A Clinical Study of Two Patient with Oligomenorrhea treated Carthami flos of Aqua-Acupuncture (홍화 약침을 병행한 희발월경 환자 치험 2례)

  • Objectives : The purpose of this study is to report the effect of oriental medicine about two patients with oligomenorrhea. Methods : Two patients had no special abnormality in BC, CBC, UA, ultra-sono, hormonal test in this study are 25-years-old and 26 years-old female who have over 40-days menstrual cycle. They had treated for 49-days, 21-days each by oriental medicine method. Their herb medicine is On-kyung-tang and Gwa-gi-eum. Main acupuncture points are Hapkok(LI4), Kihae(CV6), Gwanwon(CV3), Choksamni(ST36), Samumgyo(SP6) and moxibuation Gwanwon(CV3) and treated Carthami flos of Aqua-Acupuncture 0.05cc at Samumgyo(SP6). Results : After treatment, their menstrual cycle decreased 33-days, 36-days each. And improved dyspepsia, menstrual pain. Conclusions : Oriental medical methods are effective in two patients with oligomenorrhea had no special abnormality in BC, CBC, UA, ultra-sono, hormonal test.

Understanding of Sasang Constitutions using DNA chip Analysis (DNA chip을 이용한 사상체질의 연구)

  • Objectives : he purpose of this study was to examine the genetic variations and changes of gene expression in the human constitutions. Methods : To analysis variations of individual gene expression, we had selected three groups of volunteers. In each group have a typical constitutional characteristics. By this rime we are analyzed their gene expression patterns by using DNA chip. Results : we can acquire a new information of standard for human constitutions. 1. The 21 genes under express and 3 genes over express in So-Yang constitution 2. The 18 genes under express and 18 genes over express in So-Eum constitution 3. The 16 genes under express and 2 genes over express in Tae-Eum constitution Conclusions : Constitution, QSCCII, Character, Genome, DNA chip.

A study on the Theory of 'Ja-Yeol(刺熱)' in 32nd Chapter of 'So Moon(素問) Yellow Emperior's Nei-Ching(黃帝內經)' (황제내경(黃帝內經) 소문(素問) 자열론(刺熱論)에 대한 연구(硏究))

  • In this thesis, I intend to study the translational and clinical interpretation through the theory of Ja-Yeol, and reached the following conclusions. 1. Liver-Heat-Disease due to absess of the function of expelling and lifting off, that Liver-Yang cannot lift up to upper-warmer, and stagnate liver. I think the symptoms of yellowish urine, abdominal pain, somnolence, fever belong to the syndrome of 'Gi-Bun(氣分)', and the symptoms of ravings with surprising, distending pain of hypochondrium, restless involuntary movement of the limbs, unable to lie flat belong to the syndrome of 'Hyeol-Bun(血分)'. 2. Heart-Heat-Disease due that 'Eum-Gi(陰氣)' in heart cannot lay down and reach to stagnate at heart, inner part. I think the symptoms of unjoy, acute cardiac pain, fidgetiness, well-nausea, headeche, reddish face, anhidrosis, etc. reveal with Heart-Heat-Disease. 3. Spleen-Beat-Disease due that 'Eum-Gi' in spleen cannot lay down and Yin of spleen changs heat. I think the symptoms of heaviness of head, cheek pain, fidgetiness, cyanosis, well-nausea, fever, not to let flex and reflex with back pain, diarrhea with abdominal pain, left and right cheek pain reveal with Spleen-Heat-Disease. I think symptoms of fever, diarrhea with abdominal pain belong to the syndrome of Yin-exhausion. 4. Lung-Heat-Disease due to that 'Eum-Gi' in lung cannot lay down. When 'Wi-Gi(衛氣)' stagnates at external part, I think, the symptoms of intolerance to wind and cold, yellowish fur, fever reveal. When Wi-Gi stagnates at lung, inner part, I think, the symptoms of dispnea with cough, pain on chest and back, unable to breath deeply, hydrosis and chilling reveal. 5. Kidney-Heat-Disease, in that the symptoms of back pain, leg aching, extreme thirst and frequently drink, fever, pain and stiffness of nape, cooling and aching leg, heat on plantar pedis, not trying to speak reveal is regarded external heat disease of 'Tai-Yang-Gyeong's(太陽經)' disease that asthenic fever open 'Tai-Yang-Gyeong' and lift by not enough of 'Yang-Gi(陽氣)' lifeing up from Kidney space, the water space of five elements.

