Evaluation for the Purpose and Relevance of Obesity Treatment in Local Clinics (외래 비만 클리닉에서 행해지는 비만 치료의 목적 및 타당성에 관한 평가)

  • Metabolic syndrome is increasing nowadays and one of causes is obesity. Therefore prevention and treatment of obesity is very important to decrease of Metabolic syndrome. However currently many pharmacotherapy for obesity are popular in Korea and it may be dangerous. The aim of this study is to evaluate the purpose and relevance of obese treatment in two areas (capital & local) of Korea and suggest to guide for the right directions of pharmacotherapy in obesity treatment. We selected and surveyed 90 patients (88 females, 2 males, age of $23{\sim}60$) at random who received a prescription after consulting with a doctor at several clinics of Cheongju-area and Seoul-area, from May thru June, 2007. And we evaluated their prescriptions. In results, only 26.7% were obese (13.3%, 2325), most 73.3% patients were normal (65.6%, 18.5$m^2$). The purpose of treatment was for appearance (38.9%), health (41.1%) or both (18.9%, p<0.01). 94% patients had the past history of obese treatment. Prescriptions were consisted of CNS stimulant, anticonvulsants, antidepressant, laxatives, diuretics, or antianxietics. Obese treatment is very important for health to prevent disease such as Metabolic Syndrome. However it should not be abused. In conclusion, some patients of obese treatment is not relevant to obesity and pharmacist role is important to guide them for the right directions in obesity treatment.

A Cross-Cultural Study of Plus-Size Consumer's Perception of Body, Attitude of Accepting Obesity and Clothing Behaviors in Korea and the US (플러스 사이즈 소비자들의 신체인지와 비만수용태도 및 의복행동에 대한 한국과 미국의 비교문화 연구)

  • The purpose of this study was to prove how sociocultural perspective of obesity, differences in consumers' perception of body and attitudes of accepting obesity affected individuals' clothing behaviors through cross-cultural studies. The data collected were composed of 612 Korean and US consumers in the 20's and 30's that had experiences in purchasing plus-size products. The results were as follows. First, BMI index was lower in Korean consumers than the US consumers, but Korean consumers received more stress from being overweight compared to the US consumers, and had a more negative attitude about their body. Second, although Korean consumers had lower BMI index and degrees of obesity than US consumers, they were severely stressed by obesity and were found to have a higher level of dissatisfaction with their bodies. Third, Korean consumers responded more sensitively to obesity and had a tendency to display a more negative attitude regarding obesity, and a more passive dependence on clothing. Forth, differences in the body shape were reflected even in wearing evaluation, and US consumers showed a more positive attitude toward evaluations of size suitability and fitness. Fifth, the plus-size market for Korean consumers was still not active, and most products purchased were generic brands obtained from online shopping malls through the Internet. However, in the case of the US, in which the ratio of obese people is high and the plus-size market is growing, consumers were purchasing plus-size brands through various distribution online and offline channels. Sixth, Korean consumers were less satisfied than US consumers with shops, sizes and fitness; however, they were more satisfied with design factors. Finally, it is expected that this study can offer practical implications for marketers and product developers running plus-size market for young obese consumers in their 20 and 30s.

Evaluation of Chronic Disease and Nutritional Intake by Obesity of Korean Elderly - Data from Korea National Health and Nutrition Examination Survey 2016~2018 - (한국 노인의 비만도에 따른 만성질환 및 영양섭취 실태조사 - 국민건강영양조사 2016~2018 자료를 이용하여 -)

  • The purpose of this study was to investigate the current status of chronic diseases according to obesity in the elderly older than age 65 using data from the 7th Korea National Health and Nutrition Examination Survey (2016~2018). The subjects of the survey were 3,245 elderly older than age 65 who participated in the health survey and nutrition survey, 45.8% of the subjects were males and 54.2% of the subjects were female. 37.5% of all the elderly were obese, and the females (42.4%) were more obese than the males (31.8%) (p<0.001). Diabetes, hypertension, and hypertriglyceridemia had a similar prevalence of 24.6%, 63.0%, and 12.7% in males and females, respectively. Hypercholesterolemia was higher in the elderly females (44.2%) than in the elderly males (24.9%) (p<0.001). Through the logistic regression analysis, it was found that the prevalence of chronic diseases was higher in obesity than normal in elderly males and females (p<0.001). The total food intake increased from 'under weight' to 'obesity' in males (p<0.01) and females (p<0.001). In the case of the elderly males, the higher the degree of obesity, the higher the energy intake (p<0.05), and 'pre-obesity' consumed the most energy in elderly females. As a result of this study, the higher the obesity rate of the elderly, the higher the prevalence of chronic diseases.

