Characteristics and Prediction of Lung Cancer Mortality in China from 1991 to 2013

  • Objective: To describe and analyze the epidemiological characteristics of lung cancer mortality in China from 1991 to 2013, forecast the future five-year trend and provide scientific evidence for prevention and management of lung cancer. Materials and Methods: Mortality data for lung cancer in China from 1991 to 2013 were used to describe epidemiological characteristics. Trend surface analysis was applied to analyze the geographical distribution of lung cancer. Four models, curve estimation, time series modeling, gray modeling (GM) and joinpoint regression, were performed to forecast the trend for the future. Results: Since 1991 the mortality rate of lung cancer increased yearly. The rate for males was higher than that for females and rates in urban areas were higher than in rural areas. In addition, our results showed that the trend will continue to increase in the ensuing 5 years. The mortality rate increased from age 45-50 and peaked in the group of 85 years old. Geographical analysis indicated that people living in northeast China provinces and the coastal provinces in eastern China had a higher mortality rate for lung cancer than those living in the centre or western Chinese provinces. Conclusions: The standardized mortality rate of lung cancer has constantly increased from 1991 to 2013, and been predicted to continue in the ensuing 5 years. Further efforts should be concentrated on education of the general public to increase prevention and early detection. Much better prevention and management is needed in high mortality areas (northeastern and eastern parts of China) and high risk populations (45-50-year-olds).

연료단지 진폐증 환자 분포현황 및 노출특성 (Distribution and Exposure Characteristics of Pneumoconiosis Patients in Fuel Complexes)

  • This study was conducted to identify the pollutants generated by the fuel complex and to determine the health effects of the surrounding residents. In addition, based on the results of epidemiological surveys and health impact surveys of local residents, we analyze the distribution of patient groups and exposure characteristics according to the distance from the fuel complex boundary. Samples were collected from the briquette plant within the fuel complex and analyzed by SEM-EDXA, X-ray Fluorescence Spectrometer, and ICP. In addition, the distribution of patients and exposure characteristics were analyzed according to the distance from the fuel complex and yard boundaries. Analysis of briquette samples from the fuel complex showed that the average particle size was 10-30 ㎛, the shape was irregular, and SiO2 accounted for more than 50%. It is believed that silica, which causes pneumoconiosis, may have been scattered into the air. In particular, there was a large distribution of 5 ㎛ particles that affect respiratory diseases. According to the analysis of the residential addresses and distribution of pneumoconiosis cases, many pneumoconiosis cases were located in the area between 200 and 500 meters from the boundary of the fuel complex. In addition, 28 pneumoconiosis cases were identified as a result of the epidemiological survey and health impact survey at the fuel complex. In detail, there were 8 cases of occupational pneumoconiosis, 6 cases of environmental pneumoconiosis, and 14 cases of occupational and environmental pneumoconiosis. The confirmed pneumoconiosis cases were located between 0.3 and 1.1 kilometers from the fuel complex. It was found that environmental pollutants generated by the fuel complex adversely affect the health of local residents. In particular, there are many cases of pneumoconiosis in the area between 200 and 500 meters from the boundary of the fuel complex, and this distance is considered to be the direct and indirect impact zone of the briquette plant.

Epidemiological and Clinical Characteristics of Elderly Fall Patients Visit to the Emergency Department: A Comparison by Gender

  • Purpose: This study was to analyze clinical and epidemiological characteristics of elderly patients who were admitted to the emergency department (ED) due to falls by separating male and female. Methods: We retrospectively analyzed the fall patients aged 65 years or older from the data of the in-depth surveillance study of injured patients visit to the ED under the supervision of the Korea Centers for Disease Control and Prevention (KCDC) from 2011 to 2016 by separating male and female. Results: A total of 361,588 elderly fall patients were analyzed and, among them, 14,429 (37.3%) were males and 24,208 (62.7%) were females. Male and female showed similar frequency of damage happening season. However, they showed falling accident mostly on winter. The time of injury occurrence is mostly from 12:00 to 18:00 with 4,949 (34.3%) male and 8,564 (35.4%) female. Most falls occurred in daily activities, accounting for 7,614 (52.8%) in males and 14,957 (61.8%) in females, respectively. Unintentional damage accounted for the most part and 7,395 (51.2%) of male and 15,343 (63.4%) of female were injured indoors. Head and neck were the most common site of injuring, with 8,392 (58.2%) in males and 7,851 (32.4%) in females. According to ED examination outcomes, most of the patients were discharged, while the majority of the hospitalized patients were admitted to the general patient room. Conclusions: The elderly falls occurred mostly from 12:00 to 18:00, during winter and to elderly women. Also, they happened unintentionally indoors in everyday life, mostly. Proved clinical, epidemiological characteristics from this research will be used as useful indicator at validity research of development of prevent program of falling accident for elderly people.

