Percutaneous Transcatheter Pericardiotomy of Pericardial Effusion in a Miniature Poodle Dog (심한 심낭수 삼출이 있는 푸들개의 카테터를 통한 부분적 심낭절개술)

  • An 11-year old, intact female Poodle (weighing 2.3 kg) was referred with signs of consistent coughing, dyspnea, poor exercise tolerance, and anorexia. Diagnostic imaging and laboratory studies revealed idiopathic hemorrhagic pericardial effusion complicated with ISACHC Ib stage of chronic mitral valvular degeneration. Percutaneous transcatheter pericardiotomy (PTP) was performed at the right precordium using alligator forceps with fluoroscopic guidance. Immediately after PTP, electrocardiogram and echocardiogram showed dramatic improvement of cardiac performance. Patient was released with the prescription of furosemide (1 mg/kg, bid, PO), enalapril (0.5 mg/kg, bid, PO), cephradine (20 mg/kg, bid, PO) for mild mitral regurgitation and post-management of infection. Diagnostic studies performed at 2 weeks after PTP revealed no further accumulation of pericardial effusion and improvement of clinical signs. The dog is currently medicated with enalapril and monitored regularly.

Comparison between Rectus Femoris and Tibialis Anterior in Terms of the Levels of Activity Varying Depending on Walking Patterns (forward and backward) and Varied Treadmill Slopes (전.후방보행 방법과 트레드밀 각도변화에 따른 넙다리곧은근과 앞정강이근의 활성도 비교)

  • Purpose: This study examined how the degrees of muscle activity of Rectus Femoris and Tibialis Anterior during the four phases of walking vary according to three different treadmill slopes of $0^0$, $7^0$, and $15^0$. Methods: Subjects were 14 randomly selected healthy students attending G University in Seoul, Korea who had never had articular problems with lower limb and had no difficulties walking at the time of study. Results: 1) With respect to Rectus Femoris, in every phase of both forward and backward walking, there were significant differences among all of the slope degrees (p <.05), while the activity increased with increased slope degrees in every phase of backward walking. 2) For Tibialis Anterior, only in P2 and P3 of both forward and backward walking there were significant differences in every slope (p <.05). Conclusion: Both Rectus Femoris and Tibialis Anterior were found to be more active during backward walking compared to backward walking. In addition, the activity degree of Rectus Femoris was high between the early part of two foot support phase and the early part of one foot support phase, whereas that of Tibialis Anterior was high between the early part of one foot support phase and the latter part of both foot support phase.

Comparison of Aerobic and Anaerobic Capacity between Sasang Consititutions and ACE Gene Polymorphismn (사상체질과 ACE 유전자 다형성 분류에 따른 유·무산소성 능력비교)

  • Purpose. This study of purpose was to to compare of Aerobic and Anaerobic Capacity between Sasang Constitutions and ACE Gene Polymorphism. Methods. 24 healthy males were participated in this experiment who were in their twenties and had no disease, then they were divided by ACE type and by Sasang constitutions with QSCC II. According to these group, the body compositon, Aerobic capacity including VO2max, Anaerobic Capacity were measured. Results. In this study, According to ACE type, ID type were 8, II were 8 and DD type were 8 persons. According to ACE type, there were significant difference among type, especially, II, ID type more than DD. II type is more higher than any type and DD type is lower in the ratio of Blood Lactic recovery. DD type was more excellent than other type in Anaerobic power. According to Sasang Constitutions, there were 8 SoYang, 5 Taeum, 11 Soeum and then no TaeYang constitution. In Aerobic capacity, Taeum constitution had significantly high means and Anaerobic threshold. In the ratio of Blood lactic recovery, Taeum constitution was excellent and SoYang had poor recovery capacity. SoYang had more excellent than other constitution significantly in Anaerobic capacity. Comparing ACE type with Sasang constitution, Soyang constitution included 4 ID types, Taeum inclued 2 II types and 2 ID types then Soeum included 6 DD types. Compared of Aerobic and Anaerobic capacity between Sasang constitutions and ACE type, Soyang constitutions were similar to ID type, Taeum similar to II type and ID type and then Soeum were DD type. Conclusions. This study had made it clear that there were similar feature between ACE type and Sasang constitutions. Also it's possible to predict the Aerobic capacity that may be foreseen by ACE type with Sasang questionnaire method but not Anaerobic capacity.