Relationship between body fat distribution and menstrual disorder in Obese Pre-menopausal Korean Women (한국 성인 비만여성의 체지방 분포와 월경 장애의 상관성 분석)

  • Background: Obesity is associated with menstrual disorder. Especially, upper-body obesity affects on female reproductive function. Objectives: The goals were to investigate relation between fat distribution and menstrual disorder in obese pre-menopausal Korean females. The hypothesis were tested that there is a relation between upper body obesity and menstrual disorder. Design: A cross-sectional evaluation of 66 Females (baseline age $32.15{\pm}7,32)$ with body mass index $31.22{\pm}4.05\;kg/m^2$. Body composition was measured using bioimpedence analysis (BIA) and anthropometry was done by same observer. VAS and Multidimensional verval rating scale(MVRS) were checked for menstrual pain. Dual-Energy X-ray Absorptiometry (DXA) was measured for evaluating body fat distribution. Menstrual disorder and body fat distribution were compared using statistical methods. Results: TLFR and WH ratio was higher in menstrual cyclic disorder group than controls. There was negative relationship between VAS and TLFR. Conclusions: The data shows that disorder of menstrual cycle was associated with upper body obesity. WH ratio could be one of the factor of menstrual disorder. VAS was correlated with lower body obesity. Further studies for role of upper body fat distribution on female reproductive function and relationship between menstrual pain and fat distribution thought to be needed.

Screening of anti-obesity drugs, their analogues and prohibited ingredients in slimming foods (식품 중 부정 혼입된 비만치료제 및 사용금지 성분 실태조사)

  • Globally, obesity has been recognized as a serious health problem. Recently, slimming foods for weight loss and maintenance were found to contain anti-obesity drugs, their analogues, and prohibited ingredients. To avoid inspections by the Government, structurally modified analogs have been continuously synthesized. For rapid determination of adulterated slimming products, we simultaneously analyzed 21 illegal compounds using liquid chromatography (LC) equipped with photo diode array, and LC coupled with tandem mass spectrometry. The validation of the method was performed with regard to selectivity, linearity, limit of detection, limit of quantification, precision, and accuracy. We purchased 128 samples from the Korean market and online sources in the year 2015. In 31 samples, 3 illegal compounds were detected as follows: 9.9-135.3 mg/g of sibutramine, 0.2-17.5 mg/g of yohimbine, and 1.8 mg/g of icariin. This simultaneous detection method might prove to be a simple and rapid analysis for monitoring illegal compounds in slimming foods.

The Development of Educational CD-Program for Obesity Prevention and Management for Primary School Students (초등학교 비만예방 및 관리를 위한 교육용 CD 프로그램 개발)

  • Purpose: The study is designed to develop an educational CD-Program for prevention and control of obesity among primary school students. Method: The study is conducted from June 15, 2000 to April 15, 2002. Based on the course of program development suggested by Dick and Cray (1990), the study followed the planning, development, education and evaluation of a program. Result: The developed CD-Program consists 2 parts each for lower and higher grades of primary school students. The introduction part of the first trial for lower grade students uses quiz to encourage their motivations, the body proceeds with motion pictures and animations to trigger their interests. The introduction part of the second trial for the lower grades consists of remembering the exhibition lecture. The first trial for higher grades of primary school students builds on the contents of the low grades. Its body part, how to determine obesity and calculate ones own obesity, puts ones own weight and height in by the mouse. For the second trial of the higher grades, the body consists of life-style, diet, and regiments. Conclusion: The merits of this CD-Program are that to be possible an interaction between teachers and students.

