A study on the usefulness of standard evaluation tools for ambulance ride practices of paramedic students (응급구조(학)과 학생들의 구급현장실습 표준 평가도구의 유용성에 관한 연구)

  • Purpose: This study tests the validity of a standard evaluation tool of ambulance ride practices with new evaluation items and supports its application by 119 preceptors. Methods: We tested the validity of standard evaluation items collected from 19 on-site professionals assessing ambulance ride practices. New evaluation items, 'understanding of emergency rescue equipment' and 'ability to communicate', were added as additional criteria. The modified Delphi technique was used to test the three evaluation areas and ten evaluation items. We used the analytic hierarchy process to analyze the weighting value of the reconstructed evaluation tool model. Results: All three evaluation areas and 10 evaluation items within the standard evaluation tools used for ambulance ride practices corresponded with the consistency index, degree of convergence, and agreement in the modified Delphi panel. Conclusion: These results provide evidence of the consistency and usefulness behind preceptors' use of this standard evaluation tool in ambulance ride practice.

Correlation between injury and the injury severity score in geriatric traffic accident patients transported by 119 rescue services (119로 내원한 노인교통사고 환자의 손상과 손상중증도계수의 상관관계 분석)

  • Purpose: The purpose of the study was to investigate the correlation between injury and injury severity score (ISS) in geriatric traffic accident patients transported by 119 emergency medical technicians (EMTs). Methods: The subjects were 240 traffic accident patients over 65 years old transported by 119 rescue services emergency departments in Busan from January 1, 2014 to August 31, 2015. Results: ISS increased in motorcycle traffic accidents (p=.026), truck injuries (p=.005), and head and neck injury (p<.001). Vital signs were evaluated by 119 rescue EMT. ISS increased in cases of unresponsive unconsciousness (p<.001), hypotension (p=.001), and bradycardia (p<.001). The need for bleeding control and dressing by the EMTs increased ISS significantly (p=.022). Conclusion: In the initial evaluation of geriatric traffic patients, ISS can increase in motor cycle accidents and truck injuries. Due to high ISS, patients with head and neck injury, chest injury, hypotension, or tachycardia should be transferred to advanced level hospitals.

Comparing statistical data on 119 ambulance runs and ambulance crew in Korea and Japan (한국과 일본의 소방 구급 출동 및 구급인력 규모 비교 연구)

  • Purpose: This study aimed to compare and analyze statistical data on 119 ambulance runs and ambulance crew, which are the components of the emergency medical services system in Korea and Japan. Methods: Data from National Fire Agencies of both Korea and Japan were collected and statistically compared. Results: With regard to the ratio of 119 ambulance runs, Korea's ratio has been gradually and continuously growing beyond that of Japan (Korea 4708.11, Japan 4706.47) since 2014. The ratio of firefighting ambulances in Korea was 2.59 ($2.59{\pm}0.10$), and was 4.76 ($4.76{\pm}0.12$) in Japan. The ratio of 119 ambulance crews in Korea was 15.55 ($15.55{\pm}2.03$), and was 47.24 ($47.24{\pm}1.06$) in Japan. Among the ambulance crews, the ratio of paramedics was 33.81 ($33.81{\pm}5.85$) in Korea and was 38.86($38.86{\pm}4.10$) in Japan. Conclusion: The ratio of 119 ambulance runs in Korea has already exceeded that of Japan, but the numbers of 119 ambulance crews and paramedics qualified for special emergency treatment are still insufficient. Therefore, supply and demand policy that promotes the development of the firefighting ambulance service system is necessary.

