Colorimetric evaluation of white spot lesions following external bleaching with fluoridation: An in-vitro study

  • Objective: This study was performed to investigate the effects of external tooth bleaching with flouridation on the appearance of white spot lesions (WSLs) in vitro. Methods: In total, 125 bracket-bonded bovine incisor enamel blocks with artificial WSLs were randomly divided into a control group and four treatment groups (home bleaching, home bleaching + fluoridation, in-office bleaching, and in-office bleaching + fluoridation). A spectroradiometer (SR) and digital images (DIs) were used to evaluate colorimetric parameters (Commission Internationale l'Eclairage $L^*a^*b^*$) for all specimens. Color measurements were obtained before WSL formation (T1), after WSL formation (T2), and after completion of the external tooth bleaching treatment (T3). Results: The SRbased color change after bleaching was significantly greater in the treatment groups than in the control group (p < 0.05). SR-based lightness ($L^*$) and redgreen ($a^*$) values were significantly higher at T2 than at T1 (p < 0.001), with no significant changes in yellow-blue ($b^*$) values. At T3, SR-based $L^*$ values had increased while $a^*$ and $b^*$ values had decreased in the treatment groups (p < 0.001). The DI-based color difference between the sound enamel and WSL areas (DE*DI) increased significantly from T1 to T2 in all groups (p < 0.001) and significantly decreased from T2 to T3 in the treatment groups (p < 0.001). No significant differences in ${\Delta}E^*DI$ at T3 were shown between the four treatment groups (p > 0.05). Conclusions: The findings of this study suggested that external tooth bleaching with fluoridation can alleviate the conspicuity of WSLs.

치위생과와 비치위생과 학생의 치아미백에 대한 인식도 및 실태조사 (A servey on the actual conditions & recognition of tooth bleaching in female college students)

  • Some female college students have to recognize and understand the actual conditions of the tooth bleaching, and effective consultation and training to provide basic data for the purpose of investigation. this study made a survey about recognize and understand the actual conditions of the tooth bleaching and oral health knowledge and generalization methods for 649 female college student in Daejeon and Jeonbuk areas during the period between March. 15 and April. 10,2008. The results were as follows: 1. Tooth Status was found low, self-discontent respondents Status 44.2% and average 31.1%, self-contentment were 14.8%. Tooth color was average respondents were 69.8%, yellow 29.0%, White 1.7% (p=0.001, p=0.030). 2. Tooth bleaching experience has not experienced the most the military was 86.4%, to the desired Tooth bleaching for the external beauty 44.2%, confidence of smile time 37.5%. self-discontent 10.7%(p=0.000, p=0.000). 3. Tooth health status satisfaction was dental hygiene students higher than non dental hygiene, and scaling knowledge of the Tooth bleaching effect was non dental hygiene higher than non dental hygiene(p=0.039, p=0.000). 4. General knowledge for Tooth bleaching was found high 96.1%, as for the recognition route, 55.6% were through broadcast medium(p=0.025, p=0.000). 5. Medical institution chosen for Tooth bleaching treatment method appears the most preferred by 79.9% to the dental hospital dental clinic. 6. Important to consider that the choice of Tooth bleaching was Tooth bleaching duration of 37.1% cost 33%, And when Tooth bleaching hoped to be long-lasting. In this research the high recognized of Tooth bleaching treatment, but very low Tooth bleaching experience female college students for the Tooth bleaching had the wrong information. Therefore, Tooth bleaching treatment and counseling that can be used to development and education were required to provide the correct information.

Can discolored dental composites be bleached in depth?

  • Objectives: Previous in vitro studies determined the whitening effects of bleaching products on stained resin composite surfaces. This in vitro study aimed to verify the effectiveness of a whitening system on composite resin previously subjected to pigmentation, specifically examining the depth of whitening effectiveness within the material structure. Materials and Methods: A commercially available nano-filled composite resin was used. Specimens were stained using a coffee-based solution and a 10% carbamide peroxide-based gel was employed as the whitening agent. The pigment's penetration and the effect of the bleaching gel were evaluated by measuring color (CieLab values) from the outer edge to the inner part of the specimens. Color measurements were taken at 14 points, starting from 0.1 mm from the external perimeter up to 3.0 mm. Results: Analysis of variance tests showed a statistically significant difference between the Control Group (CG), Pigmentation Group, and Whitening Group. The whitening agent was effective up to 1.5 mm in depth, with Whiteness index (W) values not statistically different from those of CG up to 0.5 mm in depth. Conclusions: Whitening agents on nano-filled resin composite previously pigmented appear effective in restoring the W to values similar to the original, particularly in the superficial layers of the sample.

