To investigate the presence of infectious agents in human atherosclerotic arterial tissues. Atherosclerotic plaques were removed from 128 patients undergoing carotid endarterectomy or other bypass procedures for occlusive disease, and from twenty normal arterial wall samples, obtained from transplant donors with no history of diabetes, hypertension, smoking, or hyperlipidemia. Using the polymerase chain reaction (PCR) or reverse transcription-PCR, these samples were analyzed for the presence of Chlamydia pneumoniae, cytomegalovirus, enterovirus, adenovirus, herpes simplex viruses types 1 and 2, and Epstein-Barr virus. The amplicons were then sequenced, and phylogenetic analyses were performed. Enteroviral RNA was found in 22 of 128 atherosclerotic vascular lesions $(17.2\%),$ and C. pneumoniae and cytomegalovirus were each found in 2 samples $(1.6\%).$ In contrast, adenovirus, herpes simplex viruses, and Epstein-Barr virus were not identified in any of the atherosclerotic samples. Enterovirus was detected in 6/24 $(25.0\%)$ aortas, 7/33 $(21.2\%)$ carotid arteries, 6/40 $(15.0\%)$ femoral arteries, and 3/31 $(9.7\%)$ radial arteries of patients with chronic renal failure. There were no infectious agents detected in any of the control specimens. Using phylogenetic analysis, the enterovirus isolates were clustered into 3 groups, arranged as echovirus 9 and coxsackieviruses Bl and B3. Enteroviral RNA was detected in $17.2\%$ of atherosclerotic plaques, but was not observed in any of the control specimens. This suggests a connection between enteroviral infection and atherosclerosis. These findings differ from those of other studies, which found more frequent incidence of C. pneumoniae and cytomegalovirus infection in atherosclerotic plaques.
Epstein-Barr virus (EBV) infection can be presented with various clinical manifestations and different levels of severity when infected. Infectious mononucleosis, which is most commonly caused by EBV infection in children and adolescents, is a clinical syndrome characterized by fatigue, malaise, fever, sore throat, and generalized lymphadenopathy. But rarely, patients with infectious mononucleosis may present with gastrointestinal symptoms and complicated by gastritis, splenic infarction, and splenic rupture. We encountered a 16-year-old girl who presented with fever, fatigue, and epigastric pain. Splenic infarction and EBV-associated gastritis were diagnosed by using esophagogastroduodenoscopy and abdominal computed tomography. Endoscopy revealed a generalized hyperemic nodular lesion in the stomach, and the biopsy findings were chronic gastritis with erosion and positive in situ hybridization for EBV. As splenic infarction and acute gastritis are rare in infectious mononucleosis and are prone to be overlooked, we must consider these complications when an infectious mononucleosis patient presents with gastrointestinal symptom.
Objective: To conduct a meta-analysis to investigate the value of EBV DNA in diagnosis of nasopharyngeal cancer (NPC) in Asian populations, and provide important evidence for screening. Methods: Prospective or respective case-control or cohort studies regarding the detection role of EBV DNA for NPC were included in our study. We conducted a comprehensive literature search in PubMed, EMBASE, and the Chinese Biomedical Database (CBM database between January 1980 and March 2012. Results: A total of 18 studies with 1492 NPC cases and 2641 health controls were included. Almost of the included studies were conducted in China, and only one other conducted in Thailand. The overall results demonstrated that the pooled sensitivity, specificity, positive likelihood (+LR) and negative likelihood (-LR) were 0.73 (0.71-0.75), 0.89 (0.88-0.90), 8.84 (5.65-13.84) and 0.19(0.11-0.32), respectively. The overall EBV DNA detection showed the largest area of 0.932 under the summary receiver operator curve (SROC). The accuracy of detection by plasma for NPC (0.86) was higher than in serum (0.81), with largest areas under the SROC of 0.97 and 0.91, respectively. Conclusion: Our results demonstrated the EBV DNA detection in plasma or serum has high sensitivity and specificity in diagnosis of NPC, especially in Chinese populations with a high risk of cancer.
Post-transplant lymphoproliferative disorder (PTLD) is a common complication of therapeutic immunosuppression after organ transplantation. Gene expression profile facilitates the identification of biological difference between Epstein-Barr virus (EBV) positive and negative PTLDs. Previous studies mainly implemented variance/regression analysis without considering unaccounted array specific factors. The aim of this study is to investigate the gene expression difference between EBV positive and negative PTLDs through partial least squares (PLS) based analysis. With a microarray data set from the Gene Expression Omnibus database, we performed PLS based analysis. We acquired 1188 differentially expressed genes. Pathway and Gene Ontology enrichment analysis identified significantly over-representation of dysregulated genes in immune response and cancer related biological processes. Network analysis identified three hub genes with degrees higher than 15, including CREBBP, ATXN1, and PML. Proteins encoded by CREBBP and PML have been reported to be interact with EBV before. Our findings shed light on expression distinction of EBV positive and negative PTLDs with the hope to offer theoretical support for future therapeutic study.

