Prognostic Factors Affecting Surgical Outcomes in Squamous Cell Carcinoma of External Auditory Canal

  • Objectives. Carcinomas of the external auditory canal (EAC) are rare, and management remains challenging. Previous studies seeking prognostic factors for EAC cancers included cancers other than carcinomas. In this study, we analyzed the treatment outcomes of, prognostic factors for, and survival rates associated with specifically squamous cell carcinoma (SCC) of the EAC. Methods. A retrospective review of 26 consecutive patients diagnosed with SCCs of the EAC in a 10-year period was performed in terms of clinical presentation, stage, choice of surgical procedure, and adjunct therapy. Overall survival (OS) and recurrence-free survival (RFS) were calculated and univariate analysis of prognostic factors was performed. Results. The median age of the 26 patients with SCCs of the EAC was 63 years (range, 40 to 72 years), and 16 males and 10 females were included. According to the modified University of Pittsburgh staging system, the T stages were T1 in 11, T2 in six, T3 in four, and T4 in five cases. The surgical procedures employed were wide excision in three cases, lateral temporal bone resection (LTBR) in 17, and extended LTBR in four, and subtotal temporal bone resection in two. Two patients underwent neoadjuvant chemotherapy, and two underwent adjuvant chemotherapy. One patient received preoperative radiation therapy, and eleven received postoperative radiation therapy. Of the possibly prognostic factors examined, advanced preoperative T stage and advanced overall stage were significant predictors of RFS, but not of OS. Conclusion. The advanced T stage and overall stage were associated with decreased survival after surgical treatment in patients with SCC of the EAC, highlighting the importance of clinical vigilance and early detection.

The cover of an ear thermometer probe as a split-thickness skin graft mold in external auditory canal reconstruction

  • Maintaining the patency of the external auditory canal (EAC) during reconstruction is important because of its physiological role in hearing and immunological protective functions. The curved shape of the EAC presents a challenge when performing a skin graft. One of the key points for a successful skin graft is to ensure compression on the wound bed, and many novel methods, including prefabricated ear molds, have been reported for this purpose. In this study, we present a case of a skin graft performed to reconstruct a skin defect following excision of actinic keratosis in the EAC, using the cover of an ear thermometer probe as a mold for the graft to match the curvature of the EAC. This is an economical and practical method for secure compression dressing of a skin graft in the EAC.

외이도용적에 따른 외이도공명의 변화 (Resonance Changes in the External Auditory Canal Associated with the Ear Canal Volume)

  • The external ear generates resonance gain because of anatomical characteristics. The ear canal resonance is influenced by the length and volume of the ear canal, the pinna, the concha cavity, the body trunk, and the speed of sound wave. This study is focus on the influence of the volume of ear canal. 17-healthy-adult (32 ears) were participated. They did not have any medical and ear disease history. The maximum resonance frequency of the ear canal was 2675 (${\pm}265$) Hz at azimuth $0^{\circ}$ and 2784 (${\pm}268$) Hz at azimuth $45^{\circ}$. The resonance gain was 18.1 (${\pm}3.9$) dB at azimuth $0^{\circ}$ and 17.9 (${\pm}3.8$) dB at azimuth $45^{\circ}$, respectively. The ear canal volume was 0.78 (${\pm}0.2$) cc and 1.32 (${\pm}0.8$) cc including static compliance. The ear canal resonance was changed depending on the ear canal volume. It was also statistically correlated at azimuth $0^{\circ}$ (p=0.038) and $45^{\circ}$ (p=0.013), respectively. The resonance gain was not correlated with the ear canal volume. The change of resonance frequency according to the ear canal volume will be useful information in the field of audiological rehabilitation especially for hearing aids fitting. In addition, we expected this study can provide the basic information for the study of the external ear resonance characteristics.

A Case of Engorged Female Hard Tick in the External Auditory Canal of an Infant

  • An oval-shaped mass with a smooth surface was found in the ear canal of a 22-month-old male infant. Although the mass appeared to be almost entirely blocking the ear canal, it was successfully removed under a surgical microscope without general anesthesia at the outpatient department. Under an optical microscope with hematoxylin and eosin staining, the specimen was observed to have a cuticle with a serrated surface and a pore canal, as well as parts of the capitulum, salivary glands, muscles, midgut, and the legs. The specimen was identified as a hard tick of the family Ixodidae, based on gross and histological findings. This paper is the first report in Korea on the diagnosis and treatment of a tick bite in the ear canal.

