Effects of exercise training on the biochemical pathways associated with sarcopenia

  • [Purpose] Sarcopenia is considered one of the major causes of disability in the elderly population and is highly associated with aging. Exercise is an essential strategy for improving muscle health while aging and involves multiple metabolic and transcriptional adaptations. Although the beneficial effects of exercise modalities on skeletal muscle structure and function in aging are well recognized, the exact cellular and molecular mechanisms underlying the influence of exercise have not been fully elucidated. [Methods] We summarize the biochemical pathways involved in the progression and pathogenesis of sarcopenia and describe the beneficial effects of exercise training on the relevant signaling pathways associated with sarcopenia. [Results] This study briefly introduces current knowledge on the signaling pathways involved in the development of sarcopenia, effects of aerobic exercise on mitochondria-related parameters and mitochondrial function, and role of resistance exercise in the regulation of muscle protein synthesis against sarcopenia. [Conclusion] This review suggested that the beneficial effects of exercise are still under-explored, and accelerated research will help develop better modalities for the prevention, management, and treatment of sarcopenia.

Comparison of Abdominal Muscle Thickness Using Ultrasound Imaging During Bridging Exercises With a Sling and Ball in Healthy Young Adults

  • Background: Bridging exercises are used to enhance the functional stability of the lumbopelvic region in clinical settings. Although most of the studies on bridging exercises have compared the complete activation of the trunk muscles, some recent studies have examined the functional stability of the trunk and the lumbopelvic region and assessed the appropriate recruitment of the local and global muscles during different task levels. Objects: The purpose of this study was to investigate the changes in muscle thickness in the transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles during a common bridging exercise on an unstable surface and to determine whether these changes differ based on the surface used. Methods: Twenty-five healthy young adults (8 males, 17 females) were recruited. The subjects were randomly assigned to either the exercise progression with a sling bridge group or the ball bridging exercise progression group, each with three stages of increasing difficulty. Each position was measured three times with an ultrasonic diagnostic imaging system, and the mean values were recorded for analysis. Results: No significant differences were observed between the TrA, IO, or EO muscle thickness ratios between the sling and ball exercise groups (p > 0.05). There were also no significant differences in the EO muscle thickness ratios between the tasks irrespective of whether the sling or ball was used. However, the TrA and IO thickness ratios in both groups were significantly greater during stages 2 and 3 compared to stage 1. Conclusion: The results suggest that the use of slings and balls during bridging exercises is effective in activating the deep abdominal muscles.

  • A Study on Scoliosis Milwaukee Brace , Therapeutic Exercise -Oh, Kuyung- Hwan Scoliosis implies an appreciable lateral deviation in the spine. There are numerous theories of the etiology of scolisosis. but the true causative factors remain unknown ;thus this affliction currently cannot be prevented. Treatment essentially consists of early recognition, correction of existing curves, and prevention of the further progression of the curves . Tremendous strides are being made in the treatment of severe scoliosis but the desire for early nonoperative treatment is of paramount important. Many conservative nonoperative procedures have been advocated. Among th-em .Milwaukee brace and therapeutic exercise are the most effective ones .

  • 운동은 건강한 삶의 영위에 필요하지만 코로나19와 같은 전염병 유행 상황에서 비대면 환경에서 진행되는 것이 권장된다. 그러나 기존의 비대면 방식의 운동 콘텐츠에서는 운동 동작의 인식은 가능하지만 이를 해석해서 피드백 정보를 제공해주는 과정이 자동화되지 않았기 때문에 피드백이 트레이너의 눈대중으로 이루어지는 한계가 있다. 따라서 본 논문에서는 이러한 문제를 해결하기 위해서 운동 내용 및 이를 구성하는 동작을 추적하기 위해 공식화된 규칙을 만드는 방법을 제안한다. 이러한 규칙을 만들기 위해서는 전체적인 운동 내용의 진행 규칙을 먼저 만들고, 운동을 구성하는 동작의 추적 규칙을 만든다. 동작의 추적 규칙은 동작을 여러 단계로 나누고 단계를 나누는 키 프레임 자세를 정의하는 것에서 출발하여 키 프레임 자세로 대표되는 상태와 상태 간의 전이 규칙을 만듦으로써 생성될 수 있다. 이렇게 생성한 규칙은 모션 캡쳐 장비를 이용한 자세 및 동작 인식기술의 사용을 전제로 하며 이러한 기술 적용의 자동화를 위한 논리적인 전개에 사용된다. 본 논문에서 제안한 규칙을 사용하면 운동 과정에서 나타나는 동작을 인식하는 것뿐만 아니라 동작의 전 과정에 대한 해석의 자동화가 가능하여 인공지능 트레이닝 시스템 등 보다 진보된 콘텐츠 제작이 가능해진다. 이에 따라 운동 과정에 대한 피드백의 질을 높일 수 있다.

