How to Improve the Emergency Medical Service System: Levels of Knowledge and Techniques in Prehospital Care for 119 Relief Squad Members and Emergency Room Nurses (병원 전 단계 응급의료 서비스 개선방안에 관한 연구 : 119 구급대원과 응급실 간호사의 응급처치 지식 및 기술 숙련도 비교)

  • During the past decade or so in Korea, 119 relief squad members were supposed to respord first urgent or emergency cases. The primary purpose of this study was to assess what levels of knowledge and techniques in prehospital care the 119 relief squad members showed. Data regarding the knowledge and technique levels were collected from both the 119 relief squad members (n=63) and the emergency room nurses(n=46). The Results indicated that the 119 relief squad when compared to emergency nurses, showed higher scores for knowledge and techniques in some areas of prehospital care but not in other areas, However, no differences in knowledge and technique were found when the ANOVAs were calculated with two covariates : duration of their career in emergency medical services and frequency of their exposure to lectures on emergency medical technology. In addition, many respondents in the 119 relief squad group rated themselves poorly in knowledge and techniques of prehospital care, The findings imply that qualitatively better curricula should be given to the 119 relief squad members before they are allowed to play an important role in the emergency medical service system. These findings are also discussed in the context of improving the emergency medical service system.

A Study on Textbook for Training of the Emergency Medical Dispatchers in Korea (응급통신관리자 양성을 위한 교재개발 방안연구)

  • The purposes of this study which was conducted by inquiring into some curricula for Emergency Medical Dispatchers(EMDs) are to introduce a curriculum into Korea and develop a textbook for training of the EMDs in Korea. The conclusions from this study were summarized as follows; (1) There are some professional demands in our Emergency Medical Dispatch field therefore, now it is necessary to set up an education system for the EMDs in Korea and give them a suitable name to this change. (2) Some historical turning points which are condensed from the developmental process in the USA. including "Medical Self Help" with no formal dispatch protocols, Medical Priority Dispatch System, Practice Standard, Dispatch Protocols will serve as a good reference for establishing and education system for the EMDs. (3) To effectively train the EMDs in Korea, we needed to be introduced to Emergency Medical Dispatch : National Standard of Curriculum from the U.S. Department of Transportation & the National Highway Traffic Safety Administration which is made up Basic Emergency Medical Concepts, Information Gatherin & Dispatch, Introduction to the Emergency Medical Dispatch Protocol Reference System(EMDPRS) & 32 Chief Complaint Types. (4) The introduced curriculum closely related to the medical director, the direct indirect medical control, the scope of practice in the USA, should be revised for the Emergency Medical Service System of Korea. But to reduce the developmental process & time, it is necessary to postpone the consideration on these factors until publishing a new textbook. (5) This study which gives the cornerstone about some developmental methods on textbook for EMDs will be helpful to build up an education system for EMDs such as Curriculum, EMD Certification, National Academy of Emergency Medical Dispatch to fit the Emergency Medical Service System of Korea in the future.

Association between Helicopter Versus Ground Emergency Medical Services in Inter-Hospital Transport of Trauma Patients (응급의료 전용헬기와 지상 앰뷸런스를 이용한 병원 간 이송에서 외상 환자의 예후 비교)

  • Purpose: To improve outcome of severe trauma patient, the shortening of transport time is needed. Although helicopter emergency medical services (HEMS) is still a subject of debate, it must also be considered for trauma system. The aim of this study is to assess whether transport method (HEMS versus ground EMS) is associated with outcome among inter-hospital transport. Methods: All trauma patients transported to regional emergency center by either HEMS or ground EMS from September 2011 to September 2014. We have classified patients according to two groups by transport method. Age younger than 15 years and self-discharged patients were excluded. Results: A total of 427 patients were available for analysis during this period. 60 patients were transported by HEMS and 367 patients were transported by ground EMS. HEMS group had higher mortality than ground EMS group (23.3% vs 3.5%; p<0.001), and included more patients with excess mortality ratio adjusted injury severity score (EMR-ISS) above 25 (91.7% vs 48.8%; p<0.001). In the multivariable regression analysis, HEMS was not associated with improved outcome compared with ground EMS, but only EMR-ISS was associated with a mortality of patients (odds ratio, 1.06; 95% confidence interval, 1.04-1.09). Conclusion: In this study, helicopter emergency medical services transport was not associated with a decreased of mortality among the trauma patients who inter-hospital transported to the regional emergency center.

