전치부는 심미적인 관점에서 치열 및 연조직과의 조화가 매우 중요하다. 상악 전치부의 심미적인 치료는 술 전 정확한 진단과 치료계획이 필수적인데 술자는 특히 치관부 크기의 적절한 비율과 치은 형태의 대칭에 대해 충분히 고려해야 한다. 만약 치은의 형태가 비심적이고 부자연스러운 경우에는 치은 절제술, 골삭제를 동반한 치관 연장술, 교정적 정출술 등을 시행할 수 있다. 치관의 형태 및 위치가 부적절한 경우에는 술 전 진단 모형 및 왁스 업을 통해 교정 치료나 완전도재관, 라미네이트 베니어 등의 심미적 보철치료를 계획할 수 있다. 본 증례들에서는 치아의 위치 이상과 연,경조직의 부조화를 가진 전치부를 교정 치료 및 치은 성형과 심미적 보철 치료를 시행함으로써 만족할만한 결과를 얻었기에 이를 보고하고자 한다.

The preservation of a healthy periodontal attachment is the most significant factor in the long-term prognosis of a restored tooth. The 'Biologic Width' is composed of the connective tissue attachment and the epithelial attachment in the dentogingival junction. The violation of the biologic width may result in a progressive inflammatory process and crestal bone loss. So a careful soft tissue management is needed to preserve it for the gingival health and an esthetic restoration. The following clinical cases show the five different situations of the violation of the biologic width and their management.

Intrinsic discoloration of permanent anterior teeth is a continuing esthetic problem. Tetracycline ingested during amelogenesis has long been recognized as predisposing factor in intrinsic staining. Methods for restoring original tooth color have included (1) complete PFM crown coverage (2) porcelain laminate veneers (3) direct bonding of composite resin, and (4) bleaching. In the case of tetracycline-stained upper anterior teeth, We authors, obtained the satisfactory results by means of gingivectomy, preliminary bleaching and porcelain laminate veneer restoration.

BACKGROUND. Though implant dentistry is very successful and predictable in treatment of patients with destroyed dentition, there are some cases with limitations to implant therapy. In these cases, alternative treatment modality should be considered. CASE DESCRIPTION. A patient with destroyed dentition was rehabilitated with a lateral rotational path removable partial denture. According to the diagnosis, we determined to raise vertical dimension for esthetic and functional restoration. The final restoration was performed after four months of provisional period. CLINICAL IMPLICATION. The edentulous patients with compromised esthetics and functions can be successfully treated with a rotational path removable partial denture through adequate treatment planning and precise laboratory procedure.

최근 치과 임상 영역에 디지털이 접목되면서 디지털을 활용한 임상 진료가 점차 일반화되어가고 있다. 현재까지의 디지털 치과진료는 구강스캔 등의 자료채득, 가이드를 활용한 수술, CAD CAM을 사용한 보철물 제작 등의 직접적인 부분에 다소 집중된 감이 있으나, 치과 진료의 시작인 진단 및 치료계획 수립 단계에서도 디지털의 활용은 큰 효용을 가지고 있다. 이번 증례에서는 전치부 수복을 위해 내원한 환자에게 구치부를 포함한 상악 전체의 수복을 이해시키고, 교합고경을 재설정하여 수복하는 과정의 가이드라인으로 사용한 digital mockup의 활용 예를 보고하고자 한다.

As the interest for esthetic restoration is increasing, the usage of composite resin is increasing. The usage of composite resin is not limited to anterior teeth but is spreading to posterior area using direct ＆ indirect methods. Generally, dual or chemical cure resin cement has been used for setting composite or porcelain inlay restoration. However, chemical cure resin cement has limited working time and it's difficult to remove excess cement from the tooth and the restoration. The dual cured composite is also difficult to remove from the tooth surface.(omitted)

