Spinal accessory neuropathy (SAN) is commonly caused by an iatrogenic procedure, and that caused by tumors is very rare. We present a case of a 49-year-old man suffering from weakness in the right trapezius and sternocleidomastoid muscle. An electrophysiology study confirmed proximal SAN. Fluorodeoxyglucose (FDG)-positron emission tomography (PET)/computed tomography (CT) revealed a diffuse large B-cell lymphoma compressing the right spinal accessory nerve. Ultrasonography showed definite atrophy on the trapezius and sternocleidomastoid muscles. In addition, post-chemotherapy FDG-PET/CT showed increased FDG uptake in the right upper trapezius, suggestive of denervation. This is the first report of SAN caused by direct compression by a diffuse large B-cell lymphoma, comprehensively assessed by an electrophysiology study, ultrasonography, and FDG-PET/CT.
CT를 기반으로 감쇠 보정이 시행되는 PET/CT 검사에서는 금속 삽입물에 의한 선속 경화 현상 (Beam Hardening Artifact)으로 인공물이 발생 된다. 이는 인접한 부위의 과대 혹은 과소 평가를 유발하는 감쇠 보정으로 인해 $^{18}F$-FDG의 섭취 변화를 가져오며 영상의 질과 비뇨 생식기 질환의 진단능을 저하시킨다. 따라서 본 연구에서는 PET/CT 영상의 질 향상을 위한 금속 인공물 저감 (Metal Artifact Reduction, MAR) algorithm 재구성방법의 유용성을 평가하고자 한다. 인공 고관절 삽입물에 대한 인공물을 평가하기 위해 SPECT/PET Phantom에 고관절 삽입물을 고정 하여 PET/CT를 진행했다. W/O MAR algorithm 영상과 MAR algorithm 영상에서 CT상에 나타나는 Bright streak, Dark streak, Metal과 Background의 영역에서의 표준화 섭취 계수(Standardized Uptake Value)의 변화를 분석하였다. 또한 본원에 내원한 고관절 전치환술을 시행한 15명의 환자에게서 W/O MAR algorithm과 MAR algorithm, 비 감쇠 보정 영상을 비교 평가하였다. W/O MAR algorithm 영상에서 SUV는 Bright streak 영역에서는 $0.98{\pm}0.48$ g/ml, Dark streak 영역에서는 $0.88{\pm}0.02$ g/ml, Metal 영역에는 $0.24{\pm}0.16$ g/ml, Background 영역은 $0.91{\pm}0.18$ g/ml로 측정되었다. 하지만 MAR algorithm 영상에서는 Bright streak 영역에서는 $0.88{\pm}0.49$ g/ml, Dark streak 영역은 $0.63{\pm}0.21$ g/ml, Metal 영역에서는 $0.06{\pm}0.07$ g/ml, Background는 $0.90{\pm}0.02$ g/ml로 측정되었다. MAR algorithm 적용 시 SUV가 평균적으로 감소 하였으며, W/O MAR algorithm 영상에서는 Bright streak영역에서 Background 보다 높게 측정되어 false positive uptake로 나타났으나 MAR algorithm 영상에서는 Background와 비슷한 농도와 SUV가 나타났고, false positive uptake가 관찰되지 않았다. 따라서 MAR algorithm을 적용하여 인공 고관절 삽입물 주변 조직의 과대 혹은 과소 감쇠 보정으로 인한 섭취의 증가를 줄일 수 있었다. 하지만 Dark streak 영역에서의 SUV 저하를 줄이기 위한 방안은 더욱 많은 연구가 이뤄져야 할 것이다. 고관절 삽입 인공물로 인해 발생하는 Bright streak 영역에서의 false positive uptake를 감소시키고 이와 인접해 있는 비뇨 생식기 질환에 W/O MAR algorithm과 MAR algorithm, 비 감쇠 보정 영상이 동시에 제공된다면 더욱 진단능을 향상 시킬 수 있으리라 생각된다.