Emergency Medical Technician's Works within Hospital at Partial Areas (일 지역 병원내의 응급구조사 업무 - 광주광역시를 중심으로 -)

  • This study examines and analyzes EMTs works in hospital with 48 subjects working for hospitals at partial areas. It aims to provide basic materials for practice-centered learning to students at Dept. of Emergency Care and information of roles of EMTs. As a result of study, it was found that EMTs works included classification of severity of patients when they came to hosptial, supervision and move of serious case, move of patients and a member of cardiopulmonary resuscitation team as well as filling empty of medical staff and playing medical assistant under direct control of doctors and they played important roles in medical examination. These results showed that EMTs could extend their areas of service within hospital and they could have a positive influence on their seniors' employment.

A Research on the Actual Condition of the Prehospital Emergency Care and Education in 119 Emergency Medical Services (119구급대의 병원 전 응급처치 실태 및 교육 현황 분석)

  • This is the study of actual condition and improvement on emergency medical treatment by 119 emergency medical service personnel. The subjects in this study were 299 emergency medical service personnel. Data were collected from May 1 to August 31 of 2010, and analyzed by SPSS 12.0 program. The major area of study of them were study of emergency medical services 41.5%, study of nursing 10.0%, fire fighting related studies 15.4%, health related studies 1.0%, and others 32.1%. The certificate of them were 1st class emergency medical technician(EMT) 38.1%, 2nd class EMT 33.8%, nurse 9.4%, first aid education 14.0%, and others 4.7%. Frequency of Prehospital emergency care, oxygen supply(274), splint apply(229), spinal immobilization(229), external bleeding control(223), medication(7), intravenous(4). Professionally trained EMT makes possible to secure high quality emergency medical treatment in the prehospital phase. Therefore, it is essential for the quality improvement of prehospital emergency care that well trained EMT ride on the ambulance together and take the responsibility for the treatment and transferring of emergency patients. In order to improve the proficiency of 119 emergency medical services personnel, it is also necessary to provide continuous job training programs for the prehospital emergency medical treatment.

Security Structure for Protection of Emergency Medical Information System (응급의료정보시스템의 보호를 위한 보안 구조)

  • Emergency medical information center performs role of medical direction about disease consult and pre-hospital emergency handling scheme work to people. Emergency medical information system plays a major role to be decreased mortality and disability of emergency patient by providing information of medical institution especially when emergency patient has appeared. But, various attacks as a hacking have been happened in Emergency medical information system recently. In this paper, we proposed security structure which can protect the system securely by detecting attacks from outside effectively. Intrusion detection was performed using rule based detection technique according to protocol for every packet to detect attack and intrusion was reported to control center if intrusion was detected also. Intrusion detection was performed again using decision tree for packet which intrusion detection was not done. We experimented effectiveness using attacks as TCP-SYN, UDP flooding and ICMP flooding for proposed security structure in this paper.

Analysis of the Korean Triage and Acuity Scale by type of ambulance (한국형 중증도 분류도구를 이용한 구급차별 중증도 분석)

  • Purpose: The purpose of this study is to investigate the characteristics and appropriateness of the Korean-type severity classification by ambulance based on the medical records of 43,561 emergency patients who were brought to the emergency medical center via ambulance between January 1, 2015 and December 31, 2017. Methods: This study analyzed the classification characteristics of the Korean severity classification tool by applying them to emergency patients who visited the emergency medical center. Results: As a result of the study first, among the categories of home hospitals according to the results of visits, "other," "low consciousness," and "dyspnea" in the order of 129 ambulances were statistically significantly higher. In the order of "low consciousness" and "trauma," the "trauma" category was 5.3% higher than that of 129 ambulances. Conclusion: Among the classification items, "others," "low consciousness," and "dyspnea" were significantly higher in the group of patients who boarded 129 ambulances, and "others," "low consciousness," and "traumatic" were significantly higher in the 119 ambulances.