The Epstein-Barr virus (EBV) is oncogenic and can transform B cells from a benign to a malignant phenotype. EBV infection is also associated with lymphoid interstitial pneumonia (LIP). Here, we report the case of a 14-year-old boy who was diagnosed with a latent EBV infection and underlying LIP, without any associated immunodeficiency. He had been EBV-seropositive for 8 years. The first clinical presentations were chronic respiratory symptoms and recurrent pneumonia. The symptoms worsened in the following 2 years. The results of in situ hybridization were positive for EBV, which led to a diagnosis of LIP. The diagnosis was confirmed by the results of a thoracoscopic lung biopsy. The EBV titer of the bronchoalveolar lavage specimens obtained after acyclovir treatment was found to be fluctuating. The patient had latent EBV infection for 8 years, until presented at the hospital with intermittent abdominal pain and distension. Physical examination and pelvic computed tomography revealed a large mesenteric mass. A biopsy of the excised mass led to a diagnosis of Burkitt's lymphoma (BL). The patient received combination chemotherapy for 4 months, consisting of vincristine, methotrexate, cyclophosphamide, doxorubicin, and prednisolone. He is now tumor-free, with the LIP under control, and is being followed-up at the outpatient clinic. This is the first report of a Korean case of chronic latent EBV infection that developed into LIP and BL in a nonimmunocompromised child.
To determine the prevalence of genetic polymorphisms in Epstein-Barr virus (EBV) strains in the Korean population, the restriction site polymorphisms for BamHI and XhoI enzymes were analyzed with 16 EBV isolates from cancer patients and 7 EBV isolates from healthy carriers, using polymerase chain reaction techniques. None of the 23 isolates were found to carry an extra BamHI site in the BamHI F-fragment (f-variant). Of the 12 type-1 isolates from the cancer patients, 10 lost both the LMP1 XhoI site and the BamHI site between the BamHi W1* and I1* fragments (a W1*I1* fusion variant or type C). The latter W1*I1* fusion variant was due to a mutation of thymidine to adenine, as evidenced by a sequence analysis. The remaining two type-1 isolates showed either no variation at both sites or the loss of only the XhoI site. In contrast, two type-2 isolates and two intertypic recombinants with a type-1 allele at the EBNA2 locus and type-2 alleles at all or some of the EBNA3 loci retained both enzyme sites. In similar analyses of the 7 isolates from the healthy carriers, five of six type-1 isolates lost these two sites, however, one type-2 isolate did not. These results clearly indicate a strong association of both the LMP1 XhoI site loss and the W1*I1* fusion variant with the type-1 rather than the type-2 EBV strains circulating in the immunocompetent Korean carriers.
유색미의 항암활성을 in vitro에서 조사하기 위한 기초실험으로서, 유색미 겨의 유기용매 추출물에 의한 DNA strand scission 및 발암 promotion의 억제효과를 조사하였다. 실험에는 유색미로 상해향혈나와 수원415호의 두 품종과 함께 대조구로 추청을 사용하였다. 품종별 변이원 억제활성은 70% 에탄올추출물 및 모든 유기용매 분획물에서 추청<상해향혈나<수원415호의 순서로 증가하였다. 그러나 DNA strand scission에 대한 억제활성은 70% 에탄올 추출물에서 상해향혈나<추청<수원415호의 순서로 증가하였다. 클로로포름 분획물에 대하여 조사한 결과, 상해향혈나에는 거의 DNA strand scission을 억제하는 활성이 없었으며, 예상과는 달리 추청은 오히려 상해향혈나보다 약간 활성이 높았다. 그리고 수원415호의 클로로포름분획물은 70%에탄올 추출물의 경우와 비슷한 수준의 강한 억제활성을 보였다(약 50% 억제). 발암 promotor에 의하여 유도된 Epstein-Barr 바이러스의 early diffusible antigen의 발현수준을 지표로 하여 70% 에탄올 추출물의 발암 promotion의 억제효과를 측정한 결과, 추청<상해향혈나< 수원415호의 순서로 억제활성이 높아짐을 알 수 있었다. 세 품종 모두의 클로로포름분획물에서 발암 promotion을 완전히 차단하는 효과가 발견되었지만, 세포독성이 동반되지 않은 promotion에 대한 억제효과는 수원415호에서만 유일하게 관찰되었다.
