The Effects of Foot Intrinsic Muscle and Tibialis Posterior Strengthening Exercise on Plantar Pressure and Dynamic Balance in Adults Flexible Pes Planus

  • Background: In previous studies regarding flexible pes planus, Foot orthosis, special shoes have been used as interventions for correcting malalignment and intrinsic muscles strengthening exercise have been regarded as interventions for foot function and supporting medial longitudinal arch during walking. However, some recent studies reported that strengthening extrinsic muscles as well as intrinsic muscles is more effective and active intervention for flexible pes planus. In particular, the tibialis posterior muscle of foot extrinsic muscles plays essential roles in maintaining the medial longitudinal arch during dynamic weight bearing and balance. In addition this muscle acts longer than other supination muscles during the stance phase in the gait cycle. Objects: This study aimed to investigate the effect of foot intrinsic muscle and tibialis posterior muscle strengthening exercise for plantar pressure and dynamic balance in adults with flexible pes planus. Methods: 16 young flexible pes planus adults (7 males, 9 females) were recruited and were randomized into two groups. The experimental group performed foot intrinsic muscle and tibialis posterior muscle strengthening training, the control group performed only foot intrinsic muscle strengthening training. All groups received strengthening training for 30 minutes five times a week for six weeks. Results: The experimental group had significantly lower plantar pressure of medial heel area than the control group in stand (p<.05). The experimental group had significantly higher dynamic balance ability than control group (p<.05). Conclusion: The results of this study provide evidence to suggest that foot intrinsic muscle and tibialis posterior muscle of extrinsic muscle strengthening exercises may improve plantar pressure distribution and dynamic balance ability in adults with flexible pes planus.

신발의 굴림 특성이 족저반력 및 회내운동에 미치는 영향 (The Effects of Shoes with Rolling Feature on the Foot Reaction Force and Pronation)

  • The purpose of this study was to analyze the effects of shoes with curved out-sole on the pressure, reaction force(sum of pressure) on foot and relations between the rolling speeds and pronation of foot. The foot pressure, reaction force and pressure center on the foot surface of shoe were measured with NOVEL padar system, and 3 type shoes were used to compare the position and speed of pressure center and the foot reaction force, which were s(target) shoe with soft cushions in middle part of out-sole and curved out-sole, m shoes with two type- soft, hard, hardness out-sole and curved out-sole and n shoes with flat out-sole. The subjects were 13 female university students, had weared the 3 type shoes for 6 weeks on two-weeks shifts for adaptation before experiment and put on 3-type shoes repeatedly and randomly and walked on treadmill with 3.5km/h and 80 steps/min. The data were captured with 30Hz and readjusted with 5kgf threshold reaction force. The results can be summarized as follow. 1. There were no difference in maximum reaction force on initial contact period and total foot impact, but statistical difference in maximum reaction force on takeoff period : s, m, n in ascending order. 2. There were some difference in rolling speeds for support periods. At initial contact, the rolling speed of s shoes was fastest but at periods between first and second maximum reaction force, that of m shoes fastest. 3. There was a negative relation between rolling speeds and the length of lever arm on initial reaction force related to pronation. It seems shoes with various curved shapes and hardness could make effects on the rolling features and the rolling speed also have some relationships with walking efficiency, absortion of impact and pronation.

경추 자세변화에 따른 성인의 족저압력 균형 및 폐활량의 차이 (Differences of Foot Plantar Pressure Balance and Lung Capacity According to Change of Cervical Posture in Adults)

  • Objective: The purpose of this study was to differences of foot plantar pressure balance and lung capacity according to cervical posture in adults. Method: The subjects consisted of 33 adults in their 20s and 50s who use M centers in B-gu and H-gu, B-City, and they measured foot plantar pressure balance and lung capacity according to cervical posture (cervical normal curvature posture, cervical flexural posture) in adults. Results: In this study, the difference of foot plantar pressure balance according to cervical posture were analyzed. In the difference between left and right foot pressure balance. It was 1.50% increased in the cervical flexural posture than in the cervical normal curvature posture, and a statistically significant difference was observed. In the difference between the anterior and posterior foot pressure balance. It was 4.28% increased in the cervical flexural posture than in the cervical normal curvature posture, and a statistically significant difference was observed. The difference of lung capacity according to cervical posture were analyzed. In the PEF, It was 58.63 l/min decreased in the cervical flexural posture than in the cervical normal curvature posture, and a statistically significant difference was observed. In the FEV1, It was 0.15 ℓ decreased in the cervical flexural posture than in the cervical normal curvature posture, and a statistically significant difference was observed. Conclusion: The results of this study suggest that had a positive effect on differences of foot plantar pressure balance and lung capacity at cervical normal curvature posture in adults. In future research, itis believed that research on the elderly who have collapsed the normal curvature posture due to aging, as well as teenagers whose normal curvature posture due to the use of smartphones, will contribute to the balance of foot pressure and improvement of the right cervical habits. In future studies, it is also believed that it will be necessary to measure lung capacity after performing exercise according to the cervical posture, thereby providing sufficient oxygen during exercise to enhance the persistence and efficiency of the movement.

