양산지역 대학원 일개 학년 학생을 대상으로 동적검사 및 정적검사를 통한 족부지표 분석 (Analysis of the Foot Indices Using Dynamic and Static Check of Postgraduate School Students in Yangsan)

  • Objectives : To evaluate the foot indices using dynamic and static checking on healthy adult people. Methods : Foot indices analysis was performed for 46 students. Static checking practiced for Foot Pressure(FP) and Toe Pressure Difference(TPD) and dynamic checking was used for Vertical Axis Angle(VAA). Results : On gender, there was generally no statical significant difference, except RFP(Right Foot Pressure). The RFP of male was bigger than that of female. On age, in addition, there was similar tendency, only RFP had a positive correlation (r=0.35, p<0.05). Comparing foot indices on right and left, we found TPD and VAA had a tendency of pronation on left, but FP had a opposite tendency. On consistency of each foot index, discrepancy between TPD and VAA (39.47%) was lesser than combination of the others. Conclusions : According to foot indices among 46 healthy student, we found out that there is partially correlation between dynamic and static checking. We hope that the data we found will contribute to standardization of podiatry indices which provide medical guidance for cure of foot disease, supporting further podiatry research.

기능성 전문테니스화의 족저압력분포 분석 (A Study of In-sole Plantar Pressure Distribution in Functional Tennis Shoes)

  • The aim of this study is to evaluate tennis shoes's plantar pressure distribution in tennis prayers and to determine the influence of the shoe on various tennis movements. When investigating the biomechanics of movement in tennis, one of the first things to do is to understand the movement patterns of the sport, specifically how these patterns relate to different tennis shoes. Once these patterns are understood, footwear company can design tennis shoes that match the individual needs of tennis players. Plantar pressure measurement is widely employed to study foot function, the mechanical pathogenesis for foot disease and as a diagnostic and outcome measurement tool for many performance. Measurements were taken of plantar pressure distribution across the foot and using F-Scan(Tekscan Inc.) systems respectively. The F-Scan system for dynamic in-shoe foot pressure measurements has enabled us to assess quantitatively the efficacy of different types of footwear in reducing foot pressures. The Tekscan F-Scan system consists of a flexible, 0.18mm thick sole-shape having 1260 pressure sensors, the sensor insole was trimmed to fit the subjects' right, left shoes. For this study 4 university male, high level tennis players were instructed to hit alternated forehand stroke, backhand stroke, forehand volley, backhand volley, smash, service movement in 4 different tennis shoes. 1. When impact in tennis movement, peak pressure distribution of landing foot displayed D>C>B>A, A displayed the best low pressure distribution. A style's tennis shoes will suggest prayer with high impact. If prayer with high impact feeling during pray in tennis wear A style, it will decrease injury, will have performance improvement. 2. When impact in tennis movement, plantar pattern of pressure distribution in landing foot displayed B>A>C>D in stability performance. During tennis, prayer want to stability movement suggest B style tennis shoes when tennis movement impact keep stability of human body. B style tennis shoes give performance improvement 3. When impact in tennis movement, plantar pattern of center of force(C.O.F.)trajectory in landing foot analyzed this : 1) When stroke movement and volley movement in tennis, prayer better to rearfoot movement. 2) when service movement, prayer midfoot strike movement. 3) when smash movement, prayer have forefoot strike movement.

Rear Foot Wedge 각도가 보행시 전족저 최대압력에 미치는 영향 (The Effect of Rear Foot Wedge Angle on Peak Plantar Pressures on the Forefoot During Walking)

  • The purpose of this study was to find the effect of rear foot wedge angle on peak plantar pressures on the forefoot during walking. Twenty normal healthy subjects (10 female, 10 male) were recruited. Peak plantar pressure was measured using pressure distribution platforms (MatScan system) in medial forefoot (under the first, second metatarsal head) and lateral forefoot (under the third, fourth, fifth metatarsal head). The subjects walked at the comfortable velocity under seven conditions; bare footed, $5^{\circ}$, $10^{\circ}$ and $15^{\circ}$ wedges under the medial and lateral sides of the hindfoot. The three averaged peak plantar pressures were collected at each condition at stance and toe off phases. The results showed that a significant increase in lateral forefoot plantar peak pressure investigated in the medial wedge and a significant decrease in lateral forefoot plantar peak pressure investigated in lateral wedge at stance phase (p<.05). These results suggest that rear foot wedge may be useful to modify the peak plantar pressure on the forefoot.