목적: 호흡게이트PET(이하 RGPET)을 이용하여 호흡에 의한 PET영상의 인공산물의 감소 효과를 호흡모형 팬톰을 제작하여 분석하였다. 특히 4D-CT를 시행하여 얻은 동일 호흡위상의 CT영상을 이용하여 RGPET의 감쇠 보정에 이용할 수 있도록 CT영상을 재구성하는 방법을 제시하였다. 대상 및 방법: 반복주기 6초, 진동 폭 26mm의 운동 팬톰에 각각 3 ml syringe와 10, 30 ml의 vial에 18.5 MBq (0.5 mCi) 18-F FDG를 주입한 후, 게이트의 유무에 따라 Discovery ST (GE Medical System, Milwaukee. WU) PET-CT 스캐너를 사용하여 PET/CT스캔을 시행하였다. 이때 호흡추적장치로는 적외선 CCD카메라 방식의 Real-Time Position Management (Varian Medical Systems, Palo Alto, CA)을 사용하였다. 호흡게이트PET 및 4D-CT스캔은 10% 호흡위상백분위 별로 총 10세트의 영상을 각각 획득하였다. 이와 같이 운동주기를 10개의 소 구간으로 분할하여 얻은 PET과 CT영상으로부터 각 물체의 위치를 분석하였고, 물체의 크기에 따른 운동 인공산물의 크기와 PET 계수 값의 감소간의 상관관계를 분석하였다. 결과: RGPET과 4D-CT상에서 물체의 중심위치를 호흡위상별로 분석한 결과, 오차범위 내에서 실제 위치와 잘 일치하였다. 게이트를 시행하지 않은 PET에서 관측된 물체의 크기는 상대적 운동크기에 비례하여 증가하여, 운동범위가 물체 크기의 2배가 되면 부피를 2.5배 가량 과대 평가하였다. 반면, 최대 uptake수치는 50% 가량 줄었다. 결론: RGPET을 통해 PET영상에서 나타나는 호흡으로 인한 인공산물의 대부분을 제거할 수 있음을 확인할 수 있었으며, 4D-CT스캔을 통해 획득한 동일위상의 CT 영상을 이용하여 보다 정확한 감쇠 보정 및 영상융합 결과를 얻었다.
목 적: PET이 PET-CT로 오면서 가장 큰 차이를 보이고 있는 점은 감쇄보정을 위한 투과영상의 차이이다. PET CT에서는 CT영상을 감쇄보정에 이용하고 있으며, CT 요인들이 PET 영상의 감쇄보정시 영향을 줄 수 있을 것으로 예상된다. 본 연구에서는 CT의 관전압과 관전류의 변화에 따라 PET의 정량성 평가로 이용되는 SUV(standard uptake value)의 값이 변화가 있는지를 평가하였다. 재료 및 방법: Data spectrum's lung phantom을 이용하여 $^{18}F$-FDG 1.909 mCi(팬텀 1)와 $913\;{\mu}Ci$(팬텀 2) 각각 물과 함께 넣고 흔들어 균일한 상태를 만든 후, 관전압을 80 kVp, 100 kVp, 120 kVp, 140 kVp까지, 관전압변화 당 관전류 10 mA에서 100 mA까지 각각 10 mA씩 변화를 주어 CT 영상을 얻었다. PET 영상을 각각의 CT영상을 이용하여 재구성 하였으며, 같은 부위에서의 SUV값을 구하여 그 차이를 비교하였다. 결 과: 팬텀 1의 SUV는 관전압 80, 100, 120, 140 kVp CT영상을 이용했을 때, 각각 $12.26{\pm}0.009$, $12.27{\pm}0.005$, $12.27{\pm}0.006$, $12.27{\pm}0.009$로 나타났다. 팬텀 2에서는 관접압을 달리 하였을 때 각각 $4.52{\pm}0.043$, $4.53{\pm}0.004$, $4.52{\pm}0.007$, $4.52{\pm}0.005$의 SUV를 보였다. 이들은 모두 관전압을 달리 하였을 때 통계적으로 유의한 SUV 변화를 보이지 않았다. 또한 관전류의 변화에서도 팬텀 1, 2 모두에서 유의한 SUV 차이는 나타나지 않았다. 결 론: 본 연구 결과 CT 요인의 변화가 PET 영상에서 SUV에 유의한 영향을 미치지 않았다. 따라서 PET CT 이용 시 SUV에 유의한 영향 없이 임상적 조건에 맞게 CT 요인들을 변화 시키는 응용이 가능할 것으로 생각된다.