역도 인상동작 불안정성 수준에 따른 발바닥 체성감각요인 분석 (Factor Analysis of the Somatosensory for Foot according to the Instability Level of Snatch Lifting)

  • Objective: It is to find factors related to stability through analysis of plantar pressure factors according to the level of instability when performing Snatch. Method: Foot pressure analysis was performed while 10 weightlifters performed 80% of the highest level of Snatch, and motion was classified and analyzed in 3 grades according to the level of instability. Results: First, in Bad Motion, the movement distance of the pressure center in the direction of ML and AP was larger significantly in Phase 2. Second, in Phase 2, the number of zero-crossing in the AP direction was larger statistically significantly in Good Motion. Third, in the bad motion in Phase 3, the number of zero-crossing in the ML direction showed a significantly larger value. Fourth, in Phase 4, it was found that the more stable the lock out motion, the greater the activity of foot controlling in the left and right directions. Fifth, Phase 3, the greater the Maximum/Mean foot pressure value, the more stable the pulling action. Sixth, in Phase 2, the foot pressure was concentrated with a wide distribution in the midfoot and rearfoot. Seventh, the triggering number of the forefoot region was small in the last pull phase. Eighth, the number of triggers in the toe area was significantly higher during Good Motion in Phase 4. Conclusion: Summarizing the factors of instability in Snatch, there was no significant difference in Phase 1 for each condition. In order to enhance the stability in Phase 2, the sensory control ability in the AP direction is required, and focusing the foot pressing motion with a wide distribution in the middle and rear parts increases the instability. In Phase 3, it was found that the more unstable, the more sensory control activity was performed in the ML direction, the stronger the forefoot pressing action should be performed for a stable Snatch. In Phase 4, It is important that the feet sensory control activity in ML directions and the control ability of the toes in order to have stable Lock out motion.

아로마발반사요법과 발반사요법이 농촌의 본태성고혈압 노인환자의 혈압, 맥박, 혈중지질농도에 미치는 효과 (The effects of Aroma foot reflexology and foot reflexology on blood pressure, pulse rate and blood lipid level of elderly essential hypertensive patients in a rural area)

  • 본 연구의 목적은 아로마발반사요법과 발반사요법이 농촌의 본태성고혈압 노인 환자의 혈압, 맥박, 그리고 혈중지질농도에 미치는 효과를 확인하기 위함이다. 연구방법은 비동등성 대조군 전 후 유사 실험연구로, 2008년 6월 30일부터 8월 9일까지 6주간 충청남도 G군 소재 C보건진료소에 등록된 본태성고혈압 환자로 실험군 I(아로마 발반사요법) 24명, 실험군 II(발반사요법) 27명, 대조군(기존의 처치) 20명을 대상으로 하였다. 수집된 자료는 SPSS/WIN 12.0 프로그램으로 분석하였다. 연구결과는 6주 간 아로마발반사요법과 발반사요법을 각각 적용한 후 실험군 I과 실험군 II 모두 대조군에 비해 수축기 혈압(p<.05)과 이완기 혈압(p<.05)에서 유의한 차이를 보였고, 맥박수도 유의하게 감소하였으나(p<.05), 혈중지질농도의 변화는 세군 간에 통계적으로 유의한 차이가 없었다(p>.05). 결론적으로 아로마 발반사요법과 발반사요법은 농촌의 본태성고혈압 노인 환자의 혈압 하강과 맥박수 감소에 긍정적인 기여를 하는 것으로 나타났으므로 향후 아로마발반사요법과 발반사요법을 본태성고혈압 환자의 간호중재방법으로 활용 할 수 있을 것이다.

간호학생의 임상실습 스트레스경감을 위한 발마사지의 효과 (The Effect of Foot Massage on Stress in Student Nurses in Clinical Practice)

  • The purpose of this study was to identify the effects of foot massage on the stress of student nurses in clinical practice. The research design was a nonequivalent control group pretest-posttest design. The subjects of this study were 52 student nurses from three junior colleges and one university. The subjects were assigned to one of two groups : the experimental group(26 students), and the control group(26 students). The study was conducted from October to November 1999. For the experimental group the foot massage was performed once a day for 3 days, starting from the first day of clinical practice. Subjects' mood, fatigue, blood pressure and pulse rate were measured as dependent variables. The Instrument for mood and fatigue state was a 100mm visual analogue scale. For the experimental group pre and post tests were done just before and after every 3 foot massages and on the 7th clinical practice day. For the control group pretest was done on 1st clinical day and posttest was done on clinical practice 7th day. The data was analyzed by the t-test and paired t-test and the results are summarized as follows: 1. The mood state scores were significantly higher for the experimental group as compared to the control group after the foot massage(t=13.73, p<.001). 2. The fatigue state scores were significantly lower for the experimental group as compared to the control group after the foot massage(t=8.69, p<.001). 3. The systolic blood pressure was significantly different between the experimental group and the control group after the foot massage(t=3.0073, p<.01). 4. The diastolic blood pressure was not significantly different between the experiment group and the control group after the foot massage. 5. The pulse rate was not significantly different between the experiment group and the control group after foot massage. 6. The mood state scores on the 1st, 2nd, 3rd massage day was significantly improved right after the foot massage. 7. The fatigue state scores on the 1st, 2nd, 3rd massage day were significantly lower right after the foot massage. 8. Systolic blood pressure on the 1st and 2nd massage day was significantly decreased right after the foot massage. Diastolic blood pressure and pulse rate were decreased right after massage but the change was not statistically significant. On the basis of the above findings, this study suggests that foot massage can be an effective stress relieving method for student nurses in clinical practice.

