임플랜트 지대주의 삭제과정이 결합부 안정성에 미치는 영향 (THE EFFECT OF PREPARATION PROCEDURE ON IMPLANT-ABUTMENT JOINT STABILITY)

  • Statement of problem: Little is known about the effect of abutment preparation procedure on do-torque values in different implant platform and the relationship of final do-torque values with different implant platform size. Purpose: This study evaluated the effect of abutment preparation procedure on do-torque values in different implant platform and the relationship of final do-torque values with different implant platform size. Material and method: Six ITI implants (2 narrow-neck implants, 2 regular-neck implants, 2 wide-neck implants) and six Branemark implants (2 narrow platforms, 2 regular platforms, 2 wide platforms) were embedded in each acrylic resin block with epoxy resin. Eighteen $synOcta^(R)$ abutments (6 narrow-neck implant-abutments, 6 regular-neck implant-abutments, 6 wide-neck implant-abutments) and eighteen esthetic abutments (6 narrow platform-abutments, 6 regular platform-abutments, 6 wide platform-abutments) were tightened to each implant with digital torque gauge. Initial do-torque values were measured using digital torque gauge. After preparation of abutments, Final do-torque values were measured with digital torque gauge. Results and conclusion: 1. Screws loosening or abutments motion were not detected in all experimental group, but some scratches of implant-abutment joints were detected in all group 2. Reduction ratios of final do-torque values were greater than initial do-torque values in all measured group, except in narrow-neck implant-abutment group (p<0.05). 3. Reduction ratios of final do-torque values in wide-neck implant-abutment group were greater than regular-neck implant-abutment group (p<0.01). 4. The greatest standard deviation value was detected in wide platform group in both implant systems.

악안면 보철용 폴리우레탄과 실리콘의 접착도에 관한 실험적 연구 (A STUDY ON THE ADHESIVENESS OF SILICONE AND POLYURETHANE SHEET IN MAXILLOFACIAL PROSTHESES)

  • The material of choice for functional and esthetic reconstruction of maxillofacial defects is silicone. Silicone has appropriate physical properties for maxillofacial prosthesis but it has weak edge strength. Therefore, a proper combination of silicone and polyurethane sheet is recommended to improve this weakness. Various primers are also used to enhance the adhesive strength between silicone and polyurethane sheet. The purpose of this study was to determine the adhesive strength of silicone and polyurethane sheet. Silicone elastomer mixture was made by admixing MDX4-4210 elastomer (40%) and Silastic Medical Adhesive Type A(60%). This silicone elastomer mixture was attached to polyurethane sheet, using one of three different primers(1205, S-2260, or A-304), treated for 1, 2, 4, 6, and 8 hours. These were then polymerized in room temperature, dry-heat oven or microwave oven. Six specimens per each group, a total of 270 specimens were prepared for final test. The differences of T-peel bonding strengths were then determined by a test. The differences of T-peel bonding strengths were then determined by a test method that was recommended by American Society for Testing and Materials C794-80. The results were statistically analyzed using the ANOVA and Mutiple Range Tests(Tukey' HSD). The reults were as follow. 1. Type of primer, primer reaction time, and methods of polymerization showed significant correlation on the T-peel bonding strengths in adhesiveness between silicone and polyurethane sheet. 2. A-304 primer showed statistically higher in T-peel bonding strength than otehr type of primers except for the polymerization in microwave oven with reaction times of 2, 6 hours(p<0.05). 3. No significant differences in T-peel bonding strength were observed among the polymerization methods. 4. The effect of reaction time by the primer type and polymerization method showed statistically significant differences in bonding strength among different reaction times. And in most cases, reaction time of 1 or 2 hours showed higher T-peel bonding strength.

Comparative analysis of transmittance for different types of commercially available zirconia and lithium disilicate materials

  • PURPOSE. Translucency and colour stability are two most important aspects for an aesthetic dental restoration. Glass ceramic restorations are popular amongst clinicians because of their superior aesthetic properties. In the last decade, zirconia has generated tremendous interest due to its favorable mechanical and biological properties. However, zirconia lacks the translucency that lithium disilicate materials possess and therefore has limitations in its use, especially in esthetically demanding situations. There has been a great thrust in research towards developing translucent zirconia materials for dental restorations. The objective of the study was to evaluate and compare the transmittance of a translucent variant of zirconia to lithium disilicate. MATERIALS AND METHODS. Two commercially available zirconia materials (conventional and high translucency) and 2 lithium disilicate materials (conventional and high translucency) with standardized dimensions were fabricated. Transmittance values were measured for all samples followed by a microstructural analysis using a finite element scanning electron microscope. One way analysis of variance combined with a Tukey-post hoc test was used to analyze the data obtained (P=.05). RESULTS. High translucency lithium disilicate showed highest transmittance of all materials studied, followed by conventional lithium disilicate, high translucency zirconia and conventional zirconia. The difference between all groups of materials was statistically significant. The transmittance of the different materials correlated to their microstructure analysis. CONCLUSION. Despite manufacturers' efforts to make zirconia significantly more translucent, the transmittance values of these materials still do not match conventional lithium disilicate. More research is required on zirconia towards making the material more translucent for its potential use as esthetic monolithic restoration.

