Targeting motor and cognitive networks with multichannel transcranial direct current stimulation along with peripheral stimulation in a subacute stroke survivor: single case study

  • Objective: Reacquisition of motor functions following stroke depends on interhemispheric neural connections. The intervention highlighted in the present case is an insight for augmenting motor recovery by stimulating the lesioned area and adjacent areas governing the motor behaviour of an individual. The purpose of this study was to determine the changes in the motor and cognitive outcomes through multi target stimulation of cortical areas by application of multichannel transcranial direct current stimulation (M-tDCS) in a stroke survivor. Design: A case report. Methods: The patient was a participant of a trial registered with the clinical trial registry of India (CTRI/2020/01/022998). The patient was intervened with M-tDCS over the left primary motor cortex i.e. C3 point and left dorsolateral prefrontal cortex i.e. F3 point with 0.5-2 mA intensity for the period of 20 minutes. SaeboFlex-assisted task-oriented training, functional electrical stimulation over the lower extremity (LE) to elicit dorsiflexion at the ankle and eversion of the foot, and conventional physiotherapy rehabilitation including a tailored exercise program were performed. Outcome assessment was done using the Fugl-Meyer assessment scale (FMA) for the upper and lower extremity (UE and LE), Montreal Cognitive Assessment (MOCA), Wisconsin Gait Scale (WGS) and the Stroke Specific Quality of Life (SSQOL) measures. Assessment was taken at Day 0, 15 and 30 post intervention. Results: Improvement was observed in all the outcome measures i.e FMA (UE and LE), MOCA, SSQOL and WGS across the span of 4 weeks. Conclusions: M-tDCS induced improvement in motor functions of the UE and LE, gait parameters and cognitive functions of the patient.

  • The purpose of this study is to identify trends in the application of artificial intelligence by analyzing upper extremity movement assessment and artificial intelligence convergence research using a systematic literature review method. The research was conducted using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. Among the 380 articles searched in three databases, 8 articles were finally selected according to the selection and exclusion criteria. For the evaluation of upper extremity movement, motion performance evaluation, FMA, and ARAT were used. For quantification, data were extracted using various tools, and upper extremity movement classification, recovery prognosis prediction, and evaluation tool score were predicted using artificial intelligence. This study is meaningful in that it systematically reviewed studies that objectively evaluated upper extremity movement using artificial intelligence and identified the direction in which artificial intelligence is being applied. Based on this, the introduction of artificial intelligence technology in the assessment of upper extremity movements is expected to help objectively identify the intervention effect and the patient's recovery.

The Effect of the Task-oriented Treadmill Gait on the Improvement in Gait Function, Balance Ability and Functional Activities in Subacute Stroke Patients: Results of a Randomized Controlled Trial (과제지향적 트레드밀 보행이 아급성기 뇌졸중 환자의 보행기능, 균형능력 및 기능적 활동 향상에 미치는 영향: 무작위 대조 실험 )

  • PURPOSE: This study aimed to determine the effects of task-oriented treadmill training on the gait and balance ability and functional activity in 20 patients with subacute stroke. METHODS: The study subjects were twenty stroke patients, ten randomly placed in the experimental group and ten in the control group. Both the experimental and control groups received 30 minutes of traditional physical therapy and an additional 15 minutes of functional electrical stimulation therapy. The experimental group was given task-oriented treadmill training, while the control group received general treadmill training. Each session lasted for 25 minutes, three times a week, over four weeks, totaling 12 sessions. RESULTS: Both groups showed statistically significant differences in the 10-metre walk test (10MWT), timed up and go test (TUG), Fugl-Meyer Assessment (FMA), and Modified Barthel index (MBI). However, statistically significant differences in the Functional Ambulation Categories (FAC) and Berg Balance Scale (BBS) were seen only in the experimental group. There were statistically significant differences in the between-group differences value comparisons in the 10MWT, BBS, TUG, FMA, and MBI. CONCLUSION: Task-oriented treadmill training positively impacts gait, balance, and daily function in subacute stroke patients. This study highlights the benefits of training on unstable surfaces and offers valuable insights for stroke rehabilitation and gait training.

