Effects of Moxibustion and Theraband Exercise on Physical and Psychological Variables of the Aged with Degenerative Osteoarthritis (뜸요법과 세라밴드운동이 퇴행성 골관절염 노인의 신체적, 심리적 변수에 미치는 효과)

  • Purpose: To examine the effects of a nursing intervention including moxibustion and theraband exercise on pain, fatigue, flexibility, and depression in aged patients with degenerative osteoarthritis. Methods: This study had a one group pretestposttest design. Thirty people over 60 yr-of-age with degenerative osteoarthritis were recruited from a community health center from November 1, 2005 to June 14, 2006. The 6 week treatment intervention included twice weekly 60 min sessions of of theraband exercise (30 min) and moxibution (30 min). Data was analyzed by SPSS Ver. 12.0. Results: Intervention was associated with significant decreases in pain (t=8.28, p<.001) and fatigue (t=5.19, p<.001), and significant increases in knee flexibility (t=-3.54, p=.001), dorsal flexibility (t=-9.05, p<.001), and plantar flexibility (t=-4.09, p<.001). Depression was significantly decreased after intervention (t=-4.09, p<.001). Conclusions: Moxibustion and theraband exercise should be utilized as an effective nursing intervention for the aged with degenerative oateoarthritis.

The Moderating Effect of Gender in the Relationship between Physical Education and Adolescents' Internalizing and Externalizing Problem Behaviors: Using Multi-level Growth Modeling (체육시간과 청소년의 외현화·내재화 문제와의 관계에서 성별의 조절효과: 다층성장모형의 적용)

  • This study examined the relationship between physical education and externalizing(aggression) and internalizing(depression, social withdrawal) problem behaviors among adolescents. The moderating role of gender and time in the association between physical education and problem behavior was also identified. This study used data from the second, third, and fourth waves of the middle school student cohort (N=2,133, N=2,151, N=1,979) of the Korean Children-Youth Panel Survey(KCYPS). Main analyses involved multilevel growth model with interaction terms. The dependent variables were aggression, depression, and social withdrawal. The independent variables were gender and physical education (exercise hours) at school. The control variables were abuse, school-adjustment, annual household income and parents' highest level of education. The major longitudinal findings of this study are as follows: First, there was significant change according to the passage of time only in aggression among externalizing and internalizing problems. Second, gender differences exist in aggression and depression. Third, exercise hours of physical education had a negative relationship with internalizing problems. Fourth, there were no gender differences over time in both externalizing and internalizing problems. Fifth, the interaction between exercise hours of physical education and time was statistically significant for social withdrawal. Sixth, the interaction between exercise hours of physical education and gender was statistically significant for depression. The results of this study may become an academic basis for suggesting policy directions that promote increased exercise hours in physical education classes at school.

Neurogenic effect of exercise via the thioredoxin-1/ extracellular regulated kinase/β-catenin signaling pathway mediated by β2-adrenergic receptors in chronically stressed dentate gyrus

  • [Purpose] Chronic stress is a precipitating factor for depression, whereas exercise is beneficial for both the mood and cognitive process. The current study demonstrates the anti-depressive effects of regular exercise and the mechanisms linked to hippocampal neurogenesis. [Methods] Mice were subjected to 14 consecutive days of restraint, followed by 3 weeks of treadmill running, and were then subjected to behavioral tests that included the forced swimming and Y-maze tests. Protein levels were assessed using western blot analysis and newborn cells were detected using 5-bromo-2'-deoxyuridine (BrdU). [Results] Three weeks of treadmill running ameliorated the behavioral depression caused by 14 days of continuous restraint stress. The exercise regimen enhanced BrdU-labeled cells and class III β-tubulin levels in the hippocampal dentate gyrus, as well as those of thioredoxin-1 (TRX-1) and synaptosomal β2-adrenergic receptors (β2-AR) under stress. In vitro experiments involving treatment with recombinant human TRX-1 (rhTRX-1) augmented the levels of phospho-extracellular signal-regulated kinases 1 and 2 (ERK1/2), nuclear β-catenin, and proliferating cell nuclear antigens, which were previously inhibited by U0216 and FH535 (inhibitors of ERK1/2 and β-catenin/T cell factor-mediated transcription, respectively). The hippocampal neurogenesis elicited by a 7-day exercise regimen was abolished by a selective inhibitor of β2-AR, butoxamine. [Conclusion] These results suggest that TRX-1-mediated hippocampal neurogenesis by β2-AR function is a potential mechanism underlying the psychotropic effect of exercise.

