  • Purpose: The purpose of this study was to identify the factors affecting sleeping patterns among hospitalized elderly. Methods: A descriptive correlational study was conducted. Participants were 121 hospitalized elderly in a hospital located in a county. A structured questionnaire was used for data collection. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation, and stepwise multiple regression. Results: Almost a half of hospitalized elderly in this study suffer from sleep disturbances. As results of univariate analyses, physical symptoms, anxiety, depression, and environmental factors disturbed sleep among hospitalized elderly. However, the result of stepwise multiple regression analysis identified that anxiety and environmental factors were significant predictors of sleeping patterns for hospitalized elderly and these two factors accounted 32.5% of variance of sleeping patterns among hospitalized elderly. Conclusion: Sleep assessment should be conducted on admission to identify sleep difficulties and thereby to improve quality of nursing care. Nurses and other health care personnels should make efforts to decrease anxiety and to eliminate environmental barriers of sleep among hospitalized elderly. Educational programs for nursing staff that offer information about sleep and sleep promoting interventions for the elderly patients are critical. Moreover, sleep promotion intervention program should be developed, applied and evaluated.

  • Purpose: The purpose of this study was to investigate the factors increasing fall risk in the residential environment risk and the perceived fall risk among the older adults who received home care services to provide information for developing a comprehensive falls intervention program. Methods: The subjects were 227 community-dwelling elderly aged 65 years and over who were taken care of by home-visiting nurses of the national health centers. The data were collected from July to August in 2012 using the Choi's residential environmental risk scale (2010) and the Hong's fall risk scale (2011). Results: Requires an assistive devices to walk, modified residential environment, health security, approval certificate of LTC, residential safety perception, residential environment risk, and perception of fall risk were statistically significant risk factors. A multiple logistic regression analysis showed that room & kitchen, physical perception, medication & ADL perception, floor-related environmental perception, and daily living tool-related perception were statistically significant predictors of fall. Conclusion: The results showed that the residential environment and the perceived fall risk were associated with fall experiences among the elderly. It is necessary to develope multifactorial intervention programs considering both environmental and perceived risk factors as well as physical risk factors to reduce and prevent falls among the elderly.

  • For the purposes of enhancing usability of Numerical Weather Prediction (NWP), the quantitative precipitation prediction scheme by machine learning has been proposed. In this study, heavy rainfall was corrected for by utilizing rainfall predictors from LENS and Radar from 2017 to 2018, as well as machine learning tools LightGBM and XGBoost. The results were analyzed using Mean Absolute Error (MAE), Normalized Peak Error (NPE), and Peak Timing Error (PTE) for rainfall corrected through machine learning. Machine learning results (i.e. using LightGBM and XGBoost) showed improvements in the overall correction of rainfall and maximum rainfall compared to LENS. For example, the MAE of case 5 was found to be 24.252 using LENS, 11.564 using LightGBM, and 11.693 using XGBoost, showing excellent error improvement in machine learning results. This rainfall correction technique can provide hydrologically meaningful rainfall information such as predictions of flooding. Future research on the interpretation of various hydrologic processes using machine learning is necessary.

  • The purpose of this study was to examine the environmental characteristics of adolescents in low-income families, identify the high-risk & protective factors among environmental contexts surrounding adolescents, and investigate the relative importance of high-risk & protective factors to adolescents'psychological and behavioral adjustment separately. The present study was the primary research of developing prevention program for adolescents'deviant behaviors in low-income families. Subjects of this study consisted of 176 adolescents drawn from 8 social-welfare institutions in Chungbuk province. The pilot study was done to examine the applicability of survey instrument. Data were analyzed by the frequency, percentage, Pearson correlation, stepwise regression using SPSS/WIN program. The results were as followings: 1. There was statistically correlated with each other in environmental high-risk and protective factors except an housing environment. The results implies that environmental contexts itself surrounding adolescents in low-income families can be either high-risk factors or protective factors. 2. The adolescents in low-income families perceived that stresses from consumer and school environments were high-risk factors among other environmental contexts. 3. The adolescents in low-income families perceived that resources from friend and school were protective factors among other environmental contexts. 4. The stresses from friend and eating behaviors were significant factors predicting adolescents'relative psychological adjustment. However, the behavioral adjustment was not predicted by environmental contexts. 5. The resources from school, consumer, and eating behaviors were significant factors predicting adolescents'relative psychological adjustment. Also, the resources from school, eating behavior, and family were predictors of behavioral adjustment. This research implies that the findings can be based on the development of prevention program for adolescents deviant behaviors in low-income families.

  • Objects: The prevalence of asthma has increased in recent decades globally. The objective of the present study is to elucidate whether hospitalization for bronchiolitis in infancy and low socioeconomic status interact for bronchial hyperreactivity during teenage years. Method: We studied 522 children age 13-14 years attending schools in rural and urban areas to investigate the risk factors for bronchial hyperreactivity (BHR), defined as a provocation concentration of methacholine that causes a decrease of 20% ($PC_{20}$) in forced expiratory volume within 1 second. Clinical examination, skin prick test, spirometry, and methacholine challenge were performed on all study subjects, who provided written consent. We used multivariate logistic regression to investigate the risk factors for BHR, and analyze the interaction between hospitalization for bronchiolitis in infancy and low socioeconomic status. Results: Forty-six (10.3%) positive BHR cases were identified. In the multivariate logistic analysis, as independent predictors of BHR, adjusted odds ratio of bronchiolitis diagnosed before 2 years of age in low income families was 13.7 (95% confidence interval, 1.4 to 135.0), compared to reference group, controlling for age, gender, parental allergy history, skin prick test, and environmental tobacco smoke (ETS) exposure. Interaction was observed between bronchiolitis before 2 years old and low socioeconomic status on children's bronchial hyperreactivity (p-interaction=0.025). Conclusions: This study showed that bronchiolitis diagnosed before 2 years of age and low socioeconomic status interacted on children's bronchial hyperreactivity. Prevention of acute respiratory infection in early childhood in low socioeconomic status is important to prevent BHR as a precursor of asthma.