Comparison of Number of Repetitions and Repetition Rate in 5 Sets of 65%1RM Bench Press and Biceps Curl Exercise

  • The purpose of this study is to investigate the changes in the number of repetitions and the repetition rate according to the exercise mode when performing 65%1RM resistance exercise at the 1-minute rest interval and the 3-minute rest interval. Sixteen healthy male subjects were treated with Bench press and Biceps curl of 65%1RM intensity at 1 and 3 minute rest intervals. The number of repetitions for each set of 1 minute rest interval showed a significant decrease from 1set to 5set in bench press. biceps curl showed a significant decrease from 1set to 4set. The repetition rate according to the exercise mode with a 1-minute rest interval showed a significant difference from 2sets to 4sets. In the repetition rate for each set, bench press showed a significant decrease from 1set to 5set. biceps curl showed a significant decrease from set 1 to set 4. The number of repetitions according to the exercise mode with a 3-minute rest interval showed a significant difference from 2sets to 5sets. In the number of repetitions for each set, bench press showed a significant decrease from 1set to 5set. biceps curl showed a significant decrease from 1set to 4set. The repetition rate according to the exercise mode with a 3-minute rest interval showed a significant difference from 2sets to 5sets. In summary, the decrease in the number of repetitions according to the set progression in the resistance exercise of the endurance depends on the exercise mode, and the increase of the rest interval or the decrease of the weight-intensity should be considered when aiming for more exercise.

The Ability of Muscle Functional MRI to Detect the Slight Effect of Exercise on Trunk Muscle Activity

  • Purpose: In this study, we provide a way to assess even a slight effect of exercise on trunk-muscle activity. Materials and Methods: Seven healthy male participants (mean age, 24.7 ± 3.2 years; height, 171.2 ± 9.8 cm; and weight, 63.8 ± 11.9 kg) performed 15 sets of an exercise with 20 repetitions of 90° hip and right-knee flexion while lying supine. The exercise intensity was measured using the 10-point Rating of Perceived Exertion Scale after the first and 15th sets of exercises. Although cross-sectional areas and functional T2 mapping using ultrafast imaging (fast-acquired muscle functional magnetic resonance imaging, fast-mfMRI) have been proposed for imaging to evaluate exercise-induced muscle activity in real time, no previous studies have reported on the evaluation of trunk-muscle activity using functional T2 mapping. As a method for assessing trunk-muscle activity, we compared functional T2 mapping using ultrafast imaging (fast-mfMRI) with cross-sectional areas. Results: Although the muscle cross-sectional areas were increased by the exercise, there was no significant difference at rest. On the other hand, for all sets, the changes in T2 were significant compared with those at rest (P < 0.01). These results demonstrate that T2, calculated from fast-mfMRI images can be used to detect even a small amount of muscle activity induced by acute exercise, which was impossible to do with cross-sectional areas. Conclusion: Fast-mfMRI, which can also display functional information with detailed forms, enabled non-invasive real-time imaging for identifying and evaluating the degree of deep trunk-muscle activity induced by exercise.

운동중재가 투석 전단계 만성 신질환 노인의 신체기능 및 신기능 향상에 미치는 효과: 체계적 문헌고찰 및 메타분석 (Effectiveness of exercise for improving physical and renal function in older adults with pre-dialysis chronic kidney disease: A systematic review and meta-analysis)

  • Purpose : Exercise may prevent the worsening of chronic kidney disease (CKD) and progression of cardiovascular diseases in patients with CKD. This review aims to identify the best type of exercise modality and summarizes the beneficial effects of exercise on physical and renal function among older adults with pre-dialysis CKD. Methods : A systematic search of PubMed, Embase, CINAHL, Cochrane Library, Web of Science, SCOPUS, and domestic database was performed for randomized controlled trials (RCTs) assessing the effect of exercise intervention on older adults with pre-dialysis CKD published until February 2023. A random-effects meta-analysis was conducted. The risk of bias was assessed using a Cochrane tool for assessing the risk of bias in RCTs (RoB 2.0). Results : The systematic review included 11 RCTs (n = 591, average age 60.2-76), of which 8 could be included for meta-analysis. Exercise was significant in increasing peak oxygen consumption and knee muscle strength among physical functions, and also in improving glomerular filtration rate among kidney functions. Conclusion : Exercise has beneficial effects on physical and renal function among older adults with pre-dialysis CKD. In the future, it is necessary to verify the effectiveness of exercise by subdividing it by type, intensity, duration, and delivery.

