입.퇴원 module 시스템의 효과 분석 -ocs 입원 관리 시스템 효과 분석- (The evaluation of admission-discharge module system by OCS)

  • Background : There were so many patients who are waiting for admission in Emergency room in spite of more than one hundred empty beds everyday. This study was conducted to evaluate admission-discharge module system by OCS which reduce empty beds. Methods : The data of bed utilization in general beds from 2004 were reviewed. For evaluation of performance at admission-discharge module system by OCS, the change of Occupancy of bed were calculated. Results : The percentage of Average Bed Emptiness was changed from 13.8% to 9.2%. The residents in surgery(100%) and in internal medicine(75.5%) approved this system. Conclusion : The personnel in hospital recognized that it was very important to manage bed. The management of beds by OCS was helpful to reduce empty beds and was important.

응급의료센터 내원환자 진료시 소요시간과 관련된 요인 (Factors Related to Waiting and Staying Time for Patient Care in Emergency Care Center)

  • 3차 의료기관 응급의료센터의 총 진료소요시간을 단축하여 업무의 효율성을 높이고 환자적체를 해소하는 방안을 마련하기 위하여 l997년 4월 1개월 동안 대구광역시 소재 영남대학교병원 응급의료센터 내원환자 1,742명을 대상으로 환자의 특성, 응급진료와 관련된 내용 및 응급진료시간, 그리고 상호관련성을 분석하였다. 평균 초진소요시간은 83.3분이었고, 남자 83.1분, 여자 84.9분여였으며, 평균 총 진료소요시간은 전체 698분이었고, 남자 718.0분 여자 670.5분이었다. 총 진료소요시간은 고령일수록 증가하였으며 의료보호환자에서 초진 및 퇴실시간이 가장 많이 소요되었고 산재환자는 가장 적게 소요되었다. 전원시 소견서를 구비하지 않은 경우에 초진소요시간어이 많았으며, 총 진료소요시간은 외래어에서 전과된 경우, 타병원으로부터 전원된 경우, 전원시 소견서를 구비한 경우와 OCS를 부분적으로 사용한 경우에 많았다. 약물중독환자, 심폐소생술을 시행한 환자, 내과환자 수혈을 받은 환자 및 복합진료 여부가 3개과 이상이었던 환자에서 총 진료소요시간이 많았으며 당직인턴수가 4명이었던 경우가 총 진료소요 시간이 5명이었던 경우에 비해 더 많이 걸렸다. 입원한 경우, 입원명령후 공실이 없었던 경우에서 역시 총 진료소요시간이 증가하였으며 총 진료소요시간과 유의한 상관관계를 갖는 연속변수로는 환자의 연령, 방사선검사수 및 일반검사수였다. 초진소요시간에 대한 중회귀분석결과, 응급의학과 환자, 응급환자, 내원시 심폐소생술 시행환자유무, 내원시 기관내삽관 여부 등이 유의한 독립변수였다. 총 진료소요시간에 대한 중회귀분석결과, 공실유무, 일반검사수, 최종진료과, 타병원 전원유무, 방사선검사수, 퇴원약 유무, 입원실 종류, 입원유무, 담당전공의 연차, 내원원인, 내원시 심폐소생술 시행환자 유무, 수술여부, 병원직원 지인유무 및 특수검사수가 유의한 독립변수였다. 이상의 결과로 보아 응급의료센터내의 환자적체현상을 해결하기 위한 방안으로는 응급환자와 비응급환자를 분류하는 제도적 장치가 필요하며, 필수적인 경우에 한해 일반검사 및 방사선검사를 실시하도록 하며, 병동내의 과별 지정병상을 유동적으로 운영하여 응급의료센터 환자가 우선적으로 입원될 수 있도록 각과의 협조가 필요할 것으로 사료된다.

The Survey of Dentists: Updated Knowledge about Basic Life support and Experiences of Dental Emergency in Korea

