The Applications of the Duplex Stainless Steel as Hyperthermia Materials

  • The use of Duplex stainless steel as a thermo-implant categorizes into two clinical applications: hyperthermia and thermal ablation or destruction. The goal of hyperthermia is to destroy the heat-sensitive abnormal cells and minimize normal cell death maintaining heat between $42^{\circ}C$ and $46^{\circ}C$. Thermal ablation takes place when the local tissue temperature increases greater than $46^{\circ}C$. This elevated temperature denatures protein irreversibly resulting cellular death. The author introduced several thermo-implants such as thermo-rod, thermo-stent, thermo-coil and thermoacupuncture-needle. Those thermo-implants are made of duplex stainless steel which can produce regulated heat by itself within an induction magnetic field. Thermal ablation characteristics of the thermo-rod on tumor hyperthermia depend on configurations of the thermo-rods and the magnitude of the induction magnetic strength. The exothermic properties of the thermo-implants can be characterized using the calorimetric test and the heat affected zone(HAZ) analyses in vitro. Thermal radiation studies using thermo-coils and thermo-stents show the capability of the occlusion of animal blood vessels and inhibiting the proliferation of the abnormal smooth muscle cell growth and inflammatory cell reactions maintaining the heat between $42^{\circ}C$ and $46^{\circ}C$ minimizing a normal cell death in the study on external iliac artery of the New Zealand White (NZW) rabbit. Thermal stimulation study using thermo-acupuncture needles suggests the potential applications of the automated acupunctural therapies.

신상부 복부대동맥류의 하이브리드 수술 - 1예 보고 - (Hybrid Surgery for Supra-renal Abdominal Aortic Aneurysm - A case report-)

  • 확장성 심근증이 있던 70세 남자 환자가 좌측 신장결석으로 입원하여 검사 중 신상부 복부대동맥류가 발견되었다. 환자의 상태가 좋지 않아 혈관 내 스텐트를 이용한 하이브리드 수술을 시행하였다. 수술은 비뇨기과에서 좌측 신장결석을 제거 후에 복부대동맥의 내장동맥과 양측 신동맥등 복부대동맥의 가지혈관의 근위부를 결찰하고 이어 좌측 외장골동맥 근위부로부터 2개의 Y-그라프트를 이용하여 역행성 우회수술로 가지혈관들의 재관류를 하고는 복부대동맥에는 스텐트 그라프트를 설치하였다. 이에 저자들은 신상부 복부대동맥류 환자의 하이브리드 술식을 치험하였기에 보고하는 바이다.

Deep Vein Thrombosis Due to Hematoma as a Rare Complication after Femoral Arterial Catheterization

  • Hematoma is quite a common complication of femoral arterial catheterization. However, to the best of our knowledge, there have been no previous studies regarding deep vein thrombosis (DVT) caused by compression of a vein due to a hematoma. We report a case of a hematoma developing after femoral arterial catheterization and causing extensive symptomatic DVT. A 59-year-old male was seen in our Emergency Department with right lower leg swelling 15 days after coronary stent implantation performed using right femoral artery access. Computed tomographic (CT) scanning revealed a large hematoma (45 mm in its longest diameter) compressing the common femoral vein and with DVT from the right external iliac vein to the popliteal vein. Due to the extensive DVT involvement, we decided to release the compressed common femoral vein by surgical evacuation of the large hematoma. However, even following evacuation of the hematoma, as the DVT did not resolve soon, further mechanical thrombectomy and catheter-directed thrombolysis were performed. Angiography then showed nearly resolved DVT, and the leg swelling was improved. The patient was discharged with the anticoagulation medication, warfarin.

Feasibility and Safety of Robotic Surgery for Gynecologic Cancers

  • Background: To determine surgical outcomes, perioperative complications, and patient outcomes in gynecologic cancer patients undergoing robotic surgery. Materials and Methods: Surgical outcomes, including docking time, total operative time, console time, estimated blood loss (EBL), conversion rate and perioperative complications were retrospectively reviewed in 30 gynecologic cancer patients undergoing robotic surgery. Patient outcomes included recovery time and patient satisfaction, as scored by a visual analogue scale (VAS) from 0-10. Results: The operations included 24 hysterectomies with pelvic lymphadenectomy (PLD) and/or para-aortic lymphadenectomy, four radical hysterectomies with PLD, and two radical trachelectomies with PLD. Mean docking time was $12.8{\pm}9.7min$, total operative time was $345.5{\pm}85.0min$, and console time was $281.9{\pm}78.6min$. These times were decreased in the second half of the cases. There was no conversion rate. Three intraoperative complications, including one external iliac artery injury, one bladder injury, and one massive bleeding requiring blood transfusion were reported. Postoperative complications occurred in eight patients, most were minor. Only one patient had port herniation that required reoperation. Mean hospital stay was $3.5{\pm}1.7days$, and recovery time was $14.2{\pm}8.1days$. Two-thirds of patients felt very satisfied and one-third felt satisfied; the mean satisfaction score was 9.4 +0.9. Two patients with stage III endometrial cancer developed isolated port site metastasis at five and 13 months postoperatively. Conclusions: Robotic surgery for gynecologic cancer appears to be feasible, with acceptable perioperative complication rate, fast recovery time and high patient satisfaction.

개에서 다중나선형 CT촬영에 의한 동맥혈전색전증의 평가 (Assessment of Systemic Arterial Thromboembolism with Multi-Slice Spiral CT in a Dog)

  • 호흡곤란과 후지마비 및 발등부위의 괴사를 나타낸 수컷 진돗개가 전북대학교 동물의료센터에 내원하였다. 초기 검사에서 심장 사상충 감염이 확인되었다. 흉부방사선 외측상에서 폐동맥의 확장과 후엽의 간질패턴, 그리고 복배상에서 주 폐동맥의 뚜렷한 확장등이 관찰되어 심장사상충증을 뒷받침하였고, 복부 외측상 및 복배상에서 복부세부음영 소실이 관찰되어 복수를 의심하였다. 초음파상에서 복수와 불규칙한 간변연 그리고 신장양극의 피질에서 쐐기모양의 국소적 고에코상을 관찰하였으며 복부대동맥에서 분지하여 주행하는 바깥장골동맥의 3상형 동맥파형이 분지부 근위에서 관찰되었으나 이 후 대퇴동맥의 파형은 확인되지 않았다. 혈액화학검사에서 백혈구증다증, 빈혈, 혈색소 뇨, 고빌리루빈혈증, 저 알부민혈증, 전해질불균형, 그리고 간장 및 신장효소치의 상승등이 관찰되어 광범위한 장기의 손상이 의심되었다. 특히 글루코스는 정상적인 전지와 마비를 보이는 후지에서 비교한 결과 후지의 글루코스 수치가 현저하게 낮았다. 전산화단층촬영술 후 3차원으로 재구성한 영상을 이용하여 후지 마비의 원인으로 여겨지는 후지 동맥의 혈전색전증과 폐동맥혈전색전증 및 신장경색 등을 확인하였다. 예후불량으로 판단되었으며 실험적 중재적 방사선술을 시도하였으나 마취에서 깨어나지 못했다. 3차원 재구성 CT 영상은 색전증의 빠르고 정확한 진단에 유용하며 효과적인 치료 계획을 수립하는데도 큰 도움이 된다고 판단된다.
