Translation and validation of the Turkish version of the Psychosocial Impact of Dental Aesthetics Questionnaire

  • Objective: The aim of this study was to translate the Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ) into Turkish, validate the questionnaire, and provide a cross-cultural adaptation. Methods: The translation process included the following steps, which were performed by a translation committee: (1) translation into Turkish, (2) back translation into English, (3) pretesting, and (4) cross-cultural adaptation. The Turkish version of the PIDAQ was produced subsequent to the translation process. Validity and reliability were measured using the Perception of Occlusion Scale and the aesthetic component of the Index of Orthodontic Treatment Need. The questionnaire was administered to 260 individuals (age range, 18-30 years; mean age, $20.50{\pm}1.9$ years). Structural validity was assessed via factor analysis, and internal consistency was measured using Cronbach's alpha coefficient. Results: Factor analysis revealed a four-factor structure, with factor loadings for included items ranging from 0.380 to 0.868. Few questions were shuffled among domains various factor loadings. Cronbach's alphas for the Turkish version of the PIDAQ ranged from 0.534 to 0.904. Mean scores for the PIDAQ subscale and total scores differed significantly according to Index of Orthodontic Treatment Need and Perception of Occlusion Scale scores. Conclusions: This study provided a Turkish version of the PIDAQ, which could be a useful tool in the evaluation of the psychosocial impact of malocclusion in young Turkish adults.

Customized abutment and screw-type implant prostheses after cementation based on the digital intra-oral impression technique (구강 내 디지털 인상채득을 통한 맞춤형 지대주와 시멘트 합착 후 나사형 임플란트 보철 수복 증례)

  • Fixed dental prostheses such as inlay, onlay, crown, and bridge fabricated by CAD/CAM technique combined with digital impressions is getting popular due to the recent rapid progress of digital impression taking system. For the scope of implant prosthesis, however, digital intra-oral scan hasn't been actively utilized for the fabrication of superstructures. In this case report, 6 cases of titanium-milled custom abutment based on the iTero intra-oral scan data were introduced, five of them were restored with screw-type prosthesis after cementation (SCRP) and the clinical results were satisfactory on restoring the function and esthetics.

Treatment of the cleft lip and palate patient with few remaining posterior teeth using hybrid telescopic crown denture (치주질환에 이환된 소수 잔존치를 가지는 구순구개열 환자에서의 상악 혼성 이중관 의치를 이용한 수복증례)

  • For individuals with cleft lip and palate, the removable partial dentures (RPDs) have been an important treatment option. Some modifications from conventional prosthetic treatment may be necessary to achieve satisfactory functional and esthetic results in cleft patients. In case of cleft palate patient with periodontally compromised and only posterior few remaining teeth, removable partial prosthesis connected to telescopic crown can be the alternative treatment option. When connected to the RPD, telescopic crowns increase the prosthetic stability and retention, optimize favorable force transmission to the long dental axis, and improve esthetics. And the cross arch stabilization of double crown denture helps to stabilize both divided maxillary ridges in CLP patient. This case present one adult CLP patient using an RPD connected with hybrid telescopic crowns with friction pins to improve not only retention and stability but also aesthetics.

  • In the treatment of esthetically important areas such as maxillary anterior teeth, they should be corresponded with surrounding tissues, and shape of the smile line, soft tissue, and hard tissue, also the anatomical shape and proportion of the teeth should be considered as well. Esthetic analysis includes facial analysis which evaluates the proper parallelism between the occlusal plane and the horizontal reference line, dentolabial analysis which assesses the position of the incisal edge and the coherence between the occlusal plane and the commissural line, tooth analysis which evaluates not only esthetics but also morphology and appearance for proper function, and gingival analysis which forms ideal outline of gingival margins. A maxillary anterior diastema can be esthetically restored through the systematic diagnostic approach and treatment planning, and orthodontic, prosthetic, and conservative treatment can be applied for the treatment.