Effect on Cognitive Function in Elderly People (노인의 인지기능에 미치는 영향)

  • Objectives : The purpose of this study was to identify the effect on cognitive function in elderly people above 65 years of age. Methods : The data were collected from June 1 to August 30 2014. Data were collected by questionnaire from 994 elderly people. The instruments for this study were ADL, IADL, depression, MMSE-DS. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, multiple linear regression with the SPSS/WIN 18.0 program. Results : The significant factors influencing MMSE-DS were education(${\beta}=-0.314$, p<.001), IADL(${\beta}=0.196$, p<.001), age(${\beta}=-0.168$, p<.001), exercise time(${\beta}=0.089$, p=.004), economic conditions(${\beta}=0.086$, p=.007), depression(${\beta}=-0.084$, p=.001), income(${\beta}=-0.083$, p=.003), sleep time(${\beta}=-0.079$, p=.001), medical insurance(${\beta}=0.059$, p=.024) and leisure(${\beta}=0.057$, p=.039). Conclusions : Based on the study, subjects with cognitive impairment detected at an early stage it is necessary to intervene in consideration of the individual characteristics and to develop and apply the program that improve the living habits such as exercise and sleep, and decreasing depression.

The Effect of a Health Maintenance Program on Physical Function and Mental Health of the Elderly in Nursing Homes (건강유지 프로그램이 시설노인의 신체 건강과 정신 건강에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of a Health Maintenance Program on physical functions and mental health of the elderly in nursing homes. Method: Sixty elderly(over 65 years old) in a randomized control study participated in a 16-week group-based intervention including functional exercises and health education. The participants were divided into 3 groups(Health Maintenance Program Group, Supportive Music Exercise Group, and Control Group) of 20 elderly each. Data was collected from Dec. 1st, 2005 to Mar, 30th, 2006. Physical function of lower body strength was assessed using a 30-second chair test, flexibility was assessed using a sit-and-reach test, and static balance was assessed by the ability to balance on one leg with open and closed eyes. Depression was assessed using the Korean Form of the Geriatric Depression Scale and self esteem was assessed using Rosenberg's Self Esteem Questionnaire. Data was analyzed by Chi-square test, One-way ANOVA, and Repeated measure two factor analysis. Results: A Health Maintenance Program significantly increased muscle strength, flexibility and static balance, but depression and self-esteem scores were not significantly changed. Conclusion: Findings demonstrated that a Health Maintenance Program was more effective on physical function than mental health of the elderly in nursing homes.

Analysis of Mental Health Level and Life Satisfaction According to the Amount of Physical Activity and Muscular Strength Level

  • Objective: About one-third of college students are exposed to stress to the point that their academic performance is negatively affected. To provide useful information for managing the mental health of young people by analyzing the perceived stress level, depression level, and life satisfaction according to the amount of physical activity and the level of muscular strength in male college students. Design: A cross-sectional study. Methods: Among 304 students who participated in the health exercise class at Sahmyook University in Seoul, the amount of physical activity and the level of muscular strength were divided into 3 quartiles, respectively, and the perceived stress level, depression level, and life satisfaction were analyzed through one-way ANOVA. The physical activity level and muscle strength level of the subjects were ranked, divided into 3 groups, and the difference in variables according to the group was verified. Results: There was no difference in perceived stress level, depression level, and life satisfaction according to the amount of physical activity, but statistically significant differences were shown in perceived stress scale(p=0.008) and life satisfaction(p=0.030) according to muscular strength. Conclusions: Policies to increase physical activity are important to improve and manage the mental health of young people, but it is judged to be more meaningful to provide an environment that can improve muscular strength.

Relationship of Stress, Depression and Yangsaeng(養生) in Korean College Women (여대생의 스트레스, 우울과 양생 간의 관계)

  • Purpose: The purpose of this study was to investigate the relationship of stress, depression and Yangsaeng in Korea. Yangsaeng is a traditional health care regimen for the promotion of health and prevention of illness by means of specific principles and methods, whose purpose was to improve longevity and healthy life. Method: The subjects of this study were 523 college women in Korea. Data were collected by using a self-reported questionnaire and analyzed through descriptive statistics, Pearson's correlation coefficient, t-test, ANOVA and Cronbach's ${\alpha}$. Results: There was statistically significant difference in stress and depression according to drinking and positive thinking. There was statistically significant difference in Yangsaeng according to grade, drinking, exercise and positive thinking. Stress was positively related depression. Stress and depression was negatively related Yangsaeng. Conclusion: The results of this study suggest that Yangsaeng as oriental health care regimen could lower level of stress and depression. Therefore, Yangsaeng could be recommended a way of mental health promotion in Korean culture.