The Relationship between Physical Characteristics and Walking Ability in Elderly: A Cross-Sectional Study (노인들의 보행 능력과 신체적인 특성 간의 상관관계: 단면 연구)

  • This study aimed to investigate the relationship between physical characteristics and walking ability in the elderly population. Subjects were 77 elderly (38 men and 39 women) who are capable of walking independently with and without walking aids. Correlation and stepwise multiple linear regression analyses were used to analyze the relationship between physical characteristics (age, gender, height, weight, body mass index, muscle mass, waist/hip ratio, heart rate, vital capacity, flexibility, maximum oxygen consumption, one-leg standing time, and strength of knee flexor and extensor) and walking velocity of subjects. Age, height, vital capacity, one-leg standing time, and strength of knee flexor and extensor showed significant correlations with walking velocity of subjects (p<.05). Further, the strength of knee flexor explained 27% of the variance, and up to 32% of the walking velocity could be explained when the strength of knee extensor were added to the model. The findings suggest that walking velocity of elderly depends on the strength of lower limb's strength and a variety of physical characteristics.

  • For using the knowledge of experts diversely and efficiently, it is very important that the system could represent the complex knowledge in well structured manner and also adapt well to the frequent schema changes. In this paper, we propose an efficient method for knowledge base management by using XML based templates. In our proposed method, to solve the inefficiency of management of many XML documents, we represent the complex knowledge on XML-based templates, and manage the templates structurally by using a meta-template that defines relationship among the templates. In order to show the effectiveness of our method, we developed a knowledge base to formulate an exercise prescription for a subject and an application program to classify the subject's fitness. The experimental study shows that XML-based templates provide a flexible, extensible and structured way of representing expert knowledge. The inference using XML-based templates can be controlled systematically and efficiently by using meta-template.

A Survey of Hypertension Treatment in Korean Medicine (고혈압 한의 진료 실태조사)

  • Objectives: This study surveyed doctors of Korean medicine on how they treat hypertension in their Korean clinical practices. Methods: An invitation to take an Internet-based questionnaire was sent to doctors who were registered in the Association of Korean Medicine in September 2016. Participants were first asked general profile questions, which included age, gender, work experience as a practitioner, and specialties. Second, they were asked whether they had experience with hypertension treatment and additional questions related to hypertension. Results: A total of 991 replies were received to the questionnaire. Most of the Korean medical doctors (95%) had blood pressure units and had monitored blood pressure. A majority of the Korean medicine doctors (55%) had provided hypertension treatment. Acupuncture and herbal medicine were commonly used for the treatment of hypertensive patients. In addition, bloodletting, moxibustion, pharmacopuncture, cupping, Chuna, Qigong exercise, meditation, and Tai Chi were used. Many doctors also recommended lifestyle modification. The respondents said that Korean medicine improves general health conditions (42.6%) and has low adverse effects (29.9%). One of the most common opinions about the revitalization measures of Korean medicine was expanding health insurance benefits (20.8%). Conclusions: Through this survey, we found out information about the present situation of hypertension treatment in Korean medicine. In the future, it is possible that hypertension treatment using Korean medicine will be demanded and used by practitioners if it is covered by public health insurance.

Population Stereotypes as a Perceiver's Cognitive Structure on Manipulating Devices for Daily Use (일상생활용 장치 작동에 따른 지각자의 인지구조에 대한 집단고정관념)

  • Purpose: This study standardizes the position and direction of devices based on general user expectations and stereotypes. Population stereotypes are cognitive structures that contain the perceiver's knowledge, beliefs, and expectations about human groups. In this paper, the stereotypes that people typically expect when manipulating 'cylindrical key-in-knob locks' and 'lever-type water faucet handles' were investigated and data regarding their expectations were collected. Methods: Two sets of 600 participants, between 13 and over 60 years old, were recruited for the experiment. Each group was evenly subdivided into six age groups. Each participant was presented with cylindrical door locks mounted on a miniature door and actual lever-type water faucet handles mounted on a mockup sink. Results: If the cylindrical lock was positioned 'vertically,' 59.2% of the participants expected the device to be locked, and if the lever type faucet handle was positioned 'up,' 63.0% of the participants expected the device to be turned on. Thus, daily-use devices should be designed consistent with user expectations of operation. There was a significant difference between genders for manipulating the faucet handle between up and down. Conclusion: A more general stereotype may be defined by repetitive measurements under the same test conditions with fixed time intervals, as well as accounting for people with cognitive problems.

