기공운동 제항의 효과 분석 및 활용 방안 - 중국 문헌을 중심으로 - (Health Effects and Utilization of the Levator Ani Muscles Training in Health Qigong Exercise - based on Chinese Literatures -)

  • Objectives : The Levator Ani Muscles Training(LAMT) is one of the well known very effective prevention and treatment method of some specific diseases in the anal organs, digestion system, urinary organs and genital organs, which means LAMT could be considered as an independent exercise. Methods : Chinese databases were searched and found one good source named CNKI(China National Knowledge Infrastructure) where there were downloaded 154 articles, out of which were finally selected 82 used for the introduction of the historical background, health-effectiveness, working mechanism, training methods, etc. Results : LAMT as an independent exercise, is considered very effective for the prevention of diseases in the anal organs, e.g. hemorrhoids, anal fissure, rectal prolapse; urinary organs, e.g. urinary incontinence, frequent micturition; digestion system, e.g. visceroptosis, diarrhea, chronic enteritis; genital organs, e.g. prostatitis, impotence, premature ejaculation, feminine vaginal relaxation, sexual indifference, etc. It is also told LAMT is effective for coronary heart disease, hyperyension and varicose veins of lower extremity to a certain degree. LAMT is also very similar to the Kegel exercise in couple of aspects. They are equally effective for the urinary incontinence and impotence. Conclusions : LAMT as an independent exercise has developed and become more and more health-effective, especially through the combination with the Qigong exercise, and thus many training methods have been introduced. There are not many scientific research and development with very limited accomplishments even in China. It is strongly required to strengthen the research and development activities so that LAMT will become one of the very effective natural healing soonest possible.

트레드밀 운동이 뇌졸중 환자의 폐기능과 보행능력에 미치는 효과 : 메타분석 (Effects of Treadmill Exercise on Pulmonary Function and Gait Capacity in Stroke Patients : A meta-analysis)

  • Purpose : This study aimed to conduct a comprehensive meta-analysis to evaluate the effect size for pulmonary function and gait capacity of treadmill exercise in stroke patients. In addition, we aimed to examine the current status of treadmill interventions and compare the effect sizes of treadmill training to provide evidence-based practice for future development and application. Methods : The meta-analysis study was conducted using research articles that applied treadmill exercise to stroke patients and were published between January 2005 and February 2020. For a systematic meta-analysis, we conducted a search using the PICOS framework and selected 25 domestic stroke- and treadmill-related studies. The Comprehensive Meta-Analysis program was used to calculate the effect sizes for lung function and gait capacity (6-minute walk test and 10 m walk test). As Cohen's d has a tendency to overestimate the effect size, we used Hedge's g to increase the accuracy in computing the effect size. Results : Based on the results of the meta-analysis, the total effect size of treadmill exercise was 0.535, which was statistically significant, with a medium effect size (p<0.05). The effects of treadmill exercise in stroke patients were divided into dependent variables, namely pulmonary function (0.372) and gait capacity (0.584). In addition, for gait capacity, the effect sizes were evaluated for both the 6-minute walk test (0.756) and the 10 m walk test (0.514). Conclusion : This study provides objective evidence of the effectiveness of treadmill-based gait training programs. We hope that the results of this study will help support the development and implementation of treadmill-based gait training in stroke patients. Treadmill training is expected to improve not only pulmonary function, but also the gait capacity of stroke patients. Long-term investigations on the effects of treadmill training in stroke patients are necessary.

