매일의 임상에서 흔히 접하게 되면서도 항상 갈등을 야기하는 증례들이 있다. 그러나 많은 경우에 있어서 그 어려움 들은 우리가 치과 학문의 여러 분야들을 종합적으로 활용하지 못하고 단편적으로 접근을 하기 때문에 반복적으로 경험하는 것일 수도 있다. 현대의 치의학은 어느 특정한 한 분야를 선도적으로 주도해나가는 치과의사뿐 아니라, Super GP 로서 치의학의 모든 분야를 종합적으로 활용해 나갈 줄 이는 치과의사를 요구한다. Super GP를 추구하는 많은 치과의사들이 치주, 보존, 보철 등의 여러 전문적인 분야의 Interdisciplinary Approach를 통해 훌륭한 결과를 얻어내고 있지만 교정은 그중에서도 심미치료를 지향하는 치과의사들에게는 결코 빼놓을 수 없는 분야 중 하나일 것이다. 다음의 증례들은 치주, 보존, 보철 등의 분야와 함께 교정의 도움이 없이는 결코 얻을 수 없는 심미적인 결과였기에 함께 나누고자 한다.

현재 CAD/CAM의 다양한 발전이 무서운 속도로 우리들에 게 다가오고 있다. 하지만 우리 치과영역에서 극도의 자연스러운 심미에 관해서는 결국 사람 손에 의해 극복이 되어야 하고 그것을 위해 우리는 끝없는 도전과 자신만의 신념이 필요하다고 생각한다. 그래서 사람들은 심미보철에 관한 술식을 국내외 서적이나 강의를 통해 많이 접하게 되는데 치과기공사에게 필요하고 쉽게 이해할 수 있게 요약해 놓은 것은 많지 않은 것 같다. 이에 대해 고민하다 심미보철작업을 위한 지침인 Esthetic Check List를 생각하여 수년전부터 조금씩 정리해 보았고 이 지침을 통해 균형 있고 조화로운 보철을 제작해 오고 있는데 이 해법을 본 연구에서 소개하고자 한다.

Application of ceramic materials for fabrication of dental restoration materials has been a focus of interest in the field of esthetic dentistry. The ceramic materials of choice are glass ceramics, spinel, alumina, and zirconia. The development of yttrium tetragonal zirconia polycrystal (YTZP)-based systems is a recent addition to all-ceramic systems that have high strength and are used for crowns and fixed partial dentures. Computer-aided design/computer-aided manufacturing (CAD/CAM)-produced, YTZP-based systems are popular with respect to their esthetic appeal for use in stress-bearing regions. The highly esthetic nature of zirconia and its superior physical properties and biocompatibility have enabled the development of restorative systems that meet the demands of today's patients. Many in vitro trials have been performed on the use of zirconia; however, relatively fewer long-term clinical studies have been published on this subject. The use of zirconia frameworks for long-span fixed partial dentures is currently being evaluated; in the future, more in vivo research and long-term clinical studies are required to provide scientific evidence for drawing solid guidelines. Further clinical and in vitro studies are required to obtain data regarding the long-term clinical use of zirconia-based restorations.

Generally, the esthetics is thought to mean health. On the assumption that dental esthetics must also be in harmony with general health, dental restorations have to be made in order for patients to appear to be healthy and vivacious in their mouth and result in good appearances. Therefore, I wish that patients live their life without a sense of disharmony in esthetic and biological aspect. This article is to explain some cases that manifest improvement of quality in dental treatment by means of team approach.

Replacements for natural dentition must fulfil many criteria. An important aspect, especially for the patient, is aesthetic consideration. The shape and colour can be important factors in the aesthetic success of an anterior restoration. In order to allow for proper shape; the outline form, the spatial position of individual teeth and the surface characteristics must be considered. For the colour; translucency, value, chroma and hue must be expressed exactly. The dentist and technicians should catch up these very specific characteristics of the patient and convert them accurately to the restorations with the simplest way.

The preservation of a healthy periodontal attachment is the most significant factor in the long-term prognosis of a restored tooth. The 'Biologic Width' is composed of the connective tissue attachment and the epithelial attachment in the dentogingival junction. The violation of the biologic width may result in a progressive inflammatory process and crestal bone loss. So a careful soft tissue management is needed to preserve it for the gingival health and an esthetic restoration. The following clinical cases show the five different situations of the violation of the biologic width and their management.

Intrinsic discoloration of permanent anterior teeth is a continuing esthetic problem. Tetracycline ingested during amelogenesis has long been recognized as predisposing factor in intrinsic staining. Methods for restoring original tooth color have included (1) complete PFM crown coverage (2) porcelain laminate veneers (3) direct bonding of composite resin, and (4) bleaching. In the case of tetracycline-stained upper anterior teeth, We authors, obtained the satisfactory results by means of gingivectomy, preliminary bleaching and porcelain laminate veneer restoration.