Effects of Circuit Weight Training on Isokinetic Muscle Strength and Body Composition in elderly (서킷 웨이트트레이닝이 노인들의 등속성 근력과 신체구성에 미치는 영향)

  • The purpose of this study was to investigate the effects of circuit weight training(CWT) on isokinetic muscle strength and body composition in elderly. The subjects who engaged in this experiment exercised at $40\%$ of 1-RM, 12 repetitions, followed by 15 sec as the subject moved to the each break training program which was consist of the circuit of 10 stations performed on 3 set a day, circuits 3 days a week during 10 weeks. The assessment of isokinetic factor was in concentric flexors and extensors of right and left knee joint. Tests were performed on the Cybex 770 Isokinetic Dynamometer and body composition were estimated the three parts of chest, abdomen and anterior thigh by using skinfold caliper, calculated the average and followed by Seri and Brozek way. Statistical analysis were performed using analysis of variance paired t-test, accepting level for all significant was above ${\alpha}=.05$ and ${\alpha}=.01$. Following is as a result of 10 weeks circuit weight training. 1. At the $60_{\circ}$ /sec, the right and left knee isokinetic concentric flexors and extensors peak torque increased significantly(p<.01). 2. At the $180_{\circ}$ /sec, the right and left knee isokinetic concentric flexors and extensors peak torque increased significantly(p<.01). 3. At the $60_{\circ}$ /sec, the right and left knee isokenetic concentric flexors and extensors average power increased significantly(p<.05, p<.01) and at the $180_{\circ}$ /sec, the right extensors didn't show any statistical significant. 4. At the $60_{\circ}$ /sec, the right and left knee Isokinetic concentric flexors and extensors total work increased significantly(p<.05, p<.01) but at the $180_{\circ}$ /sec right concentric flexors didn't show any statistical significant. 5. The body composition changed significantly(p<.01). These results suggest that 10 weeks circuit weight training increases the peak torque, average power, total work significantly and decreases the $\%$body fat significantly.

Improvement of physical fitness and work ability of the middle-aged and aged workers through exercise prescription program (운동처방 프로그램을 통한 중·고령 근로자의 체력향상을 통한 노동력 증진)

  • The purposes of this study were to investigate whether the program of health promotion and follow up feedback by periods could improve physical fitness related to work ability of the middle-aged and aged workers. Subjects were composed of two groups; workers aged below forty five and forty five and over who had measured their health-related physical fitness twice for 2 years (2006 & 2007) through the program of health promotion operated by Korea Occupational Safety & Health Agency. Paired t-test was used to evaluate the difference between the first measurement and re-measurement of health-related fitness which was operated after six to twelve months from the first one. We used 0.05 level for statistical significance. The results of this study were as follows. All measurement items including body fat ratio(%), $VO_2max$, grip strength, sit-up, and trunk flexion were significantly improved in both groups. The improvement(%) of $VO_2max$, grip strength of workers of forty five and above was lower than those of workers of below forty five. Servicing the program of health promotion and follow up feedback by periods may help workers' improvement of health-related physical fitness related to work ability.

U-health Service Model for Managing Health of Chronic Patients in Multi-platform Environment (멀티플랫폼 환경의 만성 질환자 건강관리를 위한 유헬스 서비스 모델)

  • U-health services have been progressed as treatment and management for specific diseases and prevention services for providing the behavior management to customers according to the increase in chronic patients. The conventional U-health services provide required services and bio-information monitoring only through remote diagnoses and counsels and that represent limitations in preventing and managing metabolic syndrome patients like chronic patients. Thus, in this study a multi platform based U-health service model for managing the health of chronic patients is proposed. The multi-platform based U-health service model can provide continuous health information, diet, and exercise services regardless of the location of customers through PCs and smart phones. In addition, it is able to provide prescription services to doctors and nurses using a CDS (Clinical Decision Support) module based on clinical information. Doctors can identify the life pattern of patients through a behavior modification program and provide customized services to patients. The U-health service model provides effective services in multi-platform environments to customers and that will improve the health of chronic patients.
