Low Expression of the bcl2 Gene in Gastric Adenocarcinomas in Mazandaran Province of Iran

  • Background: Gastric cancer accounts for about 8% of the total cancer cases and 10% of total cancer deaths worldwide. It is the second lethal cancer after esophageal cancer and is considered the fourth most common cancer in north and northwest Iran. The bcl2 family has a key role in the regulation of apoptosis and change in its expression can contribute to cancer. This study initially scheduled to determine the expression of bcl2 gene in tissue samples of adenocarcinoma cancer patients. Materials and Methods: A total of 10 samples of gastric adenocarcinoma and 10 of normal tissues from Sari hospital were selected and after DNA extraction from tissues, bcl2 gene expression assayed by real-time PCR. Results: Our results demonstrated higher expression of the bcl2 gene in control compared with cancer and marginal cancer tissues. Conclusions: On one hand BCL2 plays an important role as an oncogene to inhibit apoptosis; on the other hand, it can initiate cell cycle arrest at G0 stage. Our observed association between its expression and patient survival is quite conflicting and may be tissue-specific. The data suggest expression both tumoural and non-tumoral(marginal) groups have lowered expression than controls (P>0.05). Due to the low number of samples we could not examine the relationship with clinicopathological features. However, bcl-2 expression may be important for prognostic outcome or a useful target for therapeutic intervention.

식도암 환자의 GET 영상 평가 (GET Imaging Evaluation of Patients with Esophageal Cancer)

  • 위배출 시간 측정(Gastric Emptying Time, GET)은 비침습적이고 정량적 평가 방법으로서 주로 내시경이나 방사선적 검사로 기계적 폐쇄가 없음이 확인된 위정체 증상을 보이는 환자를 검사한다. 이와 같은 일반적인 위배출 시간 측정 검사 외에 식도암 환자(식도 절제술을 시행한)를 대상으로 수술 직후와 1년 이상의 시간이 지난 후의 위배출 시간 측정 검사를 시행하여, 수술 후 흉강내에서 위장 기능의 평가 자료로서 위배출 시간 측정 검사의 유용성을 평가하고자 한다. 본원을 내원해서 식도 절제술을 시행한 환자 93명을 대상으로 위배출 시간 측정 검사를 수술 직후와 1년 이상의 시간이 지난 후에 두 번 시행하였다. 환자의 검사 전 준비사항으로는 12시간 이상 밤새 공복과 약재나 흡연을 중단 시켜야 하고 당뇨병 환자는 인슐린 주사 후 아침 일찍 검사하는 것이 이상적이다. 검사 방법은 유동식의 위배출 시간 측정은 예민도가 떨어지므로 시행하지 않고, 고형식의 위배출 시간 측정 방법으로 $^{99m}Tc-DTPA$로 표지된 레진이 들어간 계란찜, 그리고 김밥과 점성이 높은 발효유와 함께 먹은 후 입위 자세로 3시간 동안 측정하였다. 검사의 평가 방법은 위배출 곡선 상에서 위내 방사능치가 50%가 되는 시간, 즉 반감기(T1/2)를 구하였고, 반감기가 180분 이상시 지연 위배출, 180분 이내는 중등도, 삼킴과 동시에 소장으로 넘어가는 경우를 급속 위배출로 구분하였다. 일반적인 위배출 시간 측정 영상은 위의 전정부와 위저부가 강하게 나타나는 영상에서 시간이 지남에 따라 소장으로 넘어가는 영상이지만, 식도 절제술을 시행한 식도암 환자는 흉강쪽에 강한 집적 영상을 보였다. 수술 직후 반감기(T1/2)는 급속 위배출이 12.9%, 중등도 위배출이 52.7%, 지연 위배출이 34.4%로 나타났다. 이후 1년 이상의 시간이 흐른 뒤의 반감기 결과는 수술 직후 급속 위배출 환자 중 67%가 중등도 위배출로, 지연 위배출을 보였던 환자는 69%가 중등도 위배출로 나타났다. 중등도 위배출을 보였던 환자 중 급속 위배출 이나 지연 위배출로 나빠진 경우는 24%이다. 식도 절제술을 시행한 식도암 환자의 위배출 시간 측정 검사는 반감기(T1/2)가 급속 위배출 및 지연 위배출에서 시간이 지날수록 중등도 위배출로 변하는 소견을 보였다. 이는 주로 위를 보는 위배출 시간 측정 검사가 흉강내의 식도를 대신하는 위장 기능의 평가에도 유용하게 쓰일 수 있다는 것을 의미한다. 그리고 평가 기준을 세분화하고 검사의 시간 간격을 좁힌다면 좀 더 많은 정보와 분석으로 정확한 임상 진단과 평가가 이루어질 것으로 보인다.

Factors Affecting Preferences of Iranian Women for Breast Cancer Screening Based on Marketing Mix Components

  • Background: According to recent statistics, the breast cancer rate is growing fast in developing countries. In North West of Iran, the incidence of breast cancer after esophageal and gastric cancers has the highest rate. Previous studies have also indicated that women in this region show reluctance to do breast cancer screening. There is a great need for change to promote breast cancer screening among women. Social marketing is a discipline that uses the systematic application of commercial marketing techniques to promote the adoption of behavior by the target audience. Materials and Methods: In the present qualitative study, thirty-two women with breast cancer were interviewed about their experiences of breast cancer screening. A semi-structured interview guide was designed to elicit information specific to the 4 P's in social marketing. Results: Three main categories emerged from the analysis: price, service and promotion. Subcategories related to these main categories included factors effective in increasing and decreasing cost of screening, current and desirable features of screening services, and weakness of promotion. Conclusions: Screening programs should be designed to be of low cost, to meet patients' needs and should be provided in suitable places. Furthermore, it is essential that the cultural beliefs of society be improved through education. It seems necessary to design an executive protocol for breast cancer screening at different levels of primary health care to increase the women's willingness to undergo screening.