  • Background: Various medical emergency situations can occur during dental practices. Cardiac arrest is known to comprise approximately 1% of emergency situation. Thus, it is necessary for dentists to be able to perform cardiopulmonary resuscitation (CPR) to increase the chance of saving patient's life in emergency situation. In this paper, we conducted a survey study to evaluate to what extent dentists actually understood CPR practice and if they had experience in handling emergency situations in practice. Method: The survey was done for members of the Korean Dental Society of Anesthesiology (KDSA), who had great interest in CPR and for whom survey-by-mail was convenient. We had selected 472 members of the KDSA with a dental license and whose office address and contact information were appropriate, and sent them a survey questionnaire by mail asking about the degree of their CPR understanding and if they had experience of handling emergency questions before. Statistical analyses -frequency analysis, chi-square test, ANOVA, and so on- were performed by use of IBM SPSS Statistics 19 for each question. Result: Among 472 people, 181 responded (38.4% response rate). Among the respondents were 134 male and 47 female dentists. Their average age was $40.4{\pm}8.4$. In terms of practice type, there were 123 private practitioners (68.0%), 20 professors (11.0%), 16 dentists-in-service (8.8%), 13 residents (specialist training) (7.2%) and 9 military doctors (5%). There were 125 dentists (69.1%) who were specialists or receiving training to be specialist, most of whom were oral surgeon (57, 31.5%) and pediatric dentists (56, 30.9%). There were 153 people (85.0%) who received CPR training before, and 65 of them (35.9%) were receiving regular training. When asked about the ratio of chest pressure vs mouth-to-mouth respiration when conducting CPR, 107 people (59.1%) answered 30:2. However, only 27.1% of them answered correctly for a question regarding CPR stages, C(Circulation)- A(Airway)- B(Breathing)- D(Defibrillation), which was defined in revised 2010 CPR practice guideline. Dentists who had experience of handling emergency situations in their practice were 119 (65.6%). The kinds of emergency situations they experienced were syncope (68, 37.6%), allergic reactions to local anesthetic (44, 24.3%), hyperventilation (43, 23.8%), seizure (25, 13.8%), hypoglycemia (15, 8.3%), breathing difficulty (14, 7.8%), cardiac arrest (11, 6.1%), airway obstruction (6, 3.3%), intake of foreign material and angina pectoris (4, 2.2%), in order of frequency. Most respondents answered that they handled the situation appropriately under the given emergency situation. In terms of emergency equipment they had blood pressure device (70.2%), pulse oximetry (69.6%), Bag-Valve-Mask (56.9%), emergency medicine (41.4%), intubation kit (29.8%), automated external defibrillator (23.2%), suction kit (19.3%) and 12 people (6.6%) did not have any equipment. In terms of confidence in handling emergency situation, with 1-10 point scale, their response was $4.86{\pm}2.41$ points. The average point of those who received regular training was $5.92{\pm}2.20$, while those who did not was $4.29{\pm}2.29$ points (P<0.001) Conclusion: The result showed they had good knowledge of CPR but the information they had was not up-to-date. Also, they were frequently exposed to the risk of emergency situation during their dental practice but the level of confidence in handling the emergency situation was intermediate. Therefore, regular training of CPR to prepare them for handling emergency situation is deemed necessary.

  • Injury in agriculture is a serious public health issue with a major impact on the lives of Korean farmers. It is one of the leading causes of death and is also a major cause of longand short-term disability. In 2001, the social cost of one accident in agricultural machinery was estimated as 97.7-97.8 million won that is 4 fold of farm household income in Korea. Effective prevention and control of injuries requires a system of surveillance that monitors the incidence of injuries, their causes, treatment and outcomes. This requires an integrated system of data collection, analysis and interpretation and communication. Creating effective injury surveillance system in Korea requires to establish a framework for a national agenda. Discussions regarding the development of the framework should address, but not be limited to issues related to Data Holdings and Linkages; Capacity and Skills; Communication; Interconnection; and Surveillance Products. Ideally, an injury surveillance system would meet the information requirements across all sectors, while allowing each to have the ongoing information it needs for its policy and programming needs. This study was carried out to develop a surveillance system of agricultural injuries in Korea. Study subjects were residents who lived in a typical agricultural area (Yangpyung area in Kyung-gi province). The main data sources were reports of village headmen, compared with data of 'National Emergency Management Agency', 'National Health Insurance Corporation', 'Insurance of National Agricultural Cooperative', and 'Emergency Medical Centers'. Each data were reviewed to validate the strengths and weaknesses.