  • Polychlorinated biphenyls(PCBs) are halogenated aromatic compounds with the empirical formula $C_{12}$ $H_{10-n}$C $l_{n}$(n=1~10), and are a mixture of possible 209 different chlorinated congeners. PCBs were widely used as dielectric fluids for capacitors and transformers, plasticizers, lubricant inks and paint addirives. Once released into the environment, PCBs persist for years because they are so resistant to degradation. In addition to their persistence in the environment, PCBs in ecological food chains undergo biomagnification because of their high degree of lipophilicity. In 1970s, the worldwide production of PCBs was ceased and the import of PCBs was prohibited since 1983 in Korea. In spite of these actions, many PCBs seems to be still in use. The environmental load of PCBs will continue to be recycled through air, land, water, and the biosphere for decades to come. This study was conducted to measure the concentrations of PCBs in the serum samples of 112 women by GC/MSD and GC/ECD. The main results of this study were as follows. 1. PCBs were detected in all samples. The mean $\pm$SD levels of PCBs in the serum were 3.613$\pm$0.759 ppb, and median were 3.828 ppb. 2. The correlation coefficients of the concentrations of 13 PCB congeners were from minimum, 0.7913 to maximum, 0.9985, and all was significant(p=0.0001). The correlation coefficient between the concentrations of PCBs and p,p'-DDE was 0.9641(p=0.0001). 3. There was a positive association between age and PCBs' concentrations (simple linear regression ; $R^2$=0.86, $\beta$=0.08023, p<0.001). 4. There was a positive association between total lipids in the serum and PCBs' concentrations (simple linear regression ; $R^2$=0.7058, $\beta$=0.00486, p<0.001). 5. For possible predictors of PCBs and p,p' -DDE levels in the serum, age adjusted model (Y=$\beta$$_{0}$+$\beta$$_1$age+ $B_2$X) was applied. For BMI, major residential area, wether to eat caught fish by angling, where to eat caught fish by angling(by parents in the past), fish consumption, meat consumption, meat consumption, and dairy consumption, there was no association. For total conception frequency and lactation frequency and lactation period, there was negative association.ion.

  • Many risk factors exist for chemotherapy-induced nausea and vomiting (CINV). This study utilized a multivariate projection technique to identify which risk factors were predictive of CINV in clinical practice. A single-centre, prospective, observational study was conducted from January 2007~July 2010 in Singapore. Patients were on highly (HECs) and moderately emetogenic chemotherapies with/without radiotherapy. Patient demographics and CINV risk factors were documented. Daily recording of CINV events was done using a standardized diary. Principal component (PC) analysis was performed to identify which risk factors could differentiate patients with and without CINV. A total of 710 patients were recruited. Majority were females (67%) and Chinese (84%). Five risk factors were potential CINV predictors: histories of alcohol drinking, chemotherapy-induced nausea, chemotherapy-induced vomiting, fatigue and gender. Period (ex-/current drinkers) and frequency of drinking (social/chronic drinkers) differentiated the CINV endpoints in patients on HECs and anthracycline-based, and XELOX regimens, respectively. Fatigue interference and severity were predictive of CINV in anthracycline-based populations, while the former was predictive in HEC and XELOX populations. PC analysis is a potential technique in analyzing clinical population data, and can provide clinicians with an insight as to what predictors to look out for in the clinical assessment of CINV. We hope that our results will increase the awareness among clinician-scientists regarding the usefulness of this technique in the analysis of clinical data, so that appropriate preventive measures can be taken to improve patients' quality of life.

  • The objectives of this study were to obtain normative data for 45 mother-daughter pairs on spine, femur(neck, ward's triangle, trochanter) and total body BMD (bone mineral density) measured by dual energy X-ray absorptiometry, anthropometric measurement, body composition, biochemical markers, nutrient intakes and energy expenditure, to determin the interrelations of these factors within each group, to measure familial resemblance for each variable. We observed significantly positive correlations between height, weight, head, hip and calf circumferences, tricep, femur and calf skinfold thickness, total lean body mass(=weight-total fat body mass-bone mineral content), protein and fat intakes, Ca index, serum total protein and albumin of monter-daughter pairs(p<.05-p<.001). Among mothers, age, osteocalcin, higher, weight, Ca and energy intakes were predictors of BMDs. Among daughters, weight and energy intake were predictors of BMDs. The BMD in lumbar spine(r=.48, p<.01), femoral neck(r=.38, p<.05), ward's triangle(r=.36, p<.05) of the mothers were significantly correlated with those of the daughters, after adjustment for mother's age, hight, weight, osteocalcin, Ca and energy intakes and daughter's weight, energy intake. In regression analyses, mother's BMD also were positively associated with daughter's BMD in lumbar spine, femoral neck, ward's triangle. Our findings support that mothers with low BMD tended to have daughters with low BMD. In the age groups studied, as well genetic factors as environmental factors may have an important role in determining BMD. This study suggests that women may successfully enhance their genetically determined BMD through adequate nutrient intakes and weight bearing exercise.
