Relationship between Medical Service Experience and Subjective Health Awareness of Patents with Industrial Accident (산업재해 환자의 의료서비스 경험과 주관적 건강 인식과의 관계)

  • Objectives: This study intends to analyze the relationship between medical service experience and subjective health awareness by using data from Panel Study of Worker's Compensation Insurance(PSWCI). Methods: Tte χ2-test was performed to investigate subjective health awareness and medical service experience relevance. Logistic analysis was performed to analyze the influencinge factors. Results: The subjective health awareness scored "bad" in"'lack treatment period" compared to "adequate treatment period" in medical service experience (OR = 2.603 [95% CI = 1.666-2.555]). Conclusions: To improve the subjective health awareness of patients with industrial accidents, the industrial accident compensation and medical care approval system should be improved, and long-term industrial accident insurance policies should be developed to accommodate direct and indirect medical services.

Exploration into Pre-service Early Childhood Teachers' Experience of Participation in Health Promotion Activities and Its Meaning (건강증진활동에 참여한 예비유아교사의 경험과 의미 탐색)

  • Objective: This study aimed to explore the experience and meaning of pre-service early childhood teachers who practiced health promoting activities for themselves. Methods: Research participants were 115 pre-service early childhood teachers enrolled in the health education for children course at one college in G region. The reports of activity records of their eight-week long health promotion activities were collected as qualitative data followed by content analysis being implemented. Results: First, the experience of the health promoting activities the participants took part in was categorized into 'Early phase of experience: A mix of worries and anticipation, and success and failure', 'Mid phase of experience: Being motivated by physical changes and records', and 'Late phase of experience: Continuous execution through habituation.' Second, the meaning of health promotion activities the participants took part in was categorized into 'A great opportunity for introspection', 'A meaningful start of college life', and 'A valuable chance to consider desirable roles of a teacher for early childhood health education.' Conclusion/Implications: The results of this study suggest that pre-service early childhood teachers should pay close attention to their health management, and that contents that emphasize the importance of teachers' health should be included in health education.

The Complex Characterization Analysis of the Risk Awareness Affecting an Accident Experience of Quick Service Workers (퀵서비스 종사자의 사고 경험에 영향을 미치는 안전의식의 복합적 특성 분석)

  • The purpose of this study was to investigate the effect of risk awareness on injury experience in quick delivery service workers. Risk awareness has complicate characteristics such as its level of worker and worker's decision about the level of other's risk perception. Data were collected by interview survey with structured questionnaire about injury experience, risk perception, work characteristics, and socio-demographic characteristics of quick delivery service workers by cross sectional survey design in 2012. The sample size was 120 respondent of quick delivery service workers. Statistical method for this study was hierarchical logistic regression method with 3 different models using socio-demographic characteristics and work characteristics and risk perception, etc. The difference between the level of risk perception of quick delivery service and other's was statistically significant effect on the experience of injury. Especially the higher the level of risk perception of quick delivery service workers is than other's, the lower the injury experience of quick delivery service worker is. The limitation of this study can be found in survey design. The future study for investigation of mechanism of the combined effect of risk perception of quick delivery service workers and others on injury experience.

Nursing Service Experience according to Patients' Characteristics and Relation with Satisfaction of Hospitalization and Perception of the Health System (간호사 서비스 경험의 의료기관 이용자 특성별 차이와 입원만족도, 보건의료 제도 인식과의 관계)

  • This study aimed to elucidate the differences in nursing service experience according to the participants' characteristics and experience of hospitalization and to identify the relation between nursing service experience and perceptions of the health system. It analyzed the data from a "Survey of health care service experience" conducted among 574 participants who had been hospitalized. The results showed that the mean nursing service experience of participants who had never used other hospitals before hospitalization with over ten days of hospital stay was higher compared with the other participants. Moreover, those who used other hospitals before hospitalization reported lower nursing service experience scores. In the classification based on the change of the inpatient room, the difference in the nurse service experience according to the number of hospitalization days was significant only in patients who had no change in the inpatient room in the multi-person room. The nursing service experience scores were positively related to general satisfaction with the hospitalization experience, health system satisfaction, and trust in the health system. This finding implies that the nursing service experience should be interpreted by separating the inpatient experience, and the strategies to improve the nursing service are needed. In addition, the survey should include items related to the characteristics of medical institutions such as nurse staffing, to identify factors affecting the experience of nursing services.

