Study on the Establishment of a Safety Allowance Level of Disastrous and Hazardous Facilities in Large Cities (대도시 위해.위험시설에 대한 안전도 수용기준 정립에 관한 연구)

  • In today's world, rise in the establishment of social infrastructure resulting from population saturation in large cities has led to more extensive and frequent use of chemical materials on facilities. A result, unexpected and serious accidents, hazards, contingencies and disasters are more prevalent than ever. Such phenomenon calls for more devoted and concerted efforts towards finding ways to reduce the safety hazards that are seen to take place more often than before with the increase in the number of facilities that are prone to bring disaster and hazard coupled with the conventional safety problems that continue to exist even today. In developed countries, such challenge is addressed by various appropriate countermeasures drawn up by local professional committees on industrial facilities, whose members conduct offsite and onsite evaluation un the potential industrial disasters and its seriousness and provide their advice thereof. Against this backdrop, this study aims at identifying a comprehensive safety allowance level (safety acceptable level) when imposing limitation on the development of conventional or new facilities, for the fur pose of establishing a safety allowance level of disastrous and dangerous facilities in Korea. This is done by assessing and applying the level of danger each individual is exposed to in a randomly selected region (disastrous and dangerous areas in Seoul) based on probability of quantitative hazards, as well as simulation and calculation methods which include: i) social disaster evaluation method applying Quantified Risk Assessment of Health & Safety Executive of UK and Matrix of Risk of Evaluated Sources of Hazard; ii) Fault Tree or Event Tree Analysis and etc.

Safety Techniques-Based Improvement of Task Execution Process Followed by Execution Maturity-Based Risk Management in Precedent Research Stage of Defense R&D Programs (국방 선행연구단계에서 안전분석 기법에 기반한 수행프로세스의 개선 및 수행성숙도 평가를 활용한 위험 관리)

  • The precedent study stage of defense programs is a project stage that is conducted to support the determination of an efficient acquisition method of the weapon system determined by the requirement. In this study, the FTA/FMEA technique was used in the safety analysis process to identify elements to be conducted in the precedent study stage and a methodology for deriving the key review elements through conceptualization and tailoring was suggested. To supplement the key elements derived from the existing research, it is necessary to analyze various events that may arise from key elements. To accomplish this, the HAZOP technique for safety analysis in other industrial fields was used to supplement the results of kdy element derivation. We analyzed and modeled the execution procedure by establishing input/output information and association with the key elements of the precedent study stage derived by linking HAZOP/FTA/FMEA techniques. In addition, performance maturity was evaluated for performance of precedent study, and a risk-based response manual was generated based on inter-working information with key elements with low maturity. Based on the results of this study, it is possible to meet the performance, cost, and schedule of the project implementation through application of the key elements and procedures and the risk management response manual in the precedent study stage of the defense program.

The Reliability Evaluation of TBN Valve Testing Extension in NPP (원자력발전소 터빈밸브 시험주기 연장시 신뢰도평가)

  • Recently, nuclear power plant companies have been extending the turbine valve test interval to reduce the potential of the reactor trip accompanied with a turbine valve test and to improve the NPP's economy through the reduction of unexpected plant trip or decreased operation. In these regards, the extension of the test interval for turbine valves was reviewed in detail. The effect on the destructive overspeed probability due to the test interval change of turbine valves is evaluated by Fault Tree Analysis(FTA) method. Even though the test interval of turbine valves is changed from 1 month to 3 months, the analysis result shows that the reliability of turbine over speed protection system meets acceptance criteria of 1.0E-4/yr. This result will be used as the technical basis on the extension of the test interval for turbine valves. In this paper, the propriety of the turbine valve test interval extension is explained through the review on the turbine valve test interval status of turbine overspeed protection system, the analysis on the annual turbine missile frequency and the probability evaluation of the destructive overspeed due to the test interval extension.

  • This paper is aimed to investigate the actual conditions of standalone PV(Photovoltaics) streetlight in public place and analyze cause of failure and derive the method of decrease for standalone PV streetlight. The cost of installing standalone PV streetlight in a public for the safety and convenience of life is expensive(300 million won per unit). Management of standalone PV streetlight is important because it is operating system itself, after installation, but concern degree of management is very low and development of technology has not been done. Accordingly, standalone PV streetlight in public place is not operate in night, this phenomenon is lasted for several days to months. Therefore this study, for standalone PV streetlight constantly to keep the normal operation, investigation conducted to check status of standalone PV streetlight(50 units), the voltage of the main parts and analyze the cause of the failure. In addition, the improvement of standalone PV streetlight is suggested by using FTA(fault tree analysis).

  • The turnout system, which determines the direction of the train, is not only a key system but also a vulnerable system. Failure of this system may lead to a delay of the train or even casualties. In this light, it is necessary to precisely the conditions of the turnout system. Currently, ROADMASTER of Germany is used as a diagnostic system in Korea. However, a new diagnostic system should be developed for optimized operation of the turnout system with maintenance that is suitable for the Korean railway environment. In this paper, a Fault Tree Analysis for the representative faults of the turnout system is conducted and physical quantities, which can be the cause of the fault, are classified according to the component and function. Also, the measuring factors for the monitoring are derived and a decision making theory is suggested. On the basis of the results, we propose a new turnout diagnostic system that can provide more driverse and precise information than the conventional system.