Effect of Hwanggigyejiomul-tang on Postoperative Breast Cancer-related Lymphedema (BCRL): A Systematic Review and Meta-analysis (수술 후 발생한 유방암 연관 림프 부종(BCRL)에 대한 황기계지오물탕(黃芪桂枝五物湯)의 효과 : 체계적 문헌 고찰 및 메타 분석)

  • Objectives: This study assessed the effectiveness of Hwanggigyejiomul-tang for postoperative breast cancer-related lymphedema (BCRL) by a systematic review and meta-analysis of randomized controlled trials (RCTs). Methods: A search was conducted using keywords such as "breast cancer", "lymphedema", "edema", "Huangqi Guizhi Wuwu decoction", "Huangqi Guizhi Wuwu tang", and "Hwanggigyejiomul tang" in 10 databases (PubMed, Cochrane, ScienceDirect, CNKI, CiNii, RISS, KISS, ScienceON, OASIS, DBpia) on February 11, 2024. There were no limits on the publication period and language, and the quality of the studies was evaluated using Cochrane's risk of bias tool. A meta-analysis was performed based on the outcome measurements, such as total effective rate (TER), increase in shoulder joint mobility (flexion, extension, abduction, adduction), Fugl-Meyer assessment (FMA), and visual analog scale (VAS) using Review Manager Web. Results: Eleven RCTs were selected. The treatment group (Hwanggigyejiomul-tang-gagambang or hapbang with control group intervention) showed a more statistically significant effect compared with the control group (physical therapy or western medicine) in TER (upper limb circumference change), TER (upper limb edema grade change), increase in flexion angle, increase in extension angle, increase in adduction angle, FMA, and VAS. Conclusions: Hwanggigyejiomul-tang is effective in treating postoperative BCRL. However, because of the low quality of the included studies, more clinical studies are required to increase the possibility of clinical use.

Population dynamics and stock status of spotted sardinella (Amblygaster sirm, Walbaum, 1792) in the Natuna Sea, Indonesia

  • Small pelagic fish populations have long been exploited by various fishing gears in the Natuna Sea, and since the 1970s, purse seine exploitation has expanded to offshore fishing grounds. Various small pelagic fish species are exploited from these waters including spotted sardinella (Amblygaster sirm). This research was conducted for 22 months, from March 2019 to December 2020. The fish sample was taken from the purse seine fleet's catch in the fisheries management area (FMA) 711 Natuna Sea and Karimata Strait, which was brought to Pemangkat fishing port in the West Kalimantan Province, Indonesia. This paper aims to determine the exploitation status of spotted sardinella in the Natuna Sea as a recommendation material for management policies. The methods used in this analysis are estimated age, cohort, population parameters, and spawning potential ratio of spotted sardinella. The results show three primary cohorts of spotted sardinella in the Natuna Sea, with ages ranging from 9 months to 3.1 years and a maximum attainable age (longevity) of 4.3 years. The asymptotic value (L) is 21.79 cmFL in length, with a growth rate (K) of 0.65 per year. The natural mortality rate (M) was 1.19, the total mortality rate (Z) was 2.19, and the fishing mortality rate (F) was 0.99. The exploitation rate (E) was 0.45 and the stock status was still in a sustainable condition. Therefore, the sardinella fisheries can be developed using a precautionary approach and focus on the ecosystem sustainability of FMA 711.

