Comparison of Hip Muscle Strengthening Exercise and Stretching of Lower Extremity on Pain, Navicular Drop and Foot and Ankle Functional Ability in Plantar Fasciitis (엉덩근육 강화 운동과 하지 스트레칭이 족저근막염 환자의 통증, 주상골 하강 및 발과 발목의 기능적 능력에 미치는 효과 비교 연구)

  • Background: Plantar fasciitis is one of the most common conditions of the lower limbs. The present study aimed to compare the effect of hip muscle strengthening exercise and stretching of lower extremity in plantar fasciitis, with regard to pain, navicular drop and foot and ankle functional ability. Methods: A total of 42 patients diagnosed with plantar fasciitis were randomly assigned to the experimental group (n=21) that applied the hip muscle strengthening exercise and the control group(n=21) that applied the general lower limb stretching. The intervention period was 6 weeks. Results: The results of the experiment showed that the exercises for strengthening the hip muscle had significant effects on pain, navicular drop and foot and ankle functional ability In addition, lower extremity stretching showed improvement effects in pain, navicular drop and foot and ankle functional ability, however, the difference between the groups except navicular drop did not reach statistical significance Conclusion: Both interventions tested in this study were confirmed to be effective treatment options for patients with plantar fasciitis.

  • Taping techniques have been usually used by physical therapists long time ago, which have been considered a useful adjunct to treatment programs for a variety of musculoskeletal disorders. Also, taping techniques may be employed as effective prophylactic methods in clinical setting because of ease application and cost effectiveness. The aims of this study were to describe background information for the management of some chronic low back pain patients with/without leg pain that don't respond to conservative treatment, to demonstrate McConnell taping as successful therapeutic strategies for treating these patients, and to provide detailed application methods of McConnell taping in order that physical therapists can readily use the taping in clinical setting. This study emphasized to illustrate biomechanical benefits of McConnell taping in controlling undesirable muscle activation by decreasing mechanical loads on specific muscles. McConnell taping may be helpful for the inhibition of overactive synergist or antagonists, the facilitation of inactive synergists, the promotion of proprioception, the optimization of joint alignment, pain reduction, and unloading of irritable neural tissue. This study provides taping examples of low back pain, sacroiliac joint dysfunction and lower extremity symptoms associated with these conditions, and discusses the possible mechanisms for their successful application.

A convergence study the association between addictive smart phone use, dry eye syndrome, upper extremity pain and depression among college students (일 지역 대학생의 스마트 폰 중독사용과 안구건조증, 상지통증 및 우울간의 관계에 대한 융합 연구)

  • This study was to identify the correlation between dry eye syndrome, upper extremity pain, depression and addictive smart phone use among college students. Data were collected from 286 college students using a self-report questionnaire. 15.0% of participants have an addictive smart phone use. There were significant differences by dry eye syndrome(t=-4.38, p<.001), neck pain(t=-2.60, p<.05) and depression(t=-4.15, p<.001) according to the addictive smart phone use. Dry eye syndrome(r=.332, p<.001), neck pain(r=.143, p<.05), hand pain(r=.138, p<.05) and depression(r=.402, p<.001) were positively related to addictive smart phone use. Strategies to diminish depression, and to prevent dry eye syndrome and neck pain in college students will be an important intervention component to prevent addictive smart phone use and health problems in future studies.

The Effect of Intensive Integrative Korean Medicine Treatment of Patients with Adrenoleukodystrophy: A Case Report (부신백질이영양증 환자에 대한 한의 복합 치료의 효과: 증례보고)

  • The aim of this case report was to present the effects of intensive integrative Korean medicine treatment in a patient with lower back pain, both under extremity numbness, neck pain induced by adrenoleukodytrophy. A 49-year-old male patient was treated with Cheongpajeon-H, acupuncture, pharmacoacupuncture (Shinbaro2), chuna therapy for total 50 days of hospitalization. Lower back pain, both under extremity numbness, neck pain were assessed with a Oswestry disability index (ODI), Vernon-Mior neck disability index (NDI), numeric rating scale (NRS) and EuroQoL-5 dimension (EQ-5D). With 50 days of treatment, the pain was relieved from NRS 6 to 2. EQ-5D score increased from 0.75 to 0.779, ODI decreased from 34 to 15.56, NDI decreased from 30 to 15.56. These results show that lower back pain, both under extremity numbness, neck pain can be relieved with intensive integrative Korean medicine treatment. However, further well-designed studies are required to confirm these findings.