Background: Atelectasis is an important prognostic factor that can cause pleuritic chest pain, coughing or dyspnea, and even may be a cause of death. In this study, we aimed to investigate the potential impact of atelectasis and PET parameters on survival and the relation between atelectasis and PET parameters. Materials and Methods: The study consisted of patients with lung cancer with or without atelectasis who underwent $^{18}F$-FDG PET/CT examination before receiving any treatment. $^{18}F$-FDG PET/CT derived parameters including tumor size, SUVmax, SUVmean, MTV, total lesion glycosis (TLG), SUV mean of atelectasis area, atelectasis volume, and histological and TNM stage were considered as potential prognostic factors for overall survival. Results: Fifty consecutive lung cancer patients (22 patients with atelectasis and 28 patients without atelectasis, median age of 65 years) were evaluated in the present study. There was no relationship between tumor size and presence or absence of atelectasis, nor between presence/absence of atelectasis and TLG of primary tumors. The overall one-year survival rate was 83% and median survival was 20 months (n=22) in the presence of atelectasis; the overall one-year survival rate was 65.7% (n=28) and median survival was 16 months (p=0.138) in the absence of atelectasis. With respect to PFS; the one-year survival rate of AT+ patients was 81.8% and median survival was 19 months; the one-year survival rate of AT-patients was 64.3% and median survival was 16 months (p=0.159). According to univariate analysis, MTV, TLG and tumor size were significant risk factors for PFS and OS (p<0.05). However, SUVmax was not a significant factor for PFS and OS (p>0.05). Conclusions: The present study suggested that total lesion glycolysis and metabolic tumor volume were important predictors of survival in lung cancer patients, in contrast to SUVmax. In addition, having a segmental lung atelectasis seems not to be a significant factor on survival.
Purpose: 18F-FET, a radiopharmaceutical based on a Tyrosine amino acid derivative using the Sodium-Potassium Pump-independent Transporter (System L) for non-invasive evaluation of primary, recurrent, and metastatic brain tumors, exhibits distinct characteristics. Unlike the widely absorbed 18F-FDG in both tumor and normal brain tissues, 18F-FET demonstrates specific uptake only in tumor tissue while almost negligible uptake in normal brain tissue. This study aims to compare and evaluate the usefulness of 18F-FDG and 18F-FET Brain PET/CT quantitative analysis in brain tumor diagnosis. Materials and Methods: In 46 patients diagnosed with brain gliomas (High Grade: 34, Low Grade: 12), Brain PET/CT scans were performed at 40 minutes after 18F-FDG injection and at 20 minutes (early) and 80 minutes (delay) after 18F-FET injection. SUVmax and SUVpeak of tumor areas corresponding to MRI images were measured in each scan, and the SUVmax-to-SUVpeak ratio, an indicator of tumor prognosis, was calculated. Differences in SUVmax, SUVpeak, and SUVmax-to-SUVpeak ratio between 18F-FDG and 18F-FET early/delay scans were statistically verified using SPSS (ver.28) package program. Results: SUVmax values were 3.72±1.36 for 18F-FDG, 4.59±1.55 for 18F-FET early, and 4.12±1.36 for 18F-FET delay scans. The highest SUVmax was observed in 18F-FET early scans, particularly in HG tumors (4.85±1.44), showing a slightly more significant difference (P<0.0001). SUVpeak values were 3.33±1.13 for 18F-FDG, 3.04±1.11 for 18F-FET early, and 2.80±0.96 for 18F-FET delay scans. The highest SUVpeak was in 18F-FDG scans, while the lowest was in 18F-FET delay scans, with a more significant difference in HG tumors (P<0.001). SUVmax-to-SUVpeak ratio values were 1.11±0.09 for 18F-FDG, 1.54±0.22 for 18F-FET early, and 1.48±0.17 for 18F-FET delay scans. This ratio was higher in 18F-FET scans for both HG and LG tumors (P<0.0001), but there was no statistically significant difference between 18F-FET early and delay scans. Conclusion: This study confirms the usefulness of early and delay scans in 18F-FET Brain PET/CT examinations, particularly demonstrating the changes in objective quantitative metrics such as SUVmax, SUVpeak, and introducing the SUVmax-to-SUVpeak ratio as a new evaluation metric based on the degree of tumor malignancy. This is expected to further contributions to the quantitative analysis of Brain PET/CT images.