  • This study has been carried out to investigate Pyu-Juk ( 肺積 ), Pyu-Ong ( 肺癰 ), Pyu-Ju ( 肺疽 ), Pyu-Am ( 肺癌 ) by referring to 56 literatures. The results were obtained as follows; 1. Analysis on Physical Traits of Sasang Types Using Bioelectrical Impedance Analysis (임피던스 분석을 활용한 사상인의 신체계측 연구)

  • The purpose of this study was to measure circumference of four body parts of each Sasang groups with Bioelectrical Impedance Analysis (BIA), and evaluate its usefulness for Sasang type diagnostic test. We obtained the circumference measures of neck (CN), chest (CC), waist (CW), and hip (CH) of 91 college students using BIA and standardized by sex and age. The validity of Sasang group prediction with CC was evaluated. The Tae-Eum group (104.87${\pm}$5.19, 108.47${\pm}$4.96, 116.00${\pm}$9.39, 110.4${\pm}$5.29. neck, chest, waist and hip, respectively) has significantly (p<0.001) bigger circumferences than So-Yang (97.79${\pm}$3.82, 100.02${\pm}$4.21, 101.02${\pm}$7.88, 101.95${\pm}$4.06) and So-Eum (95.63${\pm}$4.67, 96.93${\pm}$4.80, 97.63${\pm}$7.72, 99.86${\pm}$4.40) groups at very four measures. Discriminant functions with CC can be used for Tae-Eum Sasang type diagnostic test with 91.2% of overall Percentage Correctly Predicted (PCP). Tae-Eum type-specific sensitivity was 73.3% and Tae-Eum type-specific specificity was 94.7%. The strength and weakness of measurement and standardization methods for Sasang type diagnostic test, and the methods for the standardization of physical traits with sex and age were discussed. Our study showed distinctive physical features of Tae-Eum type and the usefulness of BIA for the Sasang type diagnosis.

Study on the Sasang Type Diagnosis Using Objective Biopsychological Measures (객관적인 생리심리 지표를 사용한 사상체질 진단 연구)

  • Introduction The previous study showed that the psychological Sasang Personality Questionnaire (SPQ) and physical Body Mass Index (BMI) were clinically useful for predicting Sasang types diagnosed with certified clinical specialist and QSCCII. The purpose of current study was to examine its clinical usefulness using large sized clinical sample. Methods The age, sex, height, weight, circumference of waist and hip, Sasang Personality Questionnaire (SPQ), and Sasang type of 2,049 participants listed on the Korean Medicine Data Center were acquired. The anthropometrics of BMI, Ponderal Index (PI), Waist-Hip Ratio (WHR), and Basal Metabolic Rate (BMR) were calculated. The discriminant analysis using SPQ subscales, BMI, age and sex were performed to predict Sasang types. And, the ANCOVA with age and sex as covariate was also used to examine differences among diagnosed and predicted Sasang type groups in biopsychological features. Results The discriminant analysis showed 82.0% of percentage correctly predicted. And, the biopsychological characteristics of predicted Sasang type groups were significantly similar to those of diagnosed Sasang type groups. In both predicted and diagnosed Sasang type groups, the anthropometric measures were in increasing order of So-Eum, So-Yang, and Tae-Eum types, and the psychological measures were in increasing order of So-Eum, Tae-Eum, and So-Yang types as shown in previous studies. Discussion and Conclusion The well validated biopsychological variables of SPQ and BMI showed theoretical and clinical usefulness, and were found to be useful for diagnosing Sasang types in clinical setting. The current study might be useful for the East-West integrative medicine and evidence-based teaching.

A Study on Constitution Therapy Based on Hyeong-Gi Concept (형기(形氣)의 개념으로 이해한 체질치료 연구)

  • Objectives : To find appropriate constitutional therapy by understanding Hyeong and Gi from the perspective that "Gi is inner position, and Hyeong is outer position, and Yang converts to Gi, Eum makes Hyeong(氣裏形表, 陽化氣 陰成形)". Methods : 1. Defined Hyeong(形)and Gi(氣)and studied Wonhyeong(圓形) and Banghyeong(方形), the shapes that are formed as a result of Gi-movement. 2. Used 'Hyeong-Gi' concept to analyze physiological and pathological phenomena; classifies parts of the body into Sasang by extrapolating from WonBang concept; and studied Sasangchejil. 3. Based on the above 1 and 2, studied medical direction of Sasangchejil written in Hwangjenaegyeongsomun Tongcheon Results : 1. Wonhyeong is the result of Yang(陽) movement, whereas Banghyeong is the result of Eum (陰) movement. 2. Hyeong(body) can be classified into Won(圓, circle) and Bang(方, square) or more specifically into Won of Won, Bang of Won, Won of Bang, Bang of Bang. Each corresponds to Taeyang(太陽), Soyang(少陽), Taeeum(太陰), and Soeum(少陰). 3. The constitutional therapy can be formulated by refering to "Tongcheon(通天)" and by taking into consideration that each Sasang constitution has a bias toward Mok(木), Hwa(火,) Gum(金), Su(水). Conclusions : Basic treatment for round shaped people needs to be focused on a cure of Boeum(補陰) and Bojeong(補精), whereas for angular shaped people, treatment should be about Bogi(補氣) and Boyang(補陽). Treatment for Taeyangin(太陽人) should be about strengthening Ganeum(肝陰) and reducing Paeyang(肺陽) and for Soyangin(少陽人), the treatment needs to be strengthening Sineum(腎陰) and diminish Biyang(脾陽). Taeeumin(太陰人) should receive a cure that reinforces Pyeyang(肺陽) and brings Ganeum(肝陰) down, whereas Soeumin(少陰人) should have a cure that strengthens Gi and increases Yang.