두께에 따른 GI Cement의 색상 변화 및 각 Shade 내의 색상 변화 추이도 (COLOR CHANGE WITH CEMENT THICKNESS AND COLOR SHIFT PATTERN OF EACH SHADE ERIES IN GI CEMENT)

  • Glass ionomer cements, which had been developed in 1972, were widely used as an agent for cementation, base, pit and fissure sealant, and esthetic filling material with the advantages of excellent biocompatibility, anti-cariogenic fluoride release. Specimens were made to evaluate the color change of Glass ionomer cement with a newly developed improved Fuji II Le. Specimens for color change study with cement thickness of 1, 2, 3mm were made and those for the study of color shift pattern were made with the assumption that new mid-shade can be made by mixing of equal amount of both shades in 2mm thickness. After 24 hours in a $37^{\circ}C$ incubator, CIELAB color spaces were evaluated with a spectrophotometer. The results were as follows ; 1. All specimens discolored to dark and blue with an increased thickness in all shades (p<0.05). 2. There were significant $a^*$ space (red-green direction) changes in every A, B, C, D shade series except the relationship between 1mm and 2mm thickness groups in B shade series. But the changes were irregular, i.e., each shade series has its own pattern. 3. Each shade series showed its own specific color shift pattern in all $L^*$, $a^*$, $b^*$ spaces. 4. B shade series showed less amont of changes in ${\Delta}{\pm}E$ than those of A and C shade series (p<0.05), but there was no significant difference in ${\Delta}E$ within each shade series.

적층구조를 가진 CFRP의 중앙 노치구멍이 미치는 영향에 관한 해석적 연구 (Analysis Study on Influence that the Center Hole Notch of CFRP with Laminated Structure Affects)

  • 본 논문에서는 CFRP로 구성된 항공 드론 프레임의 수직 낙하충격으로 인한 CFRP적층구조에서의 파손거동을 해석적 연구를 통해 파악하였다. 기존 플라스틱 재료와 달리 CFRP는 섬유로 구성되어 파손거동이 복잡하다. 따라서 이에 대한 실험에 앞서 해석적 연구를 통해 선행 연구가 중요하다. 본 연구의 결과로서 기존의 연구모델과 비교하여 보면, 같은 조건으로 중앙 노치구멍을 가진 적층된 CFRP 드론 프레임의 해석모델에서 가해진 응력의 값이 크게 줄어듬을 볼 수 있었다. 본 연구 결과를 토대로 드론프레임의 노치구멍에 관한 기반데이터를 융합기술에 접목함으로서 그 미적인 감각을 나타낼 수 있다.

상악 영구중절치의 외상환자에서 치아파절편을 이용한 치험례 (RESTORATION OF A FRACTURED CENTRAL INCISOR USING TOOTH FRAGMENT : CASE REPORT)

  • 상악 영구전치의 외상은 소아와 청소년기에 많이 일어난다. 그 중 치관의 파절은 18세 이하의 외상성 손상의 1/4에서 나타난다. 이러한 치관파절시 심미적 기능적으로 만족시킬 수 있는 수복을 하려는 노력이 계속되어 왔다. 파절편을 이용한 수복은 이러한 심미적 기능적인 조건을 만족시킬 수 있는 방법이다. 본 증례들은 전남대학교병원 소아치과에 상악중절치의 외상을 주소로 내원한 아동들의 치험례이다. 증례 1은 미성숙영구치치근을 가지는 만 7세의 여아로 침상출혈에 따른 수산화칼슘을 이용한 직접치수복조후 파절편을 부착한 증례이며, 증례 2는 함입과 치관파절로 인해 치수노출을 보인 만 10세 남아로 resin-wire splint 후 치수치료 하고 파절편을 부착하였고, 증례 3은 파절선이 생리적 폭경을 침범한 8세 여아로 별다른 처치 없이 파절편 부착을 시행하였다. 이상의 증례들을 통하여 치아 파절편 부착을 성공적으로 시행하였으며, 심미적으로 양호한 결과를 얻었다.

A comparison of the implant stability among various implant systems: clinical study

  • PURPOSE. To determine the change in stability of single-stage, three different design of implant systems in humans utilizing resonance frequency analysis for early healing period(24 weeks), without loading. MATERIAL AND METHODS. Twenty-five patients were included into this study. A total of 45 implants, three different design of implant systems(group A,C,R) were placed in the posterior maxilla or mandible. The specific transducer for each implant system was used. ISQ(implant stability quotient) reading were obtained for each implant at the time of surgery, 3, 6, 8, 10, 12, 24 weeks postoperatively. Data were analyzed for different implant type, bone type, healing time, anatomical locations. RESULTS. For each implant system, a two-factor mixed-model ANOVA demonstrated that a significant effect on ISQ values(group A=0.0022, C=0.017, R=0.0018). For each implant system, in a two-factor mixed model ANOVA, and two-sample t-test, the main effect of jaw position(P > .005) on ISQ values were not significant. CONCLUSIONS. All the implant groups A, C and R, the change patterns of ISQ over time differed by bone type. Implant stability increased greatly between week 0 and week six and showed slow increase between week six and six months(plateau effect).