Isokinetic Exercise에 의한 중풍 재활교육이 인체기능회복에 미치는 영향 (Effect of Rehabilitation Education Using Isokinetic Exercise on Physical Function Recovery)

  • This study was performed to figure out effects of stroke rehabilitation on education using isokinetic exercise on physical function recovery. It is considered isokinetic exercise will playa primary role in muscle strength, ROM of joint, and body balance recovery for stroke rehabilitation and so far can be used as a basic references to increase the health of all people. The study consisted of 42 stroke patient(21 training group, 21 control group) diagnosed as cerebral hemorrhage from Oriental Rehabilitation Department of Kyung Hee University. Upper extremity and lower extremity exercise was performed in the training group using isokinetic ergometer. The recovery of physical function(muscle strength, ROM of joint, body balance) data between the two groups were compared and ana lysed by paired t-test are as followed. 1. Muscle testing record showed increased in the strength of elbow flexion, knee flexion, knee extension, ankle extension of the training group com paired to control group(p < .05). In the measurement of ROM, however other parts of the body motion showed no significant changes, only shoulder extension of the training group was increased(p < .05). 2. Body balance increase was highly significant in all training group compaired to control group(p<001). Based on these findings, stroke rehabilitation education with isokinetic ergometer showed available effects on recovery of physical function rehabilitation program with isokinetic exercise will play a primary role in the recovery of physical function of stroke or brain injury patients as well as to promote the health of all people.

유산소운동과 diacylglycerol 섭취가 비만인의 혈중 지질성분 및 안정 시 대사량의 변화에 미치는 영향 (Combined Effects of Aerobic Exercise Training and Diacylglycerol Ingestion on Blood Lipid Profiles and Resting Metabolic Rate Changes in Obese Man)

  • 본 연구는 체지방을 감소시키는데 효과적이라는 diacylglycerol (DG) 섭취와 유산소운동이 체구성비의 변화 및 혈중지질 개선에 미치는 영향을 규명하고 안정 시 대사량 변화를 관찰함으로써 임상적 및 대사적 측면에서 비만 개선 효과를 규명하는데 그 목적이 있었다. 본 연구의 대상은 의학적으로 다른 질병을 동반하지 않은 비만 남자 대학생으로서 연구의 의의를 충분히 이해하고 자발적으로 참여 의사를 밝힌 사람으로 선정하였으며, 체지방 측정 후 25%미만인 자와 흡연자는 대상에서 제외하였다. 모든 피험자들은 기본검사와 사전검사가 끝난 후 세가지(D, E, DE) 집단에 6명씩 무선 배정하였다. 첫 번째 실험군(D)은 diacylglycerol 섭취 조건으로 12주 동안 diacylglycerol을 섭취하였고, 두 번째 실험군(E)은 12주 동안 러닝 트레이닝을 실시하였다. 마지막 세번째 실험군(DE)은 12주 동안 diacylglycerol 섭취와 유산소 트레이닝을 실시하였으며, 본 연구결과 모든 그룹에서 12주 동안의 그룹별 처치가 진행됨에 따라 체지방이 감소하는 경향을 보였다. 안정 시 대사량은 D그룹을 제외하고 모든 그룹에서 증가하는 경향을 보였으며 운동과 다이아실글리세롤 섭취에 따라 콜레스테롤의 감소가 일어났다. 추후 카테콜라민, CAMP, HSL, Insulin 등과 같은 지방대사와 관련된 항목들에 대한 추가적인 분석이 이루어짐으로써, DG섭취와 트레이닝의 비만개선효과를 보다 신뢰성 높게 예측할 수 있으리라 본다.

감각운동훈련이 노인의 균형 및 보행에 미치는 영향 (The Effects of Sensorimotor Training on Balance and Muscle Activation During Gait in Older Adults)