DICOM 영상과 설계 모델링을 융합한 외이도의 형태적 변화 관찰 연구 (An Observational Study on the Morphological Changes of the External Ear Canal by Converging DICOM Imaging and Design Modeling)

  • DICOM 영상은 인체의 진단과 치료에 중요한 역할을 하고 있으며 설계모델링은 목적에 따라 형상을 3차원으로 계획하는 기술이다. 본 연구는 이러한 두 가지 기술을 융합하여 외이도의 형태적 변화에 대한 횡단면, 부피, 표면적의 관계를 관찰하고자 하였다. 실험은 인체의 단면 획득 의료영상기술을 적용하여 19귀의 외이도를 추출한 3차원 형상을 스테레오리소그래피, 3-매틱(matic) 프로그램으로 센터라인 생성 및 분할 기술을 적용하였다. 그 결과 외이도의 횡단면 구조는 타원형(38.5%), 반원형(28.2%), 혼재형(17.9%), 네모형(10.2%), 주름형(5.1%)등 다양한 형태가 나타났다. 또한 외이도 길이가 길수록 위상별 횡단면 면적은 크게 나타났으며 부피와 표면적은 고막방향으로 갈수록 감소하였지만 그 감소율은 상대적으로 낮게 나타났다. 이는 외이도의 형태가 고막방향으로 갈수록 불규칙한 구조로 되어 있음을 나타냈다.

3차원 의료영상을 이용한 인체 외이도 특징에 관한 연구 (A Study of the Characteristics of the Human External Auditory Canal Using 3-Dimensional Medical Imaging)

  • 의료용 디지털 영상 및 통신 표준과 3차원 의료 영상 프로그램을 이용하여 외이도의 해부학적 특징들을 비교하였다. 실험은 연령과 성별이 다른 63귀의 영상을 이용하여 외이도의 횡축 길이, 내부 직경 둘레, 상하부 굴곡 각도를 측정한 후 비교하였다. 실험 결과 외이도 형태는 연령과 성별 뿐 만 아니라 동일인의 좌.우 에서도 다르게 나타났다. 성별 비교에서 남자 35귀, 여자 28귀에 대한 평균 길이는 남자가 4.75mm 길게 나타났다. 외이도 직경 둘레는 하부 굴곡 각도를 중심으로 내측이 외측 보다 평균 37.2% 감소된 형태로 나타났다. 상하부 굴곡 각도는 상부가 평균적으로 $25.7^{\circ}$ 높았으나 4귀에서 하부 각도가 높고, 연령이 낮은(4~14세) 8귀에서 상하부 굴곡 각도 차이가 크게 나타났다. 이는 성장하면서 외이도 굴곡 모양이 변화됨을 나타냈다. 본 연구는 기존 귀본 채취를 통한 간접 측정과 물리적 방법에 의한 직접 측정과 비교하여 안전성과 정밀성을 높일 수 있는 방법이다. 이는 3차원 의료 영상 표현 기술이 실제에 가깝게 표현할 수 있는 기술 향상 때문이며 더 발전하여 외이도 형태의 표준화 연구에도 활용될 수 있는 측정 방법이라 판단된다.

만성 화농성 중이염에 동반된 이진균증 환자 한방 치료 1례 (One Case of Otomycosis with Chronic Suppurative Otitis Media Treated with Korean Medical Treatment)

  • 본 연구는 대전대학교 대전한방병원 한방안이비인후 피부과에 만성 화농성 중이염에 동반된 이진균증으로 입원 및 통원 치료를 받은 환자 1례를 보고한다. 본 증례의 환자는 상기 증상으로 한방 단독 치료를 시행한 환자로서 침, 약침, 전침, 뜸, 한약, 사혈, 습식 부항 및 한방 외용제 치료를 시행하였다. 초진 시 호소한 좌측 귀의 소양감, 통증, 이명, 충만감 및 농성 이루는 호전된 상태로 퇴원하였고, 이후 통원 치료를 통하여 잔여 증상의 호전 및 이진균증의 소실도 확인되었다. 본 연구는 이진균증 치료에 일차적으로 적용하는 항진균제의 사용 없이 한방 단독 치료로 호전을 보였다는 점과 항진균 효과를 위해 국소 한방 외용제를 사용하였다는 점에서 이후 이진균증의 한방 치료 효과를 평가하는 추가 연구에 유의미한 자료로 사용될 것으로 사료된다.

생소한 위치에 발생한 기저양 편평세포암종 (Basaloid Squamous Carcinoma of Unusual Sites)

  • Basaloid squamous carcinoma (BSC) is an uncommon aggressive variant of squamous cell carcinoma with a predilection for hypopharynx, tongue base, and larynx. We present 5 cases of BSC of unusual sites, each from maxillary sinus, external auditory canal, submandibular gland, tonsil, and nasopharynx. Only a few cases arising in these sites have been reported to date. Patients included 3 men and 2 women with the age range of 45-69 years (mean, 56.4 years). Microscopically, the tumors were characterized by solid lobules and nests of ovoid basaloid cells with abundant desmoplastic stroma. Comedonecrosis, peripheral palisading of tumor cells, trabecular pattern, and rosette-like arrangement were commonly observed. Tumor cells had scanty cytoplasm and their nuclei were ovoid, relatively uniform, and hyperchromatic. In two cases, concomitant squamous cell carcinoma in situ was identified. Immunohistochemical stains revealed that tumor cells were strongly positive for pancytokeratin and negative or weakly positive for p63. Being aware of BSC that can arise from unusual sites would help diagnose correctly and treat properly this rare and distinct clinicopathologic entity.