Review of Reliability and Validity of Medical Service Experience Survey: Focused on the Differences by Type of Medical Institutions (의료서비스경험조사의 신뢰도 및 타당도 검토: 의료기관 특성별 차이를 중심으로)

  • Background: The efforts to build more "people centered," "patient centered" health system has been emerging all over the world. Aligning with it, the Korean government is conducing the survey called "Medical Service Experience Survey (MSES)." There are critics, however, that MSES is not scrutinizing the medical experiences of patients in various healthcare settings. For this reason, this study aims to perform an empirical analysis of the differences in answers of patients responding to various healthcare settings. Methods: There are two steps in this study. First, explanatory analysis is conducted to compare the tendency of statistical concentration on questionnaires by divided healthcare settings. Second, confirmative analysis is carried out to evaluate the construct validity, reliability, and discriminant validity of the questionnaire in each healthcare setting. The raw data of MSES, which was conducted in 2020 by the Ministry of Health and Welfare in Korea and the Korean Institute for Health and Social Affairs is used. Results: As a result of exploratory factor analysis for all outpatients, the items were classified into four factors statistically: "doctor experience," "nurse experience," "outpatient service experience," and "patient satisfaction." It was confirmed that the reliability of all factors extracted was secured. However, for patients who visited hospitals, questionnaires related to personal privacy, such as "experiences on medical staffs considering physical exposure" or "experiences related to personal information exposure," were answered in conjunction with items of "nurse experience." Besides, patients responded that administrative elements of medical services, such as "experiences of comfort in medical institutions" and "experiences of satisfactory administrative services," were related to the items of "nurse experience." The answers of patients who visited traditional medical hospitals and clinics about "doctor experience" and "nurse experience" were not discerned statistically, and the answers to "doctor experience," "nurse experience," and "medical institution experience" were entangled with the responses of patients who visited dental hospitals and clinics. On the other hand, as a result of the confirmatory factor analysis, it was found that the inquiries of MSES generally had intensive validity. Conclusion: The collection of objective and scientific data is the prominent component to enlighten the patient-centered healthcare system alongside with change of the worldwide paradigm of measuring the healthcare system performance as follows the transition of perspective of health care from provider-centered to patient-centered. This study empirically shows that the patient experience can vary as the healthcare settings. Furthermore, to make an advance in measuring the experience of patients with medical services, this article proposes the deliberate consideration of the different kinds of healthcare settings and articulate design of the survey.

Effect on Infantile Oral Health Knowledge in Pregnant Women (임부의 영유아 구강보건지식에 미치는 영향)

  • The purpose of this study was to identify influencing factors associated with infantile oral health knowledge among pregnant women. The participants were 300 pregnant women who agreed to participate in this study. The data were analyzed with descriptive statistics, t-test, ANOVA, Pearson correlation, and simultaneous multiple regression using the SPSS 21.0 ver program. Findings of the study can be summarized as follows. First, the infantile oral health knowledge of primigravida total score was $28.80{\pm}3.02$, and multigravida total score was $30.23{\pm}2.94$. Second, infantile oral health knowledge different according to education, experience of delivery, experience of oral health education and need of oral health education, Third, a positive correlation existed between need of oral health education, experience of delivery, education, and experience of oral health education. Forth, the predictors that affect the infantile oral health knowledge were experience of delivery, need of oral health education, experience of oral health education, education,. Consequently, it was necessary to encourage primigravida to take part in infantile oral health education program and oral health projects.