무수치 표백술 후 잔존 과산화수소수 제거를 위한 수종의 치수강 세척제의 효과에 관한 정량적 연구 (A QUANTITATIVE STUDY ON THE DEGRADING EFFECT OF THE VARIOUS IRRIGATING AGENTS IN THE ELIMINATION OF RESIDUAL HYDROGEN PEROXIDE FOLLOWING WALKING BLEACHING)

  • Hydrogen peroxide at high concentration during walking bleaching may cause damage to the tooth structure and to the surrounding periodontal tissues and may develop external root resorption. Clinically, It is so important to find a method of prevention or minimization of these complications. The efficacy of various chamber-irrigating agents to eliminate residual hydrogen peroxide after walking bleaching was examined and compared with water rinse in this study. Extracted human 46 premolars without any cementoenamel junction defects were treated endodontically and based with IRM to 1 mm below CEJ and totally bleached 3 times for each tooth with 30% hydrogen peroxide and sodium perborate. Upon completion of the 3rd walking bleaching procedure, the cervical portion and pulp chamber of each group of teeth were irrigated with catalase, 70% ethylalcohol, acetone, and distilled water. And then, a radicular hydrogen peroxide penetration was measured with spectrophotometer immediately after each bleaching and following treatment with each chamber-irrigating agents, and the significance of their eliminating efficacy of residual hydrogen peroxide was analyzed by Kruskal-Wallis test. The results were obtained as follows. 1. Cervical root penetration of hydrogen peroxide was increased as the bleaching procedure was repeated(P<.01). 2. The most effective irrigant that removed residual hydrogen peroxide was the catalase, and the least effective one was water rinsing (P<.01).; there was no significant difference between the acetone and ethanol group. 3. The Irrigation with antioxidant enzyme or water-displacement solutions can eliminate residual oxygen radicals from the pulp chamber effectively after walking bleaching. So, these agents can reduce adverse effects such as cervical external resorption and periapical inflammation and prevent residual $O_2$ from impeding composite resin polymerization, thus increase the bonding strength of composite resin. This, in turn reduces microleakage and discoloration of the esthetic restoration, extending its service-life.

Invasive cervical resorption: treatment challenges

  • Invasive cervical resorption is a relatively uncommon form of external root resorption. It is characterized by invasion of cervical region of the root by fibrovascular tissue derived from the periodontal ligament. This case presents an invasive cervical resorption occurring in maxillary lateral incisor, following damage in cervical cementum from avulsion and intracoronal bleaching procedure. Flap reflection, debridement and restoration with glass ionomer cement were performed in an attempt to repair the defect. But after 2 mon, more resorption extended apically. Considering root stability and recurrence potential, we decided to extract the tooth. Invasive cervical resorption in advanced stages may present great challenges for clinicians. Therefore, prevention and early detection must be stressed when dealing with patients presenting history of potential predisposing factors.

재식된 상악 중절치의 장기간에 걸친 추적례 (TOOTH REPLANTATION AFTER TRAUMATIC AVULSION: A 8-YEAR FOLLOW UP.)

  • 완전 탈구된 치아의 치료 목적은 치주인대 세포의 손상과 치수 조직의 감염에 의한 염증 반응을 예방하거나 최소화하는 것이다. 치주인대 세포의 생활력은 완전 탈구된 치아의 예후에 있어 중요한 요소인데 치주인대 세포가 생활력을 유지하는 경우 재식시 재부착을 하게 되어 최소한의 염증반응 후 신생 백악질로 회복되지만, 생활력을 상실하게 되면 심한 염증반응을 야기하게 되어 광범위한 치근 흡수와 유착이 나타나게 된다. 치근이 유착되면 특히 성장하는 어린이일 경우 치조돌기의 성장을 방해하여 저위교합을 유발하게 된다. 또한 광범위한 치근 흡수나 유착으로 인한 치아의 조기 상실은 치조골의 흡수와 인접치의 지속적인 이동을 야기하여 기능적, 심미적인 문제를 발생케 한다. 본 증례는 상악 우측 중절치의 완전 탈구 후 재식한 후 항생제 투여 및 장기간의 수산화 칼슘 적용을 통해 9년에 걸쳐 기능적, 심미적으로 양호한 결과를 얻었기에 이에 보고하는 바이다.
