족관절 유합술 (Ankle Arthrodesis)

  • Ankle arthrodesis has been considered to be the standard operative treatment for end-stage ankle arthritis, nevertheless currently increasing arthroplasty. Indication for arthrodesis is painful ankle from global arthrosis regardless of the etiology. But it is hard to be carried out in the several circumstance such as infection states, poor vascularity, severe diabetes, prematurity, etc. So thorough evaluation should be done before the surgery, including adjacent joints status. The ideal position for fusion is neutral in flexion, functional valgus, and slightly external rotation. Methods of arthrodesis would be largely divided into two categories as in situ fixation and realignment procedure. The lateral and anterior longitudinal approaches are two common procedures, and fixation modalities are also variable. The long-term results of arthrodesis have been reported. Even the close follow-up have shown subsequent degeneration of adjacent joints, benefits such as reliable pain loss, easy correctability for deformity, and improved functional status with considerable durability can be expected in the most patients.

도심지 터널 근접시공에 의한 기존 지하철 구조물 안정성 평가 (Stability evaluation of existing subway structure by adjacent excavation in urban tunnelling)

  • 도심지에서의 교통인프라 개발이 활발해 지고, 대심도 지하터널을 이용한 도시의 기반시설 확충으로 인하여 기존 운영되거나 설치된 시설물 또는 구조물 상호 간의 근접시공이 많이 이루어지고 있다. 이에 따라 근접시공으로 인한 시설물 또는 구조물에 미치는 안정성 확보여부는 중요한 이슈가 되고 있으며, 관련법에 의하여 지하터널공사에 따른 지하안전영향 평가를 반드시 수행하여야 한다. 본 연구에서는 근접시공으로 인한 정거장구조물 의 거동특성을 파악하기 위해 동북선도시철도 터널이 계획된 노선 상부의 지하철 6호선 고려대역 정거장 구조물을 대상으로 신설터널에 의한 기존 구조물에 미치는 안전영향평가를 수행하였다. 이를 위하여 정거장구조물의 안전영역 평가 및 굴착방법에 대한 검토와 3차원 수치해석을 통한 상세검토를 수행하였다. 구조물 손상도 평가, 궤도 틀림 및 기존 구조물과 건물 안정성 평가 결과 근접터널공사에 따른 기존 정거장 구조물의 안정성은 확보되는 것으로 평가되었다. 본 연구는 도심지 터널공사에서 근접시 공시 기존의 인접구조물에 미치는 영향을 사전에 검토하는 경우 기본적인 기초 참고 자료로 활용될 수 있을 것으로 기대된다.

Comparative Evaluation between 1.5T vs 3.0T MRI in Brain Metastasis According to its Size

  • The purpose of this study was to compare the detection rate of brain metastasis according to size of nodule between 1.5T and 3.0T MRI 대상 및 방법: We reviewed 44 patients with primary tumors and clinical symptoms suggesting brain metastasis. After administration of double dose gadolinium-DTPA, MR imaging was performed with 3D SPGR sequence by 3.0T MRI and then with T1 SE sequence by 1.5T MRI. Consequently, comparison was done in 1.5T T1 SE sequence and 3.0T 3D SPGR sequence. With use of the signal intensity (SI) measurements in the metastatic nodules and adjacent tissue, metastatic nodule-to-adjacent tissue SI ratio were calculated. In each patient, the number of metastatic lesions detected in 1.5T and 3.0T, and their size were assessed qualitatively by three blinded readers.

  • XML 일부분의 변경이 가능한 동적 XML 환경에서 XML의 검색과 변경 기능을 효율적으로 지원하기 위한 방법으로 XML 동적번호 부여 방식에 대한 연구가 활발히 진행되고 있다. 동적 번호 부여 방식은 ORDPATH, DLN과 같이 노드 번호를 부여할 때 인접 노드의 번호에 의존적인 방식과, PSN과 같이 인접한 노드의 번호에 독립적인 노드 번호를 사용하는 2가지의 방식이 있다. 본 논문은 앞의 2가지 동적 번호 부여 방식을 SQL 환경에서 비교하기 위한 환경을 구축하고, W3C XQuery의 변경 형식을 사용하여 그 성능을 비교한다.

인접도로의 유형에 따른 공동주택 주거단지의 도로교통소음 전달영향 및 평가 (A Study on the Prediction and Evaluation of Road Traffic Noise at the Apartment Housing Complex depending on the Types of Adjacent Roads)

  • In this study, computer simulation program was using to identify the effects of road traffic noise propagation depending on the road types which are very variable in dense city. To achieve this goal, the roads should adjacent to housing complex were categorized into 7 types and propagation noise level should predicted then after simulation of noise exposure population calculation are carried out. Followings the results. First, the road types producing the higher noise level are R1, R2, R3 and R7. The lowest one is R6. And R4, R5 showed that some amount of noise level reduction. Second. the R6 road type which is tunnel shaped showed the highest noise level reduction in vertical distance gap. Last, the order of noise exposure population ratio is R1>R2>R7 and R6 showed the lowest.