Comparison Study of Wear Resistance Among Several Denture Teeth Opposing Various Restorative Materials (대합되는 재료에 따른 합성수지 인공치의 마모저항성에 관한 비교 연구)

  • Partial or complete prosthesis is needed when teeth are lost due to various kinds of reason. Artificial teeth recover occlusion instead of natural teeth. Artificial teeth are required of esthetics, fragile resistance and abrasive resistance. Artificial tooth is made of acrylic resin or porcelain. Nowadays, acrylic resin artificial teeth are mainly used. Acrylic resin teeth are occluded with natural teeth, gold alloy, Ni-Cr alloy or porcelain etc. Acrylic resin teeth have similar translucency, gloss of natural teeth. And it has good chemical bond with denture base material, but it has low wear resistance. The aim of this study is to compare wear resistance among several denture teeth(Endura, SR-orthosit-PE, Planustar) and between artificial resin denture teeth and opposing 3 restorative materials(gold, Ni-Cr alloy, porcelain). Wear tests were conducted with a rotating wear testing apparatus(pin-on-disk type wear tester) under conditions of rpm 180, 75 minutes and constant loading of 50N. The upper part was the cusp of maxillary first molar and the lower part was a disk type restorative materials. To make similar oral environment, water was supplied continually. The acrylic resin teeth wear was determined by weighing the cusp each 5 minutes during 75 minutes test. Vicker's hardness tester was used to evaluate the surface hardness of test specimens. The SEM was used to evaluate the wear surfaces. The results were as follows: 1. Wear rates of acrylic resin teeth opposing to the restorative materials were high in order of Porcelain, Gold, Ni-Cr alloy (p<.05). 2. Wear resistance rate opposing to the Porcelain disk, was shown in order of Endura, SR-orthosit-PE, Planustar. The wear rate of opposing to porcelain disk was above two times more than that of other groups (p<.05). 3. Wear resistance rates opposing to the Gold, Ni-Cr alloy disk, was shown in order of Endura, SR-orthosit-PE, Planustar (p<.05). 4. A degree of the surface hardness is directly proportional to the degree of wear resistance. There are statistically significant differences between each groups (p<.05).

Labial and lingual/palatal bone thickness of maxillary and mandibular anteriors in human cadavers in Koreans

  • Purpose: The aim of this study is to evaluate the buccal and lingual bone thickness in the anterior teeth and the relationship between bone thickness and the tissue biotype. Methods: Three male and two female human cadaver heads (mean age, 55.4 years) were used in this study. First, the biotype of periodontium was evaluated and categorized into a thick or a thin group. Next, full thickness reflections of the mandible and the maxilla to expose the underlying bone for accurate measurements in the anterior regions were performed. After the removal of the half of the alveolar bone, the probe with a stopper was used to measure the thickness of bone plate at the alveolar crest (AC), 3 mm apical to the alveolar crest (AC-3), 6 mm apical to the alveolar crest (AC-6), and 9 mm apical to the alveolar crest (AC-9). The thickness of the buccal plates at the alveolar crest were $0.97{\pm}0.18\;mm$,$0.78{\pm}0.21\;mm$, and $0.95{\pm}0.35\;mm$ in the maxillary central incisors, lateral incisors, and canines, respectively. The thickness of the labial plates at the alveolar crest were $0.86{\pm}0.59\;mm$, $0.88{\pm}0.70\;mm$, and $1.17{\pm}0.70\;mm$ in the mandibular central incisors, lateral incisors and canines, respectively. Conclusions: The thickness of the labial plate in the maxillary anteriors is very thin that great caution is needed for placing an implant. The present study showed the bone thickness of maxillary and mandibular anteriors at different positions. Therefore, these data can be useful for the understanding of the bone thickness of the anteriors and a successful implant placement.

A SOFT TISSUE ANALYSIS ON FACIAL ESTHETICS OF KOREAN YOUNG ADULTS (한국인 성인의 악안면 연조직의 심미적 안모형태 분석에 관한 연구)