The necessity for education on endotracheal intubation through video laryngoscope - A focused on paramedic students - (비디오 후두경을 통한 기관내 삽관 교육의 필요성 - 응급구조과 학생을 중심으로 -)

  • Purpose: The aim of this study was to verify the necessity of endotracheal intubation through video laryngoscope and to provide basic data to inform the provision of video laryngoscope education. Methods: Eighty paramedic students participated in this study. A survey was conducted from November 5, 2018 to December 7, 2018. Data were analyzed with independent t-tests, and the chi-squared test. Results: The video laryngoscope is a highly usable instrument that can easily be applied during training. The instrument provides better visual evaluation of the normal airway (p=.004), the airway in case of cervical collar and head fixation (p=.000), and the airway in case of tongue edema (p=.000). The time of endotracheal intubation in the normal airway was significantly less with the video laryngoscope compared with the direct laryngoscope. The success rate of tracheal intubation was significantly higher in the video laryngoscope group than in the direct laryngoscope. Conclusion: This study suggests the necessity of education on endotracheal intubation through video laryngoscope in the professional airway maintenance training course of emergency department students. The video laryngoscope is easier to apply than the direct laryngoscope in cases of intubation in various clinical situations.

  • The purpose of this study which was conducted by framing of standardized advanced emergency care instructor qualification course outline and training competent instructor Course development based on following educational principle and it would be expected more improved aspect. Advanced Emergency care Instructor Qualification Course Development (1) based on advanced emergency care instructor job analysis and paramedic job description. (2) Learning of emergency care instructor qualification course is continuous. It is important to begin at the learner's level of knowledge and to relate new learning to information the learner needs. (3) Learning of emergency care instructor qualification course is purposeful and must make sense to the learner. Progress in learning must make sense to the learner. Progress in learning must be constantly appraised through feedback. The purpose of learning BLS information and skills must be kept in sharp focus. (4) Learning involves as many senses as possible. The more stimulating a learner activity is to the senses, the longer the information will be retained. Conservative figures indicate that 75% of what is heard is for-gotten after 2 days. It has been said that learners remember (5) Learning activities must be appropriate for the emergency situation through the PBL educational method. In BLS lecture skill learning, the greatest proportion of class time should be spent in manikin practice, using performance sheets as a learning tool or guide. (6) Learning must be stimulating. Instructors can motivate learners by helping them achieve higher levels of proficiency and encouraging other levels of course completion, such as instructor and instructor trainer. (7) Learning is affected by the physical and social environment. The physical environment should be conducive to both the kind of learning taking place and the activities used for learning. Organ dose reconstruction for the radiation epidemiological study of Korean radiation workers: The first dose evaluation for the Korean Radiation Worker Study (KRWS)

  • The Korea Institute of Radiological and Medical Sciences has started a radiation epidemiological study, titled "Korean Radiation Worker Study," to evaluate the health effects of occupational exposure to radiation. As a part of this study, we investigated the methodologies and results of reconstructing organ-specific absorbed doses based on personal dose equivalent, Hp(10), reported from 1984 to 2019 for 20,605 Korean radiation workers. For the organ dose reconstruction, representative exposure scenarios (i.e., radiation energy and exposure geometry) were first determined according to occupational groups, and dose coefficients for converting Hp(10) to organ absorbed doses were then appropriately taken based on the exposure scenarios. Individual annual doses and individual cumulative doses were reconstructed for 27 organs, and the highest values were observed in the thyroid doses (on average 0.77 mGy/y and 10.47 mGy, respectively). Mean values of individual cumulative absorbed doses for the red bone marrow, colon, and lungs were 7.83, 8.78, and 8.43 mSv, respectively. Most of the organ doses were maximum for industrial radiographers, followed by nuclear power plant workers, medical workers, and other facility workers. The organ dose database established in this study will be utilized for organ-specific risk estimation in the Korean Radiation Worker Study.

A Legal Consideraton of License and Certified Qualification of Paramedics (응급구조사의 자격과 면허에 대한 소고)

  • Purpose: The aim of this study is to identify the legal concept of a license and a qualification for health care personnel. We analyze the appropriateness of the qualification for the first-class paramedics and the sufficiency of 3-year job experience in substitution for college level academic major. Method: We reviewed Laws on Healthcare which stipulate academic and career qualifications for healthcare personnel for whom college-level academic training and job experience to get a license. Results: There is no legal consideration which permits major and educational background substituted by job experience. Only third-class hygienist who graduated from a high school or the equivalent with 3 year job experience can apply for first-class hygienist' national license examination without educational requirements. However, it is nothing to compare with first-class paramedics because the work of hygienist is not directly related to patients care and there is no need of doctor's supervision for hygienist' job. Paramedics have a major role in saving emergency patients in making prompt decision, applying appropriate emergency treatment, securing intravenous route, managing intubation, applying defibrillator, and etc. Conclusion: Ministry of Health & Welfare should grant license to paramedics and annul replacement of academic major background. In addition, it is imperative to broaden the scope of paramedics' job so that they can guarantee patient's life saving in emergency situation with quality major curriculum of college level.