Background/Objectives: This study aimed to evaluate the changes of uptake around the sternoclavicular joint (SCJ) according to 18F-FDG PET images in patients with head and neck cancer who underwent neck dissection. Materials & Methods: Retrospectively, the medical records of patients who received selective or comprehensive neck dissection were reviewed. Preoperative and 1-year postoperative 18F-FDG PET images, if available, were analyzed by nuclear medicine physicians in both qualitative and quantitative manners. Correlation between the changes of uptake around SCJ and perioperative data were statistically analyzed. Results: Thirty-seven patients satisfying the inclusion criteria were enrolled. Seven patients with increased uptake around SCJ on 1-year postoperative 18F-FDG PET showed a correlation with radical or comprehensive neck dissection, accessory nerve sacrifice, and high postoperative SUVmax. When 20 patients with increased uptake around SCJ according to quantitative measurement were compared with other patients without increased uptake, no parameter was significantly different, except postoperative SUVmax. Bivariate logistic regression analysis revealed that the clinical symptom (shoulder or sternal pain) was significantly correlated with the extent of neck dissection (OR 0.227, CI 0.053-0.966, p=0.045) and spinal accessory nerve sacrifice (OR 13.500, CI 1.189-153.331, p=0.036). Conclusions: Increased uptake around SCJ on 1-year postoperative 18 F-FDG PET was correlated with either the radical or comprehensive procedure, as well as with accessory nerve sacrifice. This suggests that subjective analysis of 18F-FDG PET can be used to detect subclinical shoulder instability.

Objective: Fluorine-18-fluorodeoxyglucose positron emission tomography (18F-FDG-PET) is a new technique for identifying different malignant tumors using different uptake values between tumor cells and normal tissues. Here we assessed the diagnostic accuracy of 18F-FDG-PET in patients with testicular cancer by pooling data of existing trials in a meta-analysis. Methods: PubMed/MEDLINE, Embase and Cochrane Central Trials databases were searched and studies published in English relating to the diagnostic value of FDG-PET for testicular cancer were collected. The summary receiver operating characteristic (SROC) curve was used to examine the FDG-PET accuracy. Results: A total of 16 studies which included 957 examinations in 807 patients (median age, 31.1 years) were analyzed. A meta-analysis was performed to combine the sensitivity and specificity and their 95% confidence intervals (CIs), from diagnostic odds ratio (DOR), positive likelihood ratios (PLR), negative likelihood ratio (NLR). SROC were derived to demonstrate the diagnostic accuracy of FDG-PET for testicular cancer. The pooled sensitivity and specificity were 0.75 (95% confidence interval (CI), 0.70-0.80) and 0.87 (95% CI, 0.84-0.89), respectively. The pooled DOR was 35.6 (95% CI, 12.9-98.3). The area under the curve (AUC) was 0.88. The pooled PLR and pooled NLR were 7.80 (95% CI, 3.73-16.3) and 0.31 (95% CI, 0.23-0.43), respectively. Conclusion: In patients with testicular cancer, 18F-FDG-PET demonstrated a high SROC area, and could be a potentially useful tool if combined with other imaging methods such as MRI and CT. Nevertheless, the literature focusing on the use of 18F-FDG-PET in this setting still remains limited.