Clinical Trial of Herbal formula (Slim-diet) on Weight Loss in Obese Pre-menopausal Korean Females (비만환자의 체중감량에 있어 슬림다이어트 처방의 유용성 평가를 위한 임상시험)

  • Background: Recently, Obesity has become a social problem in all over the world including Korea. Dietary regulation, exercise and behavior modification therapy are used in the treatment of obesity, but until now, there have been many difficulties in treating it. Objectives: To examine in obese pre-menopausal Korean females the short-term efficacy for weight loss of an herbal formula (Slim-diet). Design: A 6-week longituidinal evaluation of a herbal formula (Slim-diet) in 35 pre-menopausal females (baseline age $31.6\;{\pm}\;6.1$) with body mass index > $25kg/m^2$. Body composition was measured using bioimpedence analysis (BIA) and dual energy X-ray absorptiometry (DXA). Anthropometry was done by same observer. Total cholesterol, total lipid, triglyceride, and LDL-cholesterol were measured. Results: Herbal formula (Slim-diet) treatment produced significant loss of weight (P < 0.001), percent of body fat (P < 0.001, both BIA and DXA) and waist circumference (P < 0.001). It also produced significant reduction of total cholesterol and total lipid levels (P < 0.05).

Trends in Intervention Study for Childhood Obesity in Korea (국내 아동비만 중재연구의 동향 분석)

  • Purpose: The purpose of this study was to identify trends in intervention studies on childhood obesity in Korea. Methods: From 1996, when the first research paper on childhood obesity intervention was published, to 2015, 192 published papers were analyzed using descriptive statistics. Results: The number of studies increased sharply between 1996 and 2007, but declined slightly from then. The majority of studies involved only children as intervention participants. Most were elementary students, and overweight and obese children. Exercise therapy was the most common type of intervention. Parental participation was found in 35 studies, while teacher's participation was found in only one study. In many studies physiological indicators were used as measurement variables, but follow-up was done in only 10 studies. Finally, only a few studies applied a conceptual framework, while a quasi-experimental research design was used for most studies. Conclusion: Examination of trends in intervention studies on childhood obesity in Korea, shows there has been a quantitative increase but not enough improvement in terms of the quality of interventions. Findings in the present study suggest that it is necessary to seek diversity in terms of study participants, interventions and evaluation method along with quality improvement in research methodology.

A Case Report for the Effects of Pharmacopuncture Combined Thread Implantation Therapy to Improve Localized Obesity (부분비만 개선을 위한 매선과 약침의 병행 치료 임상 효과 증례 보고)

  • The aim of this study is to investigate the effect of pharmacopuncture combined thread implantation therapy for treating localized obesity. The study was conducted on 3 subjects with abdominal localized fat, 1 subject with thigh and 1 subject with calf localized fat. We practiced two kinds of pharmacopunture, which were cultivated wild ginseng and contacting Ephedra sinica pharmacopuncture. In addition, we practiced thread implantation to 20 points on muscle membrane layer. We performed these treatments once in 2 weeks, totally for 3 times. After treatments, abdominal circumference and waist-hip ratio were reduced by 4.6 % and 0.13, respectively. In addition, circumferences of thigh and calf were reduced by 5.8 % and 4%. We supposed that this combined therapy could be useful to deal with localized fat deposit. Long-term evaluation and further studies are required to identify effect size of this therapy.

  • The purpose of the study was to identify the motives of women's appearance-management behavior, and examine how women perceived the appearance -management behavior and pursuit of ideal body image. The depth interview method was managed to five female subjects who had experiences in plastic surgery and obesity treatment. The instance analysis used in this study. The results were as follows : There were four types of women's appearance-management behavior. First, women perceived themselves by using other people's evaluation, and it was the first motive of appearance-management behavior. It shows that appearance is not based on the real self-image but is the evaluated self-image by others. Second, women were willing to suffer the pain in the plastic surgery and obesity treatment by the expectation of appearance improvement. It means the result of reducing the difference between the actual self-figure and the ideal self-image. Third, the sexual discrimination culture had an influence on appearance-management behavior. It seems the sense of male superiority spreaded over the Korean society. Lastly, women improved self-satisfaction and self-esteem through their physical appearance as an alternative method for better life.