Adverse Effects of Preserved Vegetables on Squamous Cell Carcinoma of Esophagus and Precancer Lesions in a High Risk Area

  • Introduction: Squamous cell carcinoma of esophagus (ESCC) is one of the most common cancers in China. Preserved vegetables are processed foods and consumed in high amounts in the high risk areas for ESCC. This study aimed to investigate the relationships of preserved vegetable consumption with ESCC and precancer lesions. Methods: Cases from Yanting cancer hospital with pathological diagnosis of primary cancer, along with controls and individuals diagnosed with precancer lesions by endoscopy with iodine staining were interviewed. Trained staff collected data on dietary habits 1 year before the interview. An unconditional logistic regression model was used to estimate odds ratios of preserved vegetable consumption for precancer lesions and cancer. Results: Adjusting for potential confounders, intake of preserved vegetables (OR=2.92, 95%CI 1.32~6.47) and longer intake period (OR=5.78, 95%CI 2.26~14.80) were associated with higher risk of ESCC. Compared with lowest intake frequency, the highest was associated with a 3.0-fold risk for precancer lesions and 3.59-fold risk for ESCC (both p<0.05). Conclusion: Consumption of preserved vegetables is a risk factor for esophageal lesions in high risk areas. The carcinogenicity of preserved vegetables needs investigation in further studies and the public health strategies for reducing the consumption might be initiated in high risk areas.

Influence of Residential Environment and Lifestyle on Multiple Primary Malignancies in Taiwan

  • Background: Multiple primary malignancies (MPM) have become increasingly prevalent worldwide. This investigation was aimed at establishing the clinicopathological characteristics of MPM patients and evaluating the impact of the living environment on MPM in the Taiwanese population. Materials and Methods: From January 2009 to December 2013, a total of 8,268 cancer patients were identified in our institutional center. Of these, 125 were diagnosed as MPM and thus enrolled. Data for clinicopathological features and treatment approaches for these MPM patients living in urban or suburb zone were obtained. Findings for the air pollution status in Taiwan were also collected. Results: The most common cancer match of MPM was esophageal cancer with hypopharyngeal cancer (12.8%), followed by colorectal cancer with gastric cancer (6.4%) and colorectal cancer with breast cancer (5.6%). The air quality was significantly worse in the urban than in the suburban zone and there was a remarkably higher portion of MPM patients in the urban zone suffering from grade III and IV post-chemotherapeutic neutropenia (30.8% vs 15.1%, P=0.036). Conclusions: The tumor frequency and site distribution should be taken into the clinical evaluation because there is a relatively high risk of developing MPM. This study also highlighted the potential influence of environmental factors on post-chemotherapeutic neutropenia for patients with MPM.

Prognostic Relevance of Recurrent Sites of Gastric Cancer Treated With Curative Resection: A Single Center Retrospective Study

  • Purpose: Gastric cancer treated with curative resection exhibits several recurrence patterns. The peritoneum is the most common site of recurrence. Some reports have indicated different prognostic influences according to the recurrence sites in other cancers, such as esophageal and colorectal cancers. This study investigated whether the recurrence sites influenced the prognosis of patients with recurrent gastric cancer. Materials and Methods: The data of 115 patients who experienced tumor recurrence after curative gastrectomy were retrospectively reviewed. The sites of recurrence were divided into 4 groups: lymph node (LN), peritoneum, other single organs, and multiple lesions. Clinicopathological features were compared between the sites of recurrence. Prognosis after resection and recurrence were also compared. Results: The peritoneum was the primary site of recurrence in 38 patients (33%). The tumor differentiation and pathological stages were significantly different. Survival after surgery did not show a statistically significant difference (hazard ratio [HR] of LN: 1, peritoneum: 1.083, other single organs: 1.025, and multiple lesions: 1.058; P=1.00). Survival after recurrence was significantly different (HR of LN, 1; peritoneum, 2.164; other single organs, 1.092; multiple lesions, 1.554; P=0.01), and patients with peritoneal and multiple lesion recurrences had worse prognosis. Furthermore, peritoneal recurrence seemed to occur later than that at other sites; the median times to recurrence in LN, peritoneal, other single-organ, and multiple lesions were 265, 722, 372, and 325 days, respectively. Conclusions: The sites of gastric cancer recurrence may have different prognostic effects. Peritoneal recurrence may be less sensitive to chemotherapy and occur during the late phase of recurrence.

Epigenetic Field for Cancerization

  • Epigenetic alterations, represented by aberrant DNA methylation, are deeply involved in human cancers. In gastric cancers, tumor-suppressor genes are inactivated more frequently by promoter methylation than by mutations. We recently showed that H. pylori infection, a potent gastric carcinogenic factor, induces methylation of specific genes in the gastric mucosae. When the methylation levels were analyzed in the gastric mucosae of healthy volunteers, cases with a single gastric cancer, and cases with multiple gastric cancers, who have increasing levels of risks for gastric cancers, there was a significant increasing trend in the methylation levels among the individuals without current H. pylori infection. This finding unequivocally showed the presence of an epigenetic field for cancerization. The degree of the field defect was measured more conveniently using methylation levels of marker genes than using those of tumor-suppressor genes. The presence of an epigenetic field for cancerization has been indicated for liver, colon, Barrett's esophageal, lung, breast, and renal cancers. Since decreased transcription is involved in the specificity of methylated genes, it is likely that specific genes are methylated according to carcinogenic factors. These findings emphasize the usefulness of DNA methylation as a marker for past exposure to carcinogens and future risk of cancer development.