The change of rotational freedom following different insertion torques in three implant systems with implant driver

  • STATEMENT OF PROBLEM. Implant drivers are getting popular in clinical dentistry. Unlike to implant systems with external hex connection, implant drivers directly engage the implant/abutment interface. The deformation of the implant/abutment interface can be introduced while placing an implant with its implant driver in clinical situations. PURPOSE. This study evaluated the change of rotational freedom between an implant and its abutment after application of different insertion torques. MATERIAL AND METHODS. Three kinds of internal connection implants were utilized for the current study($4.5{\times}12\;mm$ Xive, $4.3{\times}11.5\;mm$ Inplant Magicgrip, $4.3{\times}12\;mm$ Implantium MF). An EstheticBase, a 2-piece top, a Dual abutment was used for its corresponding implant system. The rotational freedom between an implant and its abutment were measured before and after applying 45, 100 Ncm insertion torque. Repeated measures ANOVA was used for statistical analysis. RESULTS. Under 45 Ncm insertion torque, the rotational freedom between an implant and its abutment was significantly increased in Xive(P = .003). However, no significant change was noted in Inplant Magicgrip and Implantium MF. Under 100 Ncm torque, both in Xive(P = .0005) and Implatium MF(P = .03) resulted in significantly increased rotational freedom between the implant and its abutment. DISCUSSION. The design of the implant/implant driver interface effectively prevented the deformation of implant/abutment interface. Little change was noted in the rotational freedom between an implant and its abutment, even though the insertion torque was far beyond clinical application. CONCLUSIONS. The implant/abutment joint of internally connecting implants were quite stable under insertion torque in clinical situation.

Mechanical and thermal properties of polyamide versus reinforced PMMA denture base materials

  • PURPOSE. This in vitro study intended to investigate the mechanical and thermal characteristics of Valplast, and of polymethyl methacrylate denture base resin in which different esthetic fibers (E-glass, nylon 6 or nylon 6.6) were added. MATERIALS AND METHODS. Five groups were formed: control (PMMA), PMMA-E glass, PMMA-nylon 6, PMMA-nylon 6.6 and Valplast resin. For the transverse strength test the specimens were prepared in accordance with ANSI/ADA specification No.12, and for the impact test ASTM D-256 standard were used. With the intent to evaluate the properties of transverse strength, the three-point bending (n=7) test instrument (Lloyd NK5, Lloyd Instruments Ltd, Fareham Hampshire, UK) was used at 5 mm/min. A Dynatup 9250 HV (Instron, UK) device was employed for the impact strength (n=7). All of the resin samples were tested by using thermo-mechanical analysis (Shimadzu TMA 50, Shimadzu, Japan). The data were analyzed by Kruskal-Wallis and Tukey tests for pairwise comparisons of the groups at the 0.05 level of significance. RESULTS. In all mechanical tests, the highest values were observed in Valplast group (transverse strength: $117.22{\pm}37.80$ MPa, maximum deflection: $27.55{\pm}1.48$ mm, impact strength: $0.76{\pm}0.03$ kN). Upon examining the thermo-mechanical analysis data, it was seen that the E value of the control sample was 8.08 MPa, higher than that of the all other samples. CONCLUSION. Although Valplast denture material has good mechanical strength, its elastic modulus is not high enough to meet the standard of PMMA materials.

내부연결방식 임플랜트 시스템의 삼차원 유한요소법적 연구 (THREE DIMENSIONAL FINITE ELEMENT ANALYSIS OF INTERNALLY CONNECTED IMPLANT SYSTEMS)

  • Statement of problem: Currently, there are some 20 different geometric variations in implant/abutment interface available. The geometry is important because it is one of the primary determinants of joint strength, joint stability, locational and rotational stability. Purpose: As the effects of the various implant-abutment connections and the prosthesis height variation on stress distribution are not yet examined this study is to focus on the different types of implant-abutment connection and the prosthesis height using three dimensional finite element analysis. Material and method. The models were constructed with ITI, 3i TG, Bicon, Frialit-2 fixtures and solid abutment, TG post, Bicon post, EstheticBase abutment respectively. And the super structures were constructed as mandibular second premolar shapes with 8.5 mm, 11 mm, 13.5 mm of crown height. In each model, 244 N of vertical load and 244 N of $30^{\circ}$ oblique load were placed on the central pit of an occlusal surface. von Mises stresses were recorded and compared in the crowns, abutments, fixtures. Results: 1. Under the oblique loading, von Mises stresses were larger in the crown, abutment, fixture compared to the vertical loading condition. 2. The stresses were increased proportionally to the crown height under oblique loading but showed little differences with three different crown heights under vertical loading. 3. In the crown, the highest stress areas were loading points under vertical loading, and the finish lines under oblique loading. 4. Under the oblique loading, the higher stresses were located in the fixture/abutment interface of the Bicon and Frialit-2 systems compared to the ITI and TG systems. Conclusions: The stress distribution patterns of each implant-abutment system had difference among them and adequate crown height/implant ratio was important to reduce the stresses around the implants.