  • 본연구는슬관절과견관절의초기재활과정에서관절운동범위와스트레칭, 그리고유산소성운동프로그램에대한논문이다. 만약관절이손상후장기간동안고정된다면관절이굳어지고 근육은위축이 될것이다. 그러므로본 프로그램은다양한운동기술을포함하고있다. 즉관절의경직을예방하기위한운동범위확보와근육의이완을위한스트레칭, 심폐기능의감소를예방하기위한심혈관훈련(수영, 상체자전거, 고정식자전거)을포함하고있다. 초기재활과정에서는임상운동전문가와환자사이의상호관계를형성하는것이매우중요하다. 또한임상운동전문가는환자의통증과운동적응도, 체력수준, 프로그램의진행등에관해스포츠의학전문의와상의하는것이권장된다.

여성 유방암 환자의 림프부종 감소와 조절을 위한 운동의 효과: 체계적 고찰과 메타분석 (Exercise for reducing and controlling lymphedema in Women with breast cancer: A systematic review and meta-analysis)

  • 유방암은 여성에서 가장 흔한 악성종양이며, 림프부종은 가장 흔한 유방암수술 후 합병증 가운데 하나이다. 따라서 림프부종과 같은 치료 후유증의 위험도 감소와 적절한 관리는 유방암 생존 환자와 의사에게 있어서 그 중요성이 점차 커지고 있다. 이러한 림프부종의 발생을 예방하기 위하여 흔히 운동이 처방된다. 그러나 그러한 운동의 시기와 효과에 대해서는 각각 상반된 결과들이 보고되어 왔다. 저자들은 림프부종에 대한 운동 효과를 점검하여 환자 치료에 최선의 근거를 도입하기 위해 최신문헌과 상호 심사된 출판물, 전문 기구의 웹사이트를 체계적으로 고찰하여 림프부종의 예방이나 치료를 위해 시행되는 운동의 효과를 검토하였고, 그 결과 이미 림프부종이 발생해 있는 환자들에서는 임상적인 부종진행의 차이점에 유의성이 없었지만, 발생 전 환자들에서는 운동 혹은 훈련이 이차성 림프부종의 발생을 감소시키고 진행을 변경시키는 양상을 볼 수 있었다. 암치료의 발전, 암과 운동 연구, 림프부종의 관리를 위해서는 현재까지 도출된 근거들에 대한 고찰과 이해가 있어야 임상의사들이 적절한 환자 교육과 전문가의뢰를 시행할 수 있을 것이다.

A systematic review of the biological mechanisms linking physical activity and breast cancer

  • [Purpose] Epidemiological evidence has shown that leisure-time physical activity and structured exercise before and after breast cancer diagnosis contribute to reducing the risk of breast cancer recurrence and mortality. Thus, in this review, we aimed to summarize the physical activity-dependent regulation of systemic factors to understand the biological and molecular mechanisms involved in the initiation, progression, and survival of breast cancer. [Methods] We systematically reviewed the studies on 1) the relationship between physical activity and the risk of breast cancer, and 2) various systemic factors induced by physical activity and exercise that are potentially linked to breast cancer outcomes. To perform this literature review, PubMed database was searched using the terms "Physical activity OR exercise" and "breast cancer", until August 5th, 2020; then, we reviewed those articles related to biological mechanisms after examining the resulting search list. [Results] There is strong evidence that physical activity reduces the risk of breast cancer, and the protective effect of physical activity on breast cancer has been achieved by long-term regulation of various circulatory factors, such as sex hormones, metabolic hormones, inflammatory factors, adipokines, and myokines. In addition, physical activity substantially alters wholebody homeostasis by affecting numerous other factors, including plasma metabolites, reactive oxygen species, and microRNAs as well as exosomes and gut microbiota profile, and thereby every cell and organ in the whole body might be ultimately affected by the biological perturbation induced by physical activity and exercise. [Conclusion] The understanding of integrative mechanisms will enhance how physical activity can ultimately influence the risk and prognosis of various cancers, including breast cancer. Furthermore, physical activity could be considered an efficacious non-pharmacological therapy, and the promotion of physical activity is probably an effective strategy in primary cancer prevention.
