparations. The need for the soft tissue cast is quite evident as the soft tissue emergence profile that surrounds the prepared tooth is destroyed in the process of fabrication. This article describes a modified technique to fabricate the soft tissue cast for the conventional fixed partial denture. The impression made with the polyvinylsiloxane was first poured to prepare the die cast. After retrieval of the cast, the same impression was poured second time with the resin based resilient material to cover the facial and proximal gingival areas. The remaining portion of the impression was poured with the gypsum material. This technique does not require additional clinical appointment, second impression procedure, technique sensitive manipulations with impression, or cumbersome laboratory procedures. The simplicity of this technique facilitates and justifies its routine use in fabrication of the fixed partial denture.

  • 흡착원리를 이용한 총의치 제작은 연하나 저작 등의 운동 시 의치 내면의 음압이 형성되어 의치연 주변의 봉쇄를 이루어 유지와 안정을 향상시키고자 소개된 방식이다. 하악 흡착의치는 특히 변형되기 쉬운 후구치융기를 안정 상태에서 채득하여 개구 시 탈락력을 감소시킨다. 이 증례에서는 하악 치조제 흡수가 심한 무치악 환자에서 흡착원리를 통한 총의치 제작을 통해 의치의 향상된 유지력과 안정 및 환자의 만족도를 얻었다.

  • Polymers are the dominant material for fabrication of denture bases. However, resin base can't fufill the patients' satisfactions completely and solve the pronunciation problem and prevent the denture fracture. In spite of many advantages, metal denture bases do not widespread in clinical practice. The main reasons are the difficulties in fabrication and additional time and cost, inability to rebase such prostheses. The use of the metal base can be one of options in complete denture treatments. This study helps, through reveiwing previous reports and literature about the metal base, metal base to be useful in the clinical application by recommend the materials, indications and advantage/disadvantage of the metal base and introduce variable designs. The clinical application of the metal base have many advantages, but the dentists have to select cases carefully and apply designs according to patient's various conditions. In conclusion, the use of the metal base can't alternate treatment of inadequate conventional complete denture. Adequate complete denture is very important for the treatment of the metal base complete denture.

  • Currently, computer-aided technology becomes one of main issues in clinical dentistry. About 25 years ago, the development of dental CAD/CAM systems for the fabrication of crowns and fixed partial dentures leads to be able to fabricate complete denture today. The fabrication of milled complete denture prostheses with digital scanning technology may decrease the number of patient appointments. However, the precise tooth arrangement and evaluation by patient is not promising relatively. The purpose of this review was to analyze the existing literature on computer aided technology for fabricating complete denture with historical background, current status, and future perspectives. In addition, two available commercial systems were introduced.

A comparative study on the accuracies of resin denture bases and metal denture bases

  • Since the late 1930s, acrylic resins have been the materials of choice for the fabrication of complete denture bases. It has excellent esthetic properties, adequate strength, low water sorption, and low solubility. But acrylic resin has disadvantage of processing shrinkage that reduces denture retention and accuracy of denture occlusion. Metals also have been used in denture base material. Metals used in denture bases display excellent strength and dimensional stability. The major disadvantages associated with metal denture bases include increased cost, difficulty in fabrication, compromised esthetic qualities, and inability to re-base. The purpose of this study is to compare the artificial tooth movements of complete dentures with resin bases and metal bases after curing, deflasking, polishing immersion in water for 1 week and 4 weeks. Twenty-four maxillary complete resin denture bases with artificial teeth were fabricated. Twelve of them were resin based and other twelve of them were metal based. Fine crosses were marked on the incisal edges of right central incisors and distobuccal cusps of be second molars. Measurements were done for the changes of distances of reference points at the time of wax denture, after deflasking after decasting after polishing after immersion in water for 1 week and 4 weeks Meaurements were done to the accuracy of 0.001mm with a measuring microscope. The results were as follows : 1. Metal base showed significantly less tooth movement than resin base after curing and decasting (p<0.01). 2. Metal base showed significantly less tooth movement than resin base after polishing (p<0.01). 3. After immersion in water for 1 week and 4 weeks, metal base showed less movement than resin base. Difference was significant for anterior-posterior distances (p<0.01), but not significant for molar-to-molar distance (p>0.01). 4. 1 week and 4 weeks of immersion failed to compensate the initial processing shrinkage of metal and resin bases (p>0.01).