  • Purpose: The purpose of this study was to evaluate the effect of 6-week sensorimotoor training on balance ability and lower limb muscle activation during gait in older adults. Methods: Twenty-four community-dwelling older adults between 65 and 90 years of age participated in this study. In the older adults of the experimental group (n=12), the sensorimotor training program was performed bare feet. General exercise was performed in the control group (n=12). Then, both groups exercised three times a week for forty minutes over a 6-week period. Balance ability was evaluated by One leg stand (OLS) test for determining the static balance and Timed Up & Go (TUG) test for determining the dynamic balance. In addition, muscle activation of the dominant lower limb tibialis anterior and gastrocnemius medialis muscles were measured by surface EMG to evaluate muscle activation during gait. Results: A significant improvement was seen in the one leg standing (OLS) time after exercise in both the sensorimotor training (SMT) group and general exercise (GE) group (p<0.05) and the change in the SMT group was greater than that in the GE group (p<0.05). A significant reduction was seen in the Timed Up & Go (TUG) test time after exercise in both the SMT group and GE group (p<0.05). Also, a significant increase was seen in muscle activation of tibialis anterior muscle after exercise in the SMT group (p<0.05), but no such significant increase was seen in the GE group (p>0.05). Conclusion: These results suggest that sensorimotor training improves the balance in older adults and has a more positive effect on muscular strength and gait. Sensorimotor training provided a variance of training environment and COG exercise of the body is thought to be a more effective exercise program that improves balance and gait ability in older adults.

The Effect of Warm-Up Method on Exercise Performance and Rate Pressure Product during Resistance Training

  • We are designed was to find an efficient warm-up method for resistance training for muscle hypertrophy, and 10 males with at least 3 years of resistance training experience were selected as subjects. The 75% 1RM was measured directly based on the pre-measured bench press 1RM. After that, the main experiment of 75% 1RM bench press according to the three warm-up methods was conducted one week apart, and all experiments were randomized and cross-over. Performance according to the warm-up method (3) was measured by total exercise volume, and physiological changes were determined by myocardial workload. All post-measurement data were analyzed using SPSS.22.0 and analyzed using repeated measures one-way ANOVA and contrast comparisons were made using the deviation method. The results showed that the method of gradually increasing the number of repetitions by performing the same intensity as the intensity of the main exercise in the form of muscle hypertrophy, but at submaximal repetitions, showed the highest performance.

노인의 단기집중 균형훈련이 하지수행능력에 미치는 영향 (Effects of Short-Term Intensive Balance Training for the Performance Ability of Lower Extremities in the Elderly)

  • The objective of this study was to discern the effects of a balance training program on the performance of lower extremities in order to improve strength (muscle power, flexibility, walking power), balance control and walking ability in the elderly. The subjects selected were aged 65 years and over, with no known relevant medical history that may disturb their balance, and have also been visiting the Gimhae Senior Welfare Center. The variable group consisted of 30 subjects, of whom were people who had been participating in balance training programs (One Leg Standing; OLS, Functional Reach; FR, Timed Get Up and Go; TUG) as an intervention for 8 weeks 3 times per a week. They were examined in order to identify their balance control before and after. The control group consisted of thirty subjects who preferred to exercise without any intervention relating to balance training program. The subjects were measured before and after balance training in order to determine the effectiveness of exercise and the effectiveness of exercise combined with the aid of a Balance Performance Monitor. 1. Mean time on OLS test with left and right sided extremities in the experimental group was 35.44 sec, 42.10 sec longer than control group respectively. In FR tests applied to the left and right side, mean reaching distance was increased up to 5.56 cm, 6.73 cm in experimental group respectively. Mean time on TUG test from a chair in experimental group was decreased to 2.33 sec. 2. Mean value of decline in the level balance control, both left and right side, decreased to 2.24% as examined by the Balance Performance Monitor. Mean scores for sway level after balance training decrease to .98% and for balance control both anterior and posterior directions decreased to mean 1.07% and 1.44%, respectively.