  • This study was designed to analyze morphological characteristics of Korean young adults, norms and standard deviation of variables, sexual differences, correlationship between each area of face and correlationship between hard tissue and soft tissue. The primary sample consisted of 45 males and 57 females who were early and middle twenties and had acceptable profile, no history of previous orthodontic treatment, absence of remarkably large overjet and overbite, full complement of permanent teeth, Class I skeletal and dental relationships and good vertical facial proportions. Their cephalograms were analyzed morphologically with a computer morphometrics. Then the final sample - 25 males and 38 females - were selected within 1 S.D. of E-line, ANB, P/A facial height ratio, Interincisal angle, L1 to A-Pog, ODI and APDI. The results of the study were as follows: 1 In the form and proportion of facial skeleton there were no significant differences between males and females, but in the size males were larger than females. 2. The dental protrusion patterns had no significant sexual difference and no significant correlationship between protrusion of upper lip and inclination of upper incisor. But mentolabial angle had positively correlated with interincisal angle and negatively with inclination of upper and lower incisor. 3. In the relationship between nose and soft-tissue profile, males were larger than females in nasal length, height and angular measurements. 4. In analysis of soft-tissue profile, males were larger than females in the length and thickness. In the angular measurements and proportion of soft-tissue profile, there were no significant differences between males and females.

SURGICAL CORRECTION OF A CUPID'S BOW USING A DOUBLE Z-PLASTY: REPORT OF A CASE (중복 Z-성형술을 이용한 큐피드활의 교정)

  • Cupid's bow, upper lip, columella, and ala of nose are esthetically important, in which their symmetrical plasties determine success or failure of the surgery in cleft lip and palate patients. Z-palsty and its modifications are simple and effective. The double Z-plasty is economical in surgical time and esthetics in reconstructing the cupid's bow compared with other methods. We report a 29-year-old male patient who represented reversed cupid's bow and whistle deformity after the primary repair of the cleft lip. He was corrected the cupid's bow using a double Z-plasty. The reversed cupid's bow was corrected to a normal shape and the whistle deformity disappeared. The tightness of the upper lip was relieved after the operation. The double Z-plasty was esthetically successful because the procedure was limited in the vermilion and did not produce a new scar on the skin. Furthermore, the lip tubercle became protuberant. The cupid's bow was symmetrically smoothly curved at 4 years after the operation. These results indicate that the double Z-plasty is simple, less traumatic to the surrounding tissues, and suitable for the correction of whistle deformity.

Reconstruction of Combined Oral Mucosa-Mandibular Defects Using the Vascularized Myoosseous Iliac Crest Free Flap

  • The authors present five cases of combined oral mucosa-mandible defects reconstructed with the vascularized internal oblique-iliac crest myoosseous free flap. This technique has many advantages compared to other conventional methods such as the radial flap, scapula flap, and fibula flap. Vascularized iliac crest flaps provide sufficient high-quality bone suitable for reconstructing segmental madibular defects. Although fibular flaps allow longer donor bone tissue to be harvested, the iliac crest can provide an esthetic shape for mandibular body reconstruction and also provides sufficient bone height for dental implants. Conventional vascularized iliac crest myoosseous flaps have excessive soft tissue bulk for reconstruction of intraoral soft tissue defects. The modification discussed in the present article can reduce soft tissue volume, resulting in better functional reconstruction of the oral mucosa. Another advantage is that complete replacement of the oral mucosa is observed in as early as one month post-operation. The final mucosal texture is much better than that obtained with other skin paddle flaps, which is especially beneficial for the placement of dental implant prostheses. Donor site morbidity looks to be similar to, if not less than that observed for other modalities in terms of function and esthetics. For combined oral mucosa-mandible defects, the vascularized internal oblique-iliac crest myoosseous free flap shows good results with respect to hard and soft tissue reconstruction.

Reinforcement of provisional restoration with cast metal framework: A case report (주조 금속 구조물을 이용하여 강화한 임시보철물 수복 증례)

  • Provisional restoration plays an important role in the process of restoration with fixed partial denture by providing protection and restoring function and esthetics of abutment tooth. This is especially true in patients requiring use of provisional prosthesis for a long term. However, such situations may lead to higher possibility of provisional prosthesis fracture, and if expected so, reinforcement of provisional prosthesis should be considered. In this article, a patient requiring prosthetic treatment with increase of vertical dimension of occlusion was introduced. Due to increased vertical dimension of occlusion, the provisional restoration was expected to withstand relatively larger amount of load during a relatively long observation period. In order to minimize possible occurrence of fracture, reinforcement of the provisional restoration was sought by insertion of cast-metal framework and by using bis-acryl resin. The reinforced provisional restoration showed satisfactory results during the observation period of 4 months.