Comparison of visual blood loss estimates and subjective emergency according to clothing color : quasi-experimental study using bleeding simulation (의복의 색상에 따른 시각적 출혈량 추정값의 정확도와 주관적 응급도의 차이 비교 : 출혈모의환자를 이용한 유사실험연구)

  • Purpose: The purpose of this study was to compare visual blood loss estimation and subjective emergency according to clothing color. Methods: This is a quasi-experimental study which involved the use of mock bleeding patients wearing different colors tops. Results: Differences in visual estimates according to clothing color were significant in both paramedic students (F=6.69, p=.002) and the general department students (F=20.92, p=.000). When looking specifically at the accuracy of visual estimates, the paramedic students group tended to underestimate (50% white, 62.5% black, 32.5% yellow) the actual blood volume in all experimental conditions. On the other hand, the general department group tended to overestimation (45% white, 40% black, 67.5% yellow). The subjective emergency was also found to differ between paramedic students (F=13.58, p=.000) and general department students (F=9.67, p=.000). Conclusion: Paramedics treating bleeding patients at pre-hospital stages need to pay attention to blood loss estimations depending on clothing color, a factor not to be neglected or underestimated.

Effects of 60 Minutes Cardiopulmonary Resuscitation on Blood Lactic Acid Concentration, Heart Rate, and Rating of Perceived Exertion in Rescuers

  • In this study, when cardiopulmonary resuscitation continued for a long time, the rescuer's blood lactic acid concentration, heart rate, and rating of perceived exertion were measured to identify the change in the rescuer's fatigue. Data collection was conducted from July 5 to July 9, 2021, with a total of 24 students, 12 students department of special warfare medical non-commissioned officer, and 12 students department of emergency medical technology at D University, undergoing a two-person alternative chest compression resuscitation for 60 minutes. As a result of the study, the rescuer's blood lactic acid concentration, heart rate, rating of perceived exertion, and chest compression speed were significantly changed according to the duration of CPR (p<.001, p<.001, p<.001, p<.001). blood lactic acid concentrations at every measurement cycle (30 minutes, 40 minutes, 50 minutes, 60 minutes) showed a significant positive correlation with heart rate (r=.696, p<.001, r=.672, p<.001, r=.709, p<.001, r=.782, p<.001), there was also a significant positive correlation with the rating of perceived exertion (r=.476, p<).05, r=.426, p<.05, r=.470, p<.05, r=.470, p<.05). Therefore, monitoring the fatigue of rescuers using heart rate and rating of perceived exertion will be useful for maintaining high-quality chest compression in situations where cardiopulmonary resuscitation is required for a long time.

  • In the latest date, medical body area networks (MBANs) are emerging as a new technology for diagnosis the human body. MBANs in the health care fields are based on short-range and low-power (e.g. ubiquitous computing) among small-sized devices, and have been used by means of medical services. In this paper, we proposed an emergency message transmission protocol using carrier sense multiple access/time division multiple access in MBANs. This scheme focuses on dependability and power-efficiency. In order to increase the reliability of the transmission, this scheme modified a MCTA slot of IEEE 802.15.3 standard to a SR-MCTA slot. SR-MCTA slot is assigned by MBAN coordinator according to requesting terminal nodes. The method, having the priority of transmission, occurs a collision packet randomly. Results from this proposed solution revel that reservation-based TDMA medical body area network(MBAN) protocol for transmitting emergency message was improved in terms of transmission delay.