Effects of Elastic Band Exercise Combined with Swiss Ball Exercise on Lower Extremity Muscle Strength, Balance, and Pain in Middle-Aged Women with Osteoarthritis

  • Objective: The aim of this study is to test the effects of elastic band exercise accompanied by Swiss ball exercise on lower limb muscle strength, balance and pain in middle-aged women with osteoarthritis. Design: A randomized controlled trial. Methods: Thirty-five participants were randomly assigned to the experimental group (n=18), which performed elastic band exercise combined with Swiss ball exercise, and the control group (n=17), which performed elastic band exercise only. Both groups did a 30-minute session of exercise three times a week for eight weeks and were assessed for lower extremity muscle strength, static and dynamic balance, and pain levels before the first therapy session. All participating patients underwent outcome assessment after eight weeks of therapy without any additional treatment. Results: The experimental group made a significant increase in muscle strength of the lower extremities, static and dynamic balance ability, and pain level (p<0.05). The control group made a significant improvement in lower limb muscle strength, dynamic balance ability and pain level (p<0.05) with no such improvement in static balance ability. The exercise group made a significant increase in static and dynamic balance ability and pain level compared to the control group (p<0.05). Conclusions: These results demonstrated that both Swiss ball exercise and elastic band exercise were effective for middle-aged women with osteoarthritis and found that elastic band exercise combined with Swiss ball exercise produced more significant effects on their balance and pain.

  • Stellate ganglion block (SGB) is applicated frequently to increase the blood flow and to reduce the pain in head, neck and upper extremity. The effects of SGB are able to be estimated by clinical signs and symptoms of Horner's syndrome, skin warmth, anhydrosis, etc. The effects are also estimated by sympathetic function and the blood flow. Blood flow velocities and pulsatility indices of common carotid,d axillary, brachial and radial artery were measured by Doppler flowmeter after SGB with 1% lidocaine at C6 level. Blood velocities of all arteries were increased and pulsatility indices of all arteries were decreased. This results suggest that SGB increase the blood flow of head and upper extremity and Doppler flowmeter is a good indicator of the effects of SGB.

Immediate Effect of the Application of IASTM Using Microcurrent and a Flossing Band and on Changes in the Thickness of the Lower Extremity Fascia in Patients with Intrinsic Patellofemoral Pain Syndrome (잠재적인 무릎넙다리 통증 증후군 환자에게 미세전류를 이용한 IASTM과 플로싱 밴드 적용이 하지 근막의 두께 변화에 미치는 즉각적 효과)

  • Background: This study examined the Immediate effects of IASTM using microcurrent and the flossing band on the lower extremity fascia thickness in subjects with Intrinsic patellofemoral pain syndrome. Methods: Sixty-six subjects with patellofemoral pain syndrome were randomized into three groups (22 each in the microcurrent IASTM (instrument assisted soft-tissue mobilization) group, and flossing band group, and combined microcurrent IASTM and flossing band group) to evaluate the immediate effects of the lower extremity fascia thickness before and after intervention. The thickness of the lower extremity fascia was measured using an ultrasound machine. Using SPSS Window. 22.0, a Shapiro Wilk was conducted to test the normality of all variables; within-group comparisons were made with a paired-samples t-test, and between-group interventions were subjected to a one-way analysis of variance. Results: Changes in the thickness of the fascia in the thigh area were observed before and after intervention in all three groups. There was a significant decrease, and in the combined group, there was a significant decrease in fascia thickness compared to when the IASTM group and the flossing band group were applied separately (p<.05). Conclusion: Through this study, the effect on fascia thickness was confirmed when IASTM and flossing band intervention were combined, and it is believed that it can be used as basic clinical data for patients with knee-thigh pain syndrome.