역방향 에르고미터 훈련이 편마비 환자의 족관절 가동범위와 하지 근력 및 족저압에 미치는 영향 (The Effect of Backward Pedaling Ergometer Training on Ankle ROM, Lower Extremities Strength, Foot Pressure in Hemiplegia)

  • PURPOSE: The purpose of this study was to identify the effects of backward pedaling ergometer training on ankle ROM, lower extremities strength, foot pressure in hemiplegia. METHODS: The subjects consisted of control group(n=10) and experimental group(n=10), subjects were trained 5 times a week for 2 weeks each group. Control group were trained forward, experimental group were trained backward pedaling with physical therapy in both groups. Each group measured ankle joint ROM with DUALER IQ and ankle and knee joint flexor and extensor muscle strength by Manual Muscle Test System and foot pressure by Gait view system. RESULTS: The result of this study between pre and post test that experimental group had statistically significantly differences in ankle joint range of motion and lower extremities strength. But foot pressure had not statistically significant differences. There was not significantly difference of variation between groups. CONCLUSION: Therefore these results mean that backward and forward pedaling ergometer training effected an improvement of lower extremities function in hemiplegia.

트레드밀 보행훈련이 편마비 환자의 족저압과 하지 근활성도에 미치는 영향 (The Effects of Plantar Foot Pressure and Muscular Activity on Treadmill Gait Training in Stroke Patients)

  • Purpose:This study was performed for effects of gait training on treadmill and stable surface which influenced on the lower limbs muscular activity needed in gait, plantar foot pressure with hemiplegic patients caused by cerebrovascular accident. Methods:Two groups of adult hemiplegia(n=20) were allocated randomly in this study: treadmill gait training group and control group. The gait training program was provided to experimental groups for 8 weeks (5 times a week). Measurements of pre and post experiment were plantar foot pressure. For measuring muscular activation rectus femoris, biceps femoris, tibialis anterior, gastrocnemius were detected. Results:The results of this study showed that in comparison of pre and post changes of gait training, the treadmill gait training group has noticeable changes than other groups in activity of rectus femoris and tibialis anterior, the control group revealed statistically significant differences in plantar foot pressure Toe2-5, M1, M3, M5, MF area, activity of gastrocnemius. Conclusion:These results mean gait training resulted by treadmill, stable surface provides effective muscle activation and plantar foot pressure with stroke.

보행 시 편평족에서 외적 무게 부하에 따른 족저압의 변화 (The Changes of Plantar Foot Pressure by External Loads during Walking in Flatfoot)

  • Purpose : The study was designed to investigate the changes of plantar foot pressure by different loads during walking in flatfoot. Methods : Fifteen subjects with flatfoot were recruited along with their written informed consent. They were asked to walk on plate at a self-selected and comfortable speed with loads of 0, 5, 10, and 15kg. Three walking trials were obtained and then averaged for data analysis. Foot pressure were measured from RS-Scan system (RS-Scan system, RS scan Ltd., German) and contact area, maximum force were analyzed. Results : There were significant increases on midfoot and decreases on forefoot in contact area. And there were significant increases in maximum force of foot pressure of 2nd metatarsal bone and midfoot. Conclusion : These findings revealed that flatfoot increases risk factors of metatarsal bone with different loads. Therefore, patients of flatfoot must be careful during walking with loads or activities of daily living.

Effects of Upright Body Exercise on Postural Balance and Foot Plantar Pressure in Elderly Women

  • Objective: This study investigated the effect of a 16-week upright body exercise program on body balance and plantar pressure balance in elderly women. Method: The subjects included elderly women in B Metropolitan city who participated in an upright body exercise program twice weekly for 16 weeks. The subjects' physical characteristics (height, weight, body mass index [BMI]), posture balance, and foot plantar pressure were measured before and after the experiment. Results: The upright body exercise program showed positive changes in posture balance and foot plantar pressure balance in seven elderly women. Conclusion: The second version of the upright body exercise program improved and may prevent postural imbalance in elderly women. This program could be utilized to improve posture and foot balance in elderly persons.
