Clinical Characteristics of Pediatric Patients With the Coronavirus Disease 2019 During the Third and Fourth Waves of the Epidemic in Korea: A Single Center Retrospective Study (국내 코로나바이러스감염증-19 유행 제3-4기 소아청소년 환자의 임상적 특성: 단일기관 후향적 연구)

  • Purpose: Since the coronavirus disease 2019 (COVID-19) pandemic began, new variants of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) have emerged, and distinct epidemic waves of COVID-19 have occurred for an extended period. This study aimed to analyze the clinical and epidemiological characteristics of children with COVID-19 from the third wave to the middle of the fourth epidemic wave in Korea. Methods: We retrospectively reviewed the medical records of hospitalized patients aged ≤18 years with laboratory-confirmed COVID-19. The study periods were divided into the third wave (from November 13, 2020 to July 6, 2021) and the fourth wave (from July 7 to October 31, 2021). Results: Ninety-three patients were included in the analysis (33 in the third and 60 in the fourth waves). Compared with the third wave, the median age of patients was significantly older during the fourth wave (6.7 vs. 2.8 years, P=0.014). Household contacts was reported in 60.2% of total patients, similar in both periods (69.7 vs. 55.0%, P=0.190). Eighty-one (87.1%) had symptomatic SARS-CoV-2 infection. Among these, 10 (12.3%) had no respiratory symptoms. Anosmia or ageusia were more commonly observed in the fourth epidemic wave (10.7 vs. 34.0%, P=0.032). Most respiratory illness were upper respiratory tract infections (94.4%, 67/71), 4 had pneumonia. The median cycle threshold values (detection threshold, 40) for RNA-dependent RNA polymerase (RdRp) and envelope (E) genes of SARS-CoV-2 were 21.3 and 19.3, respectively. There was no significant difference in viral load during 2 epidemic waves. Conclusions: There were different characteristics during the two epidemic waves of COVID-19.

  • With the COVID-19 pandemic being prolonged, the society is being converted into an untact society, thereby affecting the service methodology of domestic and foreign library systems. This study thus surveyed the librarians' awareness of COVID-19 response measures of public libraries, and proposed public libraries' response measures in situations where the novel infectious disease is spreading and prolonging. Specifically, the proposed measures include, first, preparing national-centered library countermeasures to jointly respond to the epidemic disease, second, strengthening various untact and online services of various public libraries through government-level budget support, third, a thorough prevention of epidemics and preemptive response from the viewpoint of employees and users, and fourth, the expansion of library services aimed at resolving the digital cultural divide among classes of people, including information unfairness.

Contrivance of Integrated Pattern Differentiation Method for Diagnostic Unification of Exogenous Contagious Diseases (다양한 유행성 감염병의 진단 일원화를 위한 통합변증방법 연구)

  • In recent years, there were frequent exogenous contagious diseases in Eastasia like SARS(severe acute respiratory syndrome), Avian influenza, Swine influenza, MERS etc. But there are various interpretations about their pathological differentiations and lead to controversy to diagnosis and medicinal use. So there needs universal and consistent understanding methods. Several conclusions are obtained from the research on differentiation theories of various epidemic diseases. Essential elements of differential diagnostic system are pathogen, characters and matters of disease and loci, especially three yin and three yang has close affinity with constitutional features or body shape. Binding these 3 categories, an integrated differentiation 3 dimensional coordinates are made. Out of these, each elements of 3 pathogen-axial lines are related with names of exogenous disease, and those of 3 feature-axial lines are related with 8 principal patterns. And those of 3 locus-axial lines implicating therapeutic method are related with steps and location of exterior and interior, 3 yin 3 yang, Defense, Qi, Nutrient and Blood, five viscera and six bowels and tissues. Additionally, 3 lines of each axis consist of factors which have their own affinity each other, so classification of pathogen, feature, locus of disease has layered interconnectedness. This classification system is included in constitutional features of individual patient. Afterwards, these cognitive structure can be used as a general theory guiding method of therapy, prevention and aftercure healthcare.

The Single Cigarette Economy in India - a Back of the Envelope Survey to Estimate its Magnitude

  • Background: Sale of single cigarettes is an important factor for early experimentation, initiation and persistence of tobacco use and a vital factor in the smoking epidemic in India as it is globally. Single cigarettes also promote the sale of illicit cigarettes and neutralises the effect of pack warnings and effective taxation, making tobacco more accessible and affordable to minors. This is the first study to our knowledge which estimates the size of the single stick market in India. Materials and Methods: In February 2014, a 10 jurisdiction survey was conducted across India to estimate the sale of cigarettes in packs and sticks, by brands and price over a full business day. Results: We estimate that nearly 75% of all cigarettes are sold as single sticks annually, which translates to nearly half a billion US dollars or 30 percent of the India's excise revenues from all cigarettes. This is the price which the consumers pay but is not captured through tax and therefore pervades into an informal economy. Conclusions: Tracking the retail price of single cigarettes is an efficient way to determine the willingness to pay by cigarette smokers and is a possible method to determine the tax rates in the absence of any other rationale.