중증 중독환자의 임상 역학적 특징과 예후에 영향을 주는 요인분석 (Clinical and Epidemiological Characteristics of Severe Poisoning Patients and Analysis of Prognostic Factors)

  • Purpose: This study examined the clinical and epidemiological characteristics of intensive care unit (ICU) patients admitted or died in the emergency medical center with acute-poisoning to investigate the variables related to the prognosis. Methods: The data were collected from poisoning patients admitted or died in the emergency medical center of a general hospital located in Seoul, from January 2014 to February 2020. The subjects of this study were 190 patients. The medical records were screened retrospectively, and the clinical and epidemiological characteristics of the patients in the emergency room (ER) and ICU were examined to investigate the contributing factors that influence the poor prognosis. Results: The study analyzed 182 patients who survived after being admitted to the intensive care unit (ICU). The results are as follows. The mental change (87.4%) was the most common symptom. Sedative poisoning (49.5%) was the commonest cause. For most patients, pneumonia (26.9%) was the most common complication. Hypotension (23.7%), tachycardia (42.1%), fever (15.8%), seizures (10.5%), dyspnea (2.6%), high poisoning severity score (PSS), type of toxic material, mechanical ventilator application (39.5%), inotropes application (39.5%), and pneumonia (55.3%) were correlated the LOS over 5 days in the ICU. 8 patients died. In the case of death pesticides and carbon monoxide were the main toxic materials; tachycardia, bradycardia, and hypotension were the main symptoms, and a mechanical ventilator and inotropes were applied. Conclusion: Patients with unstable vital signs, high PSS, and non-pharmaceutical poisoning had a prolonged LOS in the ICU and a poor prognosis.

직업성 근골격계질환의 발생 현황과 특성 (Status and Characteristics of Occurrence of Work-related Musculoskeletal Disorders)

  • Occupational musculoskeletal disorders currently account for the largest proportion of the occupational illnesses in Korea. In this research, status of musculoskeletal disorders among the occupational illnesses was examined through workers' compensation claims data. Types and characteristics of musculoskeletal disorders were looked at other data set as well. The data sets included epidemiological investigation data reported by Occupational Safety and Health Research Institute, and data collected from occupational disease surveillance reports and Korean occupational health-related scientific journals. Number of cases, incidence rate and insurance benefits for occupational musculoskeletal disorders in Korea are increasing every year. In addition, musculoskeletal disorders occurrence is shifted from large enterprises group to small-and-medium group, from manufacturing to service sector, and from production workers to office and professional workers. Although low back pain is still most common, its occurrence characteristics is gradually shifted from traumatic to cumulative while musculoskeletal disorders are somewhat seemingly moved from lumbar to upper limb body part. Musculoskeletal disorders were observed to be more diverse and prevalent in epidemiological investigations or surveillance data rather in workers' compensation claims data. Musculoskeletal disorders occurrence is related to demographic factors, occupational psychosocial factors, and ergonomic risk factors at workplace for which appropriate preventive measures needed to be made accordingly.

Cervical Cancer in Morocco: Epidemiological Profile from Two Main Oncological Centers

  • Background: In Morocco, the epidemiological profile of cervical cancer is not well established. The focus of the present study was both epidemiological and pathological characteristics. Methods: For all cases of cervical cancer treated between 2003 and 2007 in the National Institute of Oncology and the Oncology Department of the IbnRochd hospital (Casablanca), 900 cases were randomly selected. Results: The mean age was $52.1{\pm}11.8$ years. The most (90.5%) represented histological type was squamous cell carcinoma. For more than 57.0% cases the mean distance between patient's origin and center of treatment was greater than 100km. According to the FIGO classification, only 17.2% of patients were identified as being in early stages (0 and I). For 72.2% patients the follow-up did not exceed 2 years. At 1 year of following-up 55.8% of patients were alive and 43.4% were lost to following-up. Conclusion: Our study addressed the issue of the burden of cervical cancer in Morocco. The result provides a basis for decision-makers for the development of strategic measures to implement the fight against cervical cancer in Morocco.

암 생존율에 대한 한국 대중의 인식과 역학적 사실 사이에서 발생하는 불일치 : 온라인 설문을 중심으로 (Discrepancies Between Public Perceptions and Epidemiological Facts Regarding Cancer Survival Rate in Korea : An Online Survey)

  • 본 연구의 목적은 5대 암(위암, 대장암, 간암, 유방암, 자궁경부암)을 중심으로 5대 암에 대한 대중의 5년 생존율 인식을 조사하여 그 결과를 역학적 사실과 비교하고 암 생존율 인식에 미치는 요인을 규명하는 것이다. 자료는 2016년 4월 19일부터 24일까지 단면 인터넷 설문 조사를 수행하여 총 354명의 설문 응답을 수집하였다. 연구대상자의 특성을 파악하기 위한 빈도분석, 연구대상자의 5년 생존율 인식과 역학적 사실과의 비교를 위한 히스토그램 제시, 연구대상자의 5년 생존율에 미치는 요인을 검증하기 위한 다중회귀분석을 실시하였다. 분석결과, 위암, 대장암, 유방암, 자궁경부암의 생존율에 대한 대중의 인식은 역학적 사실보다 낮게, 간암의 생존율에 대한 대중의 인식은 역학적 사실보다 높게 인식하였다. 마지막으로 암 진단유무, 연령, 규칙적인 운동유무가 5년 생존율 인식에 유의한 영향을 주는 요인으로 나타났다.