  • The use of ultrasonography has recently been increasing in musculoskeletal diagnosis or intervention treatment. Ultrasound guided procedure offers a reliable alternative to fluoroscopy or computed tomography for lumbar medial branch block, facet joint block and peripheral nerves of lower extremity. Further, there is no exposure to radiation and additional equipment necessary for the protection against radiation is required. And ultrasound guided procedure needs smaller space than fluoroscopy guided procedure with real time images in the outpatient department. This article reviews ultrasound guided procedure at lumbar vertebra and peripheral nerves of lower extremity.

Smartphone Overuse and Upper Extremity Pain, Anxiety, Depression, and Interpersonal Relationships among College Students (대학생의 스마트폰 중독사용 정도에 따른 상지통증, 불안, 우울 및 대인관계)

  • This study aimed to survey the impact of smartphone overuse on upper extremity pain, anxiety, depression, and interpersonal relationships among college students. Subjects completed a structured questionnaire consisting of the Smartphone Addiction Inventory, the Musculoskeletal Symptom Checklist, the State-Trait Anxiety Inventory, the Beck Depression Inventory-II, and the Relationship Change Scale from May to June 2012. We analyzed the survey data from 525 responses, excluding unreturned or incomplete surveys. Data were analyzed using the $x^2$ test and t-test to determine the differences in smartphone overuse and its impact on upper extremity pain, anxiety, depression, and interpersonal relationships between two groups: the overuse and normal use groups. Moreover, Pearson's correlation coefficient was used to examine the correlation between smartphone overuse and upper extremity pain, anxiety, depression, and interpersonal relationships. The results placed 62 people (11.8%) in the smartphone overuse group. The extent of smartphone overuse was more severe among female than male college students, and longer time spent using smartphones per day was directly related to smartphone overuse. The smartphone overuse group evidenced higher shoulder pain than the normal use group did, but no differences were found in other sites of the upper extremities. State anxiety, trait anxiety, and depression were higher in the smartphone overuse group than in the normal use group. Subjects with a higher extent of smartphone overuse experienced increased state-anxiety, trait-anxiety, and depression. Moreover, subjects with higher state-anxiety, trait-anxiety, and depression scores were more likely to have poor interpersonal relationships. Therefore, early screening for smartphone overuse should be evaluated, because it can be useful in developing addiction prevention programs to improve posture, stress coping, positive mental health, and effective interpersonal relationships.

Effects of Shinchubogun-tang and Common Korean Medicine Treatment on Symptom Reduction in Patients with Intervertebral Lumbar Disc Herniation (신추보건탕 및 한방공통치료가 요추추간판탈출증 환자의 증상 호전에 미치는 영향)

  • This retrospective observational study was aimed to assess that Shinchubogun-tang (Shenzhuibujian-tang ; SBT) improves the symptoms related to lumbar disc herniation. We analyzed the medical records of 36 patients, who were satisfied with inclusion and exclusion criteria. The patients were diagnosed with lumbar disc herniation and prescribed with SBT in addition to Korean medical common treatments at Dunsan Korean Medicine Hospital of Daejeon University from January 1st, 2013 to November 30th, 2015. The effects of SBT was evaluated by comparing before and after taking SBT through 0-10 visual analogue scale(VAS) of the symptoms including lower back pain, radiating pain, and lower extremity numbness. The average period of common Korean medicine treatment before SBT prescription was 9.78 days and then the patients received SBT along with common Korean medicine treatment for 13.17 days on average. SBT plus common korean medicine treatment group significantly decreased VAS score of lower back pain from 5.40±1.62 to 3.28±1.70 (p<0.01), radiating pain from 5.60±1.42 to 2.35±1.79 (p<0.01), lower extremity numbness from 5.77±1.52 to 2.55±1.85(p<0.01). These results demonstrated that SBT might have a potential on improvement of lumbar disc herniation by reducing the symptoms of lower back pain, radiating pain, and lower extremity numbness.