A Study on the Effectiveness of Gas Safety Devices for Domestic (가정용 가스안전기기의 실효성에 관한 연구)

  • Gas safety devices are very effective to prevent catastrophic gas accident in domestic. The safety devices are included in domestic gas equipment such as extinguishing safety device and adapted at pipeline such as fuse cock, shut off device with gas alarm and so forth. In spite of using those safety devices, a few hundreds of gas accident was happened annually in residential house. In this study, we analysed systematically the domestic accidents which was happened in five years using fault tree analysis(FTA) method and analysed the effectiveness of individual safety device. And also, it was suggested that the rate of accident was decreased quantitatively by increasing safety device which is adapted in domestic. By analysis of 769 gas accidents in domestic, the order of effectiveness of safety device to prevent domestic gas accident was the multi-functional gas-safe-meter(micom-meter), fuse cock, gas leak alarm and CO alarm. If the above four kind of safety device are adapted to every house, about $59\%$ of accident will be reduced and the most of catastrophic gas accident will be Prevented in domestic.

  • In this study a new SIS evaluation method based on the reliability analysis has been developed. It evaluates the Safety Integrity Level (SIL) using the Fault Tree Analysis (FTA), and when the SIL falls short of the systems target level, through the reliability analysis and system retrofit, this method will satisfy the aimed SIL. A hazard evaluation was carried out on the 415V Diesel BUS to verify the SIL evaluation method based on the reliability analysis. The availability of the original 415V Diesel BUS was $99.40\%$, which comes under the category of SIL 2. After exchanging the diesel generator and the isolator switch using the developed evaluation method, the availability rose to $99.94\%$, SIL 3. By applying the method presented in this study, not only will it reduce the maintenance cost due to the prevention of accidents and reduction of loss, but also maximize the reliability of the system.

  • Major obstruction of using formal methods for hybrid real-time systems in industry is the difficulty that engineers have in understanding and applying the quantitative methods in an abstract requirements phase. While formal methods technology in safety-critical systems can help increase confidence of software, difficulty and complexity in using them can cause another hazard. In order to overcome this obstruction, we propose a framework for qualitative requirements engineering of the hybrid real-time systems. It consists of a qualitative method for requirements specification, called QFM (Qualitative Formal Method), and a safety analysis method for the requirements based on a causality information, called CRSA (Causal Requirements Safety Analysis). QFM emphasizes the idea of a causal and qualitative reasoning in formal methods to reduce the cognitive burden of designers when specifying and validating the software requirements of hybrid safety systems. CRSA can evaluate the logical contribution of the software elements to the physical hazard of systems by utilizing the causality information that is kept during specification by QFM. Using the Shutdown System 2 of Wolsong nuclear power plants as a realistic example, we demonstrate the effectiveness of our approach.

  • The purpose of this study is to perform the reliability-corrected development cost estimation of the launch vehicle at the conceptual design phase. In order to estimate the launch vehicle development cost, the estimation method based on the independent variable such as the rocket performance and dry mass has been mainly implemented up to now. This approach has made the approximate cost estimation possible, however, the cost variation according to the reliability requirement could not be reflected. In this paper, the cost estimation methodology that introduces the reliability factor in addition to the performance and mass in the TRANSCOST model is presented in order to improve the limitation of current cost estimation method. The development cost of KSLV(Korea Space Launch Vehicle)-II is estimated on the basis of this newly implemented concept with reliability as an added parameter.

Vulnerability Assessment Procedure for the Warship Including the Effect of Shotline and Penetration of Fragments (탄두의 관통 효과를 고려한 함정 취약성 평가 절차에 관한 기본 연구)

  • The survivability of warship is assessed by susceptibility, vulnerability and recoverability. Essentially, a vulnerability assessment is a measure of the effectiveness of a warship to resist hostile weapon effects. Considering the shot line and its penetration effect on the warship, present study introduces the procedural aspects of vulnerability assessments of warship. Present study also considers the prediction of penetration damage to a target caused by the impact of projectiles. It reflects the interaction between the weapon and the target from a perspective of vulnerable area method and COVART model. The shotline and tracing calculation have been directly integrated into the vulnerability assessment method based on the penetration equation empirically obtained. A simplified geometric description of the desired target and specification of a threat type is incorporated with the penetration effect. This study describes how to expand the vulnerable area assessment method to the penetration effect. Finally, an example shows that the proposed method can provide the vulnerability parameters of the warship or its component under threat being hit through tracing the shotline path thereby enabling the vulnerability calculation. In addition, the proposed procedure enabling the calculation of the component's multi-hit vulnerability introduces a propulsion system in dealing with redundant Non-overlapping components.