목 적 : KD는 주로 동양의 젊은 성인 여성에게서 경부 림프절 종대로 발현하는 것이 특징이며 수개월 내에 자연 치유되는 양호한 경과를 가지는 질환이다, 저자들은 KD로 진단된 소아환아의 임상적 특징을 알아보고, 환아들의 림프절 조직에서 바이러스 검출을 시도하여 KD와 HHV 8, EBV와의 연관성을 알아보기 위하여 본 연구를 시행하였다. 방 법 : 인제대학교 상계백병원에서 1998년 1월부터 2005년 12월까지 KD로 진단되어 치료받은 17세 이하 소아 26례를 대상으로 임상적 특징을 고찰하였다. 병력지 고찰을 통하여 후향적으로 분석하였고 추적 조사는 외래 병력 기록지와 전화 방문을 통하여 분석하였다. KD 환아의 림프절 조직으로부터 DNA를 추출하여 HHV 8 DNA를 검출하기 위해 PCR, EBV RNA를 검출하기 위해 ISH가 시행되었다. 결 과 : KD로 진단된 26명 중 남아 11례, 여아 15례로 성비는 1:1.4였고, 평균 연령은 13세였다. 환아의 연도별 분포는 2000년에 7례로 가장 많았으며, 여름에 가장 많이 진단되었다. 주증상은 발열(8/26)과 림프절의 동통(11/26)으로 발열의 기간은 평균 7.3일 이었다. 목빗근 뒷부위의 림프절 종대가 72%(18/24)였고 1례에서 경부 이외의 림프절 종대로 나타났다. 림프절의 크기는 $1cm{\times}1cm$에서부터 $6cm{\times}6cm$까지로 다양하게 나타났다. 백혈구 감소가 46%(6/13)에서 있었고 적혈구 침강 속도상승(>20 mm/hr)이 62%(8/13)에서 나타났다. 검체 확보가 가능하였던 20례 모두에서 HHV 8 DNA는 검출되지 않았으며, ISH를 이용한 EBV RNA 검사 결과도 음성이었다. KD 26례 모두 임상 경과는 양호하였으며, 1례(4%)만이 추적 중 발열과 함께 재발하였다. 전신성 홍반성 루프스를 포함한 결체 조직 질환으로의 이환은 관찰되지 않았다. 결 론 : KD는 소아에서 드물지 않게 발생하므로, 림프절 종대를 호소하는 환아에서 감별 진단에 포함되어야 한다. HHV 8과 EBV는 소아에서 진단된 KD의 원인 병원체로 작용할 가능성이 적을 것으로 생각된다.

Oral hairy leukoplakia occurs on the lateral surface of the tongue that clinically as an asymptomatic white lesion. It is mainly found in patient with human immunodeficiency virus infection. However, it rarely outbreak immunosuppressed patients after organ transplantation, or the patients taking steroids or immunosuppressants. It is the result of proliferating Epstein-Barr virus in the oral epithelium. Most of human immunodeficiency virus infected patients with oral hairy leukoplakia are highly contagious and possible to progress acquired immunodeficiency syndrome. Therefore, the early diagnosis of oral hairy leukoplakia is very important. Taking a thorough history and human immunodeficiency virus screening test is highly recommended in case oral hairy leukoplakia is detected. In this case, a 29-year-old man presented with whitish lesion on lateral border of tongue is diagnosed as oral hairy leukoplakia and human immunodeficiency virus infection.
목 적: EBV 감염 질환은 발생 연령별로 임상 소견이 다양하며 혈청검사 소견이 상이하다. EBV 감염을 진단하기 위하여 혈청학적 검사가 주로 이용되어왔는데, 근래에는 EBV 항원을 검사하는 검사법이 활발하게 이용되고 있다. 이에, 임상 소견이 EBV 감염과 관련된 질환을 시사하는 환자의 혈액에서 중합효소 연쇄반응 검사로 EBV 감염을 확인고, 혈청검사 결과와 비교하여 EBV 감염 질환의 임상 특성을 알아보고자 하였다. 방 법: 2004년 1월부터 2006년 12월까지 EBV 감염을 의심할 수 있었던 352명을 대상으로 EBV PCR을 시행하여 115명에서 양성의 결과를 얻었으며 이들 환자에서 EBV-IgM Ab 검사를 시행하였다. 백혈구, 혈소판, 비정형 백혈구, AST, ALT 등을 검사 하고 이들 임상소견과 검사소견을 EBV-IgM Ab 검사 음성인 예와 양성인 예를 비교하였다. 주요 검사 소견은 전예에서 생후 3세 미만, 3-5세, 6-9세, 10-15세의 4연령군으로 나누어 비교 평가 하였다. 결 과: EBV PCR에서 유전자가 검출된 115예에서 8세 이하의 어린이가 대부분 이었고(89%), 남녀 비는 1.3:1이었다. 115전예에서 인두염과 $37.5^{\circ}C$ 이상의 발열이 있었다. 경부 림프절 비대는 39예(34%), 복통을 포함한 복부 이상 20예(17%)의 순으로 관찰되이었다. EBV-IgM Ab는 33예(29%)에서 양성이었다. 말초혈에서 백혈구증가($WBC{\geq}10,000/mm^3$)는 54예(47%)에서 관찰되었고 비정형림프구(${\geq}20%$)는 28예(24%)에서 관찰되었다. 5세 이하에서 6세 이상의 연령군에 비해 ALT가 높았으며 EBV IgM Ab가 양성인 예가 많았다. 결 론: EBV 유전자를 PCR로 검사하여 발견한 연령별 본포의 누적 예는 8세에 이르기까지 급격하게 증가하였으며 EBV-IgM Ab가 양성인 환자에서 감염 단핵구증의 특징적인 소견을 보이는 예가 많았다. EBV 관련 감염질환 환자 가운데 연령이 어릴수록 ALT가 높고 EBV-IgM Ab의 발현이 많았으나 이 외의 임상 및 검사 소견은 차이가 없었다.