The Relationship between Anterior Disc Displacement without Reduction and Development of Anterior Open Bite (비정복성 관절원판변위와 전치부 개교합 발생간의 관계)

  • The purpose of this paper is to evaluate if there is a relationship between anterior disc displacement without reduction and development of anterior open bite, and a relation between occurrence of open bite and occlusal appliance therapy. In general, the statistically significant differences were found between the Group 1 and 2 and normal mean group. The variables that represent mandibular size and form, showed a statistical significance in all 3 groups. Also 3 groups patients had a smaller ANB, a larger FMA than normal mean group. When we compared the 3 groups with respect to all cephalometric measurements by One-way analysis of variance (ANOVA), group 1 and 2 patients had a larger FMA, a larger SN to mandibular plane angle, a larger maxillomandibular plane angle, a larger occlusal plane to mandibular plane angle, a smaller total posterior facial height/total anterior facial height(%), and a larger gonial angle than group 3. The statistically significant differences were not found between the Group 1 and 2, and skeletal patterns were similar. Thus, morphologic features of patients with vertical discrepancies may represent a risk factor for the development of anterior open bite with or without occlusal appliance treatment. In case of patients with vertical discrepancy, we may have to be more careful when inducing a change of the vertical dimension.

  • 원전 가동전/가동중검사 결과의 신뢰도(reliability)는 원전 배관기기의 건전성에 직결되는 것으로써 결함 발견 시 적용되는 파괴역학해석(FMA)은 비파괴검사 결과에 대한 100%의 신뢰를 전제하고 있다. 그러나 비파괴검사가 어느 정도 신뢰성을 가지고 있는지에 대한 평가가 국내에서는 거의 수행된 바가 없었다. 따라서, 본 연구에서는 원전의 비파괴검사 규제 요건의 기술적 근거를 확보하고, 원전 기기 건전성 평가 및 안전성 향상을 위한 합리적 규제지침을 수립하기 위하여 국내 원전 가동중검사(ISI)에 적용되거나 일반 산업계에 적용되고 있는 초음파탐상검사에 대하여 기량검증 Round Robin Test에 의한 통계적 신뢰도를 평가하고자 한다. 이를 위해 초음파검사 PD-RRT 결과의 통계적 신뢰도 평가 모델을 고찰하고, 결함검출성능 평가, 결함크기 측정 평가, 팀 오차 분석 등 초음파검사 PD-RRT 결과의 통계적 신뢰도를 평가하였다.

Authors reviewed the recent development of text-based medical image retrieval including the use of controlled vocabularies - RadLex (Radiology Lexicon), FMA (Foundational Model of Anatomy), etc - natural language processing, semantic ontology, and image annotation and markup.

  • This paper develops practical and realistic reliability models and methods for the evalusion of system reliability and system reliability-based rating of R.C box-girder bridge superstructures. The precise prediction of reserved carrying capacity of bridge as a system is extremely difficult expecially when the bridges are highly redundant and significantly deteriorated or damaged. This paper proposes a new approach for the evaluation of reserved system carrying capacity of bridges in terms of equivalent system-strength, which may be defined as a bridge system-strength corresponding to the system reliability of the bridge. This can be derived from an inverse process based on the concept of FOSM form of system reliability index. The strength limit state models for R.C box-girder bridges suggested in the paper are based on the basic bending and shear strength. and the system reliability problem of box-girder superstructure is formulated as parallel-series models obtained from the FMA(Failure Mode Approach) based on major failure mechanism or critical failure states of each girder. AFOSM and IST(Importance Sampling Technique) simulation algorithm is used for the reliability analysis of the proposed models.

Development and Evaluation of a Patient Garment Based on Functional-Modesty-Aesthetic (FMA) Model

  • Recent research suggested that hospitals have not responded to consumer demand for the improved aesthetic and comfort of their patient garments. Thus a new patient garment has been developed considering human factors based on the Functional-Modesty-Aesthetics model. Based on the reviewed literature and the model, necessary design requirements were identified. Then the patient garment was evaluated in comparison to existing ones with regard to potential patients' satisfaction levels. As a result, the developed patient garment received better evaluations in the aspects of overall aesthetic satisfaction, color satisfaction, and willingness to wear than the existing patient garments.