The cost-effectiveness of alternative control measures against the 2010-2011 epidemic of foot-and-mouth disease (FMD) in Andong, Republic of Korea

  • The cost-effectiveness of foot-and-mouth disease (FMD) control strategies was evaluated using a simulation model fitted to the 2010/11 FMD epidemic in the city of Andong, Republic of Korea. Seven FMD-control strategies were evaluated with respect to the direct cost of a FMD-control strategy, such as slaughtering, movement restriction, and vaccination. All the strategies included pre-emptive slaughtering, movement restriction, and vaccination, but the levels of each control option were different. The simulated median cost of the baseline FMD-control strategy (three kilometers of pre-emptive slaughtering area, 100 days of movement restriction and vaccination of all FMD-susceptible animals in the study area) was estimated to be USD 99.7 million. When a five kilometer vaccination area was applied (with the other control measures being the same as the baseline strategy), the simulated median cost was reduced to USD 81.1 million from USD 99.7. The simulated median costs were USD 107.6 million for a five kilometer radius slaughtering area and USD 168.8 million for 60 days of movement restriction. The FMD-control strategy cost decreased with increasing number of farms depopulated per day. The probability of passive surveillance being effective or the probability of the successful implementation of movement restrictions were increased. Cost-effectiveness analysis is a suitable tool for evaluating the financial consequences of FMD-control strategies by comparing the cost of control strategies for a specific area.

Modeling for Prediction of the Turnip Mosaic Virus (TuMV) Progress of Chinese Cabbage (배추 순무모자이크바이러스(TuMV)병 진전도 예측모형식 작성)

  • To develop a model for prediction of turnip mosaic virus(TuMV) disease progress of Chinese cabbage based on weather information and number of TuMV vector aphids trapped in Taegwallyeong alpine area, data were statistically processed together. As the variables influenced on TuMV disease progress, cumulative portion(CPT) above 13$^{\circ}C$ in daily average temperature was the most significant, and solar radiation, duration of sunshine, vector aphids and cumulative temperature above $0^{\circ}C$ were significant. When logistic model and Gompertz model were compared by detemining goodness of fit for TuMV disease progress using CPT as independent variable, regression coefficient was higher in the logistic model than in the Gompertz model. Epidemic parameters, apparent infection rate and initial value of logistic model, were estimated by examining the relationship between disease proportion linearized by logit transformation equation, In(Y/Yf-Y) and CPT. Models able to describe the progression of TuMV disease were formulated in Y=100/(1+128.4 exp(-0.013.CPT.(-1(1/(1+66.7.exp(-0.11.day). Calculated disease progress from the model was in good agreement with investigated actual disease progress showing high significance of the coefficient of determination with 0.710.

  • This research is based on the protection of the social safty with the preventive measures against Creutzfedt-jakob's and Mad cow Disease which has been drawn an attention of the public. It also has Known as an epidemic with rapid transmission an severe conteminaton to hurt the health through beef. In this work, some fruitful suggestions have been made to prevent and to minimize the harms Creutzfedt-jakob's and Mad Cow Disease : 1) We should Pay attention to the prior preventive measures by detecting the modes and the causes of the contemination lute route ; 2) there should be a step to inoculate the cattles to provide non-conteminated fresh feed for cows ; and 3) a health official should be alert to prevent the rapid transmission to the public by breaking the infected route of it.

  • Several studies have suggested the possibility of airborne transmission of infectious diseases such as tuberculosis, pandemic influenza. because the number of patients increases explosively, if infectious disease had a high basic reproduction number, pharmaceutical interventions such as vaccination, chemoprophylaxis in the early stage of epidemic. Thus, non-pharmaceutical interventions such as mask-wearing, installing air cleaners, school closure are important to control and prevent the infectious diseases. However, the current technology on the mask, air cleaning, ventilation, and etc., seems to be not originated from the understanding of infection via airborne transmission. It is important to estimate the aerodynamic behavior of saliva droplets by coughing or speaking in order to understand the phenomena of airborne infection. In addition, the prediction of transmission of infectious diseases through the air is critical to prevent or minimize the damage of infection. In this review, we reviewed the recent studies on the airborne infection by focusing on the aerodynamic characteristics of saliva droplets and modeling of airborne transmission.

Overview of RCT for Non-Alcoholic Fatty Liver Disease and Non-Alcoholic Steatohepatitis

  • Objective: This study aimed to get information on the current status of therapies to date for non-alcoholic fatty liver disease (NAFLD), including non-alcoholic steatohepatitis (NASH). Methods: All randomized clinical controlled trial (RCT)-derived papers for NAFLD or NASH were reviewed via PubMed Database. Results: 39 RCTs met the review criteria, of which 15 and 24 papers were for NAFLD and NASH, respectively. 83% of the papers were released since 2006, and 30 studies were conducted for western medicines, antioxidants and lifestyle intervention whereas nine trials were done using herbal medicine or acupuncture which showed positive outcome. Conclusions: NAFLD and NASH are new epidemic disorders which can be a target of traditional Oriental medicine. This study will be helpful for the Oriental medicine-based strategies or therapeutic development for them.

  • Accumulation of pathogens in chest refers to a syndrome of fullness, stuffiness and pain in the chest, diaphragm, epigastrium and abdomen, which is ascribable to accumulation of pathogenic heat with stagnancy of fluid or phlegm in the chest. Retching refers to vomiting with sound but without any vomitus. It is usually ascribable to adverse flow of ki due to deficiency of stomach, or by pathogenic heat or cold, and failure in descending of the stomach-ki. It may also be found in Soyang disease. Bloody purulent stool refers to passing stool with blood and pus. It is formed mainly due to invasion of the stomach and the intestines by epidemic pathogenic summer-heat, steaming of stagnateed damp-heat fighting against ki and blood, or improper diet, obstruction of bu-ki stagnation of blood and ki.