가압훈련의 혈류 압박 정도에 따른 복합근 활동전위의 변화 (Changes in Compound Muscle Action Potential Depending on Pressure Level of Blood Flow During KAATSU Training)

  • Purpose: In recent years, there has been increasing interest in using blood flow-restricted exercise (BFRE) or KAATSU training. The KAATSU training method, which partially restricts arterial inflow and fully restricts venous outflow in the working musculature during exercise at reduced exercise intensities, has been proven to result in substantial increases in both muscle hypertrophy and strength. The purpose of this study was to investigate the proper level of pressure for KAATSU training using compound muscle action potential (CMAP) analysis. Methods: Twenty-two healthy adults voluntarily participated in this study. CMAP was conducted by measuring the terminal latency and amplitude using a motor nerve conduction velocity test. For reference-line, supramaximal electrical stimulation was applied to the median nerves of the participants to obtain CMAP for the abductor pollicis brevis. For baseline, the intensity of the electrical stimulation was decreased to a level at which the CMAP amplitude was about a third of the CMAP amplitude obtained by the supramaximal electrical stimulation. The pressure levels for the KAATSU were set as a systolic blood pressure (strong pressure), the median values of systolic and diastolic blood pressure (intermediate pressure), and diastolic blood pressure (weak pressure). In the KAATSU condition, CMAP was performed under the same conditions as baseline after low-intensity thumb abduction exercises were performed at the subjects' own pace for one minute. Results: As the pressure increased, the CMAP amplitude was significantly increased, signifying that more muscle fibers were recruited. Conclusion: This study found that KAATSU training recruited more muscle fibers than low-intensity exercise without the restriction of blood flow.

자기관찰훈련을 병행한 과제보행운동이 뇌졸중 환자의 다리 근활성도와 보행에 미치는 영향 (The Effect of Task Gait Exercise Combined with Self-observation Training on Leg Muscle Activity and Gait in Stroke Patients)

  • PURPOSE: This study examined the effectiveness of task gait exercise combined with self-observation training to provide basic clinical data by analyzing the factors that task gait exercise combined with self-observation training has on the leg muscle activity and gait improvement in stroke patients. METHODS: The subjects were assigned randomly to experimental Group I, which mediated task gait exercise combined with self-observation training, and experimental Group II, which mediated task-walking movement. They received 30-minute interventions three times a week for four weeks. The leg muscle activity and 10 m walking test (10MWT) were conducted as pre-intervention tests, and they underwent a post-test four weeks later in the same manner as the pre-test. RESULTS: A statistically significant difference was observed in all muscles and 10MWT (p < .01) in experimental group I (p < .05), while there were significant differences only in the rectus femoris, biceps femoris, and 10MWT (p < .05). In a comparison of the changes between groups, there were statistically significant differences only in the tibialis anterior, soleus muscle, and 10MWT (p < .05). CONCLUSION: Self-observation training in experimental group I was effective in increasing the leg muscle activity and improving walking speed by discovering and correcting incorrect movements and following a normal gait pattern using the ankle joint. Therefore, the task gait exercise combined with self-observation training should be introduced and actively utilized for the rapid social recovery of stroke patients.

  • PURPOSE: This study aimed to provide the basic data necessary for rehabilitation by identifying the effects of complex balance exercises combined with self-observation training on balance and gait improvement in stroke patients. METHODS: This study assigned 20 people randomly into two groups: the control and experimental groups. The experimental group (10 subjects) underwent self-observation training-combined complex balance exercise. The control group (10 subjects) underwent complex balance exercises. A pretest of the balance ability and walking ability of both groups was performed. The interventions were conducted for 30 minutes three times a week for four weeks, and post-tests were conducted four weeks after all interventions were completed. RESULTS: There was a significant difference between the experimental and control groups according to the increase in Berg Balance Sale within the group and a statistically significant difference by a decrease in 10MWT (p < .01). On the other hand, there was a significant difference only in the change in Berg Balance Sale between the two groups (p < .05). CONCLUSION: Combined balance exercise combined with self-observation training and combined balance exercise alone positively affected the Berg Balance Sale and 10MWT in both groups. On the other hand, in the results between groups, there was a statistically significant difference in Berg Balance Sale in complex balance exercise combined with self-observation training. Therefore, self-observation training should be used for the rapid social rehabilitation of stroke patients.