대규모 유류유출사고 대비 환경보건 대응계획 수립을 위한 기반연구 (Scientific Basis of Environmental Health Contingency Planning for a Coastal Oil Spill)

  • Objectives : This study presents a scientific basis for the establishment of an environmental health contingency plan for dealing with accidental coastal oil spills and suggests some strategies for use in an environmental health emergency. Methods : We reviewed the existing literature, and analyzed the various fundamental factors involved in response strategies for oil spill. Our analysis included data derived from Hebei Spirit oil spill and used air dispersion modeling. Results : Spill amounts of more than 1,000 kl can affect the health of residents along the coast, especially those who belong to vulnerable groups. Almost 30% of South Korean population lives in the vicinity of the coast. The area that is at the highest risk for a spill and that has the greatest number of people at risk is the stretch of coastline from Busan to Tongyeong. The most prevalent types of oil spilt in Korean waters have been crude oil and bunker-C oil, both of which have relatively high specific gravity and contain volatile organic compounds, polycyclic aromatic hydrocarbons, and metals. In the case of a spill of more than 1,000 kl, it may be necessary to evacuate vulnerable and sensitive groups. Conclusions : The government should establish environmental health planning that considers the spill amount, the types of oil, and the distance between the spot of the accident and the coast, and should assemble a response team that includes environmental health specialists to prepare for the future oil spill.

Factors associated with Anxiety and Depression among University Students during the COVID-19 Pandemic in Korea

  • Backgrounds: As the COVID-19 pandemic continues, the university students population is one of the high risk and vulnerable groups. This study analyzed the status of student's mental health, including anxiety and depression caused by COVID-19, and its related factors among a university students in Korea. Methods : Our study subjects included university students in Korea. The survey items included general characteristics, teaching methods, physical activity, eating habits (SDBQ-L), weight-related questions, general anxiety disorder-7(GAD-7), and depression symptoms checklist. Frequency analysis, chi-square test, and logistic regression analysis were performed and data was analyzed using R version 4.1.2 program. Results : The prevalence of anxiety complaints was 5.5%, while the prevalence of depression complaints was 30.8%. In the multivariable logistic regression analysis of factors influencing anxiety, anxiety occurred 19.081 times (95% confidence interval (CI): 2.352-154.807) in the group with unhealthy diet (p<0.01). While in the multivariable analysis to identify factors affecting depression, women were 2.607 times significantly higher (95% CI: 1.255-5.415) than men, 0.407 times (95% CI:0.197-0.840) for residents of the metropolitan area, 3.418 times higher (95% CI:1.633-7.198) for unhealthy dietary people, and 2.225 times higher (95% CI: 1.080-4.582) for weight gainers (p<0.05). The type of class had no effect. Conclusions: COVID-19 and its associated interventions had negative effects on the mental health status including anxiety and depression in university students in Korea. Based on our results we recommend developing mental health interventions and prevention strategies for students in all universities in Korea.

Awareness of Breast Cancer Warning Signs and Screening Methods among Female Residents of Pokhara Valley, Nepal

  • Background: Breast cancer is the second most common cancer in the world and by far the most frequent cancer among women. Objective: The present study was undertaken to assess the awareness of breast cancer warning signs and screening methods among the women of Pokhara valley, Nepal. Materials and Methods: A cross-sectional questionnaire survey was carried out in a community setting with the female population. The questionnaire was administered in face-to-face interviews by trained research assistants. Results: Nepalese women demonstrated poor awareness of warning signs like a breast lump, lump under the armpit, bleeding or discharge from the nipple, pulling of the nipple, changes in the position of the nipple, nipple rash, redness of the breast skin, changes in the size of the breast or nipple, changes in the shape of the breast or nipple, pain in the breast or armpit, and dimpling of the breast skin. While 100% of nurses were aware about breast self-examination(BSE), mammography and warning signs of breast cancer. Levels of knowledge were significantly poorer in women with other occupations. Graduates were more aware about BSE, mammogram and warning signs of breast cancer compared to those with other educational levels. Conclusions: The findings indicated that the level of awareness of breast cancer, including knowledge of warning signs and BSE, is sub-optimal among Nepalese women.

한국 성인에서 심폐소생술에 대한 인지, 교육경험이 그 시행능력에 미치는 영향 (Impact of Awareness and Educational Experiences on Cardiopulmonary Resuscitation in the Ability to Execute of Cardiopulmonary Resuscitation among Korean Adults)