최근 삼차원적 안면 스캔 (facial scan) 및 악운동 (jaw motion) 등의 데이터를 통합하여 동적 교합을 나타내는 가상 환자를 형성함으로써 심미적인 전치부 고정성 보철물을 제작하는 방법이 소개되고 있다. 이를 통해 진단과정에서 환자와의 원활한 소통이 가능하며, 심미적인 보철 치료의 예지성을 높일 수 있고, 교합조정의 가능성을 낮출 수 있다. 본 증례에서는 상악 전치부 심미 보철 수복이 필요한 환자에서 구강 스캔 데이터와 삼차원 (3D) 안면 스캔데이터, 환자의 악운동 기록을 computer-aided design (CAD) 소프트웨어 상에서 통합하여 동적 교합을 나타내는 가상 환자를 형성하였다. 이를 통해 치료의 결과를 시뮬레이션하고 심미적인 상악 전치부 고정성 보철물을 제작 및 수복하였다. 또한, 안정적인 교합관계를 회복하고 적절한 전방유도가 형성되었는지 확인하기 위하여 각각 치료 단계별로 환자의 교합을 비교 평가하였으며 심미적, 기능적으로 만족스러운 결과를 보였기에 이를 보고하는 바이다.

상악 전치부는 심미적으로 민감한 부위이며, 치아 외형과 치은의 조화도 치료의 성공과 밀접한 관련이 있다. 이를 위해서는 정확한 진단과 치료계획이 필수적이며, 필요 시 치아 주위 연조직에 대한 처치 등이 동반될 수 있다. 연조직 처치로서 외과적 치관연장술 중 하나인 치은절제술을 고려할 수 있으며 이를 통해 심미적인 보철 수복을 할 수 있다. 상악 전치부는 심미뿐 아니라 기능 또한 고려하여야 한다. 지대치 주모형과 순측만을 수정한 진단 납형 모형을 스캔 및 중첩하여 디자인 및 가공하면, 최소한의 교합조정을 통해 기존의 편안했던 교합을 재현해 줄 수 있다. 본 증례에서는 치은절제술 및 모형 중첩을 통해 심미 수복을 진행하였고, 그 결과 심미와 기능모두에서 환자의 만족도를 높였기에 이를 보고하는 바이다.

상악 전치부의 보철 수복은 치과 분야에서 심미성이 크게 요구되는 치료이다. 상악 전치부의 보철 수복을 통하여 치아의 외형, 배열, 색조 등을 개선하면 심미성을 향상시키는 것이 가능하다. 특히 전치부 심미 수복을 위해서는 진단과정에서 환자의 안모 및 치아와 입술과의 관계, 치아와 치은 관계 등을 평가하고, 환자가 요구하는 바를 정확히 파악할 필요가 있다. 환자와 술자 및 기공사 간의 원활한 의사소통을 통하여 환자의 요구사항을 충실히 반영하기 위한 도구로서 DSD (digital smile design) 개념을 적용할 수 있다. 이 증례에서는 상악 중절치의 선천적 결손으로 인하여 대칭성을 구현하기 불가능한 상황에서 DSD를 활용하여, 환자 안모와 상악 전치부의 전체적인 조화를 추구하는 것을 목표로 하였으며, 이를 통해 환자와 술자 모두 만족할 만한 심미적인 결과를 얻었기에 이를 보고하고자 한다.

The Schwachman-Diamond syndrome is an autosomal recessive syndrome(1/20,000 births), consisting of pancreatic insufficiency, neutopenia, which may be intermittent, neutrophil chemotaxis defects, metaphyseal dysostosis, failure to thrive and short stature. Patients present in infancy with poor growth and grease, foul-smelling stools that are characteristic of malabsorption. These children can be readily differentiated from those with cystic fibrosis by their normal sweat chloride levels, lack of the cystic fibrosis gene, and characteristic metaphyseal lesions. Pathologically, the pancreatic acini are replaced by fat with little fibrosis. The neutropenia may be cyclic. Recurrent pyogenic infections otitis media, pneumonia, dermatitis(fig 1), sepsis are common and a frequent cause of death. In dental examination, these patients had a poor oral hygine and moderate generalized marginal gingivitis, also show delayed primary tooth exfoliation and oral development. This report illustrates a case that pancreatic agenesis 6 yeas-old boy with various esthetic dental problems has been served the esthetic dental restoration of 6 years.