A Study on Transmission and Transmutation of Disease in "Hwangjenaegyeong(黃帝內經)" ("황제내경(黃帝內經)"에 나타난 병(病)의 전변유형(傳變類型)에 관한 고찰(考察))

  • Many chapters of the Hwangjenaegyeong[HN] explain the process of transmission and transmutation of disease. The transmission and transmutation process in the HN can be categorized into one between the viscera and bowels, and another of the external pathogenic gi itself. The process between the viscera and bowels indicates the transport of the pathologic burden between each viscera and bowel. This again is categorized into three types. Interpromoting, intercontrolling and that by Saeng-yang(生陽), Sa-eum(死陰). Next, the transport of the pathogenic gi can be categorized into one moving inwards from the exterior according to personal traits, and that according to the three Eum and three Yang. Although there are numerous types of transmission and transmutation, there are two main criteria in understanding the process. First, whether the process is in accordance with the physiological or natural flow of the body. Interpromoting and three Eum three Yang processes are such examples. To follow the physiological flow of the body means to correspond to either the Heaven and Earth or the original physiology of the human body. Therefore, the disease progresses according to a certain date or season. This indicates a partial malfunction in the circulation of the vital energy, which is relatively easy to recover. In contrast, there are processes that go against the physiological flow, for example, intercontrolling transmission and transmutation. This process focuses on the movement of the pathogenic gi rather than the vital gi. The disease progresses regardless of the flow of the vital energy, and sequential functional damage occurs accordingly. Consequently, as the transmission and transmutation continue, formerly passed organs are left damaged, and the whole process is headed towards death. The second criteria for understanding the process is whether it is cyclic or not. To have a cyclic pattern means that the occurrence of a disease and the time of death is not fixed. Transmission and transmutation processes that have a cyclic pattern mostly follow the physiological flow of the body. As a result, they rarely end in deaths, and the process is centered on vital energy. On the other hand, those with acyclic patterns have a fixed occurrence and death point in the course of the disease. They are mostly unnatural processes, found in fatal acute diseases or consumption diseases.

Correlation between Sasang Constitution and Eight Principle Pattern Identification, Qi-Blood Pattern Identification, Bing-Xie Pattern Identification by using Oriental Diagnosis System (전문가시스템을 활용한 사상체질과 팔강변증, 기혈변증, 병사변증간의 상관관계)

  • Oriental Diagnosis System(ODS) is an artificial intelligence program that utilize entered diagnosis knowledge, determine patient's disease and decide right medicine. The purpose of this study is to find a correlation between pattern Identification in Korean medicine and each sasang types(Tae-Eum and So-Yang) by analyzing ODS diagnosis result. Eventually our study secure availability of using ODS program at clinical training or developing diagnosis program. Subject of this study is 50 patients who was performed Sasang constitution diagnosis (28 patients were Tae-Eum and 22 patients were So-Yang). We analyize patient's diagnosis records by using ODS program and obtained result about pattern Identification. We used SPSS statistics 23 in analyzing the differences of the scores of Eight Principle Pattern Identification, Qi-Blood Pattern Identification, and Bing-xie Pattern Identification in each Sasang types (Tae-Eum, So-Yang). The Heat and Heat-moisture scores were significantly different(p<0.05) and Qi-Blood Pattern Identification scores were not different in each Sasang types(p>0.05). And Weight was significantly different in each Sasang types(p<0.05). It is hard to generalize the result because subject of this study was not enough and had sample speciality(tinnitus patients). However, we explained correlation between pattern Identification in korean medicine and each sasang types based on quantifiable and objective evidence system. it can be used at education of korean medicine and evidence of practice diagnosis. Futhermore, there have been no studies about anaylizing correlation between pattern Identification in Korean medicine and each sasang types using ODS program. So it is worthy of being utilized at clinical evidence data of ODS program.