DENTCA 시스템을 이용한 총의치 제작법 (A novel method of complete denture fabrication with CAD/CAM)

  • Currently CAD/CAM technology has been used widely in dentistry. But it has mainly been focused on fabrication of fixed partial dentures and implant-supported prosthesis. DENTCA company uses new cutting edge of CAD/CAM technology to revolutionize denture production. With developing a CAD/CAM technology of DENTCA company, it is possible to make complete dentures with minimum visits to the clinic. The aim of this article is to introduce a new denture-making method by CAD/CAM.

  • CAD/CAM 기술은 이미 치과 보철학 영역에서 전반적으로 사용되고 있으며, 1994년 총의치 제작을 위한 CAD/CAM 기술이 처음 소개된 이후 의치 제작과정의 CAD/CAM 적용범위가 증가되고 있는 추세이다. 본 논문에서는 무치악 환자의 양악 총의치 제작을 위한 전통적인 다섯 단계 중 납의치 제작과정에 CAD/CAM기술을 도입하였다. 최종 인상채득하여 악간 관계를 기록한 주모형을 스캔한 후 Amann Girrbach사의 Ceramill full denture system (fds)을 이용해 납의치를 제작하였다. Ceramill fds는 스캔된 상하악 모형에 전용 소프트웨어를 이용해 인공치 배열 및 의치상을 제작하여 이를 밀링하는 방식으로 기존의 기공과정을 디지털로 전환하는 특징을 가진다. 이를 통해 총의치 제작시간의 단축 및 심미적, 기능적으로 만족스러운 의치제작이 가능했기에 본 증례를 보고하고자 하는 바이다.

  • The purpose of this article is the consolidation of several methods in fabrication of Konus denture. It is different Konus denture from traditional Clasped removable partial denture in the procedures of construction. There are multiple procedures of fabrications of inner and outer crowns in the construction of Konus denture. It is important to fabricate the inner crown, the outer crown and the denture framework in construction of Konus denture. Each procedure should be performed exactly. However, there are many procedures in fabrications of them, and thus, the operator and technician bear trial and error. This article consolidate the multiple methods of fabrications of components of Konus denture. The first method is completion of inner crown, outer crown and denture after one impression taking. The second method is the procedures of cementation of inner crown, impression taking of edentulous area, and completion of outer crown and denture. The third method is the procedures or pick up impression taking of inner crown and completion of outer crown and denture on the inner crown of working cast. Each method is acceptable, but operater and technician should be accustomed with their own systemic procedures and minimize the errors in the construction of denture.

전통적 제작법과 모놀리식(monolithic) 디지털 의치 시스템을 이용한 상·하악 총의치 동시 수복 증례 (Fabrication of complete denture using conventional method and monolithic digital denture system: a case report)

  • Computer aided design/Computer aided manufacturing (CAD-CAM) 기술의 발전에 따라 이를 활용한 총의치 제작 방법이 대두되고 있으며, 그 중 하나로 의치상과 인공치의 색조를 모두 포함하는 모놀리식 블록을 절삭하는 디지털 의치 시스템이 소개되었다. 본 증례는 74세 남성, 73세 여성 두 환자에서 전통적인 기존 의치 제작법, 디지털 방식의 모놀리식 블록을 절삭한 의치 제작법으로 각각 총 두 쌍의 상·하악 의치를 제작하여 비교한 증례이다. 디지털 방식의 총의치 제작 과정으로는 CAD-CAM 프로그램으로 개인 트레이를 디자인 및 3D프린팅 하여 최종인상 및 악간 관계 채득하였다. 이후 동일한CAD-CAM프로그램으로 총의치 디자인 및 시적의치를 3D 프린팅 하여 시적 하였다. 최종적으로는 모놀리식 블록(Ivotion, Ivoclar Vivadent, Schaan, Liechtenstein)을 밀링 기계(PrograMill PM7, Ivoclar Vivadent, Schaan, Liechtenstein)로 절삭하여 총의치 제작 및 장착하였다. 해당 증례에서는 CAD-CAM 시스템을 이용한 총의치 제작과 동시에 전통적 방법으로도 총의치를 제작하여2쌍의 의치의 안정 및 적합성, 발음, 저작, 심미성과 환자의 만족도를 비교, 분석하여 이를 보고하고자 한다.