수로터널 안정성에 미치는 요소를 고려한 근접시공범위에 대한 연구 (A Study on the Near Construction Range Considering the Factors Affecting the Stability of Water Tunnel)

  • 최근에는 도시개발 및 확장으로 지하철, 도로, 대형관로 등 기존 터널 구조물에 인접하여 건설 계획이 증가하고 있다. 기존 터널에 인접한 구조물 계획은 신규 구조물의 시공방법 및 인접 정도, 위치에 따라 터널 안정성 미치는 영향이 다르다. 특히 압력 수로터널은 지반 거동 특성 및 운영 특성에 있어 도로터널 및 철도터널과는 매우 큰 차이를 보인다. 따라서 수로터널의 거동 특성 및 신규공사 공법을 고려하여 근접시공으로 인한 안전영역에 대한 재검토가 필요하다. 본 연구에서는 기존 터널 인접 시공 기준을 조사하고 수로터널 특성을 고려하여 안정성 평가를 수행하였다. 안정성 평가는 운용 중인 터널의 콘크리트라이닝의 열화를 고려하여 수치해석적 방법으로 평가하였다. 또한, 신규 구조물의 말뚝 시공 및 발파굴착에 의한 진동 영향을 검토하여 인접 시공방법에 따른 기존 터널 안정성이 확보되는 영역을 검토하였다. 국내 다양한 근접시공 영역에 대한 기준을 바탕으로 운용 중인 수로터널 특성을 반영하여 안전영역을 재검토하였으며, 이를 바탕으로 근접 안전 시공범위를 제시하였다.

극동 아세아 컨테이너 항만의 능력평가에 관한 연구 (A Study on Evaluating the Ability of the Competitive Container Ports in Far-East Asia)

  • The rapid progress of the intermodal freight transportation in recent years has induced fierce competition among the adjacent hub ports for container transport. This brings increased attention to the evaluation of the port competitive ability. But it is not easy to evaluate the port competitive ability because this belongs to ill-defined system which is composed of ambiguous interacting attributes. Paying attention to this point, this paper deals the competitive ability of container port in Far-East Asia by fuzzy integral evaluation which is adequate to interacting ambiguous attribute problem. For this, the proposed fuzzy evaluation algorithm is applied to the real problem, based on the factors such as cargo volumes, costs, services, infrastructure and geographical sites These are extracted from the precedent study of port competitive ability, etc. The results show that the port evaluation factors come in following order ; services, costs, infrastructure, geographical sites and cargo volumes. There are some interactions(interaction coefficient, ${\lambda}=-0.664$ between evaluation attributes. The port competitive ability comes in following order : Singapore, Hongkong, Kobe, Kaoshiung and Busan. According to the sensitivity analysis, the rank between Busan and Kaoshiung changes when ${\lambda}=0.7$. From the analysis of the results, we confirmed that the proposed fuzzy evaluation algorithm is very effective in the complex-fuzzy problem which is composed of hierarchical structure with interacting attributes.

모자이크 영상의 객관적 품질평가지수 산정 방법 (Calculation of Objective Quality-Evaluation-Index for Mosaic Imagery)

  • 본 연구에서는 모자이크 영상에 대하여 정량적인 품질평가지수에 대한 적용 가능성을 제시하였다. 우선, 영상 품질평가를 위해 접합영상의 접합선을 이용한 연속성 평가와 색상을 이용한 유사성(similarity) 및 대비(contrast)를 이용하여 품질평가지수를 산정하였다. 선정된 품질평가 항목을 기반으로 최종적인 품질평가지수를 산정하여 실험자에 의한 주관적 평가와의 비교, 검증을 수행하였다. 그 결과 정량적 품질평가지수가 주관적 평가지수와 유사한 결과를 나타냄으로써 제시한 영상 품질평가지수를 이용하여 제작된 모자이크 영상에 대한 정량적이고 객관적인 모자이크 영상의 품질평가가 가능하다는 것을 보여주었다.

Evaluation of Tracheobronchial Diseases: Comparison of Different Imaging Techniques

  • Objective: To compare the clinical utility of the different imaging techniques used for the evaluation of tracheobronchial diseases. Materials and Methods: Forty-one patients with tracheobronchial diseases [tuberculosis (n = 18), bronchogenic carcinoma (n = 10), congenital abnormality (n = 3), post-operative stenosis (n = 2), and others (n = 8)] underwent chest radiography and spiral CT. Two sets of scan data were obtained: one from routine thick-section axial images and the other from thin-section axial images. Multiplanar reconstruction (MPR) and shaded surface display (SSD) images were obtained from thin-section data. Applying a 5-point scale, two observers compared chest radiography, routine CT, thin-section spiral CT, MPR and SSD imaging with regard to the detection, localization, extent, and characterization of a lesion, information on its relationship with adjacent structures, and overall information. Results: SSD images were the most informative with regard to the detection (3.95±0.31), localization (3.95±0.22) and extent of a lesion (3.85±0.42), and overall information (3.83±0.44), while thin-section spiral CT scans provided most information regarding its relationship with adjacent structures (3.56±0.50) and characterization of the lesion (3.51±0.61). Conclusion: SSD images and thin-section spiral CT scans can provide valuable information for the evaluation of tracheobronchial disease.

Cone-beam computed tomography-based radiographic considerations in impacted lower third molars: Think outside the box

  • Purpose: This study aimed to evaluate the anatomic circle around the impacted lower third molar to show, document, and correlate essential findings that should be included in the routine radiographic assessment protocol as clinically meaningful factors in overall case evaluation and treatment planning. Materials and Methods: Cone-beam computed tomographic images of impacted lower third molars were selected according to specific inclusion criteria. Impacted teeth were classified according to their position before assessment. The adjacent second molars were assessed for distal caries, distal bone loss, and root resorption. The fourth finding was the presence of a retromolar canal distal to the impaction. Communication with the dentist responsible for each case was done to determine whether these findings were detected or undetected by them before communication. Results: Statistically significant correlations were found between impaction position, distal bone loss, and detected distal caries associated with the adjacent second molar. The greatest percentage of undetected findings was found in the evaluation of distal bone status, followed by missed detection of the retromolar canal. Conclusion: The radiographic assessment protocol for impacted third molars should consider a step-by-step evaluation for second molars, and clinicians should be aware of the high prevalence of second molar affection in horizontal and mesioangular impactions. They also should search for the retromolar canal due to its associated clinical considerations.