  • In this paper, we proposed a technique to recognize three states in stance phase of gait cycle. Walking assistive devices are used to help the elderly people walk or to monitor walking behavior of the disabled persons. For the effective assistance, they adopt an intelligent sensor system to understand user's current state in walking. There are three states in stance phase; Loading Response, Midstance, and Terminal Stance. We developed a foot pressure sensor using 24 FSRs (Force Sensing/Sensitive Resistors). The foot pressure patterns were integrated through the interpolation of FSR cell array. The pressure patterns were processed to get the trajectories of COM (Center of Mass). Using the trajectories of COM of foot pressure, we can recognize the three states of stance phase. The experimental results show the effective recognition of stance phase and the possibility of usage on the walking assistive device for better control and/or foot pressure monitoring.

여러타입의 신발에 대한 족저압력과 임펄스의 비교분석 (Comparison of Plantar Foot Pressure and Impulse in Various Shoe Types)

  • We analyzed the pressure, impulse on 24 sensors location under the foot using the Parolee system. Total 7 kinds of shoes, i.e. sport shoe, high heel shoes (5cm heel, 8cm heel, 13cm heel), platform shoe, inline skate, and heelys were evaluated for 20 normal subjects. Compared with those of sport shoe, greater pressure and impulse were shown on the 1 st phalange and the 1 st metatarsal head and greater impulse on the medial tarsal bone in high-heel shoes. Greater pressure and impulse were shown on medial metatarsal bone and the lateral tarsal bone in platform shoe. Greater impulse was shown on the medial tarsal bone in inline-skate. Heelys shoe showed smaller impulse on the central area of foot. The result of this study is expected to provide useful information about the relationship between the shoe type and the foot pathologies.

복부수술 환자의 수술후 통증에 미치는 발마사지의 효과 (The Effect of Foot Massage on Post operative Pain in Patients Following Abdominal Surgery)

  • More and more non-injured operations are being implemented these days, thanks to the development of medical technology. Still, however, most operations leave direct scars on patient' bodies, as well as accompanying pain. The massage as an independent nursing intervention can stimulate the circulation of the blood of tissue and muscle and increase the relationship between a patient and a nurse. The purpose of this study is to investigate the effect of foot massage on pain in post abdominal operative patients. The nonequivalent control group, pre-post test design is used for this study. From July 7, 2000 to February 20, 2001, the 40 patients who were operated under general anesthesia in a university hospital in Seoul were studied. They were divided into two groups ; 20 patients were part of the experimental group, and the others, in the control group. In order to evaluate the effect of foot massage, severity of pain was checked with the VAS (Visual Analog Scale) and also each patients' vital signs were measured with pulse rate, systolic blood pressure and diastolic blood pressure. The collected datas were processed by SAS version 6.12 program and analyzed by the Chi-square, Fisher's exact test, t-test and repeated measures ANOVA. The results of this study were as follows. 1. The severity of pain decreased significantly in the experimental group as compared to the control group following foot massage (t=-3.317, p= .002). 2. Measured vital signs in the experimental group had more reduction of that than in the control group following foot massage. - The pulse rate in the experimental group was lower than that in the control group following foot massage (F=7.73, p=.008). - The systolic blood pressure in the experimental group was lower than that in the control group following foot massage (F=25.75, p=.000). - The diastolic blood pressure in the experimental group was lower than that in the control group following foot massage (F=15.27, p=.000). In conclusion, foot massage is an effective dependent nursing intervention for pain control of post abdominal operative patients.