  • Purpose: The purpose of this study was to investigate the relationship between depression status and cognitive function in the community female aged. Methods: This was a descriptive study. Data were collected using individual-based interviews from 120 senior female residents in the Hall for the aged in the city of S. The length of time for data collection was from June 18 to 29, 2007. The tool of data collection was Korean Version of Mini-Mental State Examination (Kwon & Park, 1989), Short-form Geriatric Depression Scale (Bae, 1996). Results: Depression & cognition showed significant differences according to age, regular exercise. The depression group scored significantly lower than the normal group in total score of cognition, orientation, memory recall. Conclusion: It is necessary to prepare the composite programs, which can improve not only cognition but depression in the community female aged.\

The Effects of Health Exercise Program on Walking ability, Depression and WHOQOL-BREF in the Fall experienced Women Elderly (건강체조 프로그램이 낙상경험 여성노인의 보행능력, 우울 및 삶의 질에 미치는 효과)

  • The purpose of this study was to investigate the effects of health exercise program to old Women's walking ability, depression and WHOQOL-BREF. Data were collected from April to June, 2007 from the falls experienced 70 women elderly. All subjects participated in 12 week health exercise program which was designed in order to develop walking ability. The data were analyzed using frequency, %, paired t-test. The results of this study were as followings; First, there was significant differences in the average time of chair stand (t=2.291, p=.025), one leg standing(Rt. leg)(t=2.236, p=.029), step length between before and after(t=4.015, p=.000) training of 12 week health exercise program. Second, there was non significant differences in depression(t=1.044, p=.300) but, significant differences in WHOQOL-BREF(t=3.528, p=.001). The WHOQOL-BREF in general quality of life(t=2.923, p=.005), physical(t=3.039, p=.003), psychological(t=2.481, p=.016), social(t=2.531, p=.014) and environment domain(t=4.259, p=.000) were significant differences. The results suggest that the 12 week health exercise program can improve the muscle endurance and balance, QOL.

The Comparison Between the Effects of Integrated Arthritis Self-helf Programs and the Effects of Arthritis Exercise Programs Through Meta-analysis (메타 분석을 통한 만성 관절염 환자의 통합 프로그램과 운동 프로그램의 효과 비교)

  • The purpose of this study was to use meta-analysis to analyze result of 17 studies which investigated the effects of integrated programs, and 11 studies which examined the effects of exercise programs on pain, depression, and disability. The 28 studies analyzed in this work were selected from the following sources. MEDLINE Search, bibliographies of related studies, main academic journals of nursing in Korea, and journals on arthritis issues. For the analysis of the data, homogeniety of effect sizes which were calculated based on data in the 28 studies was tested and its average effect size was computed by using meta analysis software package which was developed by Song(1992, 1998). The results can be summarized as follows : 1) Homogeneity tests were conducted on integrated programs on pain. In the prelimiary homogeneity tests on effect size of all 17 studies, no homogeneity was found. When homogeneity tests on the effect size of the remaining 15 studies were performed, excluding two studies which had extremely larger effect size compared to other studies, the 15 studies were found to be homogeneous(Q=16.38, p=.23). The obtained average effect size, D(Mean Standardized Difference Between Means), was .25. When homogeneity tests on effect size on pain was conducted for the excercise programs, effect size for all nine studies were found to be homogeneous (Q=7.42, p=.49) and the average effect size D=.30. Therefore, Hypothesis 1 was rejected from the results, that an average effect size of the integrated programs on pain was not significantely different from that of the exercise programs on pain. 2) Since only two studies investigated the effect of exercise programs on depression, comparison between the average effect size of integrated programs and that of exercise programs on depression could not be conducted, and hypothesis 2 could not be tested. Thereby, only the average effect size of integrated programs on depression was obtained. Eight studies were tested to be positive on the homogeneity of effect sizes(Q=18.31, p=.02) at $\alpha$<.01 and its average effect size was D=.11. 3) For the analysis of integrated program on disability, 13 studies, except for four which had an extremely large effect size compared to the others were found to be homogeneous at $\alpha$<.01 (Q=22.30, p=.04) and the average effect size on disability was D=.16. For analysis of the exercize programs on disability, eight studies, except for one which had an extremely large effect size compared to others, were found to be homo geneous(Q=7.87, p=.34) and the average effect size on disability was D=.60. Therefore, Hypothesis 3 was accepted from the results that an average effect size of exercise programs on disability was significantly larger than that of integrated programs on disability.