PET/CT에서 환자피폭 문제로 인해 저 선량의 중요성이 강조되고 있다. 본 연구에서는 기존에 사용되던 CT 데이터를 이용한 감쇠보정법인 CTAC와 새롭게 적용된 Q.AC를 환자실험과 팬텀 실험을 통해 저 선량으로 촬영 시 PET 영상에 미치는 영향에 대해 알아보고자 한다. 실험장비는 GE사의 PET/CT Discovery 710 (GE Healthcare, USA)를 사용하였으며 팬텀실험으로 감쇠보정의 정량적 평가를 위한 NEMA IEC body phantom과 균일성 평가를 위한 Uniform NU2-94 phantom을 사용하였다. 각각의 팬텀 내부에 동위원소 18-F FDG를 70.78 MBq, 22.2 MBq 주입하고 CT조건은 저 선량조건으로 80 kVp, 10 mA로부터 일반선량 조건으로 140 kVp, 120 mA 조건까지 스캔 후 CTAC와 Q.AC 두 감쇠보정법을 적용하여 재구성하였다. PET 영상에서 일반선량 조건을 기준값으로 정하고 horizomtal profile과 vertical profile을 통해 정량평가를 시행하고 기준값과의 상대적 오차를 평가하였다. 또한 환자실험으로 정상체중 환자와 과체중 환자를 구분하여 저 선량과 일반선량으로 비교 촬영한 뒤 CTAC와 Q.AC로 재구성된 PET영상에서 주요장기별 SUV에 대한 상대적 오차와 신호 대 잡음비를 비교분석하였다. 팬텀실험 결과 저선량 조건에서 CTAC와 Q.AC로 각각 재구성한 PET 영상의 profile과 상대적 오차에서 CTAC보다 Q.AC가 기준값과의 오차가 적은 그래프를 얻었다. 환자실험의 경우 일반선량 조건에서는 정상체중 환자와 과체중 환자 모두 감쇠보정법에 따른 상대적 오차값의 변화가 적었으나 저 선량 조건에서는 정상체중 환자보다 과체중 환자에서 감쇠보정법의 변경에 의한 상대적 오차의 감소폭이 커짐으로 기준값과 차이가 감소하였다. 기존의 감쇠보정법인 CTAC는 80 kVp, 10 mA의 저선량 CT를 사용하는데 있어 PET 영상의 선속경화현상이 발생한다. 이로 인해 CTAC를 이용하여 재구성된 PET 데이터는 정량화하는데 문제가 될 수 있음을 확인했다. 반면에 새로운 알고리즘이 적용된 Q.AC는 과체중 환자의 경우 80 kVp, 10 mA 정도까지는 140 kVp, 120 mA 조건으로 촬영하여 재구성한 PET 데이터 결과와 차이가 적음을 확인할 수 있었다. Q.AC를 이용한 경우 기존보다 저 선량의 CT를 이용해 PET의 재구성에 이용할 수 있으므로 환자의 피폭을 줄이는 데 큰 역할을 할 것으로 기대한다.

Positron emission tomography (PET) as the functional component of current hybrid imaging (like PET/CT or PET/MRI) seems to dominate the horizon of medical imaging in coming decades. $^{18}$Flourodeoxyglucose ($^{18}FDG$) is the most commonly used probe in oncology and also in cardiology and neurology around the globe. However, the major capital cost and exorbitant running expenditure of low to medium energy cyclotrons (about 20 MeV) and radiochemistry units are the seminal reasons of low number of cyclotrons but mushroom growth pattern of PET scanners. This fact and longer half-life of $^{18}F$ (110 minutes) have paved the path of a centralized model in which $^{18}FDG$ is produced by commercial PET radiopharmacies and the finished product (multi-dose vial with tungsten shielding) is dispensed to customers having only PET scanners. This indeed reduced the cost but has limitations of dependence upon timely arrival of daily shipments as delay caused by any reason results in cancellation or rescheduling of the PET procedures. In recent years, industry and academia have taken a step forward by producing low energy, table top cyclotrons with compact and automated radiochemistry units (Lab-on-Chip). This decentralized strategy enables the users to produce on-demand doses of PET probe themselves at reasonably low cost using an automated and user-friendly technology. This technological development would indeed provide a real impetus to the availability of complete set up of PET based molecular imaging at an affordable cost to the developing countries.

제 11 조 (회원의 의무)

제 4 장 서비스의 이용

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