기능성 족부 보조기 착용에 따른 족저압 변화 (Pedobarographic Analysis in Functional Foot Orthosis)

  • Purpose: The purpose of this study was to evaluate pressure distribution on the foot with the use of custom made foot orthosis and evaluate influential factors in young people using custom orthosis. Materials and Methods: 22 individuals comprised of young males and females were evaluated by radiograph, pedobarograph, and satisfaction rate and VAS score. The data was analyzed statistically to find influential factors for satisfaction after wearing the foot orthosis. Results: Around 50% of participants were satisfied in wearing the custom made foot orthosis. Initial VAS score of satisfaction of $36.2{\pm}19.7$ improved to $73.1{\pm}15.6$ after application of foot orthosis. There was a statistically significant difference. Talo-second metatarsal angle on AP radiograph after orthosis application was significantly related to satisfaction. On analysis of pedobarograph data, total contact area was increased and weight distribution was transferred medially on ambulation with the orthosis applied. Conclusion: Before designing the foot orthosis, individual foot factors such as foot anatomy and foot pressure distribution should be evaluated for foot comfort and better patient satisfaction.

발마사지가 자궁절제술 환자의 수술 전 불안에 미치는 효과 (The Effect of Foot Massage on Anxiety Response in Preoperative Patients undergoing Total Hysterectomy)

  • The purpose of this study was to examine the effects of foot massage on anxiety response in preoperative patients undergoing total hysterectomy. Among the fifty patients, twenty-five were the experimental group receiving foot massage for 10-minutes and twenty-five were the control group. Subjects were admitted to a hospital in Inchon city for total hysterectomy. This study was carried out from July 10th, to September 18th, 2000. The levels of anxiety were measured by the Visual Aanalogue Scale(VAS). State Anxiety Scale, blood pressure, pulse rate and respiration rate. Data were analyzed using $x^2$-test, t-test, paired t-test, unpaired t-test and ANCOVA with SPSS/pc program. The results were summarized as followings: 1. After foot massage, VAS anxiety level, state anxiety level, systolic blood pressure, pulse rate and respiration rate of the experimental group were decreased significantly than those measurements before the foot massage. 2. After foot massage, significant differences in the VAS anxiety level, state anxiety level, systolic blood pressure, pulse and respiration rate between the experimental and control groups were found. In conclusion, foot massage for 10 minutes is a useful nursing intervention that decreases anxiety in preoperative patients undergoing total hysterectomy.

아로마 발마사지가 수면대장내시경 대상자의 불안과 통증 및 수면만족도에 미치는 효과 (The Effects of Aroma Foot Massage on the Anxiety, Pain and Sleep Satisfaction during Colonoscopy under Conscious Sedation)

  • Purpose: This research is to know whether aroma foot massage has influence on the relief of anxiety and pain during colonoscopy under conscious sedation. Method: This research was designed as a quasi-experiment of non-equivalent control group pretest-posttest. Data were collected from April 1 2005 to August 30 2005. The subjects were divided into three groups (control group, foot massage group and aroma foot massage group) with 30 persons each. Anxiety was evaluated with Visual Analogue Scale (VAS), blood pressure and pulse. Pain response was measured with VAS and non-verbal pain behavior score. Sleep satisfaction was measured with a graphic rating scale. Data were analyzed through Chi-square test, t-test and repeated measure ANOVA. Results: Systolic blood pressure, pulse, subjective anxiety and pain scores from the aroma foot massage group decreased significantly. Sleep satisfaction score of the aroma foot massage group increased significantly. Diastolic blood pressure from the aroma foot massage group did not decrease. Conclusion: The results show that aroma foot massage with refined oils can increase sleep satisfaction and decrease anxiety and pain during colonoscopy under conscious sedation.

족부족관절 영역에서 음압창상처치의 적용 (Application of Negative Pressure Wound Therapy in the Foot and Ankle Field)

  • Negative pressure wound therapy (NPWT) has emerged as a valuable tool for managing complex wounds within the foot and ankle field. This review article discusses the expanding applications of NPWT in this specialized field. Specifically, it discusses the efficacy of NPWT for various wound types, including diabetic foot wounds, traumatic wounds, surgical wounds, and wounds involving exposed bone or soft tissue defects. NPWT demonstrates versatile utility for foot and ankle wound management by promoting healing, potentially reducing the need for secondary surgery, improving diabetic and neuropathic ulcer healing times and outcomes, and optimizing the healing of high-risk incisions. In addition, this review explores the underlying mechanisms through which NPWT might enhance wound healing. By synthesizing current evidence, this review provides a comprehensive overview of the role of NPWT in foot and ankle surgery and offers valuable insights to clinicians navigating the complexities of wound care in this challenging anatomical area.