A Study on the Relationship between Patient Safety and Patient-Centeredness in Hospitals (의료기관 환자안전과 환자중심성 간 관계 연구)

  • Purposes: This study purposed to analyze the relationship between patient safety and patient-centerendess. Methodology: The comprehensive scores from patient safety assessment program and patient experience survey conducted by Health Insurance Review & Assessment Service were used as independent variables and dependent variables. This study analyzed the relationship between 4 patient safety-related areas(i.e. risk standardized readmission ratio, intensive care unit, preventive antibiotic, the drug evaluation) and 6 patient experience areas(i.e. nurse services, doctor services, medication & treatment, hospital environment, patient's right, overall experience) by using robust regression analysis. Findings: According to results, the score in 'patient's right' and 'risk standardized readmission ratio' areas were found to have a significant relationship, and 'overall experience' and the 'preventive antibiotic' areas. The ratio of senior beds and specialists was a general characteristics of hospitals that had a significant relationship on patient experience assessment. Practical Implication: The relationships between patient safety and patient experience assessment were varied depending on areas. Further study is needed to make clear the supposed relationship.

Health Care System Recognition and Influential Factors of Health Care Experiences (의료서비스경험자의 보건의료제도 인식도와 영향 요인)

  • Objectives : This study was performed to investigate health care system recognition and influential factors using the data from the "2017 Health Care Experience Survey". Methods : Data on 7,000 participants in the Health Care Experience Survey were drawn and statistically examined using a t-test, ANOVA, and multiple regression analysis. Results : First, the significant factors of health care service satisfaction were education, income, region, chronic diseases, unmet medical needs, satisfaction with doctors and institutions, and the health care system's reliability and importance. Second, the influential factors of willingness to pay additional health insurance premium were age, occupation, income, health status, chronic diseases, unmet medical needs, satisfaction with health care institutions, limit to utilization of medical services, necessity of health care reform, and the health care system's reliability, satisfaction, importance. Conclusions : Since the additional burden for improving the health care has been negative to the socially disadvantaged, there should be efforts to provide stable health care funding for financial stability of the health insurances by considering public opinions and reaching social consensus.

Outpatient Health Care Satisfaction and Influential Factors by Medical Service Experience (의료서비스경험에 따른 외래 의료서비스 만족도와 영향 요인)

  • Objectives: This study investigated outpatient health care satisfaction and influential factors using data from the "2018 Health Care Experience Survey". Methods: Data on 6,705 participants in the Health Care Experience Survey were statistically examined using a t-test, ANOVA, and multiple regression analysis. Results: First, the level of satisfaction among outpatients was generally positive, and satisfaction among older people was high. Second, the significant factors of outpatient overall satisfaction were gender, consultation time, waiting time, reason for choosing institution, satisfaction with doctors, nurses, institutions, and the therapeutic result. Third, influential factors of intention to recommend were education, institution type, reason for choosing institution, satisfaction with doctors, nurses, institutions, and the therapeutic result. Conclusions: In order to improve the overall satisfaction of outpatients, the treatment method should be improved. For example, the doctor should devote more time for and engage in sufficient conversation with the patient, the nurse should be polite, and patients should be given easy-to-understand explanations.

The Influence Factors of Adolescents' Suicide Dimension (청소년들의 자살차원에 미치는 영향요인)

  • The purpose of this study is to find out the effects of the socio-demographic characteristics, self-recognition factors, and health behavior factors of Korean adolescents on suicide dimension, and grasp the effects of suicide thinking of adolescents on attempting suicide. This study used the eighth primitive data of the "Web-based Survey on Adolescents Health Behavior(2012)", and targeted 72,228 actual test respondent(93.8%). In conclusion, risk factors that greatly influence the suicide dimension of adolescents can be summarized into level of happiness, level of stress, depression, experience drinking, experience smoking, experience with drugs, suicide thinking, etc. and considering these risk factors, a preparation for preventing adolescents' suicide should be established, and along with a regular education, developing and expanding a treatment program for preventing recurrence afterwards of adolescents who have experience attempting suicide are necessary.