A Study on Alcohol-Related Emergency on the Aged (노인 알코올 응급에 관한 연구)

  • Purpose: The purpose of this study was to prevent the Aged from encountering a sudden accident in an emergency situation caused by drinking, help them have better awareness of drinking problems, and provide basic data for forming sound drinking. Method: A descriptive approach was made to the occurrence of emergency situations caused by drinking for the aged by reviewing literature concerning the subject, along with the analysis of the actual condition through the cases of emergency situations. Result: The Aged had a drink to drown their loneliness due to the loss of social belonging and solitude and attempt suicide, relied on drinking due to the loss of economic roles and reduced income to cause emergency situations, drove when drunken to bring emergency situations to themselves and others, and committed incendiarism after drinking due to social dissatisfaction and mental problems. Conclusion: It is necessary to develop leisure management programs positively and provide an opportunity for sound living culture to old people so that they can have sound entertainment culture other than drinking; such educational programs will serve to form healthy society with no emergency situation caused by the Aged drinking.

Informed Consent and Refusal of Treatment in Emergency Medical Situation (응급의료에서의 설명·동의 원칙과 응급의료거부죄)

  • By analyzing informed consent and the refusal of emergency medical treatment (called patient dumping) under the current Emergency Medical Service Act, this study suggests that an emergency medical professional is only liable for patient dumping if their duty to protect the patient's life takes precedence over the patient's right to self-determination. In emergency medical situations, as in general medical situations, medical treatment should be performed after the emergency medical professional informs the patient about the medical treatment, including its necessity and methods, and obtains consent from the patient. Refusing or evading the performance of emergency medical services on the excuse of the informed consent not considering a waiver or alteration of informed consent requirements without reasonable reasons violates the Emergency Medical Service Act and thus makes an emergency medical professional liable to administrative disposition or criminal penalty. In other words, depending on the existence of a waiver of alteration of the informed consent, patient dumping may be established. If the patient is a minor or has no decision-making ability, and their legal representative makes a decision against the patient's medical interests, the opinion of the legal representative is not unconditionally respected. A minor also has the right to decide over their body, and the decisions of their legal representatives should be in the patient's best interests. If the patient refuses treatment, in principle, the obligation of life protection of emergency medical professionals is the top priority. However, making these decisions in the aforementioned situations in the emergency medical field is difficult because of the absence of explicit regulations regarding these exceptional problems. This study aims to organize the following precedents of the Supreme Court of Korea. The court states that, when balancing the conflicting interests between the duty to provide emergency medical service and the duty to inform is unavoidable for emergency medical professionals, they should put the duty to protect the patient's life ahead of the duty to inform if the patient's life matters. Exceptionally, when a patient has seriously considered whether they should receive treatment before the emergency medical situation, their right to self-determination can be considered equal to the obligation of emergency medical professionals to provide emergency medical treatment. This research also suggests that an amendment of the Emergency Medical Service Act should include the following. First, the criteria for determining the decision-making ability of emergency patients should consist of medical content. Second, additional consent from a medical professional is unnecessary for first-aid treatment. Finally, new provisions for emergency medical obligations for minors, new provisions for the decision standard when there are conflicting opinions about the treatment of a patient, and new penalty provisions for professionals who suspend emergency medical examinations and treatments need to be established.

Driver's Protection Method of Ambulance Car Accident (응급구조 교통사고에 대한 운전자의 보호방안)

  • Exceptive clause of ambulance stated in Road Traffic Laws of ambulance car accidents is not properly applied and emergency staffs who transfer over 85% of emergency cases are to be forced to start out to the emergency field with unstable conditions which they may be punished on the criminal and civil laws. Hereby this study makes the following suggestions to activate the duties of transferring emergency cases by emergency staffs, promote their morale and diminish the victim of emergency staffs due to traffic accidents. 1) It is prescribed that ambulance car drivers should be protected legally by applying the exceptive clauses thoroughly regulated in special case clauses because ambulance cars are used for the purpose of saving the human life. 2) On the traffic accidents occurred during the transfer of emergency cases, the special insurance system is created for treating the ambulance car accidents, not to bind the emergency staff's mistake to traffic law and the victims are compensated by the nation on the basis of insurance system and emergency staffs have the systematic security. 3) On the road over six lanes, emergency lane is set on the center and ambulance car should be used as the exclusive lane. 4) Ambulance car drivers must have the habit of transferring emergency cases rapidly within the range of legal operation.