A Comparative Review of Radiation-induced Cancer Risk Models

  • Background: With the need for a domestic level 3 probabilistic safety assessment (PSA), it is essential to develop a Korea-specific code. Health effect assessments study radiation-induced impacts; in particular, long-term health effects are evaluated in terms of cancer risk. The objective of this study was to analyze the latest cancer risk models developed by foreign organizations and to compare the methodology of how they were developed. This paper also provides suggestions regarding the development of Korean cancer risk models. Materials and Methods: A review of cancer risk models was carried out targeting the latest models: the NUREG model (1993), the BEIR VII model (2006), the UNSCEAR model (2006), the ICRP 103 model (2007), and the U.S. EPA model (2011). The methodology of how each model was developed is explained, and the cancer sites, dose and dose rate effectiveness factor (DDREF) and mathematical models are also described in the sections presenting differences among the models. Results and Discussion: The NUREG model was developed by assuming that the risk was proportional to the risk coefficient and dose, while the BEIR VII, UNSCEAR, ICRP, and U.S. EPA models were derived from epidemiological data, principally from Japanese atomic bomb survivors. The risk coefficient does not consider individual characteristics, as the values were calculated in terms of population-averaged cancer risk per unit dose. However, the models derived by epidemiological data are a function of sex, exposure age, and attained age of the exposed individual. Moreover, the methodologies can be used to apply the latest epidemiological data. Therefore, methodologies using epidemiological data should be considered first for developing a Korean cancer risk model, and the cancer sites and DDREF should also be determined based on Korea-specific studies.

Epidemiological characteristics of bovine brucellosis in Korea, 2000~2004

  • This paper describes the epidemiological characteristics of bovine brucellosis in Korea during January 2000~September 2004, which encompasses the period when the incidence of bovine brucellosis increased abruptly. Data from the National Animal Infectious Disease Data Management System were used for this study. A range of epidemiological measures was calculated including annual herd and animal incidence. During the study period, there were 1,183 outbreaks on 638 farms. In beef cattle, annual herd incidence increased from 0.2 (2000) to 11.5 (2004, to September) outbreaks per 10,000 and annual animal incidence varied between 3.4 (2000) and 105.8 (2004, to September) per 100,000, respectively. On 401 (62.9%) infected farms during this period, infection was eradicated without recurrence. Recurrence of infection was significantly higher on farms where abortion was reported (53.3%), compared to farms where it was not (30.0%). On beef cattle farms, infection was introduced most frequently through purchased cattle (46.2%). Based on the results of this study, the establishment and spread of brucellosis in the Korean beef cattle population were mainly due to incomplete or inappropriate treatment of aborted materials and the movement of infected cattle.

어린이 보호구역 내에서 발생한 6-14세 어린이들의 보행 중 교통사고에 대한 역학적 조사 (An Epidemiological Study for Child Pedestrian Traffic Injuries that Occurred in School-zone)

  • Objectives: Pedestrian traffic injuries have been an important cause of childhood mortality and morbidity for decades. The aim of this study was to describe the epidemiological characteristics of child pedestrian traffic injuries that occurred during 2000 in one metropolitan city and its school-zones, and to determine the factors associated with those accidents. Methods: A cross-sectional study was performed in 2001. Police records were used to identify the cases of pedestrian injury. Children aged between 6 and 15 years, injured during road walking, were included in this study. A direct survey of the environmental factors within the school-zones in study area (n=116) was also performed. Self-administered questionnaires, via mail and telephone surveys, were used to assess the safety education programmes. The schools were divided into two groups according to the occurrence of pedestrian traffic injuries in their school-zone. Results: Pedestrian injuries (n=597) were found to account for 3.2% of all traffic injuries in the subject area. The epidemiological characteristics were not significantly different between genders. There were some significant risk factors within the environmental factors, such as local road (OR: 2.3, 95% CI=1.05-5.35), heavy traffic volume (OR: 2.2, 95% CI=1.00-5.04), poor visibility of speed-limit signs (OR: 2.8, 95% CI=1.25-6.42), no separation of pedestrian routes from cars (OR: 2.6, 95% CI=1.02-6.75) and barriers on the pedestrian routes (OR: 2.2, 95% CI=1.01-5.08). Only one factor, that of education in a safety-park (OR: 0.3, 95% CI=0.09-0.96), was significantly associated in the traffic and pedestrian safety education factors. Conclusion: Significant associations with pedestrian injury risk were identified in some of the modifiable environmental factors than in the educational factors.