  • 이 연구는 심폐소생술의 인지, 교육경험, 시행능력 간의 관계를 확인함으로써 인구학적요인, 지역적 요인, 의료 현황, 교육적 경험 등이 심폐소생술 시행능력에 미치는 영향을 파악하고자 하였다. 이러한 연구목적을 달성하기 위해 2014년도 지역사회 건강조사 자료를 활용했다. 이 자료는 대한민국 전체 인구를 대상으로 2014년 7월 기준 시, 군, 구에 거주하는 만 19세 이상의 성인 중, 조사에 참여한 228,712명에서 보건소별로 평균 900명(목표오차 ${\pm}3%$)으로 고려하여 표본을 추출해 실시하였다. 자료분석은 R 통계프로그램 3.1.3.을 사용하여 카이제곱검정, 상관분석, 다중로지스틱회귀분석 등으로 이루어졌다. 심폐소생술의 시행능력은 남성에서(3.34배), 젊은 층에서(1.06배), 교육수준이 높을수록(1.61배), 화이트 칼라 직업일수록(1.14배), 소득수준이 높을수록(1.07배), 급만성질환 및 사고중독의 비경험자일수록(0.91배), 고혈압 비진단자 일수록(1.12배), 당뇨병 비진단자 일수록(1.16배, 이상지질혈증 비진단자 일수록(0.86배), 뇌졸중 비진단자 일수록(1.54배), 주관적 건강이 높을수록(1.08배, 1.16배), 비고령사회일수록(0.90배), 심폐소생술 교육경험이 높을수록(3.25배), 심폐소생술 마네킹 실습경험이 있을수록(4.30배) 유의하게 더 높았다. 분석결과, 개인 및 지역사회 요인과 심폐소생술 인지, 교육경험 등은 심폐소생술 시행능력에 영향을 미치는 것으로 확인되었다. 이러한 결과를 바탕으로 국내 환경에 적합한 심폐소생술 교육정책 마련에 적극 고려되어야 할 것이며, 이를 통해 병원 전단계 심폐소생술 능력 향상에 기여할 수 있을 것이다.

한국의 환자 중심 의사 역량 프레임 타당화를 위한 델파이 연구 (A Delphi Study to Validate the Patient-Centered Doctor's Competency Framework in Korea)

  • Defining a competent doctor is important for educating and training doctors. However, competency frameworks have rarely been validated during the process of their development in Korea. The purpose of this study was to validate the patient-centered doctor's competency framework, which had been developed by our expert working group (EWG). Two rounds of Delphi questionnaire surveys were conducted among a panel of experts on medicine and medical education. The panel members were provided with six core competencies, 17 sub-competencies, and 53 enabling competencies, and were asked to rate the importance of these competencies on a 5-point Likert scale. Between April and July 2021, a total of 28 experts completed both rounds. The data of the Delphi study were analyzed for the mean, standard deviation, median, inter-rater agreement (IRA), and content validity ratio (CVR). A CVR >0.36 and IRA ≥0.75 were deemed to indicate validity and agreement. This study found that five enabling competencies were not valid, and agreement was not reached for three sub-competencies and two enabling competencies. In consideration of CVR and the individual opinions of panel members at each session, the final competencies were extracted through consensus meetings of the EWG. The competencies were modified into six core competencies, 16 sub-competencies, and 47 enabling competencies. This study is meaningful in that it proposes patient-centered doctor's competencies enabling the development of residents' milestone competencies, an assessment system, and educational programs.

Transitional care for high-risk elderly patients pre/post discharge by collaboration between general hospital and community pharmacy: a pilot study

  • Background: Medication-related problems (MRPs) frequently occur during the discharge period. Elderly patients, particularly, are at high risk for these problems due to polypharmacy and the use of potentially inappropriate medications. The purpose of this study was to build and implement collaboration between general hospital and community pharmacies to address MRPs among high-risk elderly patients before/after discharge. Methods: This retrospective study was conducted between June and December of 2020. The inclusion criteria were patients with aged ≥65 years; residents of Jeonju; discharged from Jeonbuk National University hospital; either on medication of exceeding 10 medications (or high-risk medications) after hospitalization through the emergency room, or having severe illness. Patients received medication reconciliation and counselling by hospital pharmacists before discharge and home-visit pharmaceutical care as follow-up by community pharmacists after discharge. Results: Twenty-two patients agreed to home-visit pharmaceutical services. Fifteen and 11 patients completed the first and second home-visit pharmaceutical care service, respectively. Forty-two MRPs were identified in 15 patients. The types of high-frequency MRPs were incorrect administration of drug, adverse drug reactions, medication non-compliance, drug-drug interactions, lifestyle modifications, and expired medication disposal. After consultation with the pharmacist, 34 out of 42 MRPs were resolved. Conclusions: Transitional care for high-risk elderly patients before and after discharge was successfully built and implemented through a collaboration between general hospital and community pharmacies. This study suggests that home-visit pharmaceutical services may have positive effects on the safe use of drugs during the transition period; however, additional research is needed to expand on these findings.
